Building on a vision.
Growth is not measured in just buildings, but in the quality,
spirit, and culture of an organization. CMH is committed to
excellence and has received outstanding achievements in
national and state quality measures. We are proud of these
accomplishments, but we also recognize that quality is a
continuous goal that we must strive to achieve every day
with every patient. This is a goal that our nurses, doctors,
volunteers, and support staff work toward each day.
Thank you for your support of Community Memorial
Healthcenter. With your ongoing support we will continue to
advance our vision of healthcare for today’s community and for
future generations.

Hi
Helping people where they live…and where we live.
The statistics don’t tell the whole story, but in the case of
Community Memorial Healthcenter, they tell a compelling one.
The sheer volume of patient visits, procedures, and outreach
events in our community demonstrates that we are a health
system that is fulfilling our mission.
Our hospital was established with a very specific cause in
mind—to help the people in our region. Last year we touched
thousands of individual lives through administering hundreds of
thousands of procedures.
We have also dramatically increased our capacity to
serve through the addition of our state-of-the-art Cancer and
Rehabilitation Center.
While the most evident changes are seen through our
physical growth, we are even more proud that we are
continuing to make major strides in quality.
Certainly, no one knows what each day will bring, but we
pledge to approach every day with a spirit of community and a
focus on each person we have the privilege to serve.
Benefits of CMH to the community.

What makes Community
Memorial a nonprofit
Healthcenter?
CMH is a community-owned,
nonprofit organization
governed by a volunteer Board
of Directors representing our
service areas of Brunswick,
Lunenburg, Mecklenburg, and
northern North Carolina. Each
board member has unique
expertise and knowledge of
the community that contributes
greatly to the success of CMH.
As a community-owned
health system, we operate
for the benefit of our service
area and reinvest all of our
net income in providing health
services for the communities
that we serve. We believe this
best reflects the vision of those
who believe and work toward
an exceptional hospital for our
community.
• CMH provides comprehensive medical, surgical, behavioral, and
long-term care services to meet the health needs of our citizens.
• CMH is licensed for 99 acute care beds, 161 long-term care
beds, and 24 behavioral health beds.
• As one of the area’s largest employers, we have more than
850 people on our staff, not including contract services.
Community.
It’s in our
name and at
the heart of
all we do!
• As one of the area’s largest stable
employers, your hospital provides
jobs at a wide range of skill levels
for more than 850 people from over
12 Virginia and North Carolina localities.
• Your hospital contributes to the tax
base through wages, salaries, and
benefits totaling nearly $39 million
over the past year.
• CMH and its employees purchase
goods and services locally, creating
income and jobs for other businesses
in the community.
• Your hospital helps recruit and retain
college graduates and offers tuition
and scholarships for retraining and
continuing education.
• A strong community hospital such as
CMH helps to recruit business and
industry to the region.
• CMH supports the communities it serves
through active membership in area
chambers and sponsorships of community
events and youth activities.
Our commitment to improving our community’s well-being goes
beyond the walls of the hospital:

As a nonprofit hospital, we provide the health services
needed by the people in our community. Many of these
services generate little or no revenue. These include a 24-hour,
fully staffed emergency department, obstetrics department,
and physician-based practices and clinics in outlying
communities to provide a comprehensive local health resource.
We make priorities of health education and wellness.
We focus on prevention as well as treatment to maximize the
long-term health of our citizens. Nurses and other healthcare
educators provide health education and screenings throughout
our community. CMH partners with area schools, churches, and
businesses to promote health and wellness activities and we
continually strive to find new ways to improve and maintain the
health of the people in our region.
We provide care to all, regardless of their ability to pay.
During the past fiscal year, CMH provided over $3.7 million in
charity care and over $9 million in other uncompensated care.
We make tangible strides in providing new services to our community.
Over the past year we have, as a community, made
aggressive strides against cancer, stroke, heart disease, and
other debilitating diseases through the introduction of our
advanced Cancer and Rehabilitation Center. Our region suffers
significantly above the state average in areas such as cancer,
heart disease, stroke, and diabetes. These facilities help where
our community hurts most.

