Dear Parents,
Welcome to the San Jose Unified School District Early Education Program. We
have designed this parent handbook to share important information with you
about the San Jose Unified School District (SJUSD) Early Education Program
policies—we want to support you in ensuring your child experiences a great
school year.
You are your child’s first teacher. Therefore, we are providing this handbook to
you, so you can help your child enjoy a highly successful preschool year. With this book, you can
work with your child to provide the kinds of learning experiences that will help him or her start
kindergarten ready for success in school and life.
During the year, we value your vital support in creating a quality Early Education program that
includes a caring, qualified staff, and a wide variety of learning experiences for your child as part of
an age-appropriate curriculum. The school district has operated this important program for over 20
years and prepared thousands of young learners for success in school.
Please let us know if you have comments or questions during the year. Besides parent meetings we
will hold during the year at your preschool site, we also organize a parent advisory group. This group
meets during the year to allow parents to provide feedback and suggestions for the program. Please
watch for information about this group, the Parent Advisory Committee (PAC). Our goal is to give
your child a variety of learning experiences and opportunities for growth. We appreciate having your
input because it is an important part of your child’s education.

The San Jose Unified School District Preschools provide a part-day (three hour) program
designed to prepare children to enter kindergarten the following academic year and to enjoy
success in school. Families are eligible for the program if they meet low-income guidelines or

Curriculum/Assessment
The San Jose Unified School District (SJUSD) Early Education Program provides a
comprehensive, age-appropriate educational curriculum for preschool-age children. The district
offers these services to children based on program eligibility requirements. The program does
not discriminate on the basis of age, sex, ethnic group, race, ancestry, national origin, religion,
color, sexual orientation, or mental/physical disability. SJUSD pre-schools also welcome the
enrollment of children with disabilities. All instructors refrain from religious worship or
instruction.
The preschool curriculum uses a child-centered and developmentally sequenced model in all
classrooms. The curriculum encompasses early-literacy and math concepts, opportunities for
science, art, music, dramatic play, and multicultural activities, as well as health education and
oral language expression. Every week instructors post lesson plans that list the upcoming
classroom activities.
Each classroom is staffed with a qualified teacher and an appropriate number of instructional
associates. Many of our staff members are bilingual (English-Spanish, or English-Vietnamese).
The SJUSD Early Education Program uses a state of California preschool assessment system
called Desired Results Developmental Profile. As part of this assessment system, each child’s
growth is monitored throughout the year. Teachers share a progress report with each child’s
family during the Parent Teacher conferences, or when necessary.
The Desired Results system is designed to ensure that children develop the following:
• Personal and social competence
• Effective learning skills
• Physical and motor competence
• The ability to grow, learn and play in a safe and healthy manner
As part of each child’s growth assessment, family-members will fill out an Ages and Stages
Questionnaire. This questionnaire will allow the Teacher and family to share and discover how
the child interacts at home. As a result, teachers can provide the appropriate support for your
child to succeed in the classroom.
The program will schedule a Parent/Teacher conference each Fall and Spring to review your
child’s growth results and his or her readiness for Kindergarten.

Program Procedures
A. Enrollment Process and Required Forms:
Parents may enroll their child(ren) in the SJUSD Preschool Program only after the clerk has
determined they are eligible based on program eligibility and needs criteria as well as available
openings, and only after they complete the following documents:
1.
A Confidential Application for SJUSD Preschool Services to determine/verify:
• eligibility, need, and priority
• current family income per month
• the number of family members in the home
• that families qualify for State Preschool or Power of Preschool based on
eligibility criteria.

The Consent for Medical Treatment Form
3.
An Emergency Card (Information on this form must be kept up-to-date.)
Should your child become seriously ill or injured, we will attempt to contact you, the
parent, first. If you are not available, we must be able to reach a person you have listed
on the Emergency Card. People you list on the Emergency Card must be at least 18
years old, and should be prepared to show their identification when picking up your
child. Important: Children will only be released to the individuals listed on the
Emergency Card; therefore, it is critical that you keep this information current.
4.
A complete Physical Examination Form from your child’s doctor that is current (within
one year from the date of enrollment). The physician’s report must state that the child is
physically and emotionally ready for interaction with a peer group, and it must identify
any special health concerns. This form must be submitted within thirty days of
enrollment or program administrators will start the enrollment termination process.
5.
A Complete Immunization Record with DATES.
6.
A Child’s Developmental History report.
7.
A Parents’ Rights Form, as required by the California State Department of Education, to
provide the parent/guardian (with appropriate identification) the right to enter and
inspect the SJUSD Preschool Program facility without advance notice to the provider.
This form is included in the enrollment packet.

