Allograft Information
General Information:
What is an allograft?
An allograft is soft tissue or bone that is removed from a human donor and used to surgically repair
damaged bones, joints and tendons. Over 1,000,000 allografts are implanted each year. In
orthopedics, an allograft is commonly used during ACL/PCL reconstructions, primary revision
knee/hip reconstruction and massive defects following tumor surgery.
Why is an allograft used?
Because of inadequate amounts of autograft (a person’s own bone or soft tissue) and the limited size
and shape of a person’s own bone, allograft is commonly used in reconstructive surgery of the hip,
knee and long bones, as well as in cases of bone loss due to trauma or tumors. Using allograft tissue
from another person will eliminate the need for a second operative site to remove autograft bone or
tendon, reduce the risk of infection at a separate surgical site, and safeguard against pain and loss of
function at or near the secondary site.
Where does an allograft come from?
A person has a choice regarding whether or not to donate their organs and tissues. These donors are
otherwise healthy people who have experienced a sudden illness or accident which ultimately led to
death.
What types of screening/safety measures are taken?
Before donation, all potential donors must undergo strict screening scrutiny that includes physical
examination, comprehensive medical history and social risk review. A detailed medical history that
includes social background and medical issues is obtained. The information collected on potential
donors is compared against criteria established by the United States Public Health Service to identify
and exclude individuals with high-risk behaviors. Furthermore, all tissue is held in quarantine until
microbiological and blood tests are completed. These tests are required by the American Association
of Tissue Banks (AATB) and the U.S. Food & Drug Administration (FDA) and include an analysis of
infectious diseases such as HIV, hepatitis B and C and syphilis. All of the information is evaluated by
a team of medical specialists in the fields of microbiology, orthopaedic surgery, pathology and tissue
banking. No allograft can be released until the tissue bank’s medical director determines the tissue to
be safe after review of the clinical and laboratory data.
How safe is an allograft?
Preparing the tissue for transplant begins with the removal of debris and organic matter. The allograft
is then soaked in various solutions to prevent the transmission of bacteria and viruses. Processing
and packaging are performed using aseptic techniques in clean room conditions to maintain biological
integrity. On occasion, low dose radiation is used to aid in sterilization. Final processed tissues are
tested for microbiological contamination in accordance with United States Pharmacopeia (USP)
guidelines to ensure compliance with regulatory requirements. Although there is a theoretical risk for
disease transmission, allograft transmitted infections are rare. The FDA has regulated this field very
closely since 1993 to ensure allograft transplant safety. Over the past decade, more than five million
musculoskeletal allografts have been distributed to surgeons for transplant into patients with a
remarkable record of safety.

Statement from UMTB’s Medical Director
The University of Miami Tissue Bank is the first full-service civilian musculoskeletal tissue bank in the
United States. This not-for-profit organization is accredited by the American Association of Tissue
Banks and is registered with the Food and Drug Administration (FDA) it is licensed by State of Florida
Agency for Health Administration, the State of New York Department of Health, the State of California
Department of Health, the State of Maryland Department of Health, and the State of Delaware
Department of Health. The tissue bank performs the tissue procurement and performs processing in
accordance with existing professional standards and according to the FDA Good Manufacturing
Practice Guidelines (GMP) and Good Tissue Practices (GTP). A detailed outline of the donor
selection criteria and quality assurance procedures performed by the tissue bank in processing
allografts is described on the following pages.

Transplantation of tissues touches the lives of many people in two ways; on one hand, it affects the
families of donors; on the other hand, it benefits the recipients. Transplanting bone into patients is
probably the most common form of transplantation today. The other most frequent is the
transplantation of corneas, skin and dura mater. Were it not for the understanding and generosity of
the nearest relatives of donors from whom the allografts were obtained, this would not be possible.
The advances in medical science and technology, plus the clinical experience gained during the last
few decades, allow for the increasing utilization of tissue transplants in surgery. To make this possible
and practical constitutes a major challenge, which requires the dedication of surgeons, pathologists,
nurses, microbiologists, technicians and other support personnel.
It also calls for a continuous effort on the part of researchers in both basic and applied sciences. The
present brochure describes briefly and in general terms the way in which the Tissue Bank and the
University of Miami Miller School of Medicine meet this challenge.
The University of Miami Tissue Bank was founded in 1970 as the U.S. Navy Tissue Bank curtailed its
service to civilian patients. These protocols and techniques were modified to keep pace with
technology and to improve safety. The technical personnel employed by the University of Miami
Tissue Bank during the first years of its operation were technicians trained by the U.S. Navy Tissue
Bank.
In its inception, bone and tissue allografts excised in Miami were freeze-dried by the U.S. Navy
Tissue Bank on a sharing basis. Tissue allografts preserved by freezing in mechanical freezers or in
the vapor phase of liquid nitrogen were processed locally. Since the activities of the Tissue Bank
have increased, the staff has expanded. So too has the scope of processing.
The past faculty members of the Department of Orthopaedics and Rehabilitation participate in the
activities of the Tissue Bank. Originally the Tissue Bank was housed in a three room operating room
suite and an office. Now the Tissue Bank has a separate, dedicated O.R. suite for procurement,
extensive aseptic processing rooms and other facilities used for the storage and preparation of

