2013-2014 Report to the Community 1 Innovative & O Committed ver the past year we have continued to explore and implement innovate models of care to improve the health of our patients and community. Despite many external changes and financial challenges, our organization and staff pursued opportunities to enhance quality, maximize efficiencies and deliver a broad range of services to meet the needs of our patients. This past year, we have accomplished many things that improve access, health, and knowledge for those we serve — including: • A renovation of our Medical Home model, maximizing the collaboration of physician-led, team-based care and further utilizing Nurse Practitioners and Collaborative Care Nurses in care plan delivery and chronic condition management. 2 • Blood pressure initiative, which offers an integrated delivery of care in our medical practices and at community-based locations. This work was recognized by the U.S. DHHS’ Million Hearts® initiative for success in achieving exceptionally high rates of adequate hypertension control for our patient population. • Successful physician and associate provider recruitment for primary care and various specialties. • Expansion of our Family Medicine Walk-In service at the Center at Keene location (coming in 2015). • Launch of a new online patient portal, myDH Keene, providing enhanced communication between our patients and providers. • Launch of Emmi, a patient engagement tool, providing patient education in preparation for surgical procedures, vaccine information, preventative screenings and more. Additionally, at the time we publish this report, we are on target to commence our closer affiliation with Dartmouth-Hitchcock (DH) at the end of this year. This affiliation expands the capabilities of our 16-year Joint Operating Agreement with DH to ensure a sustainable and robust health system for our community. Both of our organizations share a common commitment to provide value-based patient care and advance population health for the communities we serve. The main elements of the affiliation include increased quality of care and efficiencies, improved access to care and population health, reduced healthcare costs and continued local support via charitable gifts and board representation. We are excited about the opportunities this affiliation will bring our patients, organization and community. To close, it’s important for us to acknowledge and commend the outstanding talents and efforts of our physicians, associate providers, nurses, staff and volunteers who compassionately care for our patients and their families every day. Discounted ‘passport meals’ and free nutrition education for our community’s seniors. Providing health and safety information for emergency preparedness. Assisting individuals and families in accessing federal and state insurance programs. Organized walking club to maintain an active lifestyle. Free tobacco cessation counseling and education. Medication assistance for those who have trouble affording them. Classes about preparing nutritious meals, lowering blood pressure naturally, diabetes prevention, supermarket tours and other topics. Financial assistance, from free care to reduced charges to long-term payment plans, depending on each patient’s individual medical, family and financial circumstances. We improve the total health of our patients and the community, expanding access, enhancing our environments and shaping health knowledge. In 2014, CMC/DHK provided more than $2 million in community health services, health profession education, subsidized health services and community building activities. Additionally we provided an additional $21.6 million in unreimbursed Medicaid and Medicare, and $2.2 million in unreimbursed charity care. Without this help, I wouldn’t be able to afford the expensive medications I need to manage my conditions. —William O’Brien ” Hayley Compos, Medication Assistance Coordinator and William O’Brien, patient Our Community Medication Assistance Initiated in 1997, our Medication Assistance Program was the first hospital-based program in NH to provide help in accessing free medications for patients who are faced with the difficulty of being unable to afford long-term or expensive prescriptions. Program Coordinator Haley Compos supports patients with chronic conditions by providing a connection with pharmaceutical companies that offer free medications to qualifying patients. This program provides an average of over $2.5 million per year in free prescription drugs for financially qualified patients to ensure they receive the medications they need to maintain good health. Tobacco Cessation Program The goals of the Cheshire Coalition for Tobacco-Free Communities are to reduce youth initiation of tobacco products, to help anyone who wants to quit do so, and to eliminate exposure to secondhand smoke. The Coalition, led by Program Manager Kate McNally, sponsors an extensive array of treatment programs for those who wish to quit using tobacco, including one-on-one counseling, support group, phone support and assistance with nicotine replacement therapies (patches, lozenges and gum). A new learning center was recently created at the Kingsbury House at CMC/DHK, with educational materials and multimedia resources. In addition to these services, we also deliver specific programs for youth, women — specifically those who are pregnant or new moms — and