Here you will find the latest information about Callen-Lorde, New York City’s only licensed healthcare facility dedicated to meeting the needs of LGBT people and those living with HIV/AIDS. Callen-Lorde welcomes all who seek our services, regardless of any patient’s ability to pay. As you look through these pages, you’ll notice how much we’ve grown over the last year. We are proud to serve more people than ever with our primary care, dental, mental health, gynecological, case management, pharmacy, and health education services. In these difficult times, when more New Yorkers are in need of affordable, quality healthcare, Callen-Lorde is more vital than ever, and we remain committed to providing the world-class model of quality care that our patients have come to expect. Jay Laudato Executive Director Dear Friends, For over 40 years, Callen-Lorde has provided a safe, welcoming haven for literally tens of thousands of LGBT New Yorkers. When this visionary organization first opened its doors, it was a tiny volunteer-driven clinic serving mostly gay men who were experiencing discrimination and hostility in mainstream healthcare settings. Today, Callen-Lorde expands on this tradition and serves as a model for other healthcare providers across the country and around the world. As co-chairs of Callen-Lorde’s Board of Directors, we have helped guide this organization— but we cannot do this without you. Please review this report and get involved with CallenLorde as a supporter, as a donor, as a volunteer, or maybe even as a board member. Callen-Lorde is New York City’s only community health center dedicated to meeting the healthcare needs of lesbian, gay, bisexual, and transgender (LGBT) people and those living with HIV/AIDS, regardless of any patient’s ability to pay. For over 40 years, we have welcomed all, regardless of sexual orientation or gender identity, who otherwise lack access to respectful, empowering, and informed primary healthcare services. We continuously strive to create an environment that affirms humanity and individuality and inspires people to realize their own health and wellness. Our Mission Callen-Lorde Community Health Center provides sensitive, quality health care and related services primarily to New York's lesbian, gay, bisexual, and transgender communities—in all their diversity— regardless of ability to pay. To further this mission, Callen-Lorde promotes health education and wellness and advocates for gay, lesbian, bisexual, and transgender health issues. Our Licensure Callen-Lorde is licensed by the New York State Department of Health as a Diagnostic and Treatment Center to provide comprehensive outpatient medical services including a full oral health clinic; pharmacy accredited by the NYS Department of Education; and most recently, Article 31 licensure for Outpatient Adult and Adolescent Mental Health Treatment by the Office of Mental Health . Callen-Lorde is designated by the federal Health Resources and Services Administration as a Federally Qualified Health Center (FQHC) and is recognized as a level III PatientCentered Medical home by the National Committee for Quality Assurance. 4 Our History Callen-Lorde's grassroots heritage dates back over 40 years to the volunteer-based St. Mark's Community Clinic, founded in 1969 to provide screening and treatment for sexually-transmitted infections. In 1983, St. Mark’s merged with the Gay Men's Health Project to form Community Health Project, a mostly volunteer-staffed, episodic care program housing the nation's first community-based HIV clinic. In 1998, Community Health Project changed its name to Callen-Lorde Community Health Center and moved into a modern, state-of-the-art facility in Manhattan’s Chelsea district. In 2002, Callen-Lorde became a Federally Qualified Health Center, a historic recognition by federal health agencies of the importance of improving health disparities faced by LGBT persons. Over the last decade Callen-Lorde has continued to grow and add new programs, including a dental clinic, mental health services, and an on-site pharmacy—and it remains the New York metropolitan area’s only healthcare facility Michael Callen (1955-1993) Audre Lorde (1934-1992) Michael Callen was born in Rising Sun, Indiana, and began his career as a successful composer and singer. In 1982, he was diagnosed with AIDS (then known as GayRelated Immune Deficiency) and quickly became a leader in the response to the growing epidemic. In 1985, he became a founding member of the People with AIDS Coalition, the first organization to spring from the AIDS Self-Empowerment movement. As an leading authority in the community, Michael was called to testify before the President’s Commission on AIDS and both houses of the United States Congress. In 1990, Michael chronicled the stories of longterm survivors in his book Surviving AIDS. Michael continued his work in the community and in music until his death at age 38. He is survived by his partner, Richard Dworkin. The daughter of Caribbean immigrants, Audre Lorde was born in New York City and wrote her first poem at age twelve. In the 1950s while at Hunter College, Audre worked as a librarian and educator and became a leader in the early lesbian activist community in Greenwich Village. Her poetry was published regularly throughout the 1960s and 70s and gained wide recognition with the publication of her book The Cancer Journals in 1980, which chronicled her experience with breast cancer. In this groundbreaking work, she examined her own fears, anger, and ultimately, her strength and hope. Audre was named New York State’s Poet Laureate in 1991, a post she retained until her death at age 58, after a 14year struggle with breast cancer. She is survived by her children, Elizabeth and Jonathan, and her partner, Dr. Gloria Joseph. 