OCI Misc. Change of Address/Occupation – Minor- US Passport Holder
 The documents in the Checklist are mandatory for your application to be processed.
 This list of documents is not exhaustive and is subject to change based on the requirements of
the Indian Embassy / Consulate.
 The Indian Embassy / Consulate have the right to request for additional documentation.
 2 copies for Walk-in & 1 copy for Postal applicants of signed Checklist with the boxes ticked
must be submitted with your documents. The sequence of documents in each application form
being submitted to the Consulate should be in the order as mentioned below.

Complete the OCI Miscellaneous Service Application Form on the
Government Site (https://passport.gov.in/oci/)
 In order to complete the form, you will need at least ONE of the following
o U-Visa Number
o OCI Registration Number
o OCI File Number
 All the Information below WILL HAVE to be entered
o Current Passport Number
o Date of Birth
o Place of Birth
o Mother’s Name
 The INFORMATION below MUST MATCH what the applicant ENTERED
WHEN HE HAD APPLIED FOR the ORIGINAL OCI CARD, failing which one
cannot PROCEED.
 Select the Desired ‘Miscellaneous Service’ needed.
 Application form Signed by the applicant and DULY NOTARIZED

 1 most recent passport size photo(no older than 6 months) of the
applicant to be provided
 Photos to be 2 inchesx2 inches in size, in color, depicting front pose
strictly against white background only on photographic paper.

PROOF OF RENUNCIATION OF INDIAN CITIZENSHIP:
If you ever held an Indian Passport, it is MANDATORY to provide proof of
Renunciation of Indian Citizenship along with your OCI application. One of the
following MUST be produced as proof:o Certificate of renunciation issued by the Embassy/Consulate of
India
OR
o Cancelled Indian Passport with stamp stating the passport is
cancelled due to obtaining US citizenship
OR
o Deemed surrender certificate issued by the Embassy/Consulate of
India

If minor gained naturalization through his/her parents and possesses no
separate naturalization certificate, then copies of the parents
naturalization certificates and a Notarized statement signed by BOTH
parents is required stating the circumstances that their child received
his/her naturalization status and why the child does not possess his/her
own certificate.
If you acquired US Nationality through Naturalization, a copy of your
Naturalization Certificate is required.
If one has obtained more than one nationality after having Indian
Citizenship then provide ALL NATURALIZATION COPIES OR PROOF OF
NATIONALITY FROM ALL COUNTRIES that you have held citizenship.
If you do not have an Indian passport and are applying for OCI on the basis
that the parents and/or grandparents were Indian nationals – then
PARENTS AND/OR GRANDPARENTS NATURALIZATION CERTIFICATE IS
NEEDED
The US naturalization Certificate should state ‘FORMER NATIONALITY’ AS
INDIA. Certificates that state ‘Country of Birth’ as India CANNOT be
accepted.

If one parent or both parents are naturalized citizens, proof of
renunciation for parent(s) must be provided one of the following MUST
be produced as proof:-

o Certificate of renunciation issued by an Embassy/Consulate of India

The OCI Misc. application has multiple places for signature. The following sections
must be signed correctly:



Applicant must sign at the next to the 'Signature' line, including minor
applicants. Minors who cannot write their name can use a thumbprint in
black ink instead.
For minor applicants, BOTH PARENTS must sign the very bottom of the
form in the parent/guardian section even if both parents are not applying
for any OCI service. If only one parent is available to sign for the minor,
proof of custody or a death certificate must be provided. Application form
must be notarized.
Wherever parent’s sign, the signature must match with the passport
signature. Applications will be put on hold and a new application will be
asked for if parent’s signature does not match with the passport
signature.
NOTE - If the signature is OUTSIDE of the boxes provided, the application will be
REJECTED.

Copy of documents in support of the change (Address/Occupation) the
applicant is requesting.
For example – if changing to a NEW CITIZENSHIP, then a certificate
providing evidence of the new naturalization is required.

If your name in any of the supporting documents provided is different in
any way from the name in the US Passport, a copy of ONE of the following
documents must be provided
o Official Court Order for the name change
OR
o Endorsement of name change during naturalization process
If in case the middle name was changed to an initial then a notarized
statement letter is required.