A Parent’s Appeal Rights form as required.

The Personal (Children’s) Rights Form as required by the California State
Department of Education.
10. A signed Admissions Agreement. This form indicates that parents grant inspection
authority, according to Section 101195 of the Department of Social Services/
Community Childcare Licensing as stated in the Child Day Care General Licensing
Requirements. This agreement gives any authorized representative of the department
the right to enter and inspect any program without notice; to interview children; to
inspect and audit children’s records; to observe the physical condition of children; and to
commission a licensed medical professional to physically examine children without prior
consent. Appropriate identification from the licensing agency will be obtained prior to
implementation of the above.
B. Notice of Action:
The SJUSD will give a Notice of Action to inform parents of the effective date of program
enrollment and/or any change of services, including termination of services.
C. Reasons for Termination of Services:
Services may be terminated for failure to follow the SJUSD Preschool Policies and Procedures
and/or State/FIRST 5 Program Guidelines.
Also, service may be terminated if more than 10 Best Interest of the Child (BIC) days are taken
during the school year. BIC days include family vacation days when preschool is in session
and days missed due to lack of transportation, court dates, or visits with relatives who are
unable to bring the child to school. (See handbook section below for additional information
about BIC days.)
D. Appeal process:
A family may appeal to the SJUSD Early Education Program Manager for a change in
services. If a family is not compliant with program regulations, parents will be given fair
warning of the problem, as well as ample time to rectify the situation. If the problem persists,
and the family is not compliant with the necessary Funding Terms and Conditions or other
governing regulations, a Notice of Action for termination of services will be sent. Services will
be terminated and care will end 14 days from the date of the Notice of Action was given, or 19
days from the date the Notice of Action was sent by mail. Please keep in mind that parents do
have the right to appeal their termination should they feel that the action taken against them
was unjust. The request for an appeal may be made in person, over the phone, or in writing.
Parents may also choose to have an authorized representative (AR) represent them. All

appeals are conducted at the Early Education Office, however a request may be made to the
program manager for an alternative method, i.e. a phone conference, if there is a schedule
conflict or transportation issue.
Further information concerning the appeal process can be found on the reverse side of the
Notice of Action that you receive. After having reviewed the Notice of Action, if you still have
questions, please contact Early Education Office.
For your information, this handbook includes The Uniform Complaint Procedure used by all
SJUSD programs. See page 20 for more information. The Early Education Program makes this
information available in English, Spanish and Vietnamese.
E. Fraud Policy:
“Fraud” is defined as a deliberate deception for unfair or unlawful gains. Families enrolled in
the state or federally funded programs must meet the Eligibility and Need Criteria. All
documentation supplied by the Early Education Program regarding verification of need and
eligibility must be complete and true. If the parent provides fraudulent information to establish
initial or ongoing eligibility, services will be terminated.
If a family has been terminated due to fraud and disagrees with an action, and files for a fair
hearing (appeal) and the parent loses the fair hearing, he/she may responsible for reimbursing
The Early Education Program for the cost (family fees if applicable) of all services, which may
be provided during the appeal process.
F. Nutrition/ Meals:
A breakfast or lunch meal is provided daily through the SJUSD Student Nutrition Services
Department. Monthly menus are posted in the classroom, and offer a variety of foods: Milk,
fruit, juices, vegetables, grains through bread or crackers, and high protein foods like cheese,
meat, eggs, or peanut butter. Food from the school meal is to be eaten at school and may not
be taken home.
G. Sign-In and Out:
The Community Care Licensing Department mandates all licensed preschool program for
parent or their authorized representative, to sign each child in and out every day on the Sign
In/Out sheets located in the classroom. A full signature and time notation is necessary. If your
child is absent, you will need to indicate the reason for the missed class when he/she returns
to school.

