This year’s luncheon event will take place from 11:30 a.m. to 1:00 p.m. and will feature guest speaker Dr. Gary Gunderson, Vice President for Faith and Health at Wake Forest Baptist Health. Dr. Gary Gunderson, M.Div, DMin, D.Div, is an ordained minister in the American Baptist Convention and joined Wake Forest Baptist Health in 2012. He was previously the Senior Vice President for Faith and Health at Methodist LeBonheur Healthcare in Memphis, TN. In that position, he developed the internationally recognized “Memphis Model” – a large scale partnership between the healthcare community and 500 are congregations. The model demonstrates strategies for lowering costs and increasing access to quality healthcare in difficult population segments within a community. Dr. Gunderson is a noted author and has written four books used in seminary and public health, including Deeply Woven Roots, Boundary Leaders, Leading Causes of Life and most recently, Religion and the Health of the Public. Reservations for the Annual Meeting luncheon are $20 per person, and the faith community and other area businesses and supporters of Hospice of Davidson County are encouraged to attend. For more information, please call (336) 475-5444. The 29th Annual Meeting of Hospice of Davidson County Thursday, April 18, 2013 11:30 a.m. to 1:00 p.m. Davidson County Community College Conference Center Featuring Dr. Gary Gunderson, VP for Faith and Health at Wake Forest Baptist Health $20 per person includes lunch – (336) 475-5444 From the Desk of Laura Owen, CEO . . . I have a quote posted on my computer monitor that reads, “Anything not moving forward is moving backward.” If I were to sum up our agency’s work and accomplishments in 2012, the urgency and momentum reflected in that phrase says it all. In 2012, Hospice of Davidson County definitely moved forward. Although 2012 marks our 27th year as a hospice provider, it is only our third year providing in-patient services and my second year as administrator. Most of our leadership and staff are new or fairly young in their careers, too. That is why it is even more impressive that we experienced such significant growth and program improvements within one, single year. In 2012, care provided in assisted living and nursing homes increased by over 25%. Our Home Care program experienced a similar increase at over 20% from the prior year. Despite the expanded efforts of our competitors to increase market share in Davidson County, we reached our goals while caring for over 750 individuals with terminal illness; a 15% increase from prior year. We also experienced a number of significant “firsts” for our agency. Our first bereavement and grief conference with noted guest speaker Dr. Kenneth Doka, PhD. The expansion of our grief and bereavement programming to offer support groups, one-on-one counseling and grief and bereavement programming reaching over 3,700 children, youth and adults; the expansion of agency therapy programs to provide pet, music, and healing touch therapy services for over 200 individuals; and our significant investment in technology with servers moving to a virtual and secured, off-site location. With strong leadership and a shared vision, we formed and reinforced collaborative partnerships with our community as well. Members of our staff continue to positively impact our community’s quality of life through service on civic, healthcare and faithbased boards. Now, with nearly 100 full, part time and on-call employees, voices speaking to the value and impact of our mission are being heard in and beyond our county’s borders. As we embark on a new year, we recognize the added challenges of increased regulatory oversight, the presence of incentivized Medicare audits, additional reporting requirements and the significant investment in technology needed to meet those requirements. I am excited and encouraged. Given our past performance, the expertise and commitment by staff and our shared vision, and the significant support of our community, we will share in even greater accomplishments this year. From the Board President . . . The winter months are passing and we look forward to the hopes of spring time when things start to regenerate and grow. Hospice of Davidson County continues to grow and with growth comes new challenges. Growth is a good thing but it creates new issues of support. The board of directors has been busy working through all the challenges facing this growing organization. This is a great group of people that volunteer their time to support the staff of Hospice of Davidson County. I cannot express my appreciation enough for each of board members and their contributions to Hospice of Davidson County. My favorite upcoming event is the annual golf tournament on April 25, and I want to ask everyone to get involved and either put together a golf team or donate your time to help this great event. If the golf tournament isn’t for you, there are plenty of other opportunities Craig Idol, Board President to help Hospice of Davidson County with fundraisers this year, so please contact the Hospice staff for more information on how you can be involved. On a personal note, I look for inspiration and guidance every day. I have realized over time I tend to take some things for granted. I only have to look at my children and realize there are so many good things in life and to cherish them every day. I also get inspiration from the Hospice family, as they do their day to day activities to help families in need. Please take the time to look at your own lives and realize there are many people out there that need you help. Thanks to everyone who has been a part of this great organization over the past year. And we look ahead to a great 2013! 