At Wiltshire Council we’re committed to supporting vulnerable adults and older people. This is why we’re investing in care services which help you live independently in your own homes and communities for as long as possible. Supporting you to live independently in your own homes underpins our Help to Live at Home (HTLAH) programme. Since it was introduced in 2011, it has resulted in a number of new services which have been developed by local people, care providers and front-line staff. This has ensured the services we provide are focused on what you need and want. We understand that for some of you living in your own homes is simply not an option and for many of you, living in supported accommodation or a care home is your preferred choice. Therefore, this Directory contains useful information about care homes which will help you choose the right home for your needs. It also provides details of services that support those of you who care for others. If you need any more information, please do not hesitate to contact us. These companies are: Medvivo who provide a response service linked to telecare (assistive technology); Telecare; community equipment and practical help in the home provided by Medequip, independent support planning and brokerage services provided by the Wiltshire Centre for Independent Living and specialist financial advice provided by Eldercare Group and Ashcourt Rowan. Established in 2001, Celtic Care Services is a family owned and run domiciliary care provider. We are the domiciliary care provider of choice in Chippenham, Corsham, Calne and surrounding towns and villages. This is borne out by our customer base predominantly being made up of individuals who fund their own care or are in receipt of Direct Payments. Domiciliary care is a real alternative to a residential or nursing home. We will work with customers, carers and care providers to make this happen. This Directory outlines what support the council will put in place to help you. We also have staff on hand to discuss any long-term support you may require. This will help ensure you have all the information so you can make the right decisions for your care needs. Assessment If you want to take the next step and need to access services you can ask for a community care assessment. Everyone is entitled to receive an assessment regardless of their financial situation. You can be referred for an assessment by: • yourself or a relative, friend or neighbour; • a social worker or health professional such as a doctor or health visitor; or • any other agency. You can have a carer, friend, relative or advocate with you to help put your views and wishes forward. Your doctor, district nurse or other health professional who knows you well can also be involved if you wish. If you do not speak English, or if you use British Sign Language, we can arrange for an interpreter to be present. We will meet with you so that you can tell us what your needs are. Our assessments focus on what you want to achieve. We will encourage and assist you to contribute as much as possible to your assessment, which means we will talk through with you how best to meet your needs and choices. Care and support planning Following your individual assessment, we will look at whether you are eligible for services paid for by the council. Everyone who is eligible for support will be offered a choice of staff arranging care, or the transfer of funds which they can use to purchase their own care. This is called a direct payment. You will be offered support and assistance throughout this process either from the council or from the Centre for Independent Living whose contact details are on page 45. We can help those who are not eligible for financial support from us with information and advice and with assistance in finding services. Alternatively, you may wish to contact one of the local agencies listed on pages 15–23. People in greatest need are our priority Regardless of your financial situation, you are entitled to information and advice from us about any care and support needs you may have. We follow central government guidance set out in a document called ‘Prioritising of Needs Guidance’, which has been issued to local councils across the country. The aim is to make sure all decisions on who receives financial help with care and support are made fairly, openly and consistently. This means each council has to set criteria to make sure support is allocated fairly and to those in greatest need. Eligibility criteria is divided into four bands representing the seriousness of the risk to your independence: ‘critical’, ‘substantial’, ‘moderate’ and ‘low’. Currently in Wiltshire you are eligible for funded care and support services if your needs are assessed as ‘critical’ or ‘substantial’. This ensures that people in the greatest need will get most help. There are many council services which are not means tested, including bus passes and Blue Badges. However, if you are finding it difficult to manage, you could consider using the services of a care and support or domiciliary care provider. These companies employ staff who can help you with bathing, washing and dressing, as well as practical support with shopping, laundry and cooking meals. Staff will be trained in personal care and safety procedures, moving and handling, hygiene and infection control and first aid. If you need nursing care at home, many of these care providers employ registered nurses. These companies can also help after a stay in hospital with shopping, cleaning and preparing your house for your return and so on. Agencies providing care at home are monitored by the Care Quality Commission (CQC) which regularly inspects agencies and produces reports on their findings which can be found on the CQC’s website: www.cqc.org.uk. Equipment