GUEST PERMISSION FORM (NOT A BCHS STUDENT)
This form must be turned into the school office at least 1 day prior to purchasing a ticket.

1. All guests are to present a picture ID at the dance.
2. All guests are expected to follow the BCHS Student Code of Conduct and Dress Code
While at the BCHS dance.
3. The BCHS student is responsible for the actions of his/her guest.
4. Only one guest per BCHS student is allowed unless the Principal has approved an
additional guest.
5. A guest must arrive when the BCHS student host arrives and leave when the student
host leaves.
6. A student suspended or expelled from BCHS or any other high school is not eligible to
be a guest
7. Guests who attend ACE or D.C. are not permitted to attend.
8. Middle school students are not permitted to attend any BCHS dance.
9. Dances are not open to persons over the age of 20 (picture ID required).
10. The guest ticket is valid only for the guest named in this form; it may not be used by
anyone else.
11. This form must be attached to the guest ticket and be presented at the dance.
12. If students do not have a picture ID or do not comply with the above stated rules they
will be denied entrance or asked to leave immediately.
