has received United Way funding, which has been used to provide services through the Infant-Toddler Therapy Services the
Adult Services Program and the Children’s Services Program.
Community Care Connections, Inc. also receives donations designated by people working in neighboring counties through the
United Way of Southwestern Pennsylvania.

ADULT SERVICES PROGRAM
The CCC Adult Services Program – Learning and fun for
everyone.
Would you like to learn how to make a delicious low fat
cake with only two ingredients? Would you like to have a
green thumb creating a beautiful flower garden? Do you
like to play cards or board games and want to learn some
new ones? Are you into exercising or want to learn how to
stay fit? Do you enjoy visiting new places and trying new
things?
If you answered yes to any of these questions, then CCC’s
Adult Services Program might just be the right program for
you. The program offers weekly and monthly classes in
cooking, crafts, walking and fitness, gardening, personal
safety and fun ways to use your leisure time. Day trips, a
bowling league and volunteer opportunities round out the
program schedule. Wheelchair accessible transportation to
and from program activities is available for those who need
it.
The program is funded with grants from the Butler County
Human Services Department, the United Way of Butler
County and by private donations.

ATTENDANT CARE PROGRAM
The Attendant Care Program has been growing over the last six months and we
are very proud to announce that the Obra and Independence Waiver programs
are now being offered to residents of Butler County who qualify for these services. Community Care Connections has been providing Attendant Care Services to eligible Butler County residents for Act 150 and Attendant Care
Waiver Services for 20+ years. Attendant Care services are provided to persons with physical disabilities who are mentally alert and between the ages of
18 and 60 years of age. All Attendant Care services help eligible consumers to
maintain living in their own home in their community. Some consumers elect
to become employers and hire their own attendants. At that time, CCC becomes their fiscal agent
To be eligible for the Independence Waiver you must be between the ages of 18
and 60 and have acquired your disability after the age of 22. You must require
nursing facility level of care and be a resident of Butler County. The consumer
must not have a primary diagnosis of mental health or mental retardation and
must have resources less than $8000.

The OBRA Waiver eligibility has the same limitations as the Independence
Waiver but the developmental disability must have been acquired before the

CHILDREN’S SERVICES PROGRAM
Every day is a different day at the Children’s Services Program. The children’s program is for 5 –21 year olds with a
special need. This need might be for socialization, life
skills development, learning to get involved in volunteer
projects, cooking or cleaning and learn new games. The
children’s program also works on manners, safety and personal skills.
In the summer for six weeks at Camp Apple the children
like to plant flowers, play catch the ball, learn to bat at a
ball, go swimming, take field trips and much more.
The program is funded with grants from the Butler County
Human services Department, the United Way of Butler
County and by donations.

COMMUNITY HOME PROGRAM
The Community Home Program provides residential placement
for adolescents and adults who have a mentally challenging condition. CCC operates eight homes in the Butler area. There are
three individuals living in each home with staff to support them
in their daily lives. All individuals attend school or some type
of day program that is most suited to their needs.
Evenings and weekends, activities vary, from the very active to
the laid-back! Each home is set up to address the needs of the
individuals who live there and those needs may be constantly
changing, keeping us on our toes.
The Community Home Program has offered service for almost
30 years.
The program employs some very experienced staff who strive to
provide a warm, caring home for all the individuals who make it
their home.
The program is funded by a grant from the Butler County MH/
MR Program.

INFANT-TODDLER THERAPY SERVICES
PROGRAM
Do you have concerns about your child’s development? Have you ever
wondered whether he or she is crawling, walking or talking “on time?”
The Infant-Toddler Therapy Services Program focuses on your child’s
overall development and is here to help. We help children that are birth
to three years of age with developmental delays to reach their highest
potential. There are five areas of development that we focus on:
Gross and Fine Motor Skills:
sitting, walking, crawling, jumping, puzzles, grasping
objects and turning pages in a book.
Adaptive: Independent feeding, drinking and dressing.
Communication:
Following directions, pointing to objects and talking.
Cognitive
Thinking, reasoning and understanding directions.
Social/Emotional:
A child’s ability to interact with others.
Hearing/Vision:
Monitoring a child’s vision and hearing development.
We have licensed Occupational Therapists and Physical Therapists, licensed Speech Language Pathologists, a registered Dietician, Special
Educators and a Vision Therapist. We also contract with The Western
PA School for the Deaf for hearing services. All of our therapists display
a variety of expertise and knowledge of their discipline. The program is
a family centered parent-teaching program. The therapists go out into the
community to see the children in their natural environment, such as the
home, daycare and community settings.
The program is funded by Butler County MH/MR, Infant Toddler Waiver
Services, and Medical Assistance. This service is at no cost to families
and does not tap into a family’s private insurance coverage.