for independent living If you are struggling to manage everyday tasks such as bathing, climbing the stairs, or getting up from your chair, it may be that an item of equipment could make the task easier. Medequip has assessors available to advise you and provide you with the equipment you need. Your request may be dealt with immediately by the person you talk to on the telephone, but if not they will arrange for someone to visit you and assess how best to resolve your difficulties. It aims to promote your independence, improve your safety at home and provide reassurance for you and your carers. It usually consists of a base unit linked to a telephone and a personal pendant which is worn around the neck, on the wrist or attached to a belt. Sensors automatically monitor the home environment. These are linked to the base unit without the need for wiring and can detect potential dangers such as gas, smoke or overflowing water. When the alarm is activated, trained staff at a monitoring centre will respond and, if necessary, arrange for someone to visit you at any time of the day or night. They may also contact the emergency services if necessary. This call centre monitoring and response service is provided to us through Medvivo. If you are interested in Telecare, we can discuss the options with you so that you can decide which sensors will assist you. Both Telecare and the response service can be purchased for a small weekly charge. Adapting your home If you have mobility problems, you might benefit from adaptations to your home. This is another service we can provide. A case coordinator will be allocated to you to make an initial assessment of what you need. Major changes will involve occupational therapists and referral for a meanstested Disabled Facilities Grant to help with some or all of the costs. For care packages which have been arranged by us, contracts and care plans will have been completed by your care manager and the agencies used will be accredited by the Care Quality Commission. People arranging and funding their own care, either privately or through a direct payment, can choose their © 2014 Care Choices Ltd own worker or care agency. It is important that people have choice and control about how they are supported. Before deciding on which agency will support you, ask them to discuss and agree a support plan with you. This should cover the aspects to your care that are important to you. There are a wide range of activities or resources in your community and your agency should be able to discuss how you can access these so you can meet people with common interests. Can they supply references for you to check? • What is the agency charging per hour? What is included in that price? • How long has the agency been operating? • How can you contact the agency in an emergency or outside office hours? • How many carers would the agency assign to care for you and would you see the same one every day? If not, how does the staff rota operate and what happens if your carer goes on holiday or is sick? Will you be notified in advance that a different carer will be attending? • If you’re not happy, how easy is it to make a complaint? • Home care agencies providing personal care, must be registered and inspected by the Care Quality Commission. Ask to see a copy of their registration certificate. • If this is a private contract, ask for a copy of the agency’s contract terms and read these carefully with someone you trust. Get as much help as you can going over it and ask any questions you may have before signing anything. The Citizens Advice Bureau or Age UK could help with this. Other help that is available Support for carers A carer is someone who provides unpaid support to a family member, partner, friend or neighbour. This could be because they are ill, frail, or disabled, or have mental health or substance misuse problems. Anyone can become a carer. Carers can be any age and may or may not live with you. If there is someone who helps and cares for you on a regular basis, they will be a carer. We understand that the caring role can be both satisfying and exhausting. Carers are entitled to an assessment and may be eligible for a range of services. Respite care and short stays Carers sometimes need a break and this can be provided in several ways. 1. You can go into a care home yourself for a week or two to give the person who looks after you a break. 2. Home-based respite care can be arranged when a care worker moves into your home. 3. Family-based respite care may be arranged when you move into another family’s home for a period. The best option for you and your carer will depend on your needs, the urgency of the situation and personal choice and cost. There are a number of different schemes in Wiltshire; contact our customer advisers on 0300 456 0111 for advice. If you are thinking about long-term care, but have had no experience of living in a care home, you may wish to book in for a short stay to see what it’s like. This will help you make an informed decision. If you have nursing needs, you can get additional help with fees. See ‘Care homes with nursing’ on page 31. Support from the NHS NHS Continuing Healthcare is ongoing care arranged and funded by the NHS, when it has been assessed that an individual’s primary need is a health need. It can be provided in any setting. In a person’s own home, it means that the NHS funds all the care that is required to meet the assessed health needs. In care homes, it means that the NHS also makes a contract with the care home and pays the full fees for the person’s accommodation as well as their care. A national policy has been drawn up by central government that sets out the types of conditions that will meet the eligibility criteria for continuing care. Currently these