Pre-school sessions operate from either 8:00 a.m. to 11:00 a.m. or 12:00 p.m. to 3:00 p.m.,
Monday through Friday. At the end of each session, staff can only release a child to a parent or
authorized person/s (at least 18 years of age or older) designated on the Emergency Card.
Identification may be requested from those parents have authorized. As a parent, you are
responsible for informing the preschool teacher and the Early Education Office (408-535-6083)
of any necessary changes to the Emergency Card.
H. Absences:
Absences will be excused if due to illness of the child or members of the immediate family, or
because of doctor or dentist appointments. Upon the child’s return, the Sign In sheet must be
completed with the parent’s full signature for each day of the illness along with a brief reason
for the absence (i.e. a cold, flu or cough). An absence for illness longer than three days
requires a signed doctor’s note. Other excused absences include appearance for court dates,
lack of transportation, or court ordered visit(s).
SJUSD considers school days missed due to family vacations or visits from family members as
“Best Interest of the Child” (BIC) days. The program only allows 10 BIC days within the school
year. Per state guidelines, families exceeding 10 BIC days will be dropped from the enrollment
list.
I. Pick-Up:
Children must be picked up on time. If you are unable to pick up your child, you must make
arrangements with someone listed on your child’s emergency card to pick him/her up from
school at the designated dismissal time. Identification must be provided before the child will be
released to this person. Please inform your teacher of this arrangement prior to pick up.
If your child is picked up late, you will be asked to sign a Late Pick-up Notice. If you receive
three Late Pick-Up Notices, your child will not be allowed to return to class until you meet with
the Early Education Administrator. As the parent, you will need to contact the Preschool Office
at 408-535-6083 to set up an appointment with the administrator.
Important: Preschool staff will only release children to individuals listed on the Emergency Card
and they must show proper identification when picking up the children. It is critical for the
parents to keep this information current by contacting the office staff with changes.
J. Uniform Policy:
In order for our students to integrate and be recognized as being part of the school community,
students enrolled in the San Jose Unified Preschool Program must wear uniforms according to

the elementary school’s uniform colors. If you did not receive this information when you
enrolled your child, please check with the school office or the Early Education office for school
colors and other requirements.

Behavior Management Policy
The SJUSD provides this behavior management policy to ensure a safe and nurturing learning
environment for all children. Children enrolled in the SJUSD Early Education Program for
Preschool are expected to exhibit reasonable behavior in the preschool setting. We believe that
children find security and direction with consistent adult guidance and discipline.
Such guidance allows children to securely explore, observe, feel, interact and learn through the
process of being involved with people and things in their world. With sensitivity, fairness and followthrough, SJUSD Early Education instructors consistently implement discipline to promote each
child’s development of self-direction, self-control and socially acceptable behavior. Consequently,
our program prohibits corporal punishment and violation of a child’s personal rights.
The program considers the following child classroom behaviors unacceptable:
• Disruption of the program.
• Endangerment of the health or safety of himself/herself, other children or staff.
• Continuous refusal to follow staff directions or accepted rules of behavior.
Behavior Management Plan:
The process and strategies used to support the behavior management policy include:
1. Encouraging positive behavior by redirection.
2. Offering acceptable behavior alternatives (choices) whenever possible.
3. Giving positive reinforcement for appropriate behavior in the form of encouragement and
acknowledgment of the positive action.
4. Documenting the child’s actions, classroom strategies used and outcomes.
5. Enlisting parental support by informing them, both verbally and in writing, of behavioral
concerns and interventions used.
6. Arranging a personal conference with parents whenever necessary. Such a conference
will involve establishing specific behavioral goals and setting a time period for checking
progress toward those goals.
7. If inappropriate behavior continues, the teacher will consult with the program
administrator—all documentation and conference notes will be reviewed to consider the
situation and possible removal of the child from the program.
8. Extreme or violent behavior may result in immediate removal from the program for an
unspecified amount of time, to be determined on an individual basis. In such cases,
staff, parents and the program administrator must hold a meeting prior to the child’s
return to the program.
9. Failure to comply with the above policies may result in termination from the program.
Such measures will be considered in extreme situations after all reasonable alternatives
have been exhausted.

