nother year has come and gone, and I look back with great joy on what a
successful year it has been for Lakeview Regional Medical Center, its employees,
and our community. As the Chief Executive Officer, it is pleasing to see all of the
accomplishments we have achieved. I grow more humbled by the commitment and

(H2U), a national provider of employer health services, was
pleased to announce the H2U Health Center opened for the
hospital’s employees and their dependents. The H2U Health
Center can be accessed by employees and their dependents for
immediate primary care needs. In addition, the H2U Health
Center is available to a select number of local businesses who
partner with Lakeview Regional Medical Center to offer this
same benefit to their employees and dependents. Participating
employers gain convenient access and save money on
healthcare by paying a negotiated, lower cost through H2U.
We were the first hospital on the Northshore
to offer 24/7 Neurosurgery coverage.

About Leapfrog*
Stroke Accreditation
Leapfrog’s mission is to trigger giant leaps forward in the safety, quality and affordability of health care by:
• Supporting informed health care decisions by those who use and pay for health care; and,
• Promoting high-value health care through incentives and rewards.
The Leapfrog Group is a voluntary program aimed at mobilizing employer purchasing power to alert America’s
health industry that big leaps in health care safety, quality and customer value will be recognized and rewarded.
Among other initiatives, Leapfrog works with its employer members to encourage transparency and easy access to
health care information as well as rewards for hospitals that have a proven record of high quality care.
The Leapfrog Hospital Survey is the gold standard for comparing hospitals’ performance on the national standards
of safety, quality, and efficiency that are most relevant to consumers and purchasers of care. The survey is the only
nationally standardized and endorsed set of measures that captures hospital performance in patient safety, quality and
resource utilization. Hospitals that participate in the Leapfrog Hospital Survey achieve hospital-wide improvements that
translate into millions of lives and dollars saved. Leapfrog’s purchaser members use Survey results to inform their
employees and purchasing strategies. In 2014, 1,501 hospitals across the country completed The Leapfrog Hospital Survey.
Leapfrog’s Hospital Safety Score® assigns A, B, C, D and F grades to more than 2,500 U.S. hospitals based on their
ability to prevent errors, accidents, injuries and infections. The Hospital Safety Score is calculated by top patient
safety experts, peer-reviewed, fully transparent and free to the public. A full analysis of the data and methodology used
in determining grades is available online here. The newly re-launched website now includes consumer-friendly videos
and tips for patients and their loved ones. A Hospital Safety Score mobile app is also available for download.

The Joint Commission, the leading accreditor of health care organizations in
America recognized Lakeview Regional Medical Center for exemplary performance
in using evidence-based clinical processes that are shown to improve care for
certain conditions. The clinical processes focus on care for heart attack, pneumonia,
surgery, children’s asthma, stroke and venous thromboembolism, as

Lakeview Regional Medical Center has earned The Joint Commission’s Gold Seal of Approval®
for its Primary Stroke Center, the first of its kind on the Northshore, by demonstrating continuous
compliance with The Joint Commission’s state-of-the-art national standards for health care quality
and safety in disease-specific care. In addition to this certification, Lakeview Regional Medical
Center has also received the Get with the Guidelines®—Stroke Silver Quality Achievement Award for
implementing specific quality improvement measures outlined by the American Heart Association
and American Stroke Association for the treatment of stroke patients.
Lakeview Regional Medical Center was honored with an “A” grade in the Fall 2014 Hospital Safety Score, which
rates how well hospitals protect patients from errors, injuries and infections. Only 31-percent of hospitals across
the country achieved this high grade. The Hospital Safety Score is compiled under the guidance of the nation’s
leading experts on patient safety and is administered by The Leapfrog Group, an independent industry watchdog.
The first and only hospital safety rating to be peer-reviewed in the Journal of Patient Safety, the Score is free to
the public and designed to give consumers information they can use to protect themselves and their families when
facing a hospital stay.
Lakeview was honored by March of Dimes and
Louisiana Hospital Association for meeting the
recognition criteria. Champions for Quality Care!
Through the strong start for mothers and newborns
initiative, a partnership with the U.S. Department of
Health and Human Services and the American College
of Obstetricians and Gynecologists, the March of Dimes
has been spreading the word that Healthy Babies Are
Worth The Wait. This campaign urges women that if their pregnancy is healthy, it is best to wait and let labor begin
on its own rather than scheduling their delivery; babies aren’t fully developed until at least 39 weeks of pregnancy.
Lakeview was honored by winning the 2014 Innovation Award from the St. Tammany West Chamber
of Commerce. The Innovation Award celebrates a company that is a leader in technological change and/or
incorporates new technologies in their day-to-day business.

