Our Lady Of The Lake Regional Medical Center in Baton Rouge, Louisiana.
Clinical/Operational Goals
True IHE integration means that radiology reports and images are an integrated aspect of the
patient record. The Regional Medical Center is providing this level of integration with a unified
RIS/PACS system (Cerner RadNet RIS and Cerner ProVision PACS). Using the IHE standard has
made it possible to merge the complete RIS, HIS and PACS information set to form a complete
patient record. It also allow the merged data to become a part of the central database that makes
up the heart of the individual patient’s Central Data Repository (CDR). This data includes information from multiple modalities such as CT, US, NM, Digital Film, MRI, and CR.
Because of the ubiquitous use of IHE-defined implementation of standards, the institution is able
to seamlessly integrate a wide range of modality vendors such as GE, AFGA, Lumisys, and
Diasonic, with more being added as the implementation expands. As new modalities are purchased, support for Modality Worklist is mandated by the RFP. Each new modality is then implemented with Modality Worklist when it is brought into clinical use. Consultation between the
RIS/PACS vendor and multiple modality vendors included intensive use of the IHE Technical
Framework to resolve any transaction-related issues.
Other Implementation Details
The goal to become filmless has been achieved at Our Lady of the Lake Regional Medical Center.
Additionally, the institution will soon be integrating voice recognition and digital dictation technology that will greatly reduce the time between report creation and report verification. The DICOM
information is immediately available hospital wide and remotely via the Internet as an integrated
part of the patient record through the use of RIS/PACS and the HIS Electronic Medical Record
(EMR) system (Cerner PowerChart). Using the standard information exchange will allow the radiologist to have current access to the patient’s present and past health information, including DICOM
images. A Web access portal also provides clinicians complete access to high quality images associated with the radiology order anywhere within the enterprise.
Outcomes
Use of IHE transactions enabled scaling the implementation of the scheduled workflow and a
smooth transition management with professional, technical and support staff. The implementation
started with three modalities and added more throughout the year. The IHE Technical Framework
provided a common vocabulary for communication between vendors, therefore helping issues to
be resolved efficiently.
