Community Care Connections, Inc. is proud to be a member
agency of the United Way of Butler County. Since 1989, CCC
has received United Way funding, which has been used to
provide services through the Infant-Toddler Therapy Services
Program, the Adult Services Program and the Children’s Services
Program. Community Care Connections, Inc. also receives
donations designated by people working in neighboring counties
through the United Way of Southwestern Pennsylvania.

ADULT SERVICES PROGRAM
The CCC Adult Services Program provides life-skills training for adults
who have intellectual, sensory and/or physical disabilities.
The program is designed around the training needs of each participant and
has included training in meal preparation, reading, money management,
personal safety, computer usage, fitness, volunteerism, social skills for
individual and group situations and appropriate use of leisure time. Our
classes are offered at our program center located at 114 Skyline Drive in
Butler by trained and experienced staff. Activities are also offered that
allow the participants to practice their newly learned skills in their homes
and/or the community. The program also offers assistance with personal
care and wheelchair accessible transportation to and from program
activities, if needed.
The CCC Adult Services Program is funded by grants from the Butler
County Human Services Department, the United Way of Butler County
and by private donations.

ATTENDANT CARE SERVICE
COORDINATION PROGRAM
The CCC Attendant Care Service Coordination Program provides service
coordination for adults with disabilities who receive or wish to receive attendant care
services. Attendant Care Service Coordination Program assists individuals:

develop an Individual Service Plan that outlines what personal care and
housekeeping services they need to remain in their homes and communities,

connect with local providers who provide the needed direct personal care and
housekeeping services outlined in their Individual Service Plans,

secure assistive technology, adaptive equipment or home modifications as
needed.
Attendant Care service coordinators also serve as liaisons between the individual and
their provider of direct care services, ensuring that services are provided at the
frequency, duration, and quality outlined in that individual’s service plan. They
reassess individuals to ensure that their services are redesigned to meet changes in
their health or living arrangements.
The Attendant Care Service Coordination Program is funded through the PA
Department of Human Services Office of Long Term Living. Due to changes in
department regulations, CCC stopped serving as a designated fiscal agent on
December 31, 2012. The program switched from being a provider of direct service to
a provider of service coordination in July of 2012. Effective July 1, 2012, the
program was renamed as the CCC Attendant Care Service Coordination Program.
Services are provided to individuals residing in Butler, Venango, Armstrong,
Mercer, Clarion, Allegheny, Beaver and Lawrence Counties. Services are provided
free of charge to income-eligible adults, and on a sliding fee scale for others.

CAMP APPLE SUMMER DAY CAMP
The CCC Camp Apple Summer Day Camp provides a structured
summer day camp experience for children, six through eighteen years
of age, who have intellectual, physical or sensory disabilities.
During the months of June, July and August, Camp Apple is
scheduled for two or three weeks in each month. Camp starts at
10:00 am. and runs until 3:00 pm. Children participate in day camp
activities including music, outdoor discovery, crafts, adapted games
and sports, cooking, money management, personal safety,
volunteerism and friendship building. Swimming and field trips round
out the schedule.
An Individual Service Plan is created for each child based on input
from the child’s parents. Specially trained staff provide support and
instruction to small groups of children according to their age, interest
and abilities. One-on-one staffing can be provided, if needed.
The Camp Apple Summer Day Camp is funded by the United Way of
Butler County, fundraisers, and private donations. The program is
provided free of charge to eligible children.

CHILDREN’S SERVICES PROGRAM
The CCC Children’s Services Program provides life-skills training and
recreational opportunities for children, ages 6-18 years, who have
intellectual, sensory and/or physical disabilities.
Through one-on-one and small group activities, the children receive
instruction in the areas of healthy eating and simple meal preparation,
fitness and sports, handling money, stranger danger and personal safety,
music, games, volunteerism, outdoor discovery and social skill
development.
Our classes are offered at our program center located at 114 Skyline
Drive in Butler by trained and experienced staff. Field trips are planned
so the children have the opportunity to practice their skills and learn
about their community.
Throughout the school year, the program activities are offered on
Saturdays or at other times when school is not in session. During the
summer, the program changes to a day camp format and is called Camp
Apple.
The CCC Children’s Services Program is funded by a grant from the
United Way of Butler County and by private donations.

