FoundationFocus The official publication of the TGH Foundation Spring 2013 Campaign for the Palliative Care Program In the Fall/Winter edition of FoundationFocus, we announced that the Tampa General Hospital Foundation was launching two fundraising campaigns. In this edition, we would like to invite you to be a part of our success. One of these campaigns will raise $320,000 to help transform the hospital’s Palliative Care Program. Palliative care is an emerging medical specialty that focuses on providing patients suffering from serious or advanced illnesses relief from the symptoms, pain, and stresses of their illness. A skilled team of doctors, nurses, medical specialists, social workers, and pastoral care staff work in partnership with a patient and their family to enhance the quality of life, to ensure a patient’s autonomy and to provide an extra layer of support as difficult choices are faced. Palliative care is central to healthcare reform efforts. A growing body of evidence demonstrates that it increases the value of health care by increasing the quality of care while lowering costs. The results benefit patients, families, clinicians, hospitals, and healthcare systems. The TGH Foundation’s campaign is chaired by community leader Jon Yob, President and CEO of Creative Recycling Systems, Inc. Since November, Jon has been developing a strategy to complete the campaign by late fall 2013. To date, the campaign has raised $140,000—nearly half of its goal—but more funds are needed. During the campaign, you can play a vital role in helping us meet our goal. We ask you to consider supporting the campaign financially. The end result of our efforts will allow Tampa General to continue to grow its Palliative Care Unit. In 2012, the Palliative Care team consulted on more than 1,300 patients, about 15 percent more than in 2011. While there is currently no inpatient palliative care unit, TGH has one completed Palliative Care Suite and two additional rooms designated as future suites. Contributions will help pay for these rooms to be remodeled and equipped as tranquil Palliative Care Suites. Attendance at our events continues to grow, as well as the funds we raise to support the efforts of the hospital. Of course, our success relies on the ongoing efforts of both staff and volunteers of TGH, and most importantly—you, our donors. We welcome our new CEO and president, Jim Burkhart. Jim recently served as president of the University of Florida Shands Medical Center in Jacksonville. Jim and his wife, Tina, are currently in the process of relocating to Tampa. Straz, Jr., Chairman, Florida Health Sciences Center Board of Directors. 2 Shands Jacksonville CEO Succeeds Ron Hytoff “I’m proud that the hospital was able to grow during my tenure, as we literally tripled our revenues and turned annual budget deficits into profits,” Ron said. “But, perhaps the crowning achievement, which could not have happened without dedicated employees and physicians working so well together, was to be ranked the number one hospital in Florida by U.S.News & World Report’s Best Hospital’s 2012–13. With nine of our specialties listed among the top hospitals in the country, we have achieved national prominence, which seemed unattainable just a decade ago.” Jim Burkhart, the former president and CEO of Shands Jacksonville Medical Center, joined TGH March 4th as president and CEO. He replaced Ron Hytoff, who retired. Recently retired Tampa General CEO Ron Hytoff and Jim Burkhart, his counterpart at Shands Jacksonville Medical Center, first met about 10 years ago. Over the past decade, they have developed a mutual respect built upon the similar challenges they’ve successfully overcome and the recognition both hospitals have earned for quality patient care. While Ron was transforming TGH from a financially challenged hospital into one of the country’s finest medical facilities, his counterpart was saving Shands Jacksonville from the brink of bankruptcy. So, when Ron decided it was time to retire at the age of 67, he knew another teaching hospital CEO in Florida who could be among the best candidates to build upon the success he had achieved at TGH – and ensure a smooth transition. He contacted Jim Burkhart and suggested he apply. After a nationwide search that attracted about 200 applicants, the TGH governing board, Florida Health Sciences Center, hired the 58-year-old Shands CEO who took over on March 4th. Operation Santa Raised Nearly $12,000 for Pediatric Patients at TGH Ron believes the hiring will ensure a very smooth transition for employees and physicians because Jim Burkhart shares the same sense of mission. Like TGH, Shands Jacksonville has received national recognition for quality patient care and safety. Shands Jacksonville is ranked one of the Top 20 Academic Medical Centers and has a four-star rating from the University Hospital Consortium. Many thanks to the generous donors who helped make the holidays a little brighter at Tampa General Hospital by supporting Operation Santa! Over the holiday season, Operation Santa raised nearly $12,000 to benefit Tampa General Hospital’s Children’s Medical Center. The two hospitals share many similarities, including Magnet designation for quality nursing care, ranking in U.S.News (Shands Jacksonville ranked 11th in Florida), and a conversion