Forty years ago, two doctors—Phillip Rossman, MD, and Mayer B. Davidson, MD—had a great idea. They would organize a group of volunteers to do what the larger health care system on the Westside of Los Angeles County wouldn’t: provide free, quality health care to people who needed it but couldn’t afford it. The greatness of the idea resided not as much in its originality—other free clinics had begun popping up around the country a few years before—as in the ownership they took of it. After all, Drs. Rossman and Davidson didn’t have to create Venice Family Clinic. They bore no particular responsibility for the thousands of poor and uninsured people in Venice at the time. And it would have been easy for them to identify reasons not to do it—the enormity of the need, the lack of funding, the time involved; perhaps, even, that someone else should, or eventually would, take care of the problem. But they knew it couldn’t wait because, as physicians and as humanitarians, they understood what happens to people who go without the care they need, how a single injury or illness or neglected condition can change the trajectory of one’s life. They also understood that need is not defined exclusively, or even all that frequently, by emergency, and that the true test of a health care system is its ability to identify and treat patients’ needs before they become emergencies. This attitude—epitomized by the phrase “Can’t Wait”—has remained at the heart of Venice Family Clinic’s work ever since. And it is the most salient explanation of how and why Venice Family Clinic became what it is today—the largest free clinic in the country, with nearly 2,100 volunteers and 225 staff members, serving more than 24,400 patients through nine sites across the Westside of Los Angeles County and beyond. Weisman Family Center With this in mind, following is a small sampling of the many health needs that low-income and uninsured patients have on any given day, as well as a few milestones from Venice Family Clinic’s first 40 years to illustrate how, if we are to one day enjoy a more stable, equitable, and effective health care system, the time to start working on improvements is now. It always will be. Thank you for your support, Irma Colen Administration Building Robert Levine Family Health Center In 2010, Venice Family Clinic and hundreds of safety-net providers like it were joined in their embrace of this attitude by the Congress and the President of the United States, who together passed the landmark Patient Protection and Affordable Care Act. It was a turning point in the health reform movement, certainly, but by no means the end of the struggle, for the law will take years to implement and it will face battles on numerous legal and legislative fronts in the meantime. Children First Early Head Start, Inglewood Elizabeth Benson Forer, MSW/MPH Chief Executive Officer and Executive Director Venice Family Clinic provides a medical home to more than 24,400 low-income and uninsured patients through nine sites in Los Angeles County. 1 YEARS YEARS 40 40 The two were at opposite ends of their careers—Dr. Davidson just out of the army and Dr. Rossman with decades of experience in private practice—but in an identical place philosophically. It couldn’t wait in 1970. “He was a kindhearted man, a gentleman, and extremely focused,” Dr. Davidson says of the late Dr. Rossman. “He really kept after things. If he got subverted one way, he would try another. He was a very dedicated person.” On a seemingly ordinary Monday in Venice, California, in October 1970, as most businesses went dark for the evening, a small dental office on Lincoln Boulevard flickered back to life. Two physicians stepped inside and surveyed the situation. “Medical records—we need a spot that’s out of the way,” one remarked. As they left the building that first evening—October 12, to be precise—they paused to shake hands, neither knowing how long their fledgling clinic would, or could, last. “Where will we put medications?” the other asked. “Maybe we can find someone to build us a cabinet.” “It was such a bootstrap operation that I don’t think we thought about five years, ten years down the line,” Dr. Davidson says. “We just wanted to keep it going as long as we could.” “Have you ever done a pelvic exam in a dental chair?” “No, but I think we can make it work.” Amid the questions and answers, the doctors busily unpacked their bags in preparation for whoever might walk through the door. Word had gone out that a new free clinic was opening in the neighborhood, which was one of the poorest on