Many of my patients come to me seeking help with the changes that can occur after childbirth. Following childbirth, a woman’s breast can grow to uncomfortable proportions or just the opposite can happen. A woman’s breast can actually lose volume and shrink, resulting in the breast appearing “deflated.” Additionally, a large number of women come to me seeking help with the post partum changes of their abdomen. During pregnancy the skin and abdominal wall muscles are stretched. Following childbirth, the abdomen can protrude and the skin can be loose or sag. In some cases, the abdominal muscles can be so weakened that the individual may look like she is still pregnant. Despite daily workouts including sit ups and crunches, a tummy tuck may be needed to restore these muscles. ing breast reduction. Both groups of women want to have breasts that are proportional to their body size with the most natural result possible. In certain situations, a breast lift is also needed to tighten lax skin. The laxity can be the result of pregnancy or weight loss. When a breast lift is needed, I utilize the lollipop scar technique. A breast lift procedure is very similar to a breast reduction. Whether kissing the cheek, kissing the hand, or a romantic kiss, there is a highly ritualized set of rules that dictate how each should be done. “The French seem to place great importance on romance — the prelude — than Americans do… the French are more inclined to mystery, the Americans to mechanics,” writes the author. Even the approach to business is radically different in the two countries. While Americans use rationalization to sell products, the French create an emotional bond with the customer. The French value beauty in everything from elaborate cakes to breathtaking landscapes to precise handwriting, and have the ability to “make the most banal things beautiful.” Schoolchildren use fountain pens to write and buildings are illuminated in such a way as to emphasize their curves and angles. So when French men call out compliments to women on the street, they believe they are merely pointing out something of beauty, and the women feel flattered. In the U.S., this would be viewed as an invasion of privacy at best and sexual harassment at worst, especially in the workplace. The two countries possess differing attitudes toward infidelity. Americans see it as a betrayal while the French view it as an acceptable way to hold a marriage together when the romance in that marriage has faded. When the Monica Lewinsky scandal broke, the French were appalled, not that Clinton had an affair, but that he chose to do it with someone who they believed lacked beauty and elegance. In a book published in France in 2009, a female psychologist suggests that infidelity is natural for men and offers an “honor code” for the loyal but unfaithful husband. Must pleasure be sacrificed at the expense of loyalty? As Ms. Sciolino points out, the answers from advice columnists in the two countries would be vastly different! The release of the book coincided with two prominent U.S. sex scandals: Anthony Weiner and Dominique Strauss-Kahn. Ms. Sciolino was sought out for comments in television, radio, and print. On a PBS Newshour interview, she illuminated the differences between French and American attitudes toward extramarital affairs in light of the Strauss-Kahn arrest. “Frankly, it’s not a liability for a politician to have had a lot of extramarital affairs,” she explained. “In France, the fact that Dominique Strauss-Kahn was rumored to have had a very active extramarital sex life wouldn’t have been a factor in him being elected president. What are the worst fast foods? Whether you’re trying to lose weight, get in shape for swimsuit season or just brush up on your nutrition knowledge and healthy up your diet, we’ve got you covered. Your most pressing problems solved. 1. Is there one small diet fix that will help me lose weight? “Yes. Stop drinking your calories,” says Walter Willett, M.D., chairman of the nutrition department at the Harvard School of Public Health and coauthor of Eat, Drink and Be Healthy. Fruit juices, coffee drinks and regular soda are liquid calories that don’t yield much satiety, says Dr. Willett. A 12-ounce regular cola contains 110 calories; the same amount of orange juice has 140. “Research also shows that we don’t compensate as well for calories consumed in liquid form,” says Rachel K. To stay satiated, she recommends eating three similarly caloric meals and one or more planned snacks daily. If the craving lingers after making this alteration, it may be emotionally based. Try distracting yourself for 20 minutes (the usual length of a craving) by chatting on the phone or taking a walk around the block, says Lawrence Cheskin, M.D., director of the Johns Hopkins Weight Management Center