worksites interested in creating a smoke-free environment for customers and employees. Being smoke-free is the best thing I ever did for myself. ” —Shannon Driscoll – smoke-free for three years “ The one thing I would recommend is using the tools – gum or lozenges – where your body is still getting the nicotine and you’re able to get rid of ‘the habit’ of smoking. He also realized his lack of energy and that his vision was deteriorating. During a visit with his primary care physician at CMC/DHK, he found out his blood sugar was high and was diagnosed with Type 2 diabetes. He made a conscious effort to eat more healthy, avoid fats, carbohydrates and sugary foods, and drink more water. Over the next several years, his weight fluctuated, as did his energy level, and he needed to take four different medications to control his diabetes. About three years ago, after another visit with his PCP, he began taking insulin once a day. Then he found out his liver was suffering due to diabetes complications. “In June 2013, I was told I needed to be put on a meal-time insulin,” Steve said. If he didn’t start the additional medication, he was told, he would be at risk for cirrhosis. “I had a serious decision and choice.” “I said, ‘No way.’” Steve shares he decided to forgo insulin and instead focus on the doctor’s advice: monitoring every single thing he put into his mouth. He got a pedometer to help track his physical activity and began an exercise routine, including walking every day. He joined an online support group to help him in his efforts. This past year, he lost 89 pounds and now takes only one medication to support his body’s inability to process sugar. Patti Schuman, Registered Dietitian, Certified Diabetes Educator, and Coordinator of the Diabetes Self-Management Education Program at CMC/DHK, helped Steve get on track with his diet. Patti taught him about the proper portion sizes. He was weighing his food at first but he is now an expert at determining how much of each food he needs to eat. As part of his new program, he checks his blood sugar twice a day and eats more lean meat and vegetables. “A big struggle for me was managing my blood sugar, especially early in the morning,” he said. “Patti talked about all kinds of things to try. She told me that, based on my metabolism, there are certain foods I need to eat in certain quantities, and she told me how it affects my blood sugar. It was an amazing experience — learning about food and nutrition and my body — and now I feel great. Once you have the support resources established, and can focus on yourself and your goals, losing weight and gaining better health is actually pretty easy.” CMC/DHK’s diabetes self-management education program, recognized by the American Diabetes Association, offers individuals a variety of ways to learn about their diabetes and how to delay or prevent complications, such as eye, kidney, heart, and nerve damage. Included are group classes on such topics as nutrition, medication, monitoring blood sugar and incorporating physical activity; individual medical nutrition therapy with a registered dietitian; one-on-one instruction from a Diabetes Care Nurse; an informal monthly discussion group and diabetes prevention class. The Diabetes Care Network offers patients a multi-faceted approach that includes medical care, coping skills and assistance with lifestyle change. The team is comprised of specially-trained and certified clinicians including an endocrinologist, primary care providers, dietitians, diabetes educators, health psychologists, pharmacists, physical therapists, podiatrists, ophthalmologists, neurologists, and cardiologists. “We emphasize selfcare,” said Schuman. “We want to empower people to make lifestyle changes to become healthier and also identify a support system to help them stay on course and remain healthy.” Collaborative Care Nurses CMC/DHK has recently developed a new Collaborative Care Nurse position to support care of patients with chronic diseases, such as diabetes, hypertension, and COPD. These nurses work closely with the Primary Care Provider to support patients by making sure individuals have received all of the appropriate labs or tests for their conditions and providing important education and coaching for lifestyle changes. He credits J.D. Morse, Emergency Department technician, for keeping him laughing. “He was joking with me, and making me feel at ease,” he said. He was treated for his head and facial injuries by Dr. Steven Boutrus, first with medication to numb his skin. He received sutures in his lip and surgical staples in the top of his skull. “Dr. Boutrus was great – he kept me calm, was easy to understand, and gave me options,” Edson shares. His wife, Ginger, said her husband had a fainting spell during his visit and that the team was quick to respond. The number of patient visits to the ED has grown to more than 25,000 annually, and the number of patient visits on each day can fluctuate greatly. To continue to meet the emergency needs of our community 24/7, we are in the process of redesigning and renovating our ED to provide improved privacy and comfort for patients, support advanced technology — including telemedicine that brings specialists from nationally-recognized medical centers to our patients’ bedsides, and improved space to support the unique needs of patients in behavioral health crisis. The redesign and renovation work is