are: • NHS Continuing Healthcare which is provided if you have extensive healthcare needs. This is free of charge. • Continuing Care is a package of care that involves services and funding from both the NHS and ourselves. You can ask for an assessment by health and social care professionals if you think you may be eligible for either NHS Continuing Healthcare or Continuing Care. They will visit you to identify and prioritise your needs and the assessment will be used to decide what services will be provided, where and who funds them. If your needs change, regular reviews will be carried out and your care and/or funding arrangements will change to meet them. You can pick up a leaflet about Continuing Care from your GP or hospital. The team is responsible for completing assessments and reviews and for arranging specialist placements or services including short-term supported housing or vocational support. The initial point of contact to access mental health services is usually your GP and assessments are carried out in conjunction with your GP and the Avon & Wiltshire Mental Health Partnership NHS Trust, the NHS mental health provider in Wiltshire. The mental health social work team is also responsible for coordinating assessments under the Mental Health Act 1983. The Avon and Wiltshire Mental Health Partnership NHS Trust (AWP) provides specialist mental health services based on individual need which help people to find 28 the best way to recover and stay well. The initial point of contact to access mental health services is your GP. They will discuss with you whether help and treatment is best provided from primary health services, or whether a referral to AWP for specialist services is needed. AWP offers a Patient Advice and Liaison Service (PALS) which is a free, confidential and impartial service available to everyone who uses AWP services, their families, carers or anyone who needs advice, information, support or guidance with a particular issue. PALS can help if you: • are worried and not sure what to do or where to go; • need clarity about a situation; • want someone to listen to your experience of services; or • need information and help. They will be able to rule out any other illnesses or conditions that may have similar symptoms to dementia, including depression and infections. Once your GP has checked for any easily treatable causes, then they will be able to talk to you about having an assessment. They may refer you to the memory service, which is for people who are concerned about their memory and think it is affecting their day-to-day life. Here an assessor will talk with you about your memory difficulties, complete some further checks and discuss the impact of any problems or issues. Sometimes getting a diagnosis can take time, but this should not put you off. If you do not feel confident about talking to your GP, there are local charities you can talk to. Alzheimer’s Support and Alzheimer’s Society also have a dementia adviser service that offers personalised information and signposting for people with dementia and their carers – see page 44 for contact details. Residential dementia care checklist For information about dementia care services, such as care and support in your home or residential and care homes with nursing, contact Wiltshire Council on 0300 456 0111. Support in the community As it can be best for the person with dementia to stay in their own home for as long as possible, a range of community care services is available to facilitate this. We can help you to access care in your home; sitting services, which provide a break for any carer you may have; advice on equipment and adaptations; day care centres and respite care. These services can be provided directly by registered providers to those who do not require financial help. However, we will still be willing to offer an assessment of need. You may have a choice of which agency you receive your services from, whether or not we assist with the funding of your service. Please use it in conjunction with the care homes checklist on page 34. RESIDENTS The best indication of a good home is that the residents appear happy and responsive. Are there rummage boxes around the home to stimulate residents? Are residents taking part in activities? Are residents talking to each other, staff and visitors? Do residents look well dressed and groomed? Do residents get to choose their clothes every day? Are people supported to stay independent and do things for themselves e.g. getting dressed, making food etc.? ACCESS If the person with dementia needs, or is likely to need, equipment or adaptations you may want to check that: there are adequate signs and cues for different parts of the home such as dining room and bathroom the home has a policy about locking external doors. BEDROOMS You may want to find out whether the person with dementia can have a single room and whether residents are encouraged to bring in some of their own furniture and possessions to increase familiarity. ACTIVITIES Residents should be stimulated without feeling stressed. Are reminiscence activities available? Are residents able to compile memory boxes? Do staff compile life story books, including photographs and mementoes? STAFF It is important to note whether staff seem friendly and caring towards residents and whether they treat residents with respect. Do they have any training and experience in dementia care? Will the person with dementia have a member of staff particularly responsible for their care? Is there a member of staff you can talk to about your own worries about the person with dementia? MANAGER/HEAD OF HOME A manager who is caring as well as efficient can make all the difference to a home. Does the manager have a knowledge of dementia and can they deal with