Emergency Procedures
Lockdown and Shelter in Place
If your child’s school is involved in an imminent Lockdown or Shelter in Place procedure, your
child will not be released until officials have determined the situation to be safe and until staff
has been given the authorization to do so. For your safety and that of your child, please do
not attempt to pick up your child during these emergency protocols.
The SJUSD staff will initiate a lockdown in response to a 9-1-1 situation involving an active
shooter or weapons call. They will cover or shade all windows and barricade doors. These
doors will remain barricaded and no one other than authorized officials will be allowed to
communicate with the school staff during a lockdown. Shelter in Place procedure will be
implemented in response to situations nearby that may be a threat to the safety of staff and
students.
Staff will make every effort to ensure that your child remains safe. During these emergencies,
please contact the Early Education Office for information and/or instructions.
Child Injury
As the SJUSD Early Education team, we will do everything we can to provide a safe learning
environment for your child, but if he/she does sustain a minor injury during a school session,
one of our Pediatric CPR and First Aid Certified staff will be able to provide basic first aid. You
will be provided a simple notification at the regular pick up time on the day that your child
sustained a minor injury. If you want a copy of the actual injury report, you may submit a
request to the preschool staff. The report will be available for pick up the next school day.
Should your child sustain an injury to the head (anywhere above the neck), the staff will
immediately contact you or—if you are unreachable—any of your listed emergency contacts.
The contacted person will be required to go to the school to check on your child. Please be
prepared to provide a photo ID before the child can be released to your emergency contacts.
You or your emergency contact have the option to decline this mandatory process and your
child will be kept at school until the regular pick up time. You will receive a Head Injury Letter
when you do pick up your child. This letter will provide further instructions and information on
how to monitor and care for your child after receiving a head injury. You will be required to
initial the Head Injury Letter as proof of receipt. The original will be kept in the child’s folder.
You will still be required to sign your child out, whether you are picking him/her up early or at
the regular time.

Health and Medication
Health Check
Each day your child comes to school, the teacher will conduct a quick daily health check.
When you sign your child in, please let the teacher know of any health concern that might
affect your child’s participation during the day. So that staff can maintain healthy conditions in
the classroom, notify the teacher if your child has been exposed to any contagious disease.
Also, for the safety and protection of everyone, students should not attend school with one or
more of the following symptoms:

A fever of 100ºF or higher the day or night before school. (Students who have been
ill with a fever need to be fever-free, without the use of medication, for 24 hours
before returning to school.)
Cold symptoms, especially during the early stages
A cough accompanied by yellow or green mucus or discharge from the nose
A severely sore throat
Untreated ears with draining fluid or earache
Eyes that are red, swollen, crusting or draining
A skin rash, unless diagnosed as non-infectious by the doctor
Open sores not covered with a bandage
Headache, earache, or other severe pain
Diarrhea
Nausea or vomiting
Untreated head lice and/or nits (eggs) in the hair
If a student attends school with any of the above symptoms, the parent/guardian will be called
to pick him/her up. Under certain circumstances, you, the parent, will be asked to consult a
physician and to provide a written note from your doctor indicating that the student may return
to school. Students at school, who have one or more of the listed symptoms, will be sent
home.
If a student has been out of class due to a physical injury, i.e., a broken/fracture bone, an
original doctor’s note releasing the child to return to school is required. The note must specify
full release or list any limitations such as no climbing, or running, etc.
Head Lice Prevention—No Nit Policy
When staff identifies a child as having head lice/nits (eggs), the child’s parent will be contacted
and the child will be sent home from school. The School will provide the parent with information

regarding head lice, treatment, readmission requirements and resources for treatment. The
child may return to school the following day if all nits have been removed from the hair. Staff
will examine the child’s head before he/she re-enters the classroom. If a child returns to school
with nits remaining in his/her hair, he/she will be excluded from school. The child will be given
an unexcused absence if he/she does not return to school for inspection after 2 days from the
date of exclusion. Staff will send exposure notices home with classmates every time a new
case is discovered. We highly recommend parents check their child’s head for lice on a weekly
basis. Please notify your teacher if your child contracts head lice. We need your help in
keeping head lice out of our schools!
Medication Policy and/or plan
Notify the teacher of any allergies your child may have. Also, share information about the
allergy and any related doctor’s recommendations for substitutions or procedures necessary
for your child’s health. A School Medication Permission Form (see sample of form on the
following page) must be completed by the parent and the child’s doctor before staff can give
any medication.
** Only prescription medications in their original container can be administered at the preschool
site. Such medication must be clearly labeled with the child’s name, the exact dosage and
times, and other instructions.

