Some can be handled quickly and others may be lifelong battles that require assistance from others. For 95 years, HeartShare Human Services has been helping children, families and adults overcome the obstacles in their lives to enable them to reach their fullest potential and lead meaningful lives. Some have made tremendous progress and others are still battling through their obstacles.Regardless, all of them are in a better position than they were before coming to HeartShare. Each year HeartShare helps 20,000 children, adults and families with the difficulties they may be facing. We have more than 60 program locations throughout Brooklyn, Queens and Staten Island and our Energy Assistance programs are reaching out to individuals across the state. However, we can’t do it alone. We rely on the generous support from the community—individuals, corporations, foundations and government funding to support all of our programs. Guarinello President and CEO ANN U AL RE P ORT 2 0 0 9 1 Students at The HeartShare School learn both functional and academic skills in a classroom setting. Each child is supported and encouraged to become as independent as possible. Angela Kutov The HeartShare School What may seem like simple accomplishments for typically developing children may be enormous steps for children diagnosed with Autism Spectrum Disorder. When Angela first came to HeartShare, she required a great deal of individualized attention from her teachers. She had difficulty completing classroom activities and holding things in her hand, such as eating utensils and crayons. Now, with the assistance and dedication from the staff of The HeartShare School, Angela is able to complete her work independently, she loves to draw and is helpful to her other classmates. Angela has made tremendous progress in a short period of time and will continue to do so with the help of The HeartShare School. 2 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 3 Through group homes and supported apartments, adults with developmental disabilities are able to experience life on their own as independently as possible. HeartShare staff provides support and guidance to assist them in attaining their dreams. Jack Ameduri Residential Services Individuals diagnosed with Prader-Willi Syndrome (PWS) have a genetic disorder that can lead to a constant feeling of hunger, and in turn to excessive eating and obesity. For Jack, his PWS put his health in jeopardy. Four years ago, he was placed on oxygen 24-hours a day and his doctor recommended he lose 60 pounds. Through personal willpower and the tremendous support of his residential staff, Jack diligently followed his diet and exercise plan. He lost the 60 pounds and is no longer on the oxygen. While Jack’s accomplishment is something to be truly celebrated, he and his staff know that this will be an ongoing battle for the rest of his life. 4 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 5 HeartShare believes in the potential and dignity of all individuals with developmental disabilities. Its day programs offer a wide range of individualized and carefully planned experiences and opportunities. Jennifer Stasi Adult Day Services Jennifer is like many adults with developmental disabilities—she has overcome many obstacles in her life and has other challenges ahead. When Jennifer began attending HeartShare’s Union Turnpike Day Habilitation Program last year, one of the challenges she faced was being afraid of the dark and loud noises. She also did not speak articulately and would whisper and use incomplete sentences. Through the great efforts of the staff, Jennifer now uses full sentences and engages in a wide variety of activities with staff and peers. And while she used to avoid the music room because of the loud noises, she is now able to go into the room and play the drum or keyboard with the lights dimmed. Jennifer has not overcome all her fears yet, but she has shown signs of great improvement and is on the road to living a more complete and fulfilling life. 6 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 7 HeartShare’s Medicaid Service Coordinators work with individuals with developmental disabilities to ensure they receive the needed services. They also educate the individuals and their families about their rights and available options. Ruben Cabrera Medicaid Service Coordination Ruben began receiving service coordination through HeartShare in 1995. Over time, HeartShare service coordinators have advocated for Ruben and monitored his services to ensure that they were helping him meet his needs and reach his goals. He currently lives at home with his mother and sister and attends HeartShare’s Kaleidoscope Day Habilitation Program. Ruben has improved his communication skills greatly and now speaks more clearly and is likely to engage others in conversation. He also has improved his job skills and volunteers at a local movie theater and nursing home. Ruben hopes to find paid employment soon. His service coordinator will help Ruben get the support he needs for his job search. Ruben has overcome many obstacles to reach this point and will work to conquer those remaining in his way. 8 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 9 Through HeartShare Wellness, individuals with developmental disabilities are able to receive high-quality medical care provided by medical professionals who understand their unique needs. From primary medical care to specialized services, Wellness is a one-stop medical center. Corliss Lennon HeartShare Wellness When Corliss was first referred to HeartShare Wellness she expressed feelings of helplessness and boredom in her life. Corliss and her clinician agreed that she would set a goal to become travel-trained, meaning she could travel independently via buses and subways. Corliss began speech therapy to ensure she could communicate with others and occupational therapy to strengthen her hands and legs. Once the training began, Corliss’s self-esteem improved and her depression disappeared. Her attendance at her day habilitation program also increased, she smiles a lot more, and her ability to work with others has improved. Corliss is now being considered for a day program that focuses on job training because of the progress she has made utilizing the services of HeartShare Wellness. 