difficulties that may arise in an understanding way? Is there a full assessment at home before a resident is admitted? Our service is provided in the households of approved ‘adult placement carers’ who offer support or care as required by the person placed with them. Once a person has been referred to us, usually by their care management team, we match them with an adult placement carer who has a vacancy. A series of introductions are arranged to help them get to know each other before deciding whether or not to set up an adult placement. Our team then monitors each placement to help ensure that the arrangement is an enjoyable and fulfilling experience for all concerned. There is a diverse range of sheltered housing schemes in Wiltshire. Some are bungalows; some are flats with communal facilities. All of the stock is managed by different registered providers (landlords). The landlord is responsible for the upkeep and maintenance of the building and grounds and housing services such as communal lighting and heating. You pay rent to your landlord for your home and service charges to your landlord for utilities and maintenance of the scheme in which you live. Your service charge includes the provision of an alarm service to access help 24 hours a day, seven days a week. Sheltered housing can be rented or owner-occupied accommodation. It can be owned, rented, part owned/rented (shared ownership) or leasehold. Extra care housing can broadly be described as housing into which a range of care and support services can be delivered on-site 24 hours a day. Extra care is gaining a reputation for offering accommodation and care to people who would otherwise be frequent users of acute services, largely because they cannot look after themselves in unsuitable housing. Models of extra care are various, because it is a concept rather than a housing type. Generally, an extra care scheme is a development of flats and/or bungalows with a manager and care and support can be available for tenants/leaseholders 24 hours a day, usually supported by a lifeline and other technological assistance. There are two types of residential home: • C are homes (personal care only) If you are reasonably active but would like greater security and care, a care home offering only personal care may be the best option. Personal care includes bathing, feeding, dressing and help with moving but it must be paid for if your capital/savings exceed £23,250. See page 38 for more information. • Care homes with nursing If you think you may need nursing care in a home, you will need to be visited by a social worker or a care manager to determine what care you will need. This might be in your own home, or in hospital, or in a care home. You will be fully involved in planning your care needs. If a care home providing nursing care is the best solution for you, your care manager will give you information to help you find a home that meets your care requirements. The cost of the nursing care part of your fees is paid by the NHS to the home. The current amount is £110.89 per week. It is important that you get your nursing determination done before you go into the care home. You can contact the nurse care management teams for your area at the Clinical Commissioning Group. Personal care is not paid for by the NHS and must be paid for if your capital/savings exceed £23,250. See page 38 for further information. When looking for appropriate care: • remember that it could be your home for life; • check that the home is properly registered; • remember the care available must suit your needs; • a careful and honest assessment by the care provider should establish whether the home matches all your needs, but bear in mind that your requirements may change; • c onsider if the home environment is sufficiently safe/secure; • ask what activities and facilities are offered; and • ask if the care home normally does business with us at fee levels we accept and, if not, should your funds reduce and you seek financial support from us, you would have to move or find someone else (known as a third party) to help pay your care home fees. Inspecting care services and checking quality The Care Quality Commission (CQC) registers, inspects and reviews all adult social care and healthcare services in England in the public, private and voluntary sectors. This includes care homes, care homes with nursing, home care agencies and NHS services, amongst others. Since October 2010, all care providers have been registered under a system introduced by the Health and Social Care Act 2008, which brings adult social care, independent healthcare and the NHS under a single set of essential standards of quality and safety for the first time. Following an inspection, each care home and home care agency is given a report of how it rates against national essential standards of quality and safety. This Directory’s helpline 0800 389 2077 – information on choosing and paying for care Paying for care The information here has been developed by a group of public organisations and charities to help people make better and more informed decisions about paying for care and to encourage people to get information and advice about the use of their money in this regard. Personal budgets Personal budgets are an amount of money councils can allocate to help people get the support they want due to disability, frailty or vulnerability. Everyone who is eligible for funded social care will be allocated a personal budget once their needs are agreed. There is no obligation for people to manage their budgets themselves, and different options are available to help plan and arrange suitable care for each individual. People will be able to use their budget through a cash payment, services arranged by us, or a mixture of both. Direct payments If