In consideration of the acceptance of this registration entry, I, the undersigned, assume
full and complete risk and responsibility for any illness, injury or accident which occurs as
a result of the undersigned’s participation in the event or while I am on the premises of the
event. I hereby release and hold harmless Candlelighters Childhood Cancer Foundation
of Greater Fort Worth, its affiliates and parent corporation, employees, volunteers, sponsors,
and all other persons and entities associated with this event from all claims or liability for
injuries and damages sustained by me whether same be caused by negligence of me,
of the presenters, promoters, or other persons or entities associated with this event or
otherwise. Further, I hereby grant full permission to all the foregoing to use any
photographs, motion picture, recordings, or any other record of this event for any
purpose whatsoever. The Candlelighters Night Run, both the 1K and the 5K, will be held
regardless of weather conditions. Registration is final; all entry fees are non-refundable.