2 Energy Healing as an Alternative Therapy Energy healing is a therapeutic approach to healing that affects physical, emotional, mental and spiritual health. Hospice of Davidson County offers Energy Healing as an alternative therapy for patients who are interested in this type of therapy. Nola Fountain, a certified hospice aide with Hospice of Davidson County, is also a certified Quantum Touch® Energy Healing practitioner and conducts the therapy sessions for patients who are interested. In the non-invasive energy healing, the practitioner uses her hands to detect differences in the patient’s natural energy fields. These differences are detected as fluctuations in temperature, density and movement on or around the patient’s body. These subtle changes can indicate areas of imbalance or blockage in the energy field. During energy healing, the practitioner uses focused breathing and light touch to restore balance in these areas. Energy healing can realign the natural energy flow in a patient’s body, reactivating the mind, body and spirit connection to selfhealing. Through energy healing, patients can experience a reduction in anxiety and stress, decreased pain, increased relaxation and overall sense of well being, and the promotion of wound healing. Energy healing as an alternative therapy can help with stress management and relaxation, spiritual distress, pre and post surgical care, chronic pain or illness, disease prevention, end of life care, wound healing and grief or emotional distress. Patients who are interested in energy healing can request the service through their registered nurse (RN). The nurse consults with the patient’s physician or Hospice of Davidson County Medical Director and then obtains an order from the physician for the therapy session. In 2012, Nola conducted more than 200 energy healing sessions with Hospice of Davidson County patients. To learn more about energy healing as an alternative therapy, please call our office at (336) 475-5444. Understanding Grief after Loss Thursdays March 7-March 28 3:00-4:30pm HODC Adm Bldg If there is any one thing to expect in the first months of grief, it’s the unexpected! At HODC, one of our goals is to provide an opportunity for you to learn about your own unique journey into the wilderness of grief. As you have without doubt already discovered, grief is an intensely personal experience. No two people will ever grieve and mourn in the same way, even when they are mourning the death of the same person. Your own grief is unlike anyone else’s, even though you will find you share many commonalities with others in grief. Please join us in a safe compassionate environment as we explore realistic ways of coping with your loss. Call to register at 475-5444 Hospice of Davidson County is in need of direct patient care volunteers to join our hospice team to provide volunteer support in private patient homes and nursing facilities. Direct patient care volunteers provide companionship to patients or even respite care for their caregivers, to give caregivers an opportunity to take a short break. Visitation hours can be flexible and we can even arrange for you to accompany another team member to meet the patient and see if it is right for you. For more information, please contact Sumiko Harmon, Manager of Volunteer Services 336-474-2068 or sharmon@ hospiceofdavidson.org 3 Dementia: The Difficult Frontier By Helen Fitzgerald, CSW, Community Education Coordinator Dementia is a progressive disease that has many faces and stages, presenting an emotional roller coaster for caregivers. Clarifying the type of dementia, the symptoms and medications associated with the dementia, and identifying strategies for support will go a long way in helping you and your patient understand the disease and its progression. Types of Dementia Dementia is a term used to describe the loss of memory and intellectual abilities that interfere with a patient’s daily life. Alzheimer’s dementia is the most common form of dementia, usually constituting about 50-80% of dementia diagnoses. In the early stages of Alzheimer’s dementia, memory loss is mild but progresses to a point where patients lose their ability to communicate or recognize their own environment. According to the Alzheimer’s Association, this form of dementia is the sixth leading cause of death in the United States. Vascular dementia is the second most common form of dementia in the US, especially among older patients. It is usually combined with other types of dementia or cerebrovascular disease. Memory loss is the primary symptom but there is also usually a loss in another cognitive area. Depression is also a common symptom of dementia patients, as they lose their functioning and familiar role. 4 Lewy Body dementia is an irreversible brain disease, usually presenting with parkinson’s-like symptoms of tremors, rigidity, postural instability and slow movement. A Lewy Body patient may often experience mood swings and agitation with day to day activities, again impacting loss of role and control. Patients may present with visual, auditory of sensory hallucinations, such as seeing bugs crawling, thinking someone is hiding in the room, which can be distressing for the patient as well as the caregiver. Frontotemporal dementia describes frontal lobe damage to the brain. The patient may feel perfectly healthy. This is often an inherited disease with symptoms starting at a younger age (40s-60s). Individuals with this dementia often demonstrate, early on, inappropriate personal and social behaviors. They exhibit repetitive and ritualistic behaviors, such as gluttony, pocketing food, and hoarding. The disease causes weakening of muscles so swallowing, breathing and speech are diminished. These