Orientation
On the first day of class, the school district Early Education program holds an orientation
meeting so parents and children can meet the staff and other children, and become familiar
with the classroom. This will be a shortened class day with the regular daily schedule
beginning the following day.
The Orientation Meeting will include a review of:
A. The Parent Handbook
B. Program activities including:
• Curriculum
• Program objectives/activities
• Parent-teacher conferences (fall and spring)
• Parent Advisory Committee and other parent involvement
opportunities
C. Program policies and procedures:
• Safety/health policies
• Signing in and out procedures
• Behavior management (discipline) policy
• Absences, “Best Interest of the Child” (BIC) Days
• Parents’ rights and appeal procedures
• Personal (children’s) rights
• Licensing authority
D. Classroom expectations and routines

Parent Involvement
Parent Involvement is critical to the success of our program. We believe that when staff and
parents work together to support each child’s development, they provide a stable, consistent
and enriched early learning environment.
Parent Volunteers in the classroom are welcome, expected and strongly encouraged in order
to enrich the children’s classroom experience. For students’ safety, however, the SJUSD
preschool program requires the fingerprinting of long-term and seasonal volunteers. In
addition, all volunteers must submit tuberculosis test results to Human Resources prior to
working in the classroom. The program will pay for the cost of volunteer fingerprinting. Please
obtain the necessary paperwork for fingerprinting at the Early Education Office, located at 855
Lenzen Avenue.
The district considers long-term or seasonal volunteers those that voluntarily provide a service
to the school district on a regular and/or on-going basis throughout the school year, without
compensation. Volunteers do not necessarily need to perform this service on consecutive
days; however, the district’s goal is to use long-term or seasonal volunteers over an extended
period of time. Call the SJUSD Human Resources department if you have questions regarding
the volunteering process.
Parent Meetings are held at each preschool site throughout the year. The preschool program
will organize these meetings for the purpose of parent education, the delivery of general
information and social get-togethers.
Parent Advisory Committee (PAC) meetings will also be held three times during the school
year at the District Office. Your child’s classroom teacher will send home a notice stating
location and time for each PAC meeting.

The PAC functions to provide strong, ongoing communication between the program
administrator, the District Office, and individual preschool sites.
Early Education department staff selects one parent representative from each site to
attend PAC meetings.
At meetings, Parent Representatives bring suggestions/complaints to the attention of
the Early Education Administrator to be discussed, researched and/or resolved.
Translation is provided at the PAC meetings.

Remember to…
• Call your preschool classroom if your child will be absent.
• Sign your child in and out with your full name/signature. Sign in must be done by an
adult over 18 years old. If your child has been absent, include a short description (one
or two words) of why he/she was not at school.
• Sick children cannot be admitted to the classroom.
• Emergency cards must be kept up-to-date with current names and telephone numbers
so someone can be reached when necessary.
• Notify your teacher and the Early Education Department (408-535-6083) of changes in
your phone number, address or family situation.
• Dress your child in clothes that allow him/her to easily play and participate in preschool
activities. Tennis shoes make good school shoes (no sandals or flip-flops please.)
• Put your child’s name on everything that comes to school (especially jackets!)

Complaint Procedures
District (mandated):
If you feel that you (or your child) are not getting equal treatment with regards to the facilities,
textbooks and/or staff members, the Williams Settlement allows you to file a complaint with the
district to ensure that its staff meets the district’s standards at the site.
Please refer to the Complaint Procedures as indicated on the following pages of this
Handbook. Staff members also post the procedures and complaint forms on the bulletin board
in your child’s classroom.
Program (not mandated):
Our Early Education Program is coordinated and monitored by a Program Manager, Preschool
Teacher and an Associate. The SJUSD staffs the preschool program according to California
Department of Education for State Preschool and FIRST 5, Child Signature Program
guidelines.
The district staffs the program to maintain a 1:8 staff to child ratio in the preschool classroom.
All staff members are accountable to the Program Manager. If you have concerns and/or
questions, please first discuss them with the Preschool Teacher. If you feel your concerns