COMMUNITY HOME PROGRAM
The CCC Community Home Program provides residential
placement for adolescents and adults with intellectual disabilities.
CCC operates eight homes in the Butler area. Each site is home
for three individuals. Each resident has his/her own bedroom
which they may decorate according to their interests and
preferences. Common areas of each house are clean and
comfortably furnished, creating a warm family-like environment.
Accessibility modifications including ramps, grab bars or tub lifts
are installed at the homes to accommodate individuals with
special health or mobility needs.
Specially trained staff members are always present to provide
each person with the individual support and assistance they
require. Each person has an Individual Service Plan and may
attend a day program or prevocational program. Evening and
weekend activities are scheduled around the interests of each
house member. Family visits are encouraged as are trips home
for holidays or special occasions.
The CCC Community Home Program is licensed and funded by
the Department of Human Services Office of Developmental
Programs.

GOLDEN OPPORTUNITIES
ADULT DAY PROGRAM
The CCC Golden Opportunities Adult Day Program provides a safe, caring
day program environment suited for the needs of older adults with or
without disabilities.
The program is located on Whitestown Road in Butler. Participants may
attend on a full-time or part-time basis. Activities are planned around the
interests and abilities of the participants and are designed to encourage
independence. Guests from the community are regularly invited to share
their talents and entertain both staff and participants. Assistance with
personal care needs is readily available at all times and a full, hot meal is
served at lunchtime.
The CCC Golden Opportunities Adult Day Program is funded through
government grants and participant fees.

HOMEMAKER CHORE PROGRAM
The CCC Homemaker Chore Program helps adults with
disabilities maintain a safe, clean living environment so that
they may continue to live independently in the community and
avoid institutional placement.
The program provides service to low income Butler County
adults, ages 18-59, who have significant intellectual, physical,
sensory or mental health related disabilities and have no other
family member or friend to assist them. The services are
provided on a part-time basis, once every week or every other
week and include light housekeeping, laundry, grocery
shopping and light chore work.
The CCC Homemaker Chore Program is funded by the Butler
County Human Services Department and by private donations.

INFANT-TODDLER THERAPY
SERVICES PROGRAM
The CCC Infant-Toddler Therapy Services Program provides therapy and education
to children, birth to three years of age, who have a developmental delay or
disability. Therapies and instruction that are provided include physical therapy,
speech therapy, occupational therapy, special instruction, and nutrition services.
Therapy and instruction are provided weekly, bi-monthly or monthly, in the child’s
home with parents, siblings or other family members present. Therapy and
instruction can also be provided in other caregiver settings such as grandma’s house
or the home of a trusted aunt. Staff guide parents and family members through
therapy and instruction that will assist their child meet his/her developmental
milestones.
All Infant-Toddler Therapy Services Program staff have extensive pediatric
experience and are licensed or credentialed to provide service in Pennsylvania. The
program currently serves infants and toddlers residing in Butler, Allegheny, Beaver
and Armstrong Counties.
The Infant-Toddler Therapy Services Program is funded through a grant from the
Office of Child Development and Early Learning (OCDEL). Services are provided
free of charge to eligible children, birth to three years of age.

PA ELKS HOME SERVICE PROGRAM
Since 1972, Community Care Connections, Inc. and the Pennsylvania Elks
Home Service Program have collaborated to provide information, referral and
follow-along services to individuals with disabilities and their families. These
services include but are not limited to:





providing in-home medical care management,
advocating for an individual’s educational, legal and human rights,
referring individuals to appropriate human service agencies,
interpreting clinical findings and acting as a liaison between the
individual and his/her health care providers,
assessing the individual’s need for special equipment and
identifying funders to pay for that equipment.
The PA Elks Home Service Program is staffed by a registered nurse. Services
are provided in the individual’s home and are free of charge.
The PA Elks Home Service Program is funded through a grant from the
Pennsylvania Elks Major Projects, Inc.

“AFTERNOON DELIGHT-ITALIAN
STYLE” DANCE
The fourth annual “Afternoon Delight” Dance (formerly A Night to
Remember) was held on Thursday, April 19, 2014 at the Atrium, Rt 422
in Prospect. This event was held strictly for our adult consumers aged 16
and older.
Over 80 guests enjoyed dinner and dancing with an Italian theme. Many
thanks to our sponsors who underwrote the cost of the event.
We also wish to thank DJ Tom Menchyk for an afternoon of wonderful
music.
Lastly, we wish to thank the following members of our staff and our
Board of Directors for making sure our guests enjoyed a wonderful
