According to Tanner President and CEO Loy Howard, the hospital’s ability to perform 2,000 of the procedures in just a little more than five years’ time speaks to how much the service was needed in the area. Healing Hearts, Changing Lives New Moms: Exercise For Stronger Bones Hitting the gym is good for more than fitting into your pre-baby jeans. It also may fight bone loss from breastfeeding. Breastfeeding has perks — such as better health for mom and baby and a stronger emotional bond — but bone density typically declines while women lactate. In a recent study, a group of 10 breastfeeding women were asked to work out beginning four weeks after giving birth. After 16 weeks, they had stronger bones than those who’d been told not to exercise. How much exercise helps bones? Women in the study did 45 minutes of cardio three days a week, along with three days of strength training. Active Lifestyle Can Protect Against Workplace Injuries Once-a-week recreation can help protect you from repetitive strain injuries (RSIs) on the job. That’s the report from a study of nearly 60,000 full-time workers. Those who enjoyed physical activities in their time off had almost 20 percent fewer RSIs than their less active counterparts. Most RSIs occur in the wrists, hands, shoulders and elbows. Physical leisure activities can strengthen muscles that get underused — or overused — at work. And women with sleep problems are more likely to be at risk for cardiovascular disease and diabetes. Scientists think that higher levels of testosterone protect men from the damaging effects of poor sleep. Overweight and obese women are especially prone to sleep problems. Tanner • summer 2012 I Page 5 Surviving Breast Forges a Page 6 I Healthy Living • www.tanner.org st Cancer Close Bond Two local women share their stories and strength Allison Jiles exercises three days a week with a special group of people — special, because when she explains an ache in her shoulders as being “like I felt after I used to get a Neulasta shot,” they know exactly what she’s talking about. Neulasta is one of several medications often used to treat breast cancer, which Jiles battled in 2008. Of the 20 or so men and women with whom Jiles exercises — “the Ironclads,” as they call themselves — a handful are breast cancer survivors. Jiles spends a lot of time around women who have had, or currently have, breast cancer. Facing Troubling Mammography Results Jiles remembers that it was the summer of 2008 when she was diagnosed, after a mammogram at Tanner Breast Health in Carrollton indicated a possible cancer — a diagnosis she says was terrifying. “My first thought was, who’s going to raise my kids?” says Jiles, whose daughter was 15 and whose son was 9 at the time. She describes herself not as being a “glass is half full” kind of person, but a “glass is completely full and running over” kind of person. Fighting the fear and dread that followed her diagnosis, Jiles set out to find answers. She lined up interviews with prospective physicians who could treat her cancer, turning to a breast specialist in Atlanta. Coming Back Home for Care Jiles’ Atlanta oncologist told her she didn’t need to come back to Atlanta for her ongoing treatment. “She told me to stay in Carrollton, let Dr. Pierce do it — you’re going to get the exact same treatment, if not better, and the exact same drugs,” says Jiles. “That was a very comforting thing to know, that I did not have to go to Atlanta for my chemotherapy.” Randall Pierce, MD, is a board-certified medical oncologist with Northwest Georgia Oncology Centers and a member of the patient care team at Tanner Cancer Care. Back in Carrollton, Jiles received the treatment she needed — and wanted. “God really put some people in front of me who were tremendous,” she says. “He really had a path for me. He couldn’t have made it any more obvious.” Jiles’ path has led to her survival, and in that survival, she has found an obligation to help others who are facing cancer. That’s why, when the pastor’s wife at Southern Hills Christian Church in Carrollton told her that another member of the church, Orly Seale, had been diagnosed with breast cancer, Jiles was quick to reach out. And to be persistent. An Astute Physician’s Lifesaving Decision Seale, a pharmacy manager for Tanner Medical Center/ Villa Rica, was in her mid-30s in 2009, when she was Orly Seale (left) and Allison Jiles share a laugh. Her gynecologist, Eric Vera, MD, with Women’s Care of West Georgia and a member of the medical staff at Tanner, sent her for a baseline mammogram earlier than most clinical guidelines suggest. “I’m very glad that Dr. Vera sent me to get a baseline mammogram at 36,” says Seale. “The recommendation is that you get the baseline done at 40. I would’ve been dead.” With the advantage of perspective from being employed by Tanner, Seale knew she wanted to stay at Tanner to receive care. Raul Zunzunegui, MD, a board-certified and fellowship-trained breast surgeon with Comprehensive Breast Care Center, a Tanner Medical Group practice, diagnosed her cancer, performed the lumpectomy to remove the tumor and conducted a biopsy to Choirs for Cancer Returns Nov. 13 Mark your calendars for the third annual Choirs for Cancer concert on Tuesday, Nov. 13, at 7 p.m. in the Joshua R. He also performed the additional surgeries that were necessary to fight Seale’s cancer, which eventually included a double mastectomy. Stephen Kahler, MD, a board-certified plastic surgeon with West Georgia Center for Plastic Surgery and a member of the medical staff at Tanner, performed Seale’s breast reconstruction surgery, and Dr. Pierce provided her chemotherapy. Tackling the Emotional Challenges Seale at first pushed her emotions — the fear and uncertainty — behind a wall of tireless research. She looked up medications, tips, lifestyle changes, everything she could about her cancer and treatment. Aside from the research, Seale put her energy into raising her daughter, who was 7 at the time, and her son, who was 4. “I came at it from a very clinical perspective,” Seale says. “I detached myself, compartmentalized everything, and I didn’t deal with any of the fear or the feelings. I just said, ‘OK, this is what has to be done next.’ It took me a very long time to deal with all the feelings.” Finally, at a church event, Seale was prepared to talk, and Jiles was ready to listen. A Match Made in Heaven “I remember just feeling like I needed to go to the church then,” Seale says of the day she met Jiles. “I saw Allison across the room, and I immediately knew it was her. I knew she was the one I needed to talk to.” “It was probably because of my short hair,” Jiles jokes. Jiles and Seale became very close friends. Jiles made a point of going with Seale to each chemotherapy session, missing one only when she herself had to have surgery. Jiles frequently called on Seale at her home, a stalwart friend and compatriot against cancer. “You would come to see me, and I remember, you were so beautiful,” Seale tells Jiles. “I would look at you and think, ‘I want to be like you.’ And you would tell me not to give up, that this was just a season, and it would pass.” Seale tried to keep up her normal routine as much as possible during treatment, but it was challenging. “I tried to just bounce right back,” she explains. “I worked the entire time I had chemo. I had surgeries and would come right back to work, and my body just couldn’t handle it. I would feel so run down. And Allison would tell me, ‘Orly, you cannot just act like you don’t have cancer. Your body needs time to recover.’ And she was right.” Even after the cancer, Seale relied on Jiles’ strength and friendship. “I would have bone pain, and I would think … ” “Bone cancer,” Jiles says. “Or, I would have a headache, and … ” “Brain tumor,” says Jiles, finishing Seale’s sentence again. “After you’ve undergone cancer treatment, you start to think everything could be cancer.” “Even now, people I know who have had cancer will call me and tell me they’re having this bone pain, and they’re worried,” says Seale. “They ask if I think it could be cancer, and I tell them it’s probably not, and if they’re worried they can always go to the doctor to be sure, but I’ve had that same exact pain.” Finding an Answer to Anxious Waiting Times Both Seale and Jiles recall the anxiety between receiving their diagnosis and starting treatment. Seale remembers learning she had breast cancer just before Father’s Day weekend. Although she assured herself that everything would be fine, that she would get through it, she couldn’t stop considering what might happen next. “I would try on a dress, and think about how it could be altered if I had to have a mastectomy,” Seale says. “It was really pretty awful, looking back.” “When you get that diagnosis, you don’t sleep,” says Jiles. “They tell you that you have cancer, and that’s it for a little while. You get to think about it.” To help ease this period of waiting and worrying, Tanner now offers the Tanner Cancer Care Promise: Three Days from Diagnosis to Treatment Options. The goal is to provide patients with options as quickly as possible. How Three Days Works Upon receiving a physician’s referral, cancer specialists on staff at Tanner begin planning the best approach to defeating that patient’s particular type of cancer. An oncologist — a physician who specializes in cancer — reviews the referral; plans an approach to care that employs the most advanced, proven techniques and treatments; evaluates clinical trials for which a patient may be eligible; and puts together a comprehensive plan. The oncologist then passes his or her notes to a patient navigator, who provides a single point of contact for patients. The patient navigator assists in explaining diagnoses and physician orders, answering questions about cost and reimbursement, arranging access to community and health system resources, and much more. A patient navigator contacts each newly diagnosed patient referred to Tanner Cancer Care to set up the first treatment planning session. He or she also schedules all the necessary appointments and any additional diagnostics needed by the oncologists. High Praise for three days For those facing a cancer diagnosis for the first time, the Tanner Cancer Care Promise extends to weekends and holidays. A patient referred to Tanner Cancer Care on a Friday will have his or her treatment options by Monday. “When I heard that Tanner was doing this in three days, I was excited,” Jiles says. “If the patient navigators had been here three years ago when I was going through this, it would have been a different experience. To have someone there when you’ve received your diagnosis and to say, ‘OK, yes you have this, and here’s what we’re going to do next,’ would be so comforting.” People with recurring cancer or cancer that has spread may require a more specialized plan, which can take longer than three days. Nonetheless, Tanner Cancer Care is committed to preparing treatment plans for these patients as quickly as possible. An Essential Screening If Allison Jiles and Orly Seale could spread just one message about their experience, it’s this: Get a mammogram. “Hiding your head in the sand is not going to make it go away,” says Jiles. “Either you are putting your mind at rest knowing everything’s fine, or you’re taking care of business and finding out what needs to be done next.” Jiles credits her annual mammogram with saving her life. “If I had waited a year, I would not be here,” she says. “Mine would not have been found any other way. That’s an oversimplification, of course, but the heart does rely on electrical impulses — signals — to know when and where to contract and relax. Contracting and relaxing in the right order, in the right parts of the heart, is necessary to most efficiently pump blood in from, and out to, the rest of the body. There are times, however, when