to Epic that achieved meaningful use status within six months of implementation. TGH and Shands Jacksonville also are members of the Safety Net Hospital Alliance of Florida. Donations to Operation Santa allowed us to bring Santa and all the excitement of the magical holiday season to Tampa General for all of our littlest patients who were too sick to be at home for the holidays with their families. Operation Santa helps stock the toy closet not only for Christmas, but year-round and provides a toy, a card, and a smile when it’s most needed. Shands Jacksonville has 695 licensed beds and is the only level 1 trauma center in Northeast Florida and Southeast Georgia. It has a staff of 3,800 and is affiliated with the University of Florida College of Medicine. Jim Burkhart holds a doctorate of science degree in administration–health services and a master of hospital and healthcare administration degree from the University of Alabama at Birmingham. He graduated from the University of Tennessee with a bachelor’s degree in psychology and biology. He is also a fellow of the American College of Healthcare Executives. 3 2013 Night at the Circus The Tampa General Hospital Foundation’s 22nd Annual “Night at the Circus” was a roaring success! This year’s event was held on January 3, 2013, at the Tampa Bay Times Forum, and the event committee was chaired for the second year by April Neil and Janet Stephens. April’s and Janet’s enthusiasm for the thrills and excitement of this event and their dedication to the TGH Foundation helped them welcome ten new volunteers to the committee over the past two years. They also recruited new sponsors and continued to raise awareness of Tampa General Hospital and the TGH Foundation throughout the community. Each year, $2,500 sponsors and above are invited to attend the Patrons’ Party preceding the circus performance. This year 450 guests were greeted by the ringmaster and entertained by face painters, a juggler, a balloon twister, a DJ, and members of the circus cast. Net proceeds of $218,000 from this year’s event will benefit Tampa General Hospital’s Children’s Medical Center. The TGH Foundation donated over 1,200 tickets to local charities this year so some of those less fortunate would have an opportunity to attend and experience the thrill of the Ringling Bros. and Barnum & Bailey® Circus – The Greatest Show on Earth®. The small devices attach to a patient’s gown and include a headphone and a microphone. Staff members use the microphones to communicate in a normal voice with the patient. Some models also plug in to the television. The Rehabilitation Center received $8,218 to buy two hair salon-style shampoo sinks and chairs, so patients who are unable to take showers can get their hair washed. It’s the third time the center has benefitted from the council’s awards. Six hospital departments recently benefitted from $94,598 donated by the Tampa General Hospital Foundation’s Women’s Leadership Council for the purchase of patient-care equipment. During the recent project awards, the Radiology MRI department received $46,850 to purchase a video system that will help relieve anxiety and claustrophobia that many pediatric and adult patients have during an MRI. The council, comprised of 95 community women, was formed in 2009. Each year, members make a donation to fund the council’s project awards. The system includes special goggles and headphones that allow patients to listen to their favorite music, view relaxing images, or even watch movies or their favorite television programs as the MRI is performed. “In just four years, the council has contributed $355,000 to hospital departments for patient-care equipment purchases,” said Robin DeLaVergne, Executive Director of the Foundation. Each year, departments submit requests for funding, and the council members collectively choose the winning proposals. “The members make their individual contributions go further by pooling them together,” said Robin. “The hospital and our patients also benefit when we can purchase equipment that focuses on the patient experience, that doesn’t make it into the capital budget,” she said. “The new video system is expected to benefit many pediatric patients who often require anesthesia before undergoing an MRI. The new system will help relieve their fears, so many may not need sedation,” said Keith Aldahondo, MRI-CT manager. The Acute Care for The Elderly (ACE) Unit also received $1,330 to purchase handheld pocket talker devices to help hearing-impaired patients communicate with nurses and other staff. “Many of the unit’s patients have hearing difficulties. Caregivers often have to shout 8 The Pulmonary Rehabilitation Program received $18,400 for a specialized chair and mountable ventilator that makes it easier to take patients on ventilators out of their rooms or outside. “It can be difficult to take patients out of their rooms because of the equipment they need. It often takes up to four staff members to load the portable oxygen, monitors, and ventilator,” said Respiratory Therapist Brendan Keane, who submitted the funding request. With the new equipment provided by the council, getting patients ready to leave their rooms will be much easier, and it will provide them opportunities to be taken outside, he said. In addition, the Rapid Response