anticipated to be completed by late spring 2015. ne day this past summer, Edson DeAngelis was at home pushing his all-terrain vehicle up a ramp to load it into the back of his pick-up truck. “It let loose and landed on top of me,” he said. His legs were trapped under the 800-pound vehicle, and he suffered injuries to his head and face. 8 An interdisciplinary team of ED and Facilities Operations colleagues came together to design the improved ED plan. Staff and provider input was critical and work included a mock patient room that was adjusted and reworked to ensure the most efficient design for the care team, patients and their families. Maryanne Keating with Brianne Chavez, Shared Services Supervisor, Markem-Imaje Engaged & Empowered E mployees of Markem-Imaje in Keene are learning when the healthy choice is the easy choice, then wellness is more easily attainable. The company is a global manufacturer and distributor of specialized traceability, variable data and product identification equipment for customers in the packaging industry. The Keene location is one of six around the world, and with roughly 400 employees, is the largest worksite. It was awarded the Healthy Eating Active Living (HEAL) Organizational Champion Award in 2014, recognizing its accomplishments in the areas of promoting healthy living for employees, their organization and the community. “Champions are the advocates for improving the environments where we all live, learn, work and play,” said Linda Rubin, director for Healthy Monadnock 2020 (HM2020), a community engagement initiative, conceived and coordinated by CMC/DHK, to implement action strategies, prevent leading causes of disease and sustain a positive culture of health throughout the Monadnock region. Maryanne Keating, Community Health and HEAL Program Coordinator at CMC/DHK, began working last year with Brianne Chavez, MarkemImaje’s Shared Services Supervisor, to support ways the organization could engage its employees in health improvement activities in the workplace. Since then, the company’s Wellness Committee has implemented several wellness programs and activities. Among them are an on-site walking path and an eight-week nutrition course that focused on such topics as reading food labels and preparing healthy meals. On-site showers available 24-7 were added to support worksite exercise, and a lunchtime wellnessrelated program led by a physician who shared information about how to burn calories, knowing your numbers (cholesterol, blood pressure and others) and the importance of a good night’s rest. Markem-Imaje is also a tobacco-free campus, offers personal health assessments for employees and participates in a nationwide wellness competition that tracks healthy behaviors and offers rewards for completing them. Within the coming year, Keating is working with Chavez to implement Turn a New Leaf, a Monadnock HEAL program that partners with eating establishments, including cafeterias, to make it easier to choose healthier options when dining out. Turn a New Leaf is based on nutrition science supported efforts to promote whole grain, fruit, vegetable, and bean consumption, while limiting sodium, saturated fat, overall calories and avoid trans-fat intake. Menu options are analyzed and those that meet the criteria are labeled with a heart icon as healthy, healthier and healthiest. Markem-Imaje’s Wellness Committee is taking it a step further and will be providing guidelines and ideas for a healthy menu for food served at company meetings. When many organizations make a concerted effort focused on the people, policies, practices, environments or segments of the community they can influence, we can all help our region achieve the vision of becoming the healthiest community in the nation. Recognizing the importance of collaboration, HM2020 brought together representatives from 11 local businesses and organizations to participate in a five-month Learning Collaborative to share best practices and help train business owners, managers and wellness teams to lead the way toward greater health in our region’s workplaces. Together, the group developed effective approaches and committed to making new environmental and policy changes within the workplace to make the healthy choice the easy choice for their employees. She wasn’t able to bend down and tie her shoes, it hurt when she stood and friends started noticing she wasn’t walking normally. An elementary school teacher who plays tennis, swims, and goes kayaking in her spare time, her new limitations halted her active lifestyle. After discussing her concerns with her doctor, she was told she should consider hip replacement surgery. She wanted to put it off, but learned if she did, it was likely she would need the surgery within five years. This past winter, she returned to meet with Dr. Anthony Presutti, Orthopaedic Surgeon. She learned about the benefits, risks and options of a new anterior hip procedure and decided to schedule her surgery for the summer, when school is out of session. In March, Dr. Presutti and CMC/ DHK began offering the anterior hip procedure, which involves making a small incision through the front of the leg, rather than the side or back (the entry point for the more conventional posterior hip replacement surgery). This approach allows the surgeon to reach the hip joint without splitting or detaching muscles and more accurately place Working closely with the