the Westside of Los Angeles County, and neither doctor knew what to expect. When they created Venice Family Clinic, in 1970, there was only one physician in all of Venice who treated Medi-Cal patients, and he did so just one afternoon a week. So they formalized it with a name, Venice Family Clinic, giving an immediate indication of the organization’s scope and ethos. It was open five evenings a week. “There wasn’t a big fanfare. About a dozen patients showed up,” says Mayer B. Davidson, MD, one of the two there that day. “We learned as we went.” In 1993, Frederick R. Weisman donated the property at 622 Rose Avenue, Venice, just east of Venice Family Clinic’s flagship facility, and funded the creation of the Clinic’s mental health program. Frederick passed away the following year, but the Frederick R. Weisman Art & Philanthropic Foundation continues to support the Clinic in a multitude of ways, thanks to Frederick’s widow, Billie Milam Weisman, also pictured. The conditions conjure images of a small-scale humanitarian relief mission and, in many ways, that’s what it was. For years, the neighborhood’s low-income residents had bristled at not having an affordable health care facility nearby. They pointed to the fact that there was only one physician in all of Venice who accepted Medi-Cal (California’s Medicaid program), and he would only see those patients one afternoon a week. Within a couple years, Venice Family Clinic was seeing about 500 patients per month, from homeless people to hippies to low-income laborers. Twenty to thirty physicians volunteered their time. Residents of the area helped staff the front desk. Local hospitals donated lab, x-ray, and emergency services. And the location was provided free of charge by UCLA’s School of Dentistry. In 1969, a group of Venice residents went so far as to protest at the opening ceremony of a county clinic in Santa Monica. There, an internist named Phillip Rossman, MD, saw their pickets, invited them inside to share their concerns, and quickly promised to do what he could to help. Slowly, it became less of a day-to-day proposition. Several small fundraising events were held. One of the volunteers secured a research grant from the state. A small staff was hired. More volunteers got involved. And so on. Around the same time, Dr. Davidson, an endocrinologist, read an article about a teen clinic set up by volunteer physicians in Ohio and mentioned to a colleague his interest in doing something similar in Los Angeles. The colleague had heard what Dr. Rossman was trying to do in Venice and suggested they collaborate. In June 1970, they met for the first time. Having recently lost its largest grant, Venice Family Clinic was on the verge of closure when Irma Colen joined as a volunteer, in 1978. Irma would go on to create an entire fundraising program— including the Venice Art Walk and Silver Circle— and, by the time of her passing, in 2006, raise an estimated $30 million for Venice Family Clinic. The first Venice Art Walk was held in 1979. By the late 1980s, with the help of volunteers like Sheila Goldberg and artists like Peter Alexander (pictured in 1990), the event was attracting more than 5,000 attendees and raising more than $500,000 annually for Venice Family Clinic. In 1978, shortly after a new volunteer, Irma Colen, had come on board, the potential longevity of Venice Family Clinic came into full view. Under Irma’s masterful guidance, fundraising took off, and by 1982, emboldened in part by the financial success of a new event called the Venice Art Walk, the Clinic’s leadership FORTY YEARS OF BUILDING ON A GOOD THING 2 1970 1978 1979 1983 1988 1990 1993 Venice Family Clinic is established by Founder Phillip Rossman, MD, and Co-Founder Mayer B. The Clinic loses a key research grant and is on the verge of closing until Irma Colen joins as a volunteer and creates the fundraising program. The Venice Art Walk fundraising event is created by Irma Colen, Ruth Bloom, Mona Colman, Marj Fasman, Sheila Goldberg, Elaine Stone, Adele Yellin, and Meg Ross-Price. The Silver Circle fundraising event is created by Irma Colen. The Clinic breaks ground on an 11,000square-foot expansion of 604 Rose Avenue. The expanded facility at 604 Rose Avenue, comprised of the all-new Milken Family Foundation Medical Building and the renovated Irma and Lou Colen Administration Building, opens. Frederick R. The Clinic signs an affiliation agreement with UCLA, wherein UCLA provides malpractice insurance and administrative and in-kind support, and Venice Family Clinic provides training to UCLA