STARS Capital Campaign Contributing To a healthier community TM www.resourcecenter.org/stars Sys·tem - noun : a group of related parts that move or work together message from THE CAMPAIGN chairs As a local physician and the parent of a daughter with a developmental disability, I know firsthand the critical role The Resource Center plays in our County’s health care delivery system. The Resource Center is one of the few local providers that accept new Medicaid patients for primary care and dental services. If not for TRC, thousands of people would not have access to quality medical care locally. TRC’s commitment to meeting the needs of the underserved population will be made easier by the implementation of its broad-scale system integration. Because of the wide variety of services that TRC offers, the organization is well-positioned to thrive in this new era of health care that puts a premium on delivering improved health outcomes at a lower cost. Todd Jacobson, M.D., Co-Chair, STARS Capital Campaign As managing director at a local financial services agency, I can attest to the important role The Resource Center plays in our County’s economy. With a payroll of more than 1,600 employees and an operating budget approaching $96 million, The Resource Center is an economic powerhouse. TRC’s annual economic impact exceeds $200 million. As a health care provider, The Resource Center’s impact on the community is incredible. More than 15,000 area residents utilize TRC’s services, a number that represents more than 10 percent of the County’s population. Through the STARS initiative, The Resource Center will be able to meet the health care needs of even more people while ultimately spending less money in delivering that care. Randy Ordines, Co-Chair, STARS Capital Campaign message from the Executive Director As a comprehensive community-based organization committed to meeting the life needs of individuals with disabilities, as well as the health care needs of thousands of local residents, The Resource Center has been making a real difference in people’s lives since 1959. Because The Resource Center provides a variety of critical services, such as primary care, dental care, counseling and psychiatric care, social work, and employment assistance, our organization is uniquely positioned in these challenging times of system reform to be able to utilize so-called “social determinants” or critical life elements, that lead to wellness as well as social and economic self-sufficiency and success. Through the STARS Project, The Resource Center will be able to address each person’s needs along a five-point spectrum of key dimensions: basic needs, family stability, employment/community involvement, physical health, and emotional health. This coordinated approach to providing care and supports will result in a healthier community as individuals receive the assistance they need to maximize their potential within each of the key life dimensions, and empower themselves and their families to achieve their aspirations. While we’re excited at the prospect of STARS, we will need our community’s help in fully implementing this plan. Limited access to healthcare is not only a national and state issue, but also a local issue. Chautauqua County has too many residents who cannot achieve sufficient access to quality healthcare. The Affordable Care Act aims to achieve better health, better access to healthcare, and greater affordability. Our community needs help and The Resource Center is positioned to provide a meaningful solution. TRC’s Solution to a Healthier Community To help contribute to a healthier community and align with recent changes in health delivery, TRC has developed a new model of care that is built on integration instead of fragmentation. This model is called STARS (System Transformation with Accountability, Results and Satisfaction). Of great value has been their primary care work, which has been a huge help not only as a baseline in an area of physician shortage, but especially when physicians have left the area and patients are desperately looking for a doctor. Additionally, TRC provides sorely needed dental care for sectors of the community unable to find help elsewhere. The mental health and addiction services have been critical in helping to serve these increasing health problems. Our medical, dental and behavioral health services with a keen focus on families, employment and building strong community partnerships. “Ensuring the availability of quality healthcare in our communities is such an important mission, which is why it was such a high priority for me to secure the $100,000 of additional funding for The Resource Center that was included in last year’s State Budget. TRC’s dedication to improving the level of quality and service is an exciting and encouraging development for our area. Sys tem The Resource Center, operating for over a half century as Chautauqua County’s leading safety-net provider impacts over 10% of the population at the margins of system dependence by providing a wide array of key services. Many of these key services are located at the Bas Nee Carl Cappa building in Jamestown and our locations in Dunkirk, as well as other locations in Chautauqua County, but they are not integrated. TRC must move forward with full integration of co-located services for improved access and outcomes. The integrated approach makes TRC financially viable and keenly positioned for the future of sustainable healthcare in our community. TRC is building upon our reputation of being a center of excellence and a regional leader in providing a quality life experience. If TRC does not adjust and keep pace with the changes in the nation’s Behavioral Health healthcare system, we will be