AABB Accreditation
The AABB Accreditation Program promotes the highest standard of care
for patients and donors in all aspects of blood banking, transfusion medicine,
hematopoietic, cellular and gene therapies, transplantation, and relationship
testing. The AABB supports high standards for medical, technical and administrative performance, scientific
investigation, clinical application and education. The resulting Accreditation Program specifically assesses quality
systems and operational areas for compliance with Standards.
Advanced Certification in Stroke (Primary Stroke Center)
The Joint Commission has developed an advanced level of certification for programs
that must meet the requirements for Disease-Specific Care Certification plus
additional, clinically-specific requirements and expectations. This certification
improves the quality of care provided to patients, demonstrates commitment to
a higher standard of service, provides a framework for organizational structure
and management, provides a competitive edge in the marketplace, enhances staff
recruitment and development and is recognized by insurers and other third parties.
GIFT-Certified
Lakeview Regional Medical Center hospital staff played a critical role in the establishment
of breastfeeding, and this program helps to assure that the criteria for educating new
parents and hospital staff on the techniques and importance of breastfeeding have been
met. Designation as GIFT-certified requires a hospital to demonstrate that it has met
the Ten Steps to a Healthy Breastfed Baby. The GIFT Program Coordinator maintains
a close relationship with the hospital by assisting in meeting the criteria, conducting
on-site visits and periodic data collecting specific to breastfeeding rates.
Hospital Accreditation
Lakeview Regional Medical Center has earned The Joint Commission’s Gold Seal of

Image quality, personnel qualifications, adequacy of facility equipment, quality control
procedures, and quality assurance programs are assessed. The findings are reported to the
ACR Committee on Accreditation, which subsequently provides the practice with a
comprehensive report they can use for continuous practice improvement.

Lakeview Regional Heart Center opened the Cardiac Imaging office.
First Hospital to Offer Single-Site Robotic Surgery.
Lakeview Regional Medical Center was the first hospital on the Northshore to offer the latest break-through in
robotic technology. Dr. Gary Agena successfully performed a single site (single incision) robotic hysterectomy in
December 2013. Dr. James Christopher performed the first single site robotic gall bladder removal February 2014.
Dr. Ruary O’Connell performed the first robotic assisted gastric sleeve surgery in the region on June 6th.

Radiology Department added two new digital radiology rooms and one new portable digital x-ray machine.
Outpatient Rehabilitation obtained the BTE PrimusRS.

The BTE PrimusRS is the most versatile and functional smart physical therapy equipment for multi-joint testing,
orthopedic rehab, neuromuscular reeducation, and advanced musculoskeletal athletic training of the upper and
lower extremities and the core.
Rehabilitation Department initiated Women’s Health Program.
ReDoc – electronic rehab documentation system was developed to fit the clinical and business needs of PT,
OT and SLP. ReDoc’s technology and services vision continues to evolve and adapt to new industry challenges
and customer demands. We are focused on advancing our solutions to optimize workplace efficiency, increase
profitability, and enhance patient care and outcomes.
Act fast. We do. A Code Purple alert indicates to Lakeview Regional’s
physicians and hospital staff that there is a patient
with a suspected stroke. We take stroke care seriously with

Patient Portal is a secure online
reflection of data from Lakeview
Regional’s electronic health records
and is only accessible by the patient
or someone the patient authorizes as
a user. “This program is an important
step towards improving our patients’
healthcare experience,” states
Dr. George Barnes. “It reflects our
ongoing commitment to advancing
technology and helps patients and their
caregivers to become more informed and engaged, while increasing communication, efficiency and productivity,
leading to better outcomes.” Patients can now view their allergies, conditions, discharge summaries, hospital
visit histories, lab results, medications, medication instructions, lab results, radiology reports and upcoming
appointments from anywhere with internet access. Patients can also download and print this information so it is
readily available for scheduled appointments, referral visits or when supporting a loved one’s care.

Lakeview Regional Medical Center hosts the Sudden Impact Program, a four-hour
program for high school sophomores dedicated to the prevention of injuries and fatalities
related to driving under the influence and seatbelt usage. The Sudden Impact program
targets teens who are in the process of obtaining their drivers licenses. The program is
a partnership with Lakeview’s ER Medical Director and Registered Nurses, the Louisiana State Police, Louisiana Organ
Procurement Agency (LOPA) and the LSU Interim Hospital in New Orleans Trauma Center. Since January 2011,
over 600 students from four Northshore Schools have attended the program at Lakeview.
• Jennifer Vitellaro, NNP and Julie Broussard, NNP received the National S.T.A.B.L.E. Instructor Certification.
The S.T.A.B.L.E. Program was developed to meet the educational needs of health care providers who must
deliver this important stabilization care. S.T.A.B.L.E. education is critical to the mission to reduce infant
mortality and morbidity and to improve the future health of children and their families. S.T.A.B.L.E. stands
for the six assessment and care modules in the program: Sugar, Temperature, Airway, Blood pressure,
Lab work, and Emotional support. This program will be implemented for Women’s and Children and the
Neonatal Transport Team staff.
ED Director Chad Muntan and ED nurses participated in the Consequences of Impact: Mock Crash program, as well.
So far, they have presented the medical portion of the program at three private schools, reaching over 500 students,
teachers and parents. This Mock Crash program is a moving performance presented to the entire student body, parents
and teachers, which includes all the results of impaired and/or distracted driving.