residents. For most low-income people on the Westside, Venice Family Clinic is their only means of getting the care they need, when they need it, on an ongoing and affordable basis.” Which is why, over the course of 2010, Venice Family Clinic commenced several bold, new initiatives, including the construction of a new primary care clinic, the Irma Colen Health Center, in Mar Vista; the opening of a new Early Head Start office, in Inglewood; the development of a new dental clinic, in Santa Monica; and the implementation of an electronic health record system. These changes come just in time for the new health reform law, the success of which will rely heavily on providers like Venice Family Clinic to care for the millions of low-income Americans who will soon gain coverage. Indeed, if the law were fully implemented today, the health care system would not have adequate capacity to meet demand from the newly insured. Venice Family Clinic now operates nine sites in Venice, Santa Monica, Mar Vista, Inglewood, and Culver City, serving more than 400 patients per day and over 24,400 annually. In addition to primary care and more than a dozen kinds of specialty care, the Clinic provides dental care, vision care, mental health services, integrative medicine, health education, early childhood development services, even public health insurance enrollment. But despite all that has changed at Venice Family Clinic over the last 40 years, the ethos has remained nearly identical, as evidenced by the staggering level of volunteer involvement. In 2010 alone, nearly 2,100 people donated their time. More than 500 were physicians, from residents to fellows to private specialists. Another 500 showed up on a single weekend in May to help produce the Venice Art Walk & Auctions. Even patients volunteered their time, with some serving on the Patient Advisory Council or the Board of Directors. In March 2010, thanks to a $1 million gift from Lou Colen, Venice Family Clinic opened the 3,400-square-foot Irma Colen Health Center, in Mar Vista, tripling its capacity in the neediest neighborhood on the Westside of Los Angeles County. The Clinic’s Medical Director, Karen Lamp, MD, officially welcomed the community on July 6. In 1994, four years after Founder Phillip Rossman, MD, passed away, Co-Founder Mayer B. Davidson accepted the President’s Volunteer Action Award from President Clinton on behalf of Venice Family Clinic. The Clinic’s volunteer ranks would soon swell past 2,000 annually. “People like me can’t do this kind of thing by themselves,” Dr. Davidson says. “It requires a tremendous amount of collaboration. People’s commitment to this is really remarkable.” began thinking about purchasing a permanent home. In 1983, this new home, at 604 Rose Avenue, just a few blocks from the original site, was dedicated. Dr. Rossman, who is formally recognized as Venice Family Clinic’s Founder, grew ill and passed away in 1990, although he very well may have had an inkling of what the future held for the organization, including expanding east on Rose Avenue, taking over two county clinics—one, ironically, being the site in Santa Monica where Venice residents had protested in 1969— and becoming the largest free clinic in the country, all by the mid-1990s. Looking back on everything that was set in motion back in 1970, Dr. Davidson, who carries the title of Co-Founder, is almost incredulous. “No one anticipated that we would be doing what we’re doing today,” he says. “It feels wonderful, in a way. But it’s sad that 4 Dr. Davidson personifies that commitment. He still eagerly volunteers on the first Wednesday of every month, running an endocrinology clinic at the Milken Family Foundation Medical Building, in Venice. “It will sound silly, but it’s still true,” he says. “What I get out of it is that it’s the right thing to do and I feel good doing it. That hasn’t changed.” In 2004, thanks to a gift from longtime supporter Chuck Lorre, renovations to the former L.A. County clinic in Venice are completed. The site is renamed the Robert Levine Family Health Center, in honor of Lorre’s late father. In 2006, with seed funding provided by The Simms/Mann Family Foundation, Venice Family Clinic launched the Simms/Mann Health and Wellness Center— the nation’s first health, wellness, and integrative medicine program offered at a free clinic—on the site of a former L.A. County clinic, in Santa Monica. County Department of Health Services is on the verge of bankruptcy and plans to close two Westside clinics, in Venice and Santa Monica. Venice Family Clinic takes them over. The