Team (RRT), the hospital’s mobile critical care team, received $4,295 for portable monitors that instantly measure carbon dioxide levels in the blood. The Neonatal ICU also received $15,504 for additional breast pumps. TGH’s Newborn Hearing Program Receives State Award Tampa General was recently honored for its newborn hearing screening program with the Award of Excellence from the Florida Department of Health’s Newborn Hearing Screening Program. TGH was selected for the award from hospitals with 3,000 or more births a year. Every infant born here undergoes a series of hearing tests before being discharged, said Janet Sullivan, director of Tampa General’s Newborn Hearing Screening Program. who did not pass the inpatient screening, its timely and accurate reporting of hearing results to the State Department of Health, its screening of all babies born at the hospital prior to discharge, and its low rate of failed hearing screenings. “Since April, we’ve found seven babies with permanent hearing losses who needed hearing aids,” Janet said. “Early exposure to sound, particularly to speech sounds, is vital to an infant’s development of speech and language,” she said. In 2009, the TGH Foundation’s Women’s Leadership Council enhanced the program by funding the purchase of a $25,000 infant hearing screening machine to ensure that every newborn is tested before going home. Once a baby is diagnosed with hearing loss, they’re immediately referred to an audiologist for further evaluation and care. Having the screening available at the hospital is a tremendous benefit to parents. Infants with hearing problems can be automatically enrolled in the state’s Early Steps program. “Infants who need hearing aids are usually fitted within three weeks of being discharged from the hospital,” Janet said. “While the state mandates that every newborn receive a hearing test, what sets TGH apart from other hospitals is its concentration on obtaining a comprehensive hearing diagnosis of Neonatal Intensive Care Unit babies who don’t pass the initial hearing screening,” said Janet, who conducts the lengthy evaluations. “When they leave, I have enough information to fit them for hearing aids,” she said. Infants in the well-baby nursery are tested with a series of tones that stimulate their ears. A tiny microphone inserted in their ears detects whether they hear the tones. Babies who don’t pass are given another test that measures their brain wave responses to clicking sounds in their ears. TGH was recognized for excellence in the following areas: its follow-up of babies Being proactive has paid off. “Even infants in the NICU receive the hearing test that measures their brain wave responses to clicking sounds,” Janet said. “The Florida Department of Health ensures that every child who needs a hearing aid gets one,” said Laura Olson, who oversees the department’s Newborn Hearing program and presented the award to TGH. “Our goal is to keep them at their school level by getting them fitted early,” she said. The BSCIP accreditation survey is conducted every three years. The BSCIP provides funding at its designated centers for patients who have exhausted their insurance, Medicare, Medicaid, or worker’s compensation coverage. “We were accredited by BSCIP for all our programs without a single recommendation,” said Michael. Eric “Tate” Taylor, Esq., Trenam Kemker When you support the Tampa General Hospital Foundation with a financial gift, you demonstrate your appreciation for the wonderful work of Tampa General Hospital in our community. You also receive important tax benefits. Some of these benefits have been impacted by the passage of new tax laws, including the American Taxpayer Relief Act of 2012. This article discusses some of the tax issues to be considered when making charitable contributions in 2013. Higher Income Tax Rates. The American Taxpayer Relief Act of 2012 imposes a new top income tax rate on taxpayers with taxable income in 2013 above $400,000 ($425,000 for heads of households and $450,000 for married taxpayers filing jointly). These taxpayers also are subject to a higher long-term capital gains tax rate in 2013 (20% rather than 15%). In addition, under the “Patient Protection and Affordable Care Act,” net investment income for taxpayers with taxable income above $200,000 (or $125,000 for married taxpayers filing separately and $250,000 for married taxpayers filing jointly) is subject to a 3.8% additional tax in 2013. Higher income taxpayers who seek to decrease their taxable income in 2013 by increasing their charitable contributions also have to consider the modest effects of the so-called “Pease Limitation.” Under this adjustment, itemized deductions (including charitable contributions) made in 2013 are reduced by 3%, not to exceed 80%, of the amount by which the taxpayer’s adjusted gross income exceeds $250,000 ($275,000 for heads of household and $300,000 for married taxpayers filing jointly), indexed for inflation. For example, a married couple with adjusted gross income of $400,000 and itemized deductions of $50,000 must decrease their itemized deductions by 3% of $100,000, or $3,000. In many cases, this reduction will be modest. In-Kind Gifts of Appreciated Property. The tax laws described above should encourage donors to make in-kind gifts of appreciated property