you have been assessed as needing community care services and are capable of managing a direct payment, with assistance, if necessary, you can receive a payment and organise and buy services yourself. Direct payments can be used to arrange support designed to meet your individual needs. We will agree these needs with you and write them in your care plan. A direct payment is sometimes given so that you can arrange a short break (respite) or buy any equipment that you need to stay independent. They can also be used to pay for help with: own home. We take into account what it would cost if we provided the services instead. Unless you are a carer, you will usually have to pay a contribution to the cost of your services. If you are interested in learning more about having a direct payment or wish to access support with an existing direct payment, we have commissioned the Wiltshire Centre for Independent Living to provide this service. You can also contact our customer advisers on 0300 456 0111. • p ersonal tasks such as getting up, washing, dressing, going to bed etc; and • p ractical tasks such as food preparation, cooking, shopping and other related tasks to help you to live in your own home. You can choose to buy care from an agency or employ your own personal assistant. If you are a carer, you can get a direct payment to pay for support or services which will help you to carry on caring, although you cannot use the payment to support the person you care for. Direct payments cannot be used for: • health services; • p ermanent care in a care home or care home with nursing; or • s ervices which are managed by us – such as our inhouse home care service. Care can involve help in your own home as well as care homes or care homes with nursing. If you, or a member of your family, need care and support now or in the future there are a number of important things you need to consider. The following is intended to help you get information and advice in relation to your personal finances and how to pay for care if you: • are planning for retirement; • are an older person considering getting help with care and support for yourself; • have a need to fund care immediately; • are a family member, a carer or have Power of Attorney (PoA) or deputyship of somebody needing to fund their care; • have a child with disabilities and want to ensure they have financial security after you die; or • are disabled and want to plan your financial future to make the best use of compensation payments, or if you think your ability to work and support yourself financially may lessen over time. Seeking financial advice and help There are three main ways of paying for advice. • F ees – by paying the adviser a fee, either at an hourly rate or a set fee. • C ommission – by paying indirectly through commission, which is deducted by the product provider from the product(s) you invest in. As well as the initial commission charged when the product is taken out, there may be annual commissions, known as ‘trail commissions’. • F ees and commission – by paying a combination of fees and commission. Later life advisers Advisers specialise in the financial needs of older people. Those advisers who have taken steps to become independently accredited can offer added reassurance that they offer the practical help and guidance needed to make the right decisions at the right time. The later life adviser accreditation scheme has been developed by the standard-setting body, the Financial Services Skills Council (FSSC) in collaboration with the specialist financial services consultancy. 36 Not all advisers offer all three payment options. Make sure the adviser offers the payment option that you want. Ask the adviser to explain what you’re paying for, how much it will cost and how you will pay for it. If you want to pay by commission, ask your adviser how this will be calculated and whether this has any effect on the advice you will be given. Make sure you understand and are comfortable with the payment arrangements before proceeding. If you’re getting investment advice, ask your adviser if the costs include a review of your investments from time to time or if you must pay for that service separately. You can access both general information about paying for care and information about accredited financial advisers in your area through the Society of Later Life Advisers (SOLLA): www.societyoflaterlifeadvisers.co.uk. The MoneymadeclearTM service also gives information about finding independent financial advisers. We have identified two such companies accredited by SOLLA to provide these services to the people of Wiltshire. If your capital and income are below certain levels (locally set if you continue living in your own home, but nationally set if you decide to move into residential care) then we will pay some or all of your long-term care costs. Many people wrongly assume that they will need residential care, whereas there may be options available to stay at home. We, your GP or other NHS services can also offer some people help to try to regain their ability to manage more independently through the use of equipment or adaptations to your home, or through programmes of rehabilitation. Not all benefits are means tested and you may qualify for Disability Living Allowance (DLA) if you are under 65 or Attendance Allowance (AA) if you are over 65. AA/DLA (care component) cannot normally be paid if we are funding your care in a care home, or the NHS is. If you don’t qualify for financial help from us, you will normally have to pay the cost of care out of your own income and savings. If you move into residential care, the value of your home, if you own one, will be taken into account and if you seek a loan from us a charge may be placed against the property pending its sale, although