Complaint Procedures
Notice to Parents, Guardians, Pupils, and Teachers Regarding Complaint Rights
Parents, Guardians, Pupils, and Teachers:
Pursuant to California Education Code Section 35186, you are hereby notified that:
1. There should be sufficient textbooks and instructional materials. That means each pupil,
including English learners, must have a textbook or instructional materials, or both, to use
in class and to take home.
2. School facilities must be clean, safe, and maintained in good repair.
3. There should be no teacher vacancies or misassignments. There should be a teacher
assigned to each class and not a series of substitutes or other temporary teachers. The
teacher should have the proper credential to teach the class, including the certification
required to teach English learners, if they are present.
Teacher vacancy means a position to which a single designated certificated employee has
not been assigned at the beginning of the year, for an entire year or, if the position is for a
one-semester course, a position to which a single designated certificated employee has not
been assigned at the beginning of a semester for an entire semester.
Misassignment means the placement of a certificated employee in a teaching or services
position for which the employee does not hold a legally recognized certificate or credential,
or the placement of a certificated employee in a teaching or services position that the
employee is not otherwise authorized by statute to hold.

COMPLAINT PROCEDURES
Notice to Parents, Guardians, Pupils, and Teachers Complaint Rights
Phụ Huynh, Những Người Giám Hộ, Học Sinh và Các Giáo Viên:
Chiếu theo Bộ Luật Giáo Dục tiểu bang California Mục 35186, theo đây chúng tôi xin thông
báo rằng:
1. Sách giáo khoa và tài liệu hướng dẫn cần phải có đủ. Điều đó có nghĩa là mỗi học sinh, kể cả các học
sinh học Anh ngữ, phải có sách giáo khoa hoặc tài liệu hướng dẫn, hoặc cả hai, để sử dụng trong lớp
học và mang về nhà.
2. Cơ sở vật chất học đường phải sạch sẽ, an toàn và được bảo trì trong tình trạng sửa chữa còn được
tốt.
3. Không nên có tình trạng thiếu giáo viên hoặc phân công giáo viên không thích hợp. Cần phân công
một giáo viên phụ trách mỗi lớp chứ không phải là một loạt các giáo viên thay thế hoặc các giáo viên
tạm thời khác. Giáo viên đó cần có bằng cấp chứng nhận thích hợp để giảng dạy lớp học, trong đó
bao gồm cả giấy chứng nhận cần thiết để dạy các học sinh học Anh ngữ nếu có.
Thiếu giáo viên có nghĩa là một vị trí vẫn chưa được phân công một giáo viên có chứng nhận vào đầu
năm học cho cả năm hoặc, nếu vị trí đó là cho một khóa học trong một học kỳ, vị trí vẫn chưa được
phân công cho một giáo viên có chứng nhận phụ trách khi bắt đầu học kỳ đó trong toàn bộ học kỳ.
Phân công giáo viên không thích hợp có nghĩa là việc phân công một nhân viên có chứng nhận vào
một vị trí giảng dạy hoặc cung cấp dịch vụ mà nhân viên đó không có chứng chỉ hoặc năng lực trình
độ được công nhận hợp pháp hoặc việc phân công một nhân viên có chứng nhận vào một vị trí giảng
dạy hoặc cung cấp dịch vụ mà nhân viên đó không được luật pháp cho phép để đảm nhận.

Mẫu Thủ Tục Khiếu Nại Đồng Loạt
Dành cho Các Trường Hợp Khiếu Nại theo Mục 35186 của Bộ Luật Giáo Dục tiểu bang California
Mục 35186 của Bộ Luật Giáo Dục (EC) thiết lập một thủ tục khiếu nại liên quan tới tình trạng thiếu tài trợ giảng dạy, các điều
kiện cơ sở vật chất không được duy trì một cách sạch sẽ và an toàn hoặc ở trong tình trạng cần sửa chữa, và việc thiếu giáo
viên hoặc phân công nhiệm vụ cho giáo viên không đúng. Hồ sơ khiếu nại và trả lời là các tài liệu công khai theo qui định của
luật pháp. Các khiếu nại có thể được gửi nặc danh. Tuy nhiên, nếu quý vị mong muốn nhận được trả lời về khiếu nại, quý vị
phải cung cấp các thông tin liên lạc sau đây.