Our Commitment
to Quality Care
C
You don’t have to
go to a big city to receive
top quality healthcare!
We take great care to
ensure that our patients are
receiving the best possible
medical care. In fact, CMH
has received recognition
in the top 10 percent
nationally and in the state
for excellence in quality.
• CMH is in the top 10
percent of hospitals
nationally in the Medicare
and Medicaid quality
initiative, HQID (Hospital
Quality Incentive
Demonstration), in meeting
nationally standardized
quality measures.
• CMH has also been
recognized as one of
the top 10 percent of
hospitals in the state
that achieved excellence
in Anthem’s Quality-inSight® Hospital Incentive
Program (QHIP).
• Our patients recognize the
high quality of care that
they receive here at CMH.
Results of the most recent
patient satisfaction survey
showed that 98.5 percent
of patients said they were
pleased with the quality
of care provided here at
our hospital.
6
ommunity Memorial Healthcenter’s mission and culture are
built on a focus and passion for providing excellence in the
delivery of healthcare. A continuous commitment to excellence
is a core value that our employees share and embrace.
Our Medical Staff is at the helm of the overall quality of
medical care rendered to patients. Our physicians lead the
way in implementing best practices, identifying risks, and
implementing solutions to enhance care, safety, and the patient
experience.
CMH participates in a number of state and national quality
initiatives. These quality initiatives help to standardize care
throughout the country using specific best practice protocols.
These results provide benchmarks and measures for patients
to compare the quality of care delivered at hospitals for a
specific disease or health problem, and patient satisfaction
measures.
Consumers can obtain information on the performance of
hospitals through a variety of websites. More information on
these quality initiatives can be found on our website:
www.cmh-sh.org/content/qualitymeasures.htm.
Certifications and Awards
Our ongoing commitment to accreditation by the Joint
Commission and other accrediting bodies underscores our
commitment to provide quality care on an ongoing basis and to
work continuously to further provide quality of care and patient
safety that exceeds national standards.
Other certifications and awards Include:
• Cardiac Rehabilitation—National Accreditation by the American
Association of Cardiovascular and Pulmonary Rehabilitation
• Echocardiography Lab—Reaccreditation from the
Intersocietal Commission for the Accreditation of
Echocardiography Laboratories (ICAEL)
• Extended Care Facility—Quality Associate Award from

• Home Health—Quality Achiever Award and Quality
Associate Award by Virginia Healthcare Association
• Laboratory—Accredited by College of American
Pathologists (CAP)
• Mammography—Accredited by the American College
of Radiology, the Food and Drug Administration, and the
Mammography Quality Standards Act
• Respiratory Therapy—Awarded the Quality Respiratory
Award by the American Association of Respiratory Care
• Vascular Laboratory—Accredited by the Intersocietal Commission for the Accreditation of Vascular Laboratories (ICAVL)
Our physicians and staff are dedicated to providing holistic,
compassionate care in a professional manner to our patients,
their families, and our community. Patient safety is at the heart
of every initiative that we undertake in delivering quality patient
and family outcomes.
Our commitment to quality is exemplified by our nursing staff.
Nursing’s framework of “Relationship Based Care” provides
an infrastructure that guides nurses in creating a healing
environment based on three important relationships that must
be maintained in the delivery of quality patient care. Our
practice is based on our relationships with our patients and
families, with our colleagues and healthcare partners, and the
relationship with ourselves. We believe that these relationships
are built one at a time and require a life-long commitment.
Nurses are living their vision of providing the very best
up-to-date quality care in all that they do. The philosophy of
our nursing staff is based on the understanding that the art of