HOMEMAKER CHORE PROGRAM
The CCC Homemaker Chore Program provides part-time
in-home homemaker or chore services to low-income
adults, age 18 through 59 years who have physically and/or
mentally challenging conditions.
The program is funded by a grant from the Butler County
Human Services Department and private donations.

GOLDEN OPPORTUNITIES
ADULT DAY CENTER
Community Care Connections has had the privilege of operating the Golden Opportunities Adult Day Center for ten years!
GO is licensed by the Department of Aging and the Office of
Developmental Programs so the center provides services to the
geriatric population as well as individuals with mental disabilities.
Our experienced staff provides opportunities for socialization
and activities throughout the day. Lunch is provided on-site
and is always a favorite activity for our participants. Besides in
-house activities the center has frequent visits from outside
groups such as singing groups, the Red Hat Society and a variety of animal friends. The group also completes projects for
others in the community such as trimming and organizing
Campbell labels for local schools, providing craft projects to
distribute with Meals-on-Wheels and doing Easter and Christmas projects for AK Steel.

ELKS HOME SERVICE PROGRAM
The Elks Home Service Program provides home visits by a nurse
to persons with disabilities and families with children with disabilities. The program has been in existence for over 30 years,
and is funded by the Elks Lodges and affiliates such as Community Care Connections, Inc.
Any person who has a disability before the age of 21 can qualify
for services. The Elks nurse will do a home visit to evaluate the
needs of the person with disabilities, and work with the family to
find and qualify for various services. The Elks Nurse will also go
to clinic visits, or doctor visits with the family as needed. Any
school related issues also will be addressed with families with
children with disabilities, and the Elks nurse can attend school
meetings with the parent as an advocate for the child, and a resource for the parent.
When there are needs that cannot be met by traditional funding
methods the Elks nurse will work to find funding through the various foundations and charities and write referral letters and help to
fill out the paper work.
The best part of the program is the fact that the service is free, and
the Elks Nurse can work with a person with disabilities throughout
their lifespan. It is one of the few programs that does not stop at
the age of 18, or 21 or 60.
The program is funded by a grant from the Pennsylvania Elks Major Projects, Inc.

Walk, Run, Roll and Caring Rally

N
T
S
The Walk, Run, Roll and Caring Rally was held on August 15, 2009 at
the Masonic Shelter at Alameda Park. It was a beautiful day and many
friends and families and staff attended to enjoy a great time. The event
raised $2,115.65 for the programs and services that Community Care
Connections, Inc. provides to the people of Butler County.
Thank you to Barbara Spinneweber, Lori Schultz and Amy Horn. They
received first, second and third place awards and won cash prizes for
raising the most money through donations collected for the event.
A big thank you to our sponsor, Maher Duessel, for their support the
past three years.
Thank you to the Grove City Barbershop Group for the musical entertainment.
Thank you to the Meridian Fire Department and their volunteer firemen
for bringing their fire truck and giving the children who attended the
educational experience of touring the inside of it.
Thank you to our table sponsors. The Butler County Blind Association
and Baltimore Life Insurance. They both provided many giveaway
items and information about their agencies to all who attended.
Thank you to those who donated items and baskets for the raffles.
They include: Fun For All, Old Country Buffet, Kings, Kathy Armstrong, the Employee Assistance Program and Sue Rudisill. The following programs or departments also each gave a raffle basket: Administration, Infant-Toddler Therapy Service Program , Adult and