the pump just is not functioning as it should, due to faulty wiring, so to speak. There’s a short somewhere. The top half of the heart is beating at a different rate from the bottom half. Or the whole pump is going much too fast, much too slow — or in a dangerous pattern that can cause the whole heart to suddenly stop. The study of problems in — and solutions for — these electrical signals is called cardiac electrophysiology, and the ability to treat these problems is now available in west Georgia. “This is a very big advance for cardiac services at Tanner, and in expanding access to these services for the community,” says Rajat Jhanjee, MD, a fellowship-trained clinical electrophysiologist and board-certified Page 10 I Healthy Living • www.tanner.org cardiologist, who has joined Tanner Heart & Vascular Specialists. “Now, patients with common cardiac problems — such as atrial fibrillation, life-threatening abnormal heart rhythms and heart failure — will have access to care right in their own community.” Dr. Jhanjee earned a master’s degree in electrical engineering from the University of California, Los Angeles and worked as an electrical engineer for Dallas Semiconductor and Guidant before enrolling in medical school at the University of Minnesota. There, he earned his medical degree, completed his internship and residency in internal medicine, and achieved fellowships in cardiovascular research, clinical cardiology and clinical electrophysiology. “As an electrical engineer, I worked with companies that developed and manufactured the equipment that cardiologists used to address electrical anomalies in the heart,” Dr. Jhanjee says. “I became interested in the other side of what I was doing — in how the devices and technologies I was working on were being used to improve people’s lives in practice. That’s why I enrolled in medical school and studied cardiac electrophysiology.” With cardiac electrophysiology now available at Tanner, several new treatments are available, close to home. Catheter Ablation Atrial fibrillation (also called AF or “a-fib”) is the most common form of irregular heartbeat. “AF can often be treated with medicinal therapy, but if the medication is ineffective or cannot be tolerated by the patient, catheter ablation may be necessary,” says Dr. Jhanjee. Catheter ablation is a nonsurgical procedure in which a cardiac electrophysiologist threads thin, flexible wires through the arteries to the heart. An electrode at the end of the wire generates radio waves that create heat. “By placing the electrode at various areas of the heart, we can turn off the heart tissue that causes atrial fibrillation,” explains Dr. Jhanjee. Implantable CardioverterDefibrillator Procedure Patients with life-threatening abnormal heart rhythms may benefit from an implantable cardioverter-defibrillator (ICD) — a device in the body that continuously monitors the heart for life-threatening heart rhythms and, if one is detected, sends an electrical shock to the heart to restore a normal, healthy heart rhythm. “We can set both the rate at which a shock will occur and the level of shock required to restore a healthy heart rhythm,” says Dr. Jhanjee. “If the first shock does not achieve the desired effect, the ICD will send progressively more powerful shocks until the proper heart rhythm is restored.” The ICD is surgically implanted, usually just below the left collarbone. The wire that monitors the heart and carries the electrical charge is then threaded through a vein to the heart. Biventricular Pacemaker Implantation A biventricular pacemaker is a special type of pacemaker that uses three leads — one in the right atrium, one in the right ventricle and one in the left ventricle — to better help regulate a patient’s heart rhythm. In a healthy heart, the top half of the heart, called the atria, beats at the same rate as the lower chambers of the heart, called the ventricles. “Often, people with heart failure experience a difference between the rate at which the atria and the ventricles beat, exacerbating the symptoms of heart failure,” says Dr. Jhanjee. “Biventricular pacemakers are used to treat heart failure using cardiac resynchronization therapy — also known as biventricular pacing — by applying an electric charge to the ventricles. A few years ago, that would have been a difficult question to answer. Hospitals have long been required to report information on their quality of care, but cataloging and accessing that information was an elusive — and time-intensive — task. People relied on old standbys to gauge their expectations for a visit to the hospital: opinions from neighbors and Tanner in the Top 15 Thomson Reuters, the world’s leading source of intelligent information for businesses and professionals, named Tanner Health System one of the 15 Top Health Systems in the nation. Tanner was the only health system in Georgia on the list, which was compiled by researchers from Thomson Reuters who analyzed the clinical quality and efficiency of more than 300 health systems from across the country to identify the top 15. The analysis determined that the 15 Top Health Systems led in all study comparisons, including higher survival rates, reduced patient complications, better longterm outcomes for patients, improved adherence to accepted care protocols and patient-safety standards, shorter hospital stays and higher patient satisfaction scores. Data on vital issues, such as hospital-acquired infections, readmissions and patient satisfaction, are collected and provided, free of charge, by the national Medicare and Medicaid program at www.hospitalcompare.hhs.gov and through programs like Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) — the first national, standardized, publicly reported survey of patients’ perspectives of hospital care. So now, for prospective patients who want to know how safe, reliable and trustworthy a hospital is, the answers aren’t hard to find.