This commitment to continual learning and quality care is
evidenced by a number of successful initiatives and programs
this past year:
• The Nursing Clinical Ladder system recognized 40 nurses
for their accomplishments in quality improvement, community
involvement, education, and evidenced-based practice/
research. Radiology and Respiratory Therapy have also
implemented a clinical ladder system to encourage
continual growth in their practice and recognize employees
for excellence.
• A nursing website was created that highlights the Shared
Governance Model and the successes of each nursing
unit. Shared Governance is a councilor model that builds
relationships with members of the nursing department and
their clinical partners. It provides an environment for nurses
to work collaboratively with their colleagues in the decision
making process and enhances communication up and down
the organization.
• Through participation in the National Database for Nursing
Quality Indicators, our nurses demonstrated that we are
among the best in the nation in preventing harm to patients
such as pressure ulcers, falls, and in the use of restraints.
• The Nursing Resource and Partnership Council spearheaded
a major quality initiative partnering with the Pharmacy
Staff to design safer ways of administering medications to
our patients.
• Nurses also instituted a communication technique known
as Situation, Background, Assessment, Recommendation
(SBAR) that has greatly improved communication
between caregivers when patients are transferred to a
different department or when information is shared with
the physician.

elivering the highest quality care, maintaining
patient safety, and patient satisfaction remain
top priorities for us. A new vision has been set
for CMH: to achieve the Malcolm Baldrige
Award in healthcare. This is a long-term
process and will involve entire organizational
involvement to comply with nationally
recognized criteria for performance
excellence in all areas of operation. Only
seven out of 5,000 hospitals in the US so
far have achieved this award, but this is
the level of commitment of CMH to provide
quality healthcare for this community!
What is the Malcolm Baldrige
National Quality Award?
The Baldrige Award, named after Secretary of
Commerce Malcolm Baldrige, is an award given
by the President of the United States to businesses—
manufacturing and service, small and large—and to
education, healthcare, and nonprofit organizations that apply
and are judged to be outstanding in seven areas: leadership;
strategic planning; customer and market focus; measurement,
analysis, and knowledge management; human resource focus;
process management; and results.
Patient Satisfaction: Listening to those we serve.
Both as medical professionals
and friends and neighbors,
it is important to understand
how we are doing in the
eyes of our patients and their
families. To achieve this, we
constantly conduct surveys
to get feedback to help us
continue to build on our legacy
of caring.

The spirit of volunteerism
V
olunteers embody the spirit of Community Memorial
Healthcenter. They serve in different capacities but work
together to achieve the common goal of making healthcare
accessible to our entire community. From the volunteers who
provide helping hands at the main desk of our hospital, to the
fundraising and enthusiastic support of the auxiliary, to volunteers
who make critical decisions as part of our healthcenter board, we
thank each of our supporters.
The CMH Auxiliary is a special group of nearly 200 men
and women who continue a CMH tradition of service and
stewardship. These Auxilians enhance the quality of care
and caring, helping patients and their families, staff, and
visitors in many different ways. Volunteers are a vital part
of our community hospital. During the past year, these
volunteers provided over 28,909 hours of service to CMH.
The Auxiliary is a consistent major financial supporter of
CMH, having exceeded $1 million to date in contributions
to CMH to enhance healthcare for the community.
More than 45 Hospice Volunteers provide many hours of
service, helping patients and families cope with the stress of
terminal illness and death, and helping to enhance the quality
of the remaining days of life. These volunteers also conduct
numerous activities to raise funds to support hospice services and
programs so that all in the community have access to hospice care
regardless of their ability to pay.
Governing Boards:
• CMH is governed by a 21-member volunteer Board of
Directors, representing all of our service areas, who devote
many hours of service to keep our hospital strong, viable,
and community owned.
• Our 11-member volunteer CMH Foundation Board of
Directors works to build continuing support for the
healthcenter through philanthropy, stewardship, and
community education.

Integrating modern technology
with compassionate care
W
hile the legacy of CMH is quality care and compassion,
each year brings new technology to our community. In
some cases, this technology takes the form of preventative
care. In others, technology means bringing the latest
medical breakthroughs to our community to confront
diseases such as cancer, diabetes, and stroke.
As we continue to make exceptional strides in patient
care and quality, it is necessary to balance the heart