human tissue allografts under optimal conditions. In the early years, the number of donors from whom
allografts were obtained varied from 20 to 40 yearly. The Tissue Bank now has over 500 multipletissue donors annually. It provides a full spectrum of services for the reconstructive surgery of the
musculoskeletal system and for skin banking.
In 1988, the University of Miami Tissue Bank acquired new facilities for the excision of tissues. A
specially designed operating room suite reserved exclusively for the excision of tissues for
transplantation was constructed in the Dr. Joseph H. Davis Center for Forensic Pathology. This was
made possible by a grant from the Rotary Foundation of Miami.
Currently the University of Miami Tissue Bank provides a wide range of services with emphasis on
allografts used for the reconstruction of the musculoskeletal system. The first massive osteoarticular
allograft prepared by the University of Miami Tissue Bank was transplanted in 1978; the first massive
intercalary allograft in 1974. To date, the Tissue Bank has supplied over 10,000 massive allografts
(osteoarticular, intercalary and structural prosthetic composite) to surgeons nationally and
internationally. Massive osteoarticular allografts are cryopreserved and stored in the vapor phase of
liquid nitrogen. Intercalary allografts are either quick-frozen or stored in the vapor phase of liquid
nitrogen or are freeze-dried. Frozen and freeze-dried allografts used for reconstructive procedures of
the axial skeleton as well as filling of smaller defects of bone and for arthrodesis have been supplied
to surgeons on a routine basis since 1972. Skin allografts have also been distributed since that time.
With few exceptions, all allografts distributed by the University of Miami Tissue Bank are excised and
processed aseptically. Allografts are prepared and processed in accordance with the U.S. Food and
Drug Administration’s regulations, as specified under Good Manufacturing Practices. In addition, the
Tissue Bank prepares other secondarily sterilized bone allografts when specifically requested by the
surgeon.
The possibility of transmission of disease with transplanted allografts has been a major concern to the
University of Miami Tissue Bank. This was heightened by the onset of the AIDS epidemic. To forestall
this possibility, the biology and epidemiology of HIV has been the subject of study by University of
Miami researchers. Some of these findings resulted in recommendations for quality control measures
adopted by SEOPF and by the American Academy of Orthopaedic Surgeons.
In an effort to improve the quality of life and to make transplants readily available to patients, the
University of Miami Tissue Bank has assisted in the organization of new tissue banks in the United
States and abroad. It has also provided training for personnel from other tissue banks from the U.S.
and foreign countries and for surgeons interested in bone transplantation and tissue banking.
Staff members of the University of Miami Tissue Bank have been particularly active in the clinical
application of bone allografts in the reconstruction of failed total hip arthroplasties, reconstruction of
the extremities following resection of tumors and in spinal surgery. As a result, the University of Miami
has one of the largest orthopaedic oncology services in the world. In addition, transplantation of dura
mater allografts for the closure of pachymeningeal defects has been primarily developed at this
institution. The personnel of the Tissue Bank can view the accomplishments of the Tissue Bank with
pride. These have been achieved only through the participation and collaboration of personnel on all
levels. We look forward to the continuation of our activities and service to patients.

The University of Miami Tissue Bank has not, does not and will not engage in the sale of human
tissue. The sale of human tissue for financial compensation is illegal under Public Law 98-507, s.301;
42 United States Code s.274e; and Chapter 873, F.S. 59A1.005(11), F.A.C. The fees imposed on the
recipients of allografts are for the recovery of direct expenses associated with the excision,
preparation, storage, distribution and quality assurance studies of allografts. Financial records are
audited periodically in keeping with University of Miami and State of Florida policies.