patients usually lose verbal ability and become unable to speak, impaired ability to problem solve and make decisions. Dementia as a terminal illness Dementia is a progressive disease, so there will come a time when the patient’s condition will decline with a terminal prognosis. When this occurs, both the patient and family would benefit from the education, specialized care and emotional and spiritual support provided through hospice care, like the services offered by Hospice of Davidson County. Hospice care provides clinical expertise to assess and offer medications to help manage symptoms of patients living with dementia. By managing a patient’s symptoms, hospice care provides comfort to the family, nursing aides to assist with personal care needs, and grief support and education. How to cope as a caregiver of someone with dementia So what does all this mean for families and caregivers? Here are a few survival tips: 1. Demonstrate instead of giving instructions. Your loved one may no longer be able to process verbal information, but can follow your lead. 2. Things that used to be routine and simple may now seem like a major task which takes forever. This can be very frustrating for both the patient and caregiver. Allowing more time to do even the simplest of tasks may make it easier on both of you. 3. Celebrate the completion of any task, as there will come a time in the future when no amount of time will get it done. If you are frustrated, your loved one will sense your frustration and may respond with agitation or negative behaviors, so take a few minutes to walk away and calm yourself. 4. Try to assess any physical source of discomfort or invasion of the patient’s personal space. 5. 6. 7. 8. Any of these things may cause agitation and frustration. Orienting them to your reality does not change their reality. Avoid trying to get them to tell them who you are or where they are. Remember, it is okay and healthy to laugh, not at the patient, but at some of their behaviors and mostly at yourself. Increase the familiar and decrease the unfamiliar. Turn off the television. Play some old familiar music. Decrease overall sensory input, and that will in turn decrease agitation. Respect the patient’s space. Patients with dementia frequently experience “sundowning” which is increased agitation in the late afternoon or evening, usually due to fatigue, both mental and physical. Dementia patients only have partial mental capacity so spending the entire day trying to process information must be exhausting. Recognizing this behavior early on will help you identify medication needs and routines that will help minimize this stress. If you are both up all night, it won’t take long to get burned out. At some point, the only thing you will have left to give, to which the patient will respond, is love. Your presence, your voice, your touch are all familiar and comforting. As a caregiver, acknowledge your grief and loss as your relationship with your loved one changes. Identify and utilize your support resources. Local support groups and adult day care programs that offer respite care may be helpful. If you feel your loved one’s condition is worsening, it may help to talk with your medical provider about home health or hospice support. Dementia is incurable and progressive. Take time to gather the facts, surround yourself with supportive people, and prepare yourself for the progression of the disease. Hospice of Davidson County can be a valuable resource for caregiver support, grief counseling, and education related to the disease process. 5 Volunteer Spotlight: Volunteer Caring with a Canine Connection By Sumiko Harmon, Manager of Volunteer Services If you have happened to visit the Hinkle Hospice House on a Tuesday morning, chances are you have had the privilege of meeting volunteer Pam Gaynor. Pam has been volunteering with Hospice of Davidson County since 2009, but unlike most other volunteers, she doesn’t come alone. Pam is a Pet Therapist and always brings along one of her pet therapy dogs. Pet therapy has been proven to reduce pain ratings and elevate the moods of patients when the animal is present. Hospice of Davidson County has been blessed to have seen firsthand that pet therapy not only benefits our patients, but our staff and family members as well. Pam has been a pet therapist for 18 years, and her passion and dedication shines bright. Her journey began when she lived in Florida, and her vet called and asked for her help with a new project. The goal was to team with the local animal shelter and nursing home to provide the patients with pet therapy. Every Wednesday Pam began picking up a few freshly groomed puppies and taking them to the nursing home to see patients. “People loved it, and so did I,” Pam shared. Pam later joined the People for Animal Welfare Society (PAWS), attended a group, and began her volunteering career. When Pam moved to North Carolina, she and her pet therapy dog, Starr, began visiting patients at Hospice and Palliative Care of Greensboro and the Bell Pam with Cowboy 6 House, a home for adults with ortho-neurogical disabilities. Later she also began pet therapy with Cowboy, her super spunky, full of life Shih-Tzu. This awesome team continued to brighten people’s lives by offering their friendly visits. At Hospice of Davidson County, Pam made her pet therapy visits with Cowboy and Elka, a 110-pound plush coat German shepherd. Elka was big, beautiful, and as gentle as could be. Because of their protective nature, German shepherds are frequently used as pet therapy dogs. Pam first enrolled Elka in obedience and agility training, which helps develop trust in canines. At 12 weeks old, Pam decided to bring her along with Starr on her routine volunteer