forced to continue to provide our services to the community in a fragmented and out-dated mode of delivery. STARS is laser-focused on integrating teams, support plans for services, and software systems. An integrated approach that benefits individuals, families, and our community as a whole. Sys d Inf ate Integr ted Services Through Application Integration Health care systems are very complex due to the varied requirements of numerous insurance companies, government regulations and private payments. rated Systems All these organizations and governmental entities have their own unique set of regulations that create unrealistic demands on organizations like TRC. People Employment in our community expect organizations, like The Resource Center, to be at the forefront of bringing all that information together in one convenient location. There are simply too many points of data and no easy way to downsize the myriad of information. TRC is establishing a comprehensive system to integrate its existing electronic health records and case management systems. This type of exchange will improve data quality, create efficiency and allow for organizational information to be presented effectively. This is a critical component when interfacing with managed care companies. In order to stay competitive in the managed care market, TRC’s data and electronic records must meet the new standards for information exchange. An initial analysis is necessary 6 to assess all of the current systems and opportunities for interchange of electronic information. Estimated Project Cost: $1.1 million Making an impact...One life at a time... As part of TRC’s STARS model of health care delivery, the Health Services Clinic in the Carl Cappa Building on East Second Street in Jamestown now provides integration of services and care coordination. The Resource Center now offers primary care, social services, mental health and dental services all under one roof - a one-stop shop for all members of the community, who no longer have to drive from location to location to receive needed or necessary services. Meet Courtney. Courtney is employed at TRC’s Work Center on Lake Shore Drive in Dunkirk, where, for the last four years, she has assembled box inserts. As part of the STARS model, The Resource Center helps those who have been chronically unemployed or underemployed to transition to traditional and more competitive employment. TRC’s array of employment related supports and experiences help individuals adjust and improve their chances for success in the local job market. TRC’s Counseling and Psychiatric Services recently moved to be co-located with other key services. In Jamestown, behavioral health is now in the same building as TRC’s Dental and Primary Care on E. 2nd Street. In Dunkirk, TRC’s wide array of behavioral services, including PROS are all co-located within the same facility on E. 3rd Street. TRC can now start to deliver these services in a more integrated and efficient manner. Dr. Caillean McMahon, TRC’s Psychiatrist, states, “as part of the STARS model, certified counselors can now provide help to an individual based on their needs rather than a rigid model.” Charles is a single father of two boys. He has lived his entire life in poverty and has dealt with depression and anxiety. Since winning custody of his boys, Charles has been fighting to improve the lives of himself and his two sons. Connected to TRC in 2010, he began with a Care Coordinator who has helped him get his life back on track. Charles states, “I love my boys and it is my responsibility to take care of them. I have quit abusing alcohol and staying out of trouble. I would not have been able to straighten out my life without TRC.” Eric had a myriad of mental health related issues in his life. Things became so bad for him at one point, he was living out of his truck and was hospitalized on more than one occasion. Thanks to the supports Eric has received from TRC, he can now be proud to have stable housing, mental health services, and primary care services. “When Eric began working with me, he had nothing. We were able to link him with his basic needs. He now has begun a new life,” states Peter Szynski, Health Homes Care Coordinator. TRC really makes a difference! 7 A Long History of Improving Lives... The Resource Center has a long-standing reputation in the Chautauqua County community as an innovative and comprehensive provider of services to a wide variety of populations. With a successful fifty-six year history, The Resource Center’s primary focus has been on managing the often complex needs of individuals with disabilities and their families, as well as individuals with socio-economic challenges. The organization began as a grassroots organization founded in 1958 by a small group of parents looking for help in meeting the needs of their children with disabilities. TRC quickly expanded from serving 5 individuals in a storefront location to now serving over 17,000 individuals at multiple sites across Western New York with a current operating budget in excess of $94 million. TRC draws upon a talented and dedicated Board of Directors consisting of a majority number of family members of persons with disabilities, as well as various community leaders. As the third largest employer in Chautauqua County, TRC employs more than 1,700 individuals and is a leader in the community. TRC’s estimated economic impact is over $200 million annually. Major milestones celebrated by The Resource Center include attaining work center revenues in excess of $38 million in 2012, achieving Level 3 Patient-Centered Medical Home status in early 2012 and reaching a level of services that touches the lives of nearly 10% of the residents of Chautauqua County.