Tracking of the Donor
The University of Miami Tissue Bank’s records management system:

Tissue Preservation, Storage, Transit and Reconstitution
Fresh (Refrigerated Temperature)
Allografts:
Femoral condyles, talus, patella, humerus head
Preservation:
Simple hypothermia is the storage of tissues at or about 1 ˚C to 10˚C that are usually immersed in
nutrient culture media (examples: RPMI 1640- Roswell Park Memorial Institute, LR-Lactated Ringers,
etc.) with antibiotics. This method of preservation is used for short-term storage only and is intended
to preserve viable cells. The limitations of this preservation technique are evident in the short shelf-life

of the tissue graft. Hypothermia reduces the oxygen consumption of the viable cells, thereby slowing
cellular metabolism and allowing the recovery of the maximum number of viable cells.
Storage:
The individual allografts are submerged in nutrient culture media with antibiotics. Upon arrival,
allograft should have the appearance of floating in solution. If the liquid appears frozen, the graft
should be treated as useless to its application. The cells have died and are not viable. The sterile
packaging may be in plastic jars or pouches. Whatever the packaging, it is always sterile and in
twofold. The circulator handles the outer packaging while handing the inner container to the scrubbed
personnel.
Transit:
Pre-frozen foam refrigerant (ice packs/blue ice/wet ice) is packed around the fresh tissue in wellinsulated styrofoam boxes. Upon arrival to the hospital, if surgery is not immediate, remove allograft
contained in its packaging from styrofoam box and place in the refrigerator compartment (not freezer!)
until surgery.
Reconstitution:
Suitable thawing procedures for the fresh allograft, submerge into refrigerated 1˚C to 10˚C sterile
lactated ringer’s bath with or without antibiotics. From recovery, processing, storage and shipment,
the allograft has been kept at hypothermia. It is logical to reconstitute the graft in solutions at that
temperature.
Fresh Frozen
Allografts:
Bone-Patellar Tendon-Bone, Achilles Tendon with Bone Strut (Calcaneous), Semitendinosus/Gracilis
Tendons, Tibialis Tendon, Peroneous Longus Tendon
Preservation:
The freezing of tissues at temperatures below -70˚C is routinely used for the long-term storage of
musculoskeletal tissues. Storage at these temperatures is thought to inhibit enzymatic degradation of
tissues, most notably collagenase and protease activity. An uncontrolled freeze of tissues at these
temperatures does not aid in the preservation of viable cells; therefore, this technique is reserved for
tissues in which cellular viability is not crucial to the clinical performance of the allografts. Many
mechanical refrigeration devices are capable of attaining these temperatures. In addition, liquid
nitrogen storage containers are quite suitable for the storage of these tissues, attaining temperatures
as low as -196˚C.
Storage:
The individual allografts are frozen with little or no solution. The sterile packaging may be in plastic
jars or pouches. Whatever the packaging, it is always aseptic and in twofold. The circulator handles
the outer packaging while handing the inner container to the scrubbed personnel.
Transit:
Dry ice packed around the frozen tissue in well-insulated styrofoam or similar boxes has proven
effective for maintaining frozen bone during transport to the user institution. Dry Ice (solid carbon

dioxide, temperatures about -76˚C) is acceptable for storage. The only drawback is that it is a
logistical problem of checking the quantity levels of dry ice and restocking on a daily basis, depending
on the size of the container. For suppliers of dry ice in your area—most supermarkets carry dry ice for
sale or their own storage use—check the yellow pages under dry ice or carbonics. Standard
mechanical freezers were manufactured to mimic the temperatures of dry ice to reduce logistical
problems. However, for reliable long-term storage, it is necessary to reduce the temperature further.
Cryopreservation
Allografts:
Distal Femurs, Proximal Tibias, Meniscal grafts, Hemipelvis, Proximal Humerus
Preservation:
Cryopreservation is the freezing of tissues at a controlled rate, usually 1 to 2 ˚C per minute, in the
presence of a cryoprotectant solution (glycerol). This method is intended to preserve viable cells
when their viability is thought to play an important role in the success of the allograft. Accurate
controlled-rate freezing is usually computer controlled, with liquid nitrogen vapor as the coolant.
Subsequent storage of these cryopreserved tissues is usually at liquid nitrogen vapor phase
temperatures.
Storage:
The individual allografts are frozen with little or no solution. The sterile packaging is in plastic
pouches and is always sterile and in twofold. The circulator handles the outer packaging while
handing the inner container to the scrubbed personnel.
Transit:

Liquid nitrogen:
Only at about -120˚C does gradual growth of ice crystals completely cease.
Liquid nitrogen provides a range of cryogenic temperatures. A property of major importance in liquid
nitrogen is its chemical inertness. Unlike carbon dioxide or other solvents used, it does not react with
the materials with which it comes in contact. It has no effect on the pH of frozen tissue and vaporizes
without leaving a residue. Temperatures range from -130˚C to -196˚C. Tissues should be placed in
the vapor phase above liquid nitrogen because the packaging of allograft may not withstand the
incredibly ultra cold characteristic of the liquid.
Reconstitution: Suitable thawing procedures
Submerge the cryopreserved frozen allograft into a 22-37˚C sterile lactated ringer or similar solution

with or without antibiotics.
Note: Overly heated solutions will denature the tissues and render them useless.
Short Term Storage—Frozen Graft
Storage of bone in conventional freezers (about -20˚C) is popular because it requires only easily
obtainable, generally available freezers. However, at this temperature ice crystals grow and
eventually destroy the bone. In addition to a mechanical disruption by ice crystals, injury from freezing
is caused by factors which may include dehydration, metabolic aberrations resulting from storage at
temperatures at which some enzymes may continue to function and direct chemical injury.
Freeze Dried / Lyophilized
Preservation:
Freeze-drying is the removal of water from a substance under vacuum, transforming the water in its
crystalline form to water vapor without its ever passing through the liquid phase. This phenomenon is
termed sublimation. This means of preserving tissue was first implemented by the U.S. Navy Tissue
Bank. Lyophilization is quite useful in the long-term preservation and storage tissues when other
means are impractical. Once tissue is freeze-dried, it can be maintained at room temperatures
without the need for special handling and can be distributed over long distances. Freeze drying is
purported to alter the biomechanical properties of tissues; therefore, rehydration of the tissue prior to
use is required to restore these properties.
Storage:
The individual allografts packaging may be in glass jars or plastic pouches. Whatever the packaging,
it is always aseptic. The circulator handles the outer packaging while handing the inner container to
the scrubbed personnel.
Transit:
Freeze dried tissue is placed to avoid breakage and is shipped at ambient temperatures.
Reconstitution: Suitable thawing procedures
Submerge the freeze-dried allograft into a 22 ˚C–37˚C sterile saline baths (0.9% sodium chloride) with

UMTB provides the safest and most effective allografts:
The University of Miami Tissue Bank was established in the early 1970’s and has provided safe
and effective musculoskeletal grafts and skin for transplantation without interruption or significant
incident since its inception. The University of Miami Tissue Bank has never felt compelled to treat
tissues with chemicals or irradiation because we make extraordinary efforts to ensure aseptic
procurement and processing to protect the biologic activity of the graft. We always ask the question,
is secondary sterilization of biologic tissue progressive or regressive? Does the margin of “safety” that
it provides make up for the decrement in biologic activity? Does it substitute for poor donor selection
or sloppy procurement? We believe that the American Association of Tissue Banks (AATB) has
adopted appropriate guidelines for Tissue Banking in the United States and the University of Miami is
a proud member of this organization. Does your tissue come from an AATB accredited tissue bank?
We believe that a careful medical and social history is vitally important in identifying suitable
donors and preventing potential disease transmission. University of Miami retrievals are done under
the supervision of a licensed physician who performs a careful examination of the donor and assures
aseptic tissue handling. Bacteriologic surveillance begins with tissue procurement and continues
throughout every component of processing and packaging.
While others may feel compelled to pre treat grafts in detergents and peroxides then terminally
sterilize them for whatever reason, we do not. The use of detergents and peroxides may reduce
bacterial contamination in retrieved tissues however; we have demonstrated diminished biologic
activity in tissues exposed to chemicals such as peroxide. A whiter graft is not a better graft and a
sterile graft may not be biologically active. For this reason, we maintain that appropriate donor
selection, careful medical supervision and aseptic processing are vital to ensure both graft safety and
clinical efficacy.
We fervently believe that the combination of careful donor selection, supervised procurement and
aseptic processing produces ideal safe and effective tissue for transplantation. Our reputation and
