Since 1895, these have served as cornerstones for the agencies of Home Health Foundation. In part, they are held firmly in place and built upon through the generosity of philanthropic support. Support that ensures our services remain available to all, regardless of ability to pay or the effort needed to deliver them. Today, our three not-for-profit agencies, Home Health VNA, Merrimack Valley Hospice and HomeCare, Inc., are bringing expert health, hospice and home care to patients and their families in the places they call home. Much has changed in the past 120 years when our nurses traveled on foot or bicycle to care for those working in the area mills. Our reach has expanded from Greater Lawrence to more than 110 cities and towns in three states. More than 700 staff now deliver in-home care to patients facing more serious and complex medical needs than ever before. And the level of expertise and types of medical care we now provide were undreamed of even as recently as 20 years ago. Serving the community’s needs has always been our mission; how we do that has advanced by leaps and bounds. Our years of experience give us a strong foundation, with the ability and insight to look forward and develop innovative programs and leading-edge solutions to keep pace with an ever-changing healthcare landscape. And after so many years, we are trusted and known as the experts in the specialty of home care nursing. As time and healthcare has evolved, so have we, providing increasingly multifaceted and intensive care never before offered outside the hospital setting – delivering chemotherapy and life-sustaining IV therapy, navigating complex diagnoses and family dynamics, and connecting specialists outside our agency who are caring for our patients to what is happening in real-time. Within each agency of Home Health Foundation – Home Health VNA, Merrimack Valley Hospice, and HomeCare, Inc. – the many nurses, therapists, counselors and aides provide this complex care as a team to maximize the clinical expertise and help provide exceptional patient care. And leveraging the full continuum of our care across these three service lines, this unique collaboration ensures patients’ needs are addressed efficiently and appropriately. Serving our mission Since 1895 we have dedicated ourselves to serving the healthcare needs of our community. Within that community are patients of varying circumstances, and for 120 years we have honored our mission to provide services to all – regardless of their ability to pay. We are committed to offering support and caring for the underserved, the impoverished and those suffering from physical hardship. We are unique in providing services that many agencies do not offer because they are too complex; require expertise, resources and money to commit to; or may not be well-reimbursed. Yet, these needs are very real and the services critical. As an independent, not-for-profit organization we remain true to our mission by developing and maintaining programs that meet these needs and when no one else can say yes, we say we will be there. We actively manage at risk patients, patients susceptible to set backs and complications, as well as those prone to repeated hospitalizations. And in doing so, can improve the quality of those people’s lives and those who care for them. Our agencies are leading the way for a new model of healthcare in the state and in the nation. We are at the forefront of finding ways to offer the highest quality care at a more affordable cost, with the primary focus on providing a better patient experience. We strive to meet and exceed the growing demands of healthcare today as we look to the future, anticipating and adapting to the changing needs of an increasing patient population throughout Massachusetts, New Hampshire and Maine. Experience. Reliability. Record-setting snowfall in winter 2015 demonstrated the commitment and reliability of our agencies to ensure the needs of our patients were never compromised. With historic shut-downs of roadways and the region stuck under 100+ inches of snow, we continued to deliver care to those in need. Nurses shoveled their way and walked to homes in order to make critical visits despite unplowed roads. We ensured 24-hour home care coverage by sending two qualified staff at the start of the storm so they could provide uninterrupted care for the duration of the blizzard. Preparing patients for the storms and keeping in touch with them throughout enabled us to continue the vital care so many rely on. And inside, our team of schedulers, supervisors and in-house staff coordinated and triaged urgent needs while ensuring staff safety. Our agency never really closes. So blizzard conditions or sunny skies, our patients can count on us daily, whatever the weather. George C. Wadleigh Foundation The George C. Wadleigh Foundation is committed to meeting the needs of the aging population in Greater Haverhill as well as the disadvantaged in the community. Their compassionate reach has extended support to each of our three agencies – Home Health VNA, Merrimack Valley Hospice, and HomeCare, Inc. – for the past 15 years. Their funding in 2015 alone has provided subsidized care for Merrimack Valley Hospice, health clinics to meet the needs of elders through HomeCare, Inc., and technology to improve care delivery of Home Health VNA. With our roots in Haverhill, we are proud to meet the needs of the community through grants by this established and beloved foundation. 5 Ella loves to dance and ride horses. In just a few months her treatment for leukemia will be done and she’ll share her adventures with her nurse Sharon as a young friend, not as a patient. Ella has world-renowned cancer specialists in Boston. Home Health VNA brings specialty care to her New Hampshire home. Over the past 18 months, physical therapists and occupational therapists have helped rebuild Ella’s strength, to get her walking again and regaining physical coordination to play and jump. Ella now only needs to go to the Jimmy Fund Clinic for her care every three