Clinic begins providing on-site care to students at Santa Monica High School. The Clinic begins providing on-site care to residents of the Mar Vista Gardens public housing development. The former L.A. County clinic in Venice is renamed the Robert Levine Family Health Center, in honor of supporter Chuck Lorre’s father, and reopens after a year-long renovation. The former L.A. County clinic in Santa Monica is renovated and reopened as the Simms/Mann Health and Wellness Center, the nation’s first health, wellness, and integrative medicine center at a free clinic. The Clinic begins providing care to homeless people out of two exam rooms at the OPCC Annenberg Access Center, in Santa Monica. Venice Family Clinic opens an Early Head Start office in Inglewood, its first site outside of the Westside. The Clinic takes over medical operations at the Culver City Youth Health Center for students of Culver City High School and Middle School. The Clinic opens the Irma Colen Health Center, tripling its capacity in Mar Vista. The Clinic begins implementation of an electronic health record system. The Clinic begins construction of a new dental clinic in Santa Monica, within the Simms/Mann Health and Wellness Center. 5 YEARS YEARS 40 40 Dorena’s baby is due in December. Visiting Venice Family Clinic brings Dorena Villanueva, 27, full circle in her quest to find the health care she needs. She had been a patient of the Clinic as a child, so when she found out she was pregnant with her first baby, in early 2010, she knew just where to turn for help. Since then, Dorena has enjoyed the attention of a team of clinicians, coordinators, and health educators, providing everything she needs to deliver safely—from an initial risk assessment and regular screenings for anemia, diabetes, and genetic abnormalities to intensive health education and even multivitamins—and all she wants to know about having a baby. Not that she wants to know absolutely everything. At a recent visit with volunteer Erica Oberman, MD, a UCLA OB/GYN resident, Dorena declined to learn the baby’s sex, preferring to wait until her delivery day. “I like Isaac for a boy,” she says. “If it’s a girl, we’ll call her Melrose.” Shirley Cooper, 60, knows all too well the dangers of being homeless, and she has the scars to prove it. Several years ago, she had to have the big toe on her right foot amputated as a result of an infection. Shirley’s recovery depends on bed rest. So when she recently visited the emergency room at Santa Monica-UCLA Medical Center & Orthopaedic Hospital for a deep cut on the big toe of her left foot, she was thrilled when her doctor discharged her not to the street but the respite care program at Samoshel, a shelter operated by homeless service agency OPCC, in Santa Monica. As part of the program—which is a partnership between Venice Family Clinic, OPCC, and two local hospitals, aimed at preventing unnecessary hospital readmissions—Shirley enjoys her own bed and help from her OPCC case manager in finding permanent housing, plus easy access to medical care provided five days a week by a Venice Family Clinic medical team. “This has been a place for me to heal,” Shirley says, referring not just to her conditions but also to her outlook on life. “I’ve been in a dark place, but I can see a little light now.” 6 7 YEARS YEARS 40 40 Approximately ten percent of Venice Family Clinic’s 24,400 patients are diabetic, and helping them manage their disease requires a combination of prescription medications, health education, and regular screenings for complications. It also requires the help of volunteers, such as the team with the UCLA Mobile Eye Clinic, which visits Venice Family Clinic’s Milken Family Foundation Medical Building, in Venice, on the first Wednesday of every month. “If something is going on in the patient’s eyes, it’s probably going on elsewhere in their body,” explains ophthalmologist Lawrence Hopp, MD, who has been with the UCLA Mobile Eye Clinic since 1993 and has seen thousands of Clinic patients. “If they don’t keep it under control, they can go blind.” It’s been a year since Robert’s last eye exam. Robert Repaich, 63, was one of a dozen patients Dr. Hopp examined on a recent Wednesday. Robert has been a patient of the Clinic for three years and has been diabetic for the last ten. “You’re doing fine,” Dr. Hopp says. “Let us know if you’re having any blurred vision or other changes. Otherwise, we’ll see you back here in one year.” Since losing his employer-sponsored health insurance two years ago, Mercer Lawrence, 40, has often been tempted to put off getting the care he needs. He even tried for a couple months recently to live with a persistent pain in his jaw, until his abscessed tooth became too bad to ignore. Mercer puts his pain at 8 out of 10. 