Centre Walgwan Center Continuum of Care Initiative
Est. 1996
The Centre Walgwan Center is a treatment center for youth between the ages of 12 to 17 years of age. We currently operate a 3.5 month bilingual block program that is culturally based with a combination of contemporary clinical methods. We are located in the First Nation community of Gesgapegiag in eastern Quebec on We

Our initiative was developed based on the need to address the difficulties experienced by youth returning to their communities after treatment.
This initiative was made possible through Health Canada's Harmonization of Services Initiative Fund. Although the majority of youth referrals to our center are by provincial
authorities such as social services and youth protection services, as a
center we felt that our clients would benefit more if our focus was on the
empowerment of the community and aftercare services.

We have found that youth suffer a lot of anxiety when leaving our program. As a
center we have provided a safe, secure and healthy environment. Youth
returning home face major challenges to maintaining a healthy lifestyle.

Our goal: to assist communities develop protocols or processes that
would provide a blanket of support to youth seeking to maintain a
healthy lifestyle following treatment.
How do we do this?
The Walgwan Center team developed a framework of core components
that are based on the medicine Wheel and continuity for communities that
wish to develop a Community Aftercare Protocol.

provided with a framework which is used as a guide to building
community aftercare protocols for youth returning from treatment.

Adopting the process
The Walgwan Center team has developed a guide for use by the center staff. Our staff has been trained on how to implement the process with community referrals
