This test is th especially important for 11 grade students. All students in grade 9-11 take the PSAT regardless of payment; this is a requested contribution. (See PSAT Information for more.) This means that each student's name, birthdate, birthplace, address, telephone number major course of study, participation in school activities, dates of attendance, awards and previous school attendance may be released in accordance with board policy. In addition, height and weight of athletes may be made available. Appropriate directory information may be provided to any agency or person except private, profit-making organizations. Names and addresses of seniors or terminating students may be given to public or private schools, colleges, employers and military recruiters. Upon written request from the parent of a student age 17 or younger, the District will withhold directory information about the student. If the student is 18 or older or enrolled in an institution of post-secondary instruction and makes a written request, the pupil's request to deny access to directory information will be honored. Requests must be submitted within 30 calendar days of the receipt of this information. If you DO NOT elect to allow directory information to be released to any outside agency, including the military, please sign below and return to the school attendance office within 30 days. Parent signature will prohibit the District from providing directory information to the military, news media, employers, schools, parent-teacher organizations and similar parties. Please take a moment to access the policy online to ensure that you and your student know the school rules and possible disciplinary actions. The entire policy is accessible at http://www.sduhsd.net/About-SDUHSD/School-Board/Policies/5000-Students/index.html. The following are key items of importance: • The LCCHS campus is closed for Freshmen, Sophomores, and Juniors. Seniors are permitted to leave campus during the student lunch period. All students are expected to remain on campus during the nutrition break. • All students must have an Off-Campus Pass (obtained from the attendance office) to leave campus; this includes not returning to campus after lunch. A student may obtain an Off-Campus Pass if: o They have a note, signed by a parent or guardian, which states the reason for leaving, the time of departure, and a phone number for verification purposes. o A parent or guardian phones the Attendance Office (Ext. 6008, Option 1) in advance of the student’s departure to verify the reason for absence. 24 hour advance notice is appreciated. • According to California Education Code 48205, students are considered truant on their third unexcused tardy and/or their third unexcused absence. Unless an absence is cleared with a parent note within three (3) school days, the absence will be recorded as a truancy; no changes can be made after three (3) school days. It is the student’s and the family’s responsibility to provide appropriate notification (either by note or by phone call) to the Attendance Office. Appropriate attendance interventions will be implemented for any student considered to be truant. For more information, please see the SDUHSD governing policy regarding excused absences at http://sduhsd.net/documents/Policies/5000%20Students/5113_ar-1.pdf. • Students are required to have a school staff issued pass to leave classrooms during class hours. • Students are expected to follow the tardy guidelines of their classroom teacher. Students must have a written pass from an LCC staff member for the teacher to consider excusing the tardy. • Disciplinary consequences are assigned if students are considered excessively tardy. Progressive discipline will be implemented for students which may include, but not limited to, Academic Enrichment Tutoring, Friday Night School, Saturday School, and/or, Loss of Privilege List. • Academic honesty is a critical component of high school success. Students in violation of the SDUHSD Academic Honesty Policy are subject to disciplinary action.