5 Programs and Services Primary Care Comprehensive primary care services delivered in a skilled and culturally sensitive multidisciplinary team model, stressing prevention and wellness. Our providers also offer management of chronic health problems ranging from asthma to heart disease, as well as easily accessible appointments for urgent care. In 2012, Callen-Lorde served nearly 15,000 patients who made over 85,000 patient visits. Health Outreach To Teens (HOTT) Adolescent and young adult medical and mental health program targeting homeless youth, including New York State’s largest youth-focused HIV care program. Services are provided in a dedicated clinical suite on site and through a licensed mobile medical van. All services – including prescription medications - are free of charge or low-cost to young people ages 13-24. In 2012, the HOTT program provided free medical and mental health care to over 1,400 largely homeless youth, both on site and on our mobile medical unit. HIV/AIDS Care Comprehensive HIV-related primary care services with experienced providers meeting New York State Department of Health standards for HIV specialists, offered in conjunction with Care Coordination services which provide assistance to patients in navigating concrete services such as housing, health benefits, and immigration issues in order to remove barriers to care. CLCHC has been funded by the CDC since June 2011 to implement an HIV prevention intervention designed to reach high risk transgender youth of color in NYC and their partners. This intervention seeks to increase access to HIV counseling and testing and linkage to care for transgender youth of color, as well as to provide the evidence-based intervention Comprehensive Risk Counseling Services for persons at a high risk of exposure to HIV experiencing multiple and complex risk reduction needs. In addition, Callen-Lorde is working with Hunter College to conduct a PrEP ( pre-exposure prophylaxis) demonstration Project, developing, and then testing, an intervention that integrates PrEP delivery and support into a comprehensive prevention package delivered in a “real world” community health center. Women’s Health Gynecological services and comprehensive primary care focused on the unique needs of lesbians and bisexual women, as well as women of transgender experience. In addition to routine care, experienced medical staff provide colposcopy, cryotherapy, and LEEP—procedures designed to prevent cervical cancer. Case management services are also provided to uninsured patients to obtain entitlements and access follow-up diagnostic and specialty care. Our Women’s Health programs place special emphasis on breast/chest and cervical cancer screening. To assist uninsured patients, we participate with Columbia Cancer Prevention Services, which provides coverage for all screening and diagnostic testing. For patients with abnormal screening tests, our case manager ensures that patients gain access to specialty tests to determine if they have cancer and facilitates their access to treatment. Facilitated Enrollment In 2012, Callen-Lorde received a grant from the New York State Department of Health to provide Facilitated Enrollment services for the state’s public health insurance programs, Medicaid, Family Health Plus and Child Health Plus. Now, at Callen-Lorde and 14 other sites in Manhattan, Brooklyn, Queens and the Bronx, trained staff screen interested applicants for program eligibility, help them complete the application, assist in the procurement of needed documents, and submit these applications on their behalf. This service is provided free of charge. The Callen-Lorde Facilitated Enrollment is focused on providing a compassionate and culturally sensitive enrollment process and is committed to reaching out to New York’s most vulnerable residents. Since the program launched in April 2012, the Callen-Lorde Facilitated Enrollment program has helped more than 800 individuals access health coverage. 6 Mental Health Our services include individual and group psychotherapy, psychiatric assessment, psychotropic medication management, crisis intervention, substance abuse assessment, and stabilization services. In 2012, Callen-Lorde applied for and received Article 31 licensure to allow us to provide long-term mental healthcare and medication management for those in need. Supportive Case Management As part of our expanding efforts to reach out to all members of our communities, Supportive Case Management services are now specialized for transgender and gender non-conforming patients, for adolescents, for women, and for HIV positive people. Oral Health Comprehensive dental and oral health care including preventive, restorative, rehabilitative, cosmetic, endodontic, periodontal, and emergency services, as well as a dental hygiene program. Transgender Health Comprehensive, sensitive primary care including gynecology, and sexual health services, including counseling and education, hormone therapy, and referrals to social support services. Sexual Health Comprehensive screening, diagnosis and treatment of sexually transmitted infections and individual counseling, focused on the specific needs of the LGBT communities. The Sexual Health Clinic aims to assist patients stay safe and healthy while maintaining a satisfying sex life. Alternative Insemination Uniquely tailored care encompassing family-building options, fertility awareness, assistance with donor selection, and sperm storage. Both office-based insemination services and instruction in home-based, non-technical insemination are offered. This program provides sensitive care to anyone who wants to start a family but feels marginalized by other insemination clinics. Health Education, Training, and Outreach Health information, training, and education to groups and institutions outside of Callen-Lorde to ensure that LGBT people receive safe and supportive health care services beyond our doors. In addition, outreach activities recruit new audiences and new patients. In 2012, Outreach and Education completed 60 cultural competency and sensitiv- ity trainings , including seminars at: GLMA (Gay & Lesbian Medical Association; Yale School of Nursing; The Ralph Lauren Cancer Center, and The Albert Einstein Medical School. Pharmacy In November 2010, Callen-Lorde opened an onsite pharmacy in the lobby of our facility. At the end of its first year, the pharmacy was already filling over 5,000 prescriptions a month, including significantly discounted medications to people without health insurance. Prescriptions are sent electronically from our medical staff to the pharmacy ending potential communication errors between prescribers and pharmacists. 