Our hectic lives seem to leave us with less time and energy
to focus on our health, and as a result, we are spending more
time in the doctor’s office and more money on prescriptions to
manage our health issues. Our health care costs seem to keep
going up. But we should remember that ultimately, we can
make an impact in the way we utilize health care by considering
the cost and quality of the care we receive.
In this guide, read about Doug, Cathy, Ed and Amy, who have
taken small steps toward changing their health using the tools
and support J.B. Hunt provides. They did it, and you can too.
Six years ago, Doug Gibbs,

There are two ways for you to save in 2013:
ASSESS YOUR HEALTH
1
You’ll save $5 per week on your medical premiums if you take an online
Health Assessment to learn more about your overall health status. If you
cover your spouse, and he/she also takes the assessment, you’ll receive
an additional $5 discount each week.
BE TOBACCO-FREE
You’ll receive a weekly $5 discount on your medical premiums if you
are tobacco-free for at least 30 days prior to Nov. 15, 2012. If you cover
your spouse, and he/she is also tobacco-free, you’ll receive an
additional $5 discount every week.

What happens if you don’t go online and enroll?
If you and your family are currently enrolled in a J.B. Hunt medical plan but don’t go online and enroll,
you will continue to have coverage but will pay the higher, non-discounted medical rates in 2013.

J.B. Hunt offers three medical plans that will provide you and your covered family members
the protection you need for everyday health issues or when the unexpected happens.
All our medical plans have these same features:
• Certain preventive care services are covered at 100%. If you go to a network
provider, you can get services like immunizations, mammograms, Pap tests and
flu shots at no cost to you.
• Choose from a wide variety (a “network”) of medical providers and you’ll save.
Not only will network providers charge you less for their services, you’ll never pay
more than your out-of-pocket maximum.*
• Prescription drug coverage at no additional cost.

The HSA Medical Plan is your best value
This plan offers the best value at a lower premium, AND you can contribute dollars to a
Health Savings Account to help pay your portion of qualified medical expenses such as
your deductible and coinsurance, and you can change your contribution amount at any
time. You can also use your HSA dollars to pay for expenses that aren’t covered by health
plans such as some over-the-counter medications.
One great feature of a Health Savings Account is if you have money left at the end of the
year, it rolls over year-to-year—you don’t lose it.

PPO/OAP Medical Plan
This plan has a lower annual deductible than the HSA Medical Plan, but the premium
you pay will be significantly higher each pay period. You’ll pay a copay each time you see
a network primary care provider even after you’ve met the deductible.

Use pre-tax dollars and save
A Flexible Spending Account, or FSA, is a great way to use pre-tax dollars to pay for child
care or for qualified medical expenses that may not be covered by your medical plan.
The way an FSA works is pretty simple. You determine how much you want to put in your
account each week to pay for services like daycare or other qualified health care expenses.
You’ll get a debit card to pay for any expenses. It’s that easy.
There are a couple things you need to know about FSAs:
• You get the same triple tax advantage that a Health Savings Account provides.
• You can’t change your contribution amount during the year unless you have a
“qualifying” event like having a baby or if you get married. Maximum contribution
amounts for 2013 are:
• $2,500 Health Care FSA
• Up to $5,000 Dependent Care FSA (based on your tax filing status)
• If you don’t use all of the money during the year, you lose it.
You can enroll in a Health Care FSA if you think you’ll need additional tax-free dollars
to pay for medical expenses. If you enroll in a Health Savings Account and a Health
Care FSA, your FSA would be limited to use for dental and vision expenses only.

Your pharmacy benefit
Using generic drugs is a great way to save money. Generics are equivalent to brandname drugs and must meet the same FDA quality and safety standards. They will
treat your medical condition the same as brand-name drugs.
Using home-delivery can save you even more money. You can get a 90-supply of your
maintenance medications for less than you would pay at a retail pharmacy for a
90-day supply, and they’re delivered right to your home! If you are interested or ready
to enroll in home-delivery service, call Express Scripts toll free at 877-804-4111.
Keep in mind, non-preferred brand-name drugs will not be covered. Specialty injectables
are available through mail order only. You pay 20% but no more than $100 for a 30-day
supply or 20% but no more than $200 for a 31-90 day supply. If you enroll in the HSA
Medical Plan, you must first meet your annual deductible to receive this benefit.
If you choose an HSA Medical Plan or the PPO/OAP Medical Plan, your prescription drug

Good dental care is more than just a great smile—it’s important to your overall health.
Regular preventive dental care (check-ups and cleanings) can help your dentist find and
fix problems before they become serious.
J.B. Hunt offers dental coverage during your first year of employment. You can enroll in
the Limited Dental Plan, a “bridge plan” that will help bridge the gap until you are eligible
to enroll in the Full Dental Plan. Keep in mind, your coverage in the Limited Dental Plan
will end on your first anniversary of employment. In order to ensure you don’t have a
lapse in coverage, you can enroll in both the Limited and Full Dental Plans during your
initial eligibility period, and premiums and coverage for the Full Dental Plan will not start
until the day following your one-year anniversary.

J.B. Hunt offers a vision plan through Superior Vision with copays as little as $10 for exams,

is limited to once every 12 months for exams, lenses and contacts and once every 24
months for frames.

Disability insurance will replace a portion
of your income if you cannot work due to
an illness or injury.
Driver disability
Drivers can enroll in two- or five-year disability
coverage during their initial eligibility period
or during Open Enrollment. You cannot
enroll any other time during the year. There
is a 30-day waiting period from the day the
disability begins until benefits may be paid.
Short-term disability
Supplemental life insurance
All full-time employees who work at least
36 hours a week can enroll in supplemental
life insurance any time during the year.
If you enroll after your initial enrollment
period, you will be required to provide
Evidence of Insurability to prove you are
eligible for coverage. You can purchase
life insurance in increments of $25,000 up
to a maximum of $150,000 or $500,000
depending on your position.
Voluntary supplemental AD&D
All full-time employees are eligible to enroll
for supplemental AD&D coverage once they
become benefits-eligible. You can cover
yourself from $25,000 to $300,000. Your
dependents’ coverage will be based on
the amount you choose for yourself.

Full-time, active office and shop employees
are automatically enrolled in the shortterm disability plan on the 181st day of
employment. Short-term disability replaces
a portion of your J.B. Hunt income when
you are unable to work due to qualified
illness or non-work related injury and
covers the first 26 weeks of disability.
Long-term disability
If your disability lasts longer than 26 weeks,
long-term disability will provide you with
60 percent of your pre-disability monthly
earnings (less any other income you have).

If you don’t enroll…
• If you currently have coverage and don’t go online and enroll between Oct. 15 and
Nov. 15, 2012, you will automatically be enrolled in the same plans you are currently
in, but you will not be eligible for the tobacco-free discount or the Health Assessment
discount (unless you and/or your spouse took the assessment in 2012).
• If you currently have no coverage and don’t go online and enroll, you will have NO
coverage in 2013.
Keep in mind, if you miss this opportunity to enroll, you will not be able to make any
changes to your medical or most other coverage plans until the next open enrollment
period unless you have a qualifying event.