ommunity Memorial Healthcenter
Foundation’s (CMHF) mission is to
build continuing support for Community
Memorial Healthcenter through philanthropy,
stewardship, and community education.
Community and business leaders comprise
the board of the Foundation. Working together,
the volunteers of the Foundation work to secure
funding for growth and services that would
otherwise not be available in our community.
While these caring people come from different
backgrounds, they pursue a shared vision for
our hospital.
There are three types of programs that the
CMH Foundation offers for donor support:
Annual Giving, Capital and Major Giving, and
Planned Giving.
The Annual Giving Program allows donors
to support services and programs at various
levels. This is a way that anyone can support
CMH at any level they can afford. Capital and
Major Giving enables donors to give larger
gifts to support capital building projects and
equipment, support special projects, and
create endowments.
The Planned Giving Program provides
opportunities for donors to make major gifts
utilizing sound personal, financial, and estate
planning techniques to support CMH today
and to leave a legacy through a bequest or
estate gift.
Working toward a vision.
Our goals for the CMH Foundation are:
• to have the resources to allow CMH to make
state-of-the-art healthcare services available
to all citizens, regardless of their ability to pay,
• to build an endowment that will allow
CMH’s services to grow to meet the everevolving and expanding health needs of
its service area,
• to provide the resources to enable CMH
to attract and retain only the best qualified
healthcare professionals,
• to establish CMHF as the charitable
organization of choice that merits community
support and involvement.

We continue to build on a tradition of
excellence by making exceptional care even
better. Tangible strides continue to be made
in quality initiatives and patient satisfaction
as CMH was rated among the top 10 percent
of hospitals nationally in Medicare and top
10 percent among state hospitals in Anthem’s
Quality Initiatives.

Memorial and tribute gifts to the CMH Foundation pay
special tribute to a beloved family member or special friend.
Through the T.R.E.E. program, CMH employees support
projects that help make our patients safer, their families
more comfortable, and our staff more efficient.
The Pharmacy Connection
This Medication Assistance Program offers eligible
patients without prescription coverage help in obtaining
their prescriptive medications. Staff members are
patient advocates, serving as liaisons between the drug
manufacturers and qualifying patients. The program is
supported through a grant from the Virginia Health Care

This state-of-the-art facility was built with our region’s greatest
healthcare needs in mind. It is the culmination of the efforts,
passion, and commitment of more than 2,200 people who
made pledges to our campaign.
We would also be remiss to overlook the efforts of the many
craftsmen who assisted with the building and completed
the facility on time and under budget.
The new Center provides advanced facilities to offer
resources for cancer patients in their battle against
the disease, to help victims of stroke and heart attack
regain their independence, and to promote a healthy
lifestyle for everyone.
A vision for the future.
The spirit of community permeates our healthcenter.
Working together, we believe amazing things will
continue to happen. We see a day soon when our
rehabilitation and cancer services will expand. We
believe that with the support of our community we
can remain a strong, independent community hospital. We
believe the healthcare outlook for young people born into
our community today will be brighter and we believe that the
facilities will be in place to help those of us who choose to
live here, maintain the best possible
quality of life.
These visions are wonderful
to be sure, but they are
only possible with the
continued generosity of
our community—and
that means individual
people becoming
involved, engaged,
and energized
in the pursuit of
building a great
community-owned
resource. CMH is not
just a healthcenter;
it is truly YOUR
healthcenter.
The future:
Continuing the Vision.
What will tomorrow bring? No one knows
for sure, but we can be certain that Community Memorial
Healthcenter will be a part of our community’s future.
Our country is certainly experiencing difficult economic times
and it is easy to get distracted. Economic changes—some
good, some bad—will come and go. But there is one thing we
know will remain true: healthcare is essential for this community
to remain a strong place to live and raise a family.
For this reason, we ask that you continue to give generously
to the Community Memorial Healthcenter Foundation. It is more
than an investment in quality care; it is a contribution that will
have long-term impact on both individuals and the collective
viability of our community.
We understand that this is a day where one has many
choices in how and where to invest their charitable time
and monetary donations. In the end, we must ask: Will my
gift make a difference? Will it be used wisely? Will good
be done?
Your gift to the Community Memorial Healthcenter
Foundation will accomplish these goals. We encourage
you to consider a gift to the Foundation and we promise to
make the most of your generosity.