10 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 11 Through four Early Childhood Centers, HeartShare offers educational services to children ages 3 to 5 who have developmental delays or disabilities, including autism. Tamesh Roopnaraine Early Childhood Services When Tamesh came to HeartShare, he had limited attention span, eye contact and language skills. He was often aggressive and became easily frustrated and would cry and scream uncontrollably. During the two years that he had been with HeartShare, he made incredible progress; he began in a class of eight children, one teacher and three assistants and transitioned to a class of 12 children with peers who play more appropriately and have better language skills. Today, he is able to use words to communicate his thoughts and his sense of humor has emerged. Tamesh has overcome many obstacles at the young age of 5 and is an inspiration to other children. 12 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 13 HeartShare works with foster parents to provide loving homes to hundreds of children who are no longer able to stay with their birth parents. HeartShare’s staff continually works to find the best placement for each child. Lesly Macon Foster Care/Adoption Program The shortage of foster care homes leaves children deprived of the loving care that they need and deserve, creating obstacles in their lives at an early age. When Lesly learned that her grandchildren, Destiny and Raheem, were placed in foster care, she put her life on hold to ensure they stayed within the family. Since being placed with Lesly, she has given them much-needed love and emotional security. Recently Lesly adopted Destiny and Raheem to provide them with a permanent home. Through working with HeartShare, Lesly has helped these children avoid a life of uncertainty and challenges that no child should have to face. 14 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 15 For individuals and families affected by HIV/AIDS, HeartShare’s Community Follow-Up Program helps develop a plan and access to services to prevent or delay an unnecessary crisis. Donald Allen HIV/AIDS Services Living with HIV in 2009 is different than it was in 1999 or 1989. However, individuals still face numerous obstacles and challenges on a daily basis. When Donald came to HeartShare two years ago, he did not have housing or a medical provider, his financial entitlements were not in place and he was not connected with any community resources. Working with HeartShare, all of that has changed. He now has stable housing, attends all his medical appointments and remains in good health. Additionally, Donald has reconnected with his family and has wonderful relationships with his grandchildren. 16 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 17 Hundreds of children benefit from HeartShare’s after-school and summer camp programs each year. They offer a variety of activities to meet the diverse interests of the children attending each location. Savannah Carvalho YOUTH PROGRAMS Thousands of children in New York City go home to an empty house or apartment after school because their parents are working. Luckily, Savannah isn’t one of those children because she attends HeartShare’s Surfside Gardens Community Center. Here, she gets help with her homework, engages in a variety of activities and is in a safe place until her parents come home. Savannah attends both the after-school program and summer camp program which provide her with a yearround experience of opportunities that may not be available to other children. Savannah is on a path to becoming an educated young adult with many available options. 20 HEARTSHARE H U M AN SERVICES O F NEW YORK ANN U AL RE P ORT 2 0 0 9 21 GOVERNMENT PARTNERS HeartShare administers various funds established by ConEdison, Entergy and Nationalgrid to assist customers who are not able to pay their electric or gas bills. Staff members also conduct workshops to educate the public about steps to make their homes more energy efficient. Energy Assistance Programs Each year, hundreds of families benefit from HeartShare’s Energy Assistance Programs. Here, in their own words, are their expressions of how they are being helped. One 80-year-old woman, Mildred, wrote a letter of appreciation and stated, “Thank you for your grant. It lightens my load somewhat. I went through a shut-off period for two days and it was quite frightening as well as dangerous for me because I was afraid of falling or stepping on one of my cats. I am 80 years old and not too steady.” Medina wrote, “This program will help me a lot. By covering my ConEd, it will eliminate one of my many bills. Because I am per diem at my job, my incomes varies, so, sometimes I am not able to pay all my bills.