8 “Eventually, I had a big bump on my cheek, something totally foreign, and the pain got so bad that I couldn’t sleep at night,” Mercer says, after a recent visit with Venice Family Clinic’s staff dentist, Nicole ThompsonMarvel, DDS. “But I felt better right after having that tooth extracted.” Still, there are thousands of people on the Westside like Mercer—in fact, some have not seen a dentist in decades—and dental care is now widely considered the most urgent unmet health need among the low-income uninsured. So in 2010, Venice Family Clinic also began construction of a new 1,800-square-foot dental clinic, with room for a second staff dentist and 1,500 patients annually, double the current level. 9 YEARS YEARS 40 40 Specialty care is among the most difficult services to access for the low-income and uninsured. Often, the only affordable option is a County hospital, where wait times can be six months for some conditions and over a year for others. Dr. Padilla needs a second opinion. 10 But thanks to a new partnership, funded by Kaiser Permanente, primary care doctors from Venice Family Clinic and two other area clinics are being trained to provide basic care for conditions that normally would have to be referred to a County specialty physician. And when they need advice on the spot, they have access to phone consultations with volunteer specialists. Miriam Padilla, MD, a staff internist and the appointed cardiology champion at Venice Family Clinic, recently had a patient in his forties who presented with a cough and chest pain. “Something wasn’t right,” Dr. Padilla says. “Chest pain can be caused by a persistent cough, but his EKG results were consistent with a heart attack.” Dr. Padilla called her mentor in the program, William French, MD, a cardiologist and Director of the Cardiac Catheterization Lab at the County’s Harbor-UCLA Medical Center. After quickly reviewing the patient’s chart, Dr. French confirmed Dr. Padilla’s diagnosis, fast-tracked the patient once he arrived at the emergency room, and performed a catheterization just minutes later. “A simple phone consult can save a patient’s life,” Dr. Padilla explains. “And in situations where we can rule out an emergency or even the need for a referral, it saves precious resources at the County hospital and opens up spots for the neediest patients.” 11 YEARS YEARS 40 40 Six months ago, as Valeria Castro was reaching her first birthday, her mother, Elvia Perez, noticed something unusual. Every day matters at Valeria’s age. “I was worried about her not being able to crawl or walk,” Elvia explains, noting that her three older children had reached those milestones sooner. “She seemed to be taking a little longer than usual.” Around the same time, Elvia learned that Venice Family Clinic had opened a new early childhood development office not far from her home, in Inglewood. Soon after she enrolled Valeria in the program—known nationally as Early Head Start—they were referred to the Westside Regional Center for physical therapy. In addition to screening children for developmental delays, staff in the Inglewood office also make home visits, host play groups to ensure adequate socialization, and connect families with the Clinic’s medical services. “The whole point is early intervention, because zero to three is such a critical age,” explains Emory Chen, the office’s Home-Based Education Manager, at one of the recent play groups. “It’s great to see Valeria getting around the way she does now. I’m happy we have her for another year and a half.” Patty and Martin Hernandez, both 35, have been visiting Venice Family Clinic for medical care for the last 11 years, but recently found themselves needing a different kind of help. “He was unfaithful to me,” Patty explains. “I asked him to leave the house, but he didn’t want to leave me or the children, and he said he was willing to go to therapy.” So for the last two months, they have been seeing Steve Artiga, LCSW, one of the Clinic’s five staff social workers, every Tuesday afternoon. Already, each is quick to note the differences they see in themselves—Martin having learned to listen to Patty more and pay attention to her needs, and Patty having gotten her anger under control. But that’s not all that happened. Following are several additional accomplishments that made 2010 a historic year at Venice Family Clinic.
