CMS (Centers for Medicare and Medicaid Services) has implemented the new redesigned Provider Statistical and Reimbursement
(PS&R) system. The new PS&R system is web-based and allows all users to access CMS data.
To obtain the (PS&R) online from CMS, you need to register a Security Official for your facility. Before starting the registration we
recommend to have available the IRS Notice CP 575, because you must send it to CMS to validate your information, therefore it is
important that your registration data matches the information on the CP 575.

If you can’t find the IRS notice CP 575, call the IRS Business Tax Line at (800) 829-4933 to request a Confirmation Letter for an
assigned EIN number (147C letter) which is also accepted by CMS, because the CP 575 is only issued one time.
Follow steps 1 to 18 for registration of the Security Official for your agency.
Go to: https://applications.cms.hhs.gov/home.html .
On the CMS Applications Portal Introduction screen, select the [Account Management] hyperlink.
On the Account Management screen, select the [New User Registration] hyperlink.
On New User Registration menu select the [PS&R/STAR] hyperlink.
On the following window check
I Accept the above Terms and Conditions, and select [I Accept].
On the next window, select [I work for a Medicare Provider, and I want to register for PS&R], and click on [Next].
On the New User Registration window introduce your personal information and click on [Next].
Check your E-mail Inbox for an e-mail from IACS with a Verification Code, if is not in your Inbox check in Spam.
(Do not copy and Paste) the verification code. Introduce the Verification Code in the box and click on [Next].
On the next window introduce your Professional Contact Information. See the items below for a correct selection.
Office Telephone: Introduce your Facility Phone Number.
Company Name: Introduce the Facility Name as appear on the IRS Notice CP 575
Introduce the Address, City, and State.
11. Under Access Request click on the small triangle inside the box next to Role and select from the dropdown menu:

12. Below select Create an Organization if is not selected.
13. A new window will open. Introduce your Organization Information. See the Items below for a correct selection.
TIN/SNN: Introduce your Tax ID Number as appear on the IRS Notice CP 575 in this way: 20-1234567
Legal Business Name: Introduce the Facility Name as appear on the IRS Notice CP 575
CMS Certification Number: Introduce your Provider Number of 6 digits in this way: 102333
Add a Check Mark to the box.

Please select if the Organization's Address and Telephone Number Information is same as the User’s Address and Telephone
Number
On Justification for Action write: To obtain PS&R.
When finished, click on [Next].
On the next window you will require to answer at least two security questions.
After answer the security questions, click on [Next].
A new window will open, review the information and Print the Page for your Records.
When finished, click on [Submit].
You should get a confirmation number (REQ #). Print the page for your Records. Click on [Ok]
Write down on the CP 575 the confirmation number (REQ #) obtained from the registration and send the form to the address
below with copy of the (REQ#) page obtained from the registration.

Please note, that your request for Security Official will expire if the required IRS documentation (CP575 or 147C Letter are not
submitted within 60 days of your request.
The Security Officer may be called to confirm his/her identity. Once your identity has been confirmed you will get by e-mail the User
ID and a Temporary Password. The temporary password is only active for 10 days, so check your e-mail Inbox and Spam daily. After
getting the ID and the Temporary Password follow the instructions below to change your password and activate your IACS account.

You should go to https://applications.cms.hhs.gov/home.html
On the CMS Application Portal Introduction screen, select the [Account Management] hyperlink.
On the following window, select [My profile] hyperlink.
On the following window check

Introduce your ID and Password sent by e-mail. After sign in you will required to change the password.
To keep the security official registration active the password must be changed every 60 days.

access to your PS&R we must do a request online using our IACS account; CMS will send you an e-mail with the necessary steps to
approve our request, once the request is approved by your facility Security Official we can obtain the PS&R online from CMS. This

Medicare Cost Reports are generally due at the end of the fifth month after the end of the provider’s fiscal year. Listed is an example
of the “general rule” of Due Dates to CMS:

Fiscal Year End Date – December 31, 2011 Due to CMS May 31, 2012.
If you change your organizations’ Fiscal Year, you will have to file two reports to CMS within that year. Call us, we can help to
communicate the fiscal year end change to CMS and file the two reports to CMS.