in 2013. Charitable contributions of appreciated property held for longer than a year typically provide the donor with an income tax deduction equal to the fair market value of the property as of the date of the gift. Moreover, because the security is not sold prior to be contributed to charity, the charitable contribution avoids the recognition of gain that otherwise would occur if the property was sold. Avoiding this gain has several benefits: (i) it tends to prevent the taxpayer from being subject to the highest 39.6% income tax rate; (ii) it avoids the capital gains from selling the asset, which in 2013 would subject the capital gain to a 20% capital gains tax rate (if taxable income is above the thresholds mentioned above), and (iii) it avoids a 3.8% tax on net investment income. IRA Charitable Rollovers. The “IRA Charitable Rollover” strategy allows individuals who are 70 ½ or older to “roll over” $100,000 from a traditional IRA directly to a qualifying charity such as Tampa General Hospital Foundation without recognizing that amount as taxable income. No tax deduction is available for the donors who do so, but the amount “rolled over” to charity is excluded from income. Thus, there is no tax consequence to the IRA owner, and charity receives up to a $100,000 benefit. When first created under the Pension Protection Act of 2006, the IRA Charitable Rollover strategy was set to expire on December 31, 2007. However, it has been extended several times; and the American Taxpayer Relief Act extended it again, this time until December 31, 2013. This strategy may be of particular benefit to high-income taxpayers who itemize deductions in 2013 and are, thus, subject 12 to the Pease Limitation described above. In addition, by not increasing adjusted gross income, this strategy may help avoid the imposition of the 3.8% additional tax on net investment income. Planned Gifts Occurring Upon Death. The American Taxpayer Relief Act of 2012 preserved the $5 million exemption from estate tax, and the exemption continues to be indexed to inflation. For 2013, the exemption amount is $5,250,000. However, the effective estate tax rate in 2013 for assets above the exemption amount is 40%. For certain assets such as qualified retirement accounts and IRAs, the effective rate of tax due upon death can approach 70% because the assets are subject to both estate tax and the income taxes payable when distributions are made from these accounts after death. For families with significant assets, naming the TGH Foundation in your will or trust can provide a charitable estate tax deduction that avoids the 40% estate tax. Even more significantly, by designating the Foundation as a beneficiary of your IRA or retirement plan account, even greater tax savings are available because distributions to the Foundation after death are not subject to estate tax or income tax. New High-Tech Cardio-Vascular-Transplant Operating Rooms Open The hospital recently opened three new cardiovascular transplant operating rooms that feature the most advanced interventional radiology and surgical technology available. “Additional new equipment allows surgeries to be telecast live to several locations on campus for educational purposes,” said Rick Hufstetler, nurse manager of the cardiac operating room. The renovated area is on the third floor of the West Pavilion and is the first of a 40,117-square-foot, three-phase project that includes the three new operating suites and the renovation of the six existing operating suites. The project is expected to be completed in February of 2014. Each operating room also features new imaging systems. Large television screens display images of patients’ prior procedures—including CT scans, MRIs, and cardiac catheterization reports—so physicians can easily review them before and during surgery. Work on the initial phase began last year. The three new cardiovascular transplant operating rooms are in an area that housed the cardiac catheterization labs. The project’s second phase is already underway and includes renovating three existing cardiac operating rooms. It also will expand the patient pre-op areas from four to ten, with space for family seating. Blood gas levels will also be completed in the rooms instead of in the Pulmonary Department. Results will be available quickly on television monitors in the operating rooms. The largest of the new operating suites spans 926 square feet and is outfitted with high-tech interventional surgical equipment and new high-resolution X-ray equipment. Two of the new pre-op rooms will serve isolation patients who received pre-op treatment on another floor. New cameras in the pre-op areas also allow staff to monitor when an area is ready for another patient for improved patient flow. The third phase of the project includes renovating the remaining three cardiac operating rooms, expanding the staff locker area, relocating and enlarging the staff lounge and offices, and building a new cardiac waiting room. The existing cardiac waiting room will also be remodeled to include a children’s area, a new patient consultation, and pastoral care rooms. The Tampa General Hospital Foundation is contributing $315,000 to help cover some of the construction costs of the two waiting rooms. The physicians’ lounge will also get a makeover.
