Culberson, MSW, CSP spent 10 years working at a large Hospice organization as clinical social worker, Manager of Counseling Services and Director of Quality Service. He is a contributing author for: Humor Me and Chicken Soup for the Nurses Soul II. Dr. Mary Grace Mihalyo, BS, PharmD is a Hospice and Palliative Care Specialist. She earned her BS and PharmD from Duquesne University in Pittsburgh, PA where she is also an assistant professor. Dr. Mihalyo is a pioneer in the provision of medicationtherapy-management services to Hospice and Palliative Care patients. She has worked for more than twelve years developing the pharmaceutical care model of “On Demand Clinical Pharmacy Services” originally developed when she was associated with Valley Hospice in Ohio. Jennifer L. Capone, HPMT, MT-BC is the founder, owner and Director of Therapeutic Services of Blue Ridge Music Therapy. She received her Bachelor of Music Therapy degree from the University of Dayton in Dayton, Ohio. She founded the Music Therapy program and served as Director of Music Therapy at the Hazelwood Center in Louisville, Kentucky including clinical supervision of University of Louisville students.