this doesn’t necessarily have to take place immediately if you don’t wish it. If you have capital or savings of more than £23,250 you will have to pay for any care services you require, but you are still entitled to discuss your needs with someone at Wiltshire Council who is trained to advise you. For further information about options please look on our website for ‘Your Financial Contribution towards Your Social Care’. If you have capital or savings of less than £23,250 you may be entitled to funding from us. A financial assessment will be undertaken to determine what we consider you can afford to pay towards your care. If you need to move into a care home, you may be entitled to some of the following financial assistance and support: Twelve-week property disregard If your former home is included in your financial assessment, but your other capital/savings is less than £23,250, and your income is not enough to meet your care home fees, we may assist with the cost during the first twelve weeks of permanent care if we agree that care is needed. This benefit will not be paid if your care home is funded by us, but can be if the financial help provided by us is a ‘loan’ pending the sale of a property. Running out of money If your capital/savings are likely to reduce to £23,250, you must let us know at least two months in advance as we will step in to help with your care fees. We must conduct an assessment of your situation and will normally make a contribution towards your care costs. If the home you have chosen charges more than we consider is appropriate, you will need to find someone to help pay the difference – a ‘top-up’ payment. Whoever does this, whether family or a benevolent charity, they should realise that they may have to pay this for some time. Alternatively, you would have to find a cheaper home, although before this happens we will undertake a risk assessment to confirm that you are fit enough to move. Understanding your rights before moving into care is essential. There are a number of financial products and specialist companies who may be able to help. It is important to seek advice before committing yourself. Citizens Advice or Age UK can advise you further. Deferred payment agreements After the 12-week property disregard period, any financial help from us will be in the form of a loan and a charge will be placed against the value of your home and recovered once your house has been sold. However, we may limit how much we will pay and it may affect your entitlement to pension credit or income support if your property is not seen to be on the market, as it will be treated as capital by the Department of Work and Pensions (DWP). Attendance Allowance/Disability Living Allowance (care component) These are a non-means tested, non-taxable benefits from the Department of Work and Pensions paid at the lower rate of £54.45 per week for those needing care by day or night, and at the higher rate of £81.30 per week for those needing care both during the day and night. Making a complaint should not be made difficult for you and should not affect the standard of care that you receive. Care homes are required under government regulations to have a simple and easy to use complaints procedure. If you are concerned about the care that you, a friend or a relative are receiving in a residential home, it makes sense to speak to the manager of the home about your concerns before you take any further action. The problem may be resolved quite easily once they are made aware of it. However, if you need to make a formal complaint, you should initially contact the registered owners of the home. You may want to be closer to friends and family members, or you may want to relocate to another part of the country. If your care home place is state-funded, your home county local authority is still responsible for your care fees, even if you choose a care home in another region. The home you choose must be suitable for your assessed needs and comply with the terms and conditions set by the authority. The fees that your local authority will pay may vary. They may offer you the fees you would receive if you remained within the county, or they may offer you the fees that the local authority would pay in your chosen region. You must seek further advice before making your decision. If you’re self-funded, you obviously have freedom of choice. How solicitors can help A solicitor can give you impartial advice about wills, making gifts, estate planning and Powers of Attorney (PoA). Some can also offer guidance on immediate and long-term care plans, ensuring (if applicable) the NHS has made the correct contribution to your fees. Lasting Powers of Attorney (LPoA) allow you to appoint someone you trust to make decisions about your personal welfare, including healthcare and consent to medical treatment, and/or your property and financial affairs. An LPoA is only valid once registered with the Office of the Public Guardian. It allows for a person of your choice to make decisions on your behalf at a time when you may 40 be unable. The Court of Protection can issue Orders directing the management of a person’s property and financial affairs if they are incapable of managing their own affairs should they not have an LPoA. The Court procedure is presently very slow and the fees are quite expensive so preparing an LPoA is always advisable, providing you have somebody sufficiently trustworthy to appoint as your attorney. An ‘advance directive’ allows you to communicate your wishes in respect of future medical treatment, but it is not legally binding. Long-term care: whether you remain in your own home or move into sheltered or residential care, you may qualify for financial assistance in the form of social care and NHSfunded care and welfare benefits.