Vấn Đề Khiếu Nại (xin đánh dấu vào tất cả các ô áp dụng):
1. Sách Giáo Khoa và Tài Liệu Giảng Dạy
❏ Học sinh, kể cả học sinh Anh ngữ, không có các sách giáo khoa hoặc tài liệu giảng dạy phù hợp với tiêu chuẩn hoặc
sách giáo khoa được tiểu bang hoặc khu học chánh phê chuẩn hoặc các tài liệu hướng dẫn bắt buộc khác để sử dụng
trong lớp học.
❏ Học sinh sẽ không được sử dụng sách giáo khoa hoặc tài liệu giảng dạy ở nhà hoặc sau giờ học. Mỗi học sinh không
bắt buộc phải có 2 bộ sách giáo khoa hoặc tài liệu giảng dạy.
❏ Các sách giáo khoa hoặc tài liệu giảng dạy ở tình trạng cũ hoặc không thể sử dụng được, thiếu trang hoặc không thể
đọc được do hư hỏng.
❏ Học sinh chỉ nhận được các bản sao chép từ một phần của sách giáo khoa hoặc các tài liệu giảng dạy để giải quyết
tình trạng thiếu sách giáo khoa hoặc tài liệu giảng dạy.
2. Điều Kiện Cơ Sở Vật Chất
❏ Một tình trạng gây đe dọa khẩn cấp hoặc cấp cứu đối với sức khỏe hoặc sự an toàn của các học sinh hoặc nhân viên,
trong đó bao gồm: rò rỉ khí đốt, hệ thống lò sưởi không hoạt động, hệ thống thông gió, bình phun nước dập lửa hoặc
các hệ thống điều hòa không khí không hoạt động, cúp điện, tình trạng đường cống dừng lại nghiêm trọng, động vật
gây hại hoặc chim, sâu phá hoại ở mức nghiêm trọng, các cửa sổ hoặc cửa ra vào hoặc cổng ở bên ngoài bị hỏng,
không có khóa và gây nguy cơ về an ninh, xử lý các vật liệu độc hại trước đó không phát hiện được và gây ra đe dọa
tức thời đối với các học sinh hoặc nhân viên, hư hại về sự xây dựng tạo ra tình trạng nguy hiểm hoặc không thể cư ngụ
được, và bất kỳ tình trạng khẩn cấp nào khác mà khu học chánh thấy là thích hợp.
❏ Nhà vệ sinh của trường không được bảo trì hoặc cọ rửa thường xuyên, không hoạt động đầy đủ hoặc không cất trữ đủ
giấy vệ sinh, xà bông và khăn giấy hoặc máy sấy khô tay.
❏ Trường không mở cửa tất cả các nhà vệ sinh trong giờ học khi học sinh không có mặt trong lớp học, và không có đủ số
nhà vệ sinh phục vụ trong giờ học khi các học sinh ở trong lớp học.
3. Tình Trạng Thiếu Giáo Viên hoặc Phân Công Giáo Viên Không Đúng
❏ Thiếu giáo viên – Một học kỳ bắt đầu và tồn tại tình trạng thiếu giáo viên. (Thiếu giáo viên là vị trí trong đó vẫn chưa
được chỉ định cho một nhân viên có chứng nhận để phụ trách vào đầu năm học hoặc cả năm hoặc, nếu vị trí đó là
dành cho khóa học kéo dài một học kỳ, thì vị trí mà một nhân viên có chứng nhận vẫn chưa được phân công vào đầu
học kỳ trong toàn bộ học kỳ đó.)
❏ Sử dụng giáo viên không đúng – Giáo viên thiếu trình độ học vấn hoặc chương trình huấn luyện để giảng dạy các học
sinh Anh ngữ được phân công dạy một lớp có hơn 20 phần trăm học sinh học Anh ngữ trong một lớp.
❏ Sử dụng giáo viên không đúng – Giáo viên được chỉ định dạy một lớp trong đó giáo viên thiếu trình độ năng lực về môn
học đó.
