DECLARATION OF TEMPORARY GUARDIAN ACCOMPANIED MINOR ON FLIGHT
I, the undersigned, departing on the same flight and from the
same airport of departure to the same airport of destination
as that of the minor, hereby agree to take the minor in my
custody during the flight and to accept all the responsibilities
this custody entails.
I certify that the minor is in possession of all travel documents
(passport, visa, health certificate etc.), required by applicable
laws.