Holiday Bazaar – The Professional Women of St. Tammany hosted by the
Volunteer Auxiliary at Lakeview partnered together for this event held November 4th.
As one of the largest employers on the Northshore, Lakeview Regional Medical Center
feels this will be an effective venue to educate our employees, volunteers and members
of PWST. The holiday bazaar also gives the opportunity to shop and learn about
local businesses. The public was also invited. There were over 30 vendors and
$1,500 was raised for nursing scholarships.
• Bonnie Smith, PCT received National Child Passenger Safety Certification.
• Jeanne Waddell, RN completed her first year in a Master’s program for
Pediatric Nurse Practitioner.
• Christina Young, RN and Brandi Schwab near the completion of their
Master’s program for Pediatric Nurse Practitioner.
• Brandi Galiano and Sherri Defreites, RN completed inpatient obstetric
nursing certification.
• Kelly Engerran and Brandi Galiano completed the instructor course
through AWHONN in Fetal Heart monitoring.

The 5th Annual Boo Fest was held on October 25th
with over 8,000 children and adults in attendance.
Seventy decorated houses were on display to children
to visit, and 20 chili cook-off teams competed for
bragging rights. We raised over $40,000 for ACCESS

Successful Solutions).

• Lakeview Regional Medical Center is a proud sponsor of Sweet Soiree which was held at Stone Creek Club
& Spa. The event was a jazzy night of food & spirits benefiting ACCESS, an organization that raises awareness
and funds to help support families of children with disabilities.
• Lakeview Regional Medical Center hosted the St. Tammany West Chamber of Commerce “Business
After Hours” with demonstrations of the surgical robot. Surgeons, Robotic Team Lead and Intuitive
Representatives answered questions about the minimally invasive robotic surgery and even gave individuals
a chance to test drive the robot.
• United Way —Lakeview Regional Medical Center employees donated $9,275 to The United Way.
3rd Annual “You are the Apple of Our Eye” Teacher Appreciation Event
Lakeview Regional Medical Center hosted its 3rd annual $5 Jewelry & Accessory Sale
for the faculty and staff of the St. Tammany Parish school system. The “Apple of Our
Eye Teacher Appreciation Event” drew 162 attendees. The teachers and volunteers
attending the Jewelry and Accessory Sale raised $1,642 for Care for KIT; Lakeview
Regional matched that dollar amount, bringing the total donation to $3,285. Awards for
the highest level of attendance and door prizes were part of the afternoon event.
“Lakeview Regional has been an
integral partner in our mission to
assist homeless students and their
families,” states Lynn Johnson,
Kids in Transition Liaison.
“Fundraisers like ‘Apple of Our Eye’
make it possible for us to provide
the necessary funds to promote
academic success and student achievement.” On November 20th,
Johnson was presented with the donation to Care for KIT.
School Spirit winners for the highest attendance at the event were:
First Place— Fontainebleau High School, who received a check for
$1000; Second Place— a tie between Abita Springs Elementary
School and Tchefuncte Middle School. Door prize winner of the
three-hour limo service was Sheila Nolinske from Tchefuncte Middle School and the iPad winner was Summer Holiday
from Abita Springs Elementary School. “We are thrilled to be able to honor our teachers here in St. Tammany Parish, while
also providing some financial assistance to the Care for KIT program,” states Bret Kolman, Lakeview Regional Medical
Center Chief Executive Officer.

~ ICU: targeted training in care of the patient with septic shock

Women’s and Children: training in the variations in normal labor — precipitous delivery,
shoulder dystocia and post-partum hemorrhage
Skill-based simulation education and training in the staff’s normal working environment.
This level of training provides a suite of competency based programs for individuals and teams including
nursing, physicians and respiratory staff. This includes hands-on skills training, complex multi-layered
simulation training with the use of high fidelity simulators which aids in preparing staff to respond to
critical, clinical scenarios and optimally operate in a team environment.

Admitting distributes a Patient Passcode to protect the patient’s
privacy. The patient can share this code with friends or loved
ones. This code allows them to receive certain information like
updates on condition, results from lab tests, x-rays or other tests
and diagnosis, prognosis or discharge instructions.
• Our ER had the lowest greet time on the Northshore.
• For Stroke Awareness Month an eblast was sent to

Jenny completed the National Provider Relations
Management Development Program.
• The Respiratory Department and NICU team
remain ready for neonatal transport. Since the inception
of the program in January 2011, we performed
56 transports, resulting in admissions to our NICU.
2014 was a record year— we performed 19 transports.