Orthopaedics team, the Physical Therapy and Occupational Therapy teams provide care for those recovering from surgery such as a hip replacement. Outpatient Rehab continues the healing process and provides many services to improve function and mobility for our patients. Additionally, the Farnum Rehabilitation Center, pictured at right, is a unique setting focused on intensive rehabilitation using a dynamic team approach. Physiatrists, Nurses, Nurses Aids, Social Workers, Physical, Occupational, and Recreation Therapists as well as Speech Language Pathologists work together to return patients to their highest level of function and independence after a surgery or major health event. components, which can decrease dislocations and allow for better assessment of leg lengths. Results suggest a shorter hospital stay for patients and less pain in the immediate postoperative period, and a swifter recovery. Teresa had her surgery on a Tuesday and was discharged from the hospital Thursday. She walked using a cane the first week after the surgery, and after five weeks, she said she felt good. She continued her appointments with the Farnum outpatient physical therapists at CMC/DHK. Teresa was happy the recovery time was short and without movement restrictions, which is typical of this type of surgery. “I didn’t have to take any precautions,” she said. “Dr. Presutti told me to let my pain be my guide. He kept saying, ‘Motion is lotion. The more you move the faster you’ll heal.’” She now does a half-hour of walking each day and says, “I’m looking forward to playing tennis again next spring.” “ I’m happy that the recovery time was short and that everything went smoothly. It makes you feel very relaxed knowing you’re in good hands. ” —Thomas Holbrook Thomas Holbrook, patient, with Dr. Robert Guardiano, Dermatologist S ed & Specialized T homas Holbrook remembers summers on Spofford Lake working at summer camps and as a golf caddy. He said he remembers a lot of sunburns and was first diagnosed with skin cancer 20 years ago. The specialized treatments he needed required the Keene resident to travel an hour to another facility for about a half-dozen years. About five years ago, he began to visit Dr. Gregory Seymour, Dermatologist at CMC/DHK’s then new Dermatology office, housed in a modern suite of offices in a new building on Railroad Street. The department’s scope of services was expanded to include specialized treatments for skin care, including the Mohs Micrographic Surgery for skin cancer that Thomas needed. Dr. Robert Guardiano performs Mohs surgery, which allows for selective removal of skin cancer with the preservation of as much of the surrounding normal tissue as is possible. The surgery involves removing a skin cancer one layer at a time and examining these layers under a microscope immediately after they are removed. This allows a minimal amount of tissue to be affected while making sure all the cancer cells are removed. This offers the highest chance for complete removal of the cancer while sparing normal tissue, and the cure rate is 99 percent. Board Certified Dermatologists and the Dermatology team care for patients with a full range of common skin conditions, including acne, eczema, psoriasis, rashes, and skin cancer. Cosmetic procedures and laser surgery are also performed. L to R Sitting: Julie Croteau, RN; Roberta Allen, LPN; Hillary Stickney, LPN Thomas has had about eight Mohs surgeries and is grateful for the talent and caring of the Dermatology team. “They check on you during treatment,” he said. “The day after treatment, Dr. Guardiano calls me first thing to check in. I tell all my friends about the great team here in Keene – I wouldn’t think of going anywhere else.” Excessive exposure to sunlight is the single most important factor associated with the development of skin cancers, especially in those with fair complexions. Thomas now wears sunscreen daily, between 30 and 50 SPF, avoids the sun when possible, and wears one of the 20 hats in his collection whenever he’s outside to lessen direct sun exposure. He has a check-up every six months with Dr. Seymour. “They give my whole body a onceover,” he said. If he ever has concerns about anything, he added, he reaches out and has his answers quickly. A total of 503 participants, including over 80 Healthy Monadnock 2020 Champions, raised over $11,000 for Sports Medicine services to the community, including up-to-date rehabilitation equipment, local sports medical coverage, team physician coverage, and local Sports Medicine education. 26 2nd Annual Golf Tournament, held on September 11, 2013, raised over $43,000 to benefit the Healthy Monadnock 2020 Champions Program. These funds help promote social connection and support to improve the environments where we all live, learn, work and play and to make the healthy choice the easy choice for everyone. As a Healthy Monadnock 2020 Champion, anyone can make a commitment to live, share, model and inspire healthy habits. GOLF TOURNAMENT 6th Annual Bald is Beautiful Cancer Fundraiser, held on May 3, 2014, raised over $40,000. One hundred ten participants, including many employees, shaved their heads and raised money for our Patient Relief and Cancer Care Funds. Funds raised through Bald is Beautiful allow the Kingsbury Pavilion to extend its reach of care beyond the hospital walls, where the real healing begins, by providing financial assistance to local patients receiving care at the Kingsbury Pavilion who have difficulty covering daily living costs like gas, heating fuel, and groceries. 