visits. “She was a natural. She was quiet, she walked calmly, she was great,” Pam said. Pet therapy dogs require an outstanding temperament, which is something that they are born with, not taught. They also must be friendly to anyone or anything that they come in contact with, just as Elka was. At 9 months, she passed her Canine Good Citizens Test and began her lifelong journey as a pet therapy dog. She also completed an AKC tracking course, and would visit schools to show off her skills. Elka loved visiting kids as it was something a little different than her usual outings. Pam shared that the minute they walked into a school, Elka held her head high, stood her chest out, and pranced down the halls, ready to entertain. Pam with Elka We Remember . . . Elka “She was a natural. She was quiet, she walked calmly, she was great.” Pam and her pet therapy dogs have visited a number of schools, nursing facilities, and even the Victory Junction Gang, leaving an impression at every stop. Pam enjoys doing pet therapy in a hospice setting because of the peaceful environment. Pam says, “I like bringing a smile to someone’s face, when this is the worse time of their life. We walk in, and for a few moments, their life is normal again. It makes me feel good to bring a little bit of happiness back into their life.” And happiness is just what Elka brought to everyone that she met at Hospice of Davidson County. It was so captivating to watch her in action because, just as Pam said, Elka was a natural. Elka loved Pam very much, which was obvious as she never took her eyes off her. But none of us were prepared for the heartbreaking news we received last November. At 11 years old, Elka passed away unexpectedly, hours before she was scheduled to make yet another one of her routine pet therapy visits. Many tears were shed among staff and other volunteers, as we together grieved the loss of such an amazing volunteer. She touched the lives of so many in our community and she will never be forgotten. “The bond that I had with Elka is something that people will never know, but they have seen it,” Pam said. The energy that they exerted was so enlightening; you couldn’t help but smile when you witnessed them together. Pam plans to continue her pet therapy with Cowboy, and her newest addition, Ekko. Like Elka, Ekko is a plush coat German shepherd who at 9 weeks, already shows signs of being a natural at pet therapy. Pam recently brought Ekko to the Hospice of Davidson County campus, where staff was able to meet her. Ekko will begin her obedience classes at 12 weeks as she starts her journey as a pet therapy dog. Hospice of Davidson County would like to thank Pam and her pet therapy team for their continued dedication and for choosing to be a part of our team. While she constantly expresses her blessings of being able to do what she loves, we want her to know that we are indeed the lucky ones. Brynda Bullard (right) was a volunteer who helped with administrative tasks and fundraising events, including the annual Ride for Angels every July. Brynda loved to hug her friends, including fellow volunteer Teresa Parks (pictured left). Dan Atwood was a patient-family volunteer who always brighten his patient’s days. He clearly loved NC State and was an avid fan. Sandy Maxwell (pictured in green in the forefront of the image) was a patient-family volunteer who gave her heart to Hospice of Davidson County. 7 STAFF NEWS Donna Albright, RN, BSN, CHPN, recently earned her certification as a Hospice and Palliative Nurse (CHPN) from the National Board of Certified Hospice and Palliative Nurses. Hospice of Davidson County First Annual Luminary Service By Tammy Troublefield, LPTA, Community Outreach Coordinator As the holiday season was approaching we found ourselves outside under rainy skies trying to coordinated and place 400 luminaries for our first Annual Luminary Service. Our Memorial/Honorarium service was something we had talked about providing for the families in our community for a long time. We knew it wasn’t going to be an easy task but most things worth doing seldom are, so we kept going. We dealt with cold rainy weather, grass blowing into our luminaries from weed eaters and had to be cleaned out, our first attempt at “In memory of…” labels bubbled in the rain and had to be removed and replaced. As I said we knew our first year wasn’t going to be easy but providing this luminary service was important to us and a special way for us to remember us patients and reconnect with their families as well as the community. On Tuesday, December 11, 2012, Hospice of Davidson County held its first Annual Luminary Service. From 5-8:00 p.m. we invited our patient’s families and the community to come out to our campus and pick up their pre-ordered 2012 Hospice of Davidson County Christmas Keepsake Ornaments. In addition to pre-ordering a handcrafted porcelain dove you could also reserve a luminary that would be labeled with the name of a loved one and displayed on our campus through the holiday season. Tuesday evening we had several hundred people come out to our campus, enjoy refreshments selected from our Keep Your Fork Cookbook and prepared by our staff and volunteers, pick up their ornaments, had them personalized then stroll around our campus and view the 400 luminaries on display. It was a powerful evening, which brought with it a lot of smiles, tears and memories. The Christmas season has come and gone; the luminaries have come down, been cleaned and stored. Even though I miss looking out my window every evening and seeing the glow from the luminaries, I know Christmas 2013 will be here before we know it and provide our community with another opportunity to remember our loved ones in a special way.