7 PATIENT VOICES Community Advisory Boards Callen-Lorde’s Community Advisory Boards (CABs) provide patients with a forum where they can impact the services and programs offered at Callen-Lorde. Currently, CABs are available for HIV positive patients, women, adolescents, and transgender and gender non-conforming patients. The valuable experiences and input of the CAB members help Callen-Lorde provide the best possible services to our patients. In addition, the CABs provide patients with a meaningful outlet to discuss concerns about their care and to affect changes in policies and procedures—a process that can have positive clinical applications to patients who might otherwise feel isolated or powerless. “I absolutely love Callen-Lorde. Patient Satisfaction To make sure that Callen-Lorde is providing the level of quality care that our patients deserve and have come to expect, we periodically conduct patient satisfaction surveys— and the results of the latest one revealed that most patients are very happy with the care they receive at Callen-Lorde, with patient satisfaction improving significantly across every measured category as compared to prior years’ results. This input helps us continually improve the quality of our services. For example, as a result of patient feedback we created a new phone system that was rolled out in the fall of 2008, which included a call center where live staff members now answer all incoming calls during our hours of operation. “These people are saints. We provided over 85,536 visits to15,509 patients.—47% of which lack health insurance. 35% identify as women and 65% identify as men. Brown, M.D., former New York City Public Health Commissioner, helped change the image of gay men and lesbians by publicly affirming his homosexuality in 1973. In the spirit of Dr. Brown and his unwavering courage and dedication to the health of our communities, we have established our major donor society in his name. The mission of the society is to encourage major financial support for Callen-Lorde to offset the over $5 million in unreimbursed services we provide every year. Members make annual contributions of $1,000 or more to help Callen-Lorde continue its vital work offering the highest quality healthcare to the LGBT communities. Members of the Howard J. Brown Society are among Callen-Lorde’s most valued benefactors. Donors at this level provide the essential funding that translates into pioneering medical advances and unrivaled excellence in patient care. As a token of our appreciation, members receive benefits and privileges including complimentary tickets to Walk on the Wild Side, our annual summer party at the Central Park Zoo. Members also receive invitaions to two regularly scheduled member recpetions each year. Members are encouraged to invite guests to learn about CallenLorde and receive updates on our programs and services. The reception also allows us to say thank you while members have the opportunity to network with other members. But perhaps the most important reward is the satisfaction of knowing that they truly play a vital role in the health and wellbeing of the LGBT communities. On behalf of our staff, Board of Directors and especially the patients who look to us for their care, we offer our deepest thanks. 19 ANNUAL EVENTS Community Health Awards Each fall for over a decade, Callen-Lorde has hosted the Community Health Awards to honor the leaders in the field of LGBT health. The event consistently draws hundreds of CallenLorde’s strongest friends and supporters. The evening includes a cocktail reception, banquet, awards presentation, entertainment, and inspirational speeches from community leaders. Past honorees include former Surgeon General Dr. Jocelyn Elders, Pulitzer Prize, Tony Award and Emmy Award winning playwright and activist Tony Kushner, tennis star and health advocate Martina Navratilova, and Dr. Kevin Fenton, Director of the CDC’s Center for HIV/AIDS Prevention. Honors also go to local physicians and institutions that are passionate about providing the highest quality care to the LGBT communities and Honoreee Cynthia Nixon with Edie Windsor. In 2012, Callen-Lorde honored Cynthia Nixon, Martin Markowitz, MD, of the Aaron Diamond AIDS Research Center and Paul, Weiss, Rifkind, Wharton and Garrison LLP. Walk on the Wild Side Held at the beautiful Central Park Zoo, Callen-Lorde’s Walk on the Wild Side has been the unofficial kick-off for the summer Pride season since 2001. The event draws over 1,250 supporters and their children — becoming the only truly family-friendly New York City Pride event. The evening features delicious and bountiful food and drink, arts and crafts activities, face painting, a performance by the resident sea lions and appearances by celebrities such as Cynthia Nixon, Cherry Jones, B. D. Wong, John Tartaglia, Judy Gold, Denis O’Hare and the stars of the Broadway productions of The Lion King, Tarzan, Beauty and the Beast and the cast of STOMP. Spring Fever 20 In April 2008, Callen-Lorde hosted its first event exclusively for our female donors and their friends, Spring Fever, at the beautiful Cheim & Read gallery in Chelsea. Every year since, over 100 women have gathered for cocktails and hors d’oeuvres in support of Callen-Lorde’s vital Women’s Health services. In its second year, Spring Fever began to honor influential women who have positively impacted Callen-Lorde and the broader community — honorees have included NY Times blogger Kim Severson, Executive Director of M•A•C AIDS Fund Nancy Mahon, and most recently , advocate and equal marriage champion Edie Windsor.