1st Annual Caring, Candlelight and Community: Celebrating our Healthcare Legacy tribute event was held on November 16, 2013. Established to recognize those who have made a difference in the care of our community, and to commemorate milestones in the history of our nonprofit hospital and clinic, the first honorees were Audrey Hadcock, leader and advocate for community health, and Arthur Cohen, MD, retired urologist. The event also celebrated the 40th Anniversary of the hospital’s move Caring, Candlelight and Community: from Main Street Celebrating our Health Care Legacy to the Kingsbury Estate on Court Street. Over $16,000 was raised to benefit Cheshire Health Foundation. S W ed & Dedicated e express appreciation and sincere thanks to our volunteers who provide selfless service, compassion and caring to benefit our patients and our community. Our 170 volunteers come from an impressive array of backgrounds and generations, ranging in age from 16 to 94, and provide support in many departments throughout our organization. Showcased this year (pictured at right) are a group of veteran volunteers that boast a combined total of 114 years of service to our hospital and clinic. They bring a wealth of wisdom, humor and cheer that is invaluable to the care we give to those who walk through our doors. Their smiles and guidance bring reassurance to patients, their willing attitudes provide administrative support to bustling clinical areas, and their helping hands and generosity contribute to the needs of various departments. Please join us in thanking our volunteers who support our patients, staff and organization in so many meaningful ways. T he importance of good nutrition has been well documented, but it is even more critical to our patients who are sick and in need of nourishment for healing. With a commitment to safety and good nutrition, our Nutrition Services team annually serves close to 486,000 meals to our patients, visitors and colleagues within the hospital and in our Center Court café. This devoted and efficient team is quick to respond to its customer’s needs with grace and professionalism. We are grateful for their important contributions to our patients, visitors, staff and organization. 27 Message “ It’s always wise to look ahead, but difficult to look further than you can see. F 28 ” — Winston Churchill or the last several years, our Board and management have indeed looked ahead through a strategic planning process that is on target to culminate in an affiliation agreement with DartmouthHitchcock at the end of this year. This closer affiliation takes us to the next level of integration after a successful 16-year Joint Operating Agreement, and is based on shared values of local control, sustainability, access to healthcare service, reduced healthcare costs, increased efficiencies, quality care, and improved population health. The U.S. healthcare environment has experienced dramatic change, largely driven by cost pressures resulting from healthcare expenditures of over 17% of GDP. These pressures have created an environment difficult for a stand-alone hospital with a large group practice (in our case employed by Dartmouth-Hitchcock) to navigate the turbulent waters of change. Our closer affiliation with Dartmouth-Hitchcock will help the Medical Center and Clinic transition from a transactional environment (one in which healthcare providers are incentivized to “do more” and viewed as unsustainable) to a strategic environment (where healthcare providers are reimbursed for value of care based on health outcomes and quality metrics). The affiliation with DartmouthHitchcock will more closely align clinical interests of the Medical Center and physicians to better manage care and it will create more efficient use of our resources to fund the systems and processes required to from the Chair provide the best care in the lowest cost setting consistent with quality and patient best practices. By further aligning our governance, clinical, and administrative activities, we will benefit from additional collaboration with a larger integrated delivery system — yet allow for the Cheshire Medical Center and the Cheshire Health Foundation to retain their status as separate legal entities with their own boards and maintain a local voice in future change. Other challenges this year included the continued and burdensome impact of the Medicaid Enhancement Tax, changing payment models, and a very competitive physician recruitment environment. Despite these challenges, CMC/DHK continues to be acknowledged with various awards at the state and national levels for clinical and population health initiatives (shared in this report on page 3) that focus on keeping individuals and our community healthy and lowering healthcare costs. None is more rewarding than the recently announced $1.1 million grant from the Centers for Disease Control to further the goals of Healthy Monadnock 2020. On behalf of the Board, I would like to thank all staff and all healthcare providers for their dedication and hard work in the midst of these challenging times. Their work is essential not only for patients and their families, but for the entire community. Together, we can sustain a robust medical community that is essential to the health of our city and region.
