Out of Home Respite:
A non-skilled service that provides
temporary relief to the caregiver(s)
responsible for performing or managing the care of a functionally
impaired person. Respite is provided in a Division of Aging approved
out-of-home respite care setting.

I am a Medicaid recipient who
needs CCSP services to continue living in the community. What should
I do?

2.
How do I know if CCSP is best
for me?
CCSP staff complete an assessment
of your health needs. You, your
physician, and your family are
involved in the assessment process.
A care plan is then recommended to
you. If you wish to participate in the
program, staff arrange services and
evaluate your condition on a regular
basis to make appropriate changes
in services.

I am not a Medicaid recipient,
but I have limited income and assets
and need services. Am I eligible to
participate in CCSP?
Contact the SOWEGA Council on
Aging for help in determining your
eligibility.
4.
How much do CCSP services
cost?
Some individuals pay a fee for services, while others do not.
5.
How long will I be eligible to
receive CCSP?
As long as you meet eligibility and
service criteria. However, you may
choose to participate or cancel services at any time.

What is CCSP?
The Community Care Services
Program (CCSP) helps people who
are elderly and/or functionally
impaired. CCSP offers community
based care as an alternative to nursing home placement.

What Services Are
Available?
Care Coordination:
Care Coordination is a collaborative
process which assesses, plans,
implements, coordinates and monitors services and options to meet a
client’s needs. Care Coordinators
arrange and monitor services on a
continuous basis, and when necessary, make referrals to other
resources.

Personal Support Services:
A range of supportive services provided in the individual’s home. PSS
provides light housekeeping and