in Lutherville, Maryland. If distraction doesn’t do the trick, Linda Spangle, author of 100 Days of Weight Loss, says you might have “head hunger.” Identify the food you’re yearning for: Is it chewy/crunchy or smooth/creamy? If it’s the former, there’s a good chance that you’re angry, anxious, frustrated, stressed or resentful. Spangle associates a yen for smooth/creamy foods with “empty emotions” like loneliness, sadness or a lack of recognition. It couldn’t hurt. A high-sodium habit has also been associated with osteoporosis; taking in more than 3,000 milligrams daily may interfere with calcium absorption, which can weaken bones over time. And according to the National Heart, Lung and Blood Institute, most women consume 15 to 40 percent more more than the recommended 2,400 milligrams of sodium per day (the equivalent of about one teaspoon of table salt). To limit your intake, cut down on popcorn and cookies, frozen dinners, canned soups and broths and bottled salad dressing. 5. My triglycerides were high at my last checkup. How can I lower them? Triglycerides are simply a form of fat. “They are as important as high cholesterol (another type of fat) in women,” says Noel Bairey Merz, M.D., medical director of the Women’s Heart Center at Cedars-Sinai Medical Center in Los Angeles. Left unregulated, these fats can contribute to atherosclerosis, a condition in which artery walls become thicker and less elastic. When a blood test indicates that triglyceride levels are high (over 150 mg/dl), it’s a tip-off that they’re hovering in the bloodstream instead of being stored in the body for energy. Instead, try to concentrate on eating more low-energy-dense foods (fruits, vegetables, broth-based soups, salads and whole-grain cereals with low-fat milk) and you’ll fill up on fewer calories. 10. Are there any “super” foods I can have that aid weight loss? “Dairy foods like skim milk and low-fat yogurt top the list,” says Michael Zemel, Ph.D., director of the Nutrition Institute at the University of Tennessee. In a study of 34 otherwise healthy obese adults, those who consumed three servings of light yogurt (for a total of 1,100 milligrams of calcium) daily lost 22 percent more weight and 61 percent more body fat than those on a low-dairy (500 milligrams of calcium) diet. Zemel explains that a high-calcium diet — 1,200 milligrams per day — inhibits the production of calcitriol, a hormone that “tells” cells to generate more fat. Conversely, more calcitriol is released on a low-calcium diet, resulting in bigger, plumper fat cells. But my hometown is very beautiful, and sometimes you take it for granted,” said the former Dancing with the Stars competitor.“Garden City is the kind of town that is multi generational. In fact, my daughter has moved back. So did many of their school friends and many of my school friends. Perhaps, because it made some women feel better about becoming the big 5-0, it’s rarely been debated. However, my personal experience proves the 50/40 fable to be no more than a dream. Fifty is what it is and, for me, it hasn’t been enjoyable. During my 40’s I had a svelte figure, no cholesterol or blood pressure medications, no bifocals, no cataracts and no gray in my hair. I even got whistled at – and not just by lifeguards who wanted me to move into shallower water. Then I turned 50. Along with a slew of cards that were meant to humorously remind me that my life was more than half over, came the invitation to join AARP. Solicitations from plastic surgeons and requests to attend Botox parties filled my mailbox. Lawyers send questionnaires advising my need for a will. A local cemetery offered me a time-payment plan on a mausoleum. My car and life insurance rates jumped. A twenty-something year old hair stylist suggest I lighten my hair in an effort to not bring so much attention to my emerging crow’s feet. I fired her. At 50 I became in medical terms – a woman of your age, as in “a woman of your age needs to address certain health issues that may, or may not, effect you.” Colonoscopy, bone density and stress tests were recommended. I was warned to watch my salt and sugar intake. An exercise safer that possible bone breaking roller-skating was suggested. An arsenal of pills – calcium, fish oil, vitamins – were added to my daily routine. Dental problems arose. During my youth – prior to age 50 – I occasionally had a cavity. Now my gums are receding and one tooth even became loose. Expensive periodontal work became necessary. Routine dental appointments are now scheduled every 6 months instead of once a year. Menopause arrived. Some women accept it as the bridge of change that will lead to a better second part of a female life. I’m not one of those women. Menopause is hell. Pure hell. I do not feel renewed, refreshed. I’m tired, sweaty, bloated, cranky – all the time! I gained 20 pounds in one year despite eating the same food and same size portions now as I did in my 40’s. Diets haven’t I’m regarded as a cougar despite the fact I have the same interest in dating a younger man as I do in undergoing another colonoscopy. It’s all about the hormones, or lack of them. The boobs I prayed for at 14 arrived now when I have no need for the cleavage. Their additional weight only adds stress to a back already beginning to curve. I have nightmares about being able to play the part of Igor, sans special effects, in the next Frankenstein movie. Mood swings abound to the point that I sometimes wonder if I’ve suddenly become bipolar. I have no sense of humor, especially when somebody jokingly calls my hot flashes “Jackie’s little power surges.” Prior to entering the state of menopause I thought the legal defense of “menopause made her do it” to be a sham. Now I don’t consider this defense to be farfetched at all. Same with road rage. I have a new understanding of why some people do what they do. Nothing irks me more that to be dangerously cut off by a group of teens who shout “Hey Granny!” at me as they speed away. Socially, people view women at age 50 differently. I’m regarded as a cougar despite the fact I have the same interest in dating a younger man as I do in undergoing another colonoscopy. I’ve even had the displeasure of walking into a bar and, upon being noticed, the band halted a Kings of Leon song and substituted a classic rock song. Delivery men automatically refer to me Ma’am instead of Miss. I’m offered a senior citizen meal discount by young waitresses and waiters despite the fact I didn’t request it. If I stop to flip through a clothing rack in the teen section of any department store a salesperson will quickly approach to redirect me to the Misses section. That’a insulting! I am well aware I’m not 18, but there are still some items I can purchase from this section and wear without looking like a teen-wanna-be. While I’m on the subject of clothing – I don’t want to dress like my 88 year old mother! Yet the areas I’m referred to appeal to her age group, not mine. Serviceable is the descriptive word that comes to mind. Is there no place for the mature woman who is fighting to retain her former fashionable sense of style? Designers take note! In summation: If I had the necessary finances I’d have myself sucked, tucked, pulled and stitched into a Barbie doll formation makeover. However, since I’ve never won even a single lottery dollar, my body re-sculpting dream isn’t haaprning in the near future. So I accept who I am; a 50 year old woman who, despite age related woes and stereotypical prejudices, wakes up each morning grateful to be alive. And just a little whipped crème – a personal favorite that reminded me of Mom. So I had another. And the next day, another. That 2005 pumpkin pie seemed to open a floodgate of change. Before I knew it Christmas was over, and I was outdoors again, coaching my kids’ soccer teams. But somehow my coaching pants had gotten a little tight. Now, they were nylon – and I know nylon doesn’t shrink. Apparently I was on a roll. I surrendered to another few birthdays, another few pounds and eventually to the next size up. Change had happened. Or changes. I mean life was full and good, but I looked fuller than I wanted. However, I was busy raising children, running a house and working full time. I thought, “Well, it was a nice run.” Until last January. My co-anchor Elizabeth Hashagen had an idea. (Inside info – she’s also been a fitness instructor for almost 20 years.) She thought it would be great if we went to the gym every morning when we got off the air. I thought it would be great if she looked in the mirror and realized she was 3 months pregnant! But it was high school all over again. Peer-pressure. I signed up for “a trial” the next day. It amazes me what you can do, especially if you draft a work-out buddy. I went from avoiding a gym for 20 years, to working out 3 to 5 days a week, with some hardcore spin classes. It only took 3 weeks before I felt different – better. But guess what? By September I had gained 3 pounds! Elizabeth called it muscle. I called it muscle in her head. So nine months after Elizabeth’s pseudo New Years resolution for me, in September of 2011, I faced the grim reality that I also had to watch what I ate. I’ve been an organic vegetarian since my twenties, but I’d never limited my intake. In fact I’m the girl who’s taught other families how to celebrate “Ice Cream for Dinner.” Seriously. It’s taken about a year, but I’m back, and a dozen pounds lighter. I hate to admit “Those People” are right when they say it’s about tweaking your lifestyle. More activity and less junk equal less junk in your trunk. I have more energy than ever. I feel great!
