The AAOM with the Antiguo Hospital Civil de Guadalajara
Fray Antonio Alcalde announce the Prolotherapy Treatment
Workshop November 7 – 14, 2015. The Antiguo Hospital
Civil de Guadalajara Fray Antonio Alcalde will host our
patient treatment activities and education programs which
include daily lectures, instruction in patient evaluation and
diagnosis, solutions, needle placement, injection technique
and patient treatment to over 700 patients. The AAOM will
provide instruction to United States Physicians, Mexican
Physicians and Residents.
Dates
Workshop Days are November 7 – 14, 2015 –
Travel Dates are November 7 and 14. Workshop
begins November 8, 2014 at 9:30 AM at the host hotel
Application Requirements and Prerequisites
1. Attendance to at least one AAOM Annual Conference
or Annual Workshop
2. AAOM Active Member – MD or DO
3. Copy of Current State Medical License

Cancellation Policy
Requests for refunds must be submitted in writing before September 1, 2015. Your refund less a 20% processing fee will be mailed to you after the AAOM Mexico Workshop.
No refunds will be granted after September 1, 2015 for Guadalajara unless a replacement
can be found to take your place at the Workshop, and then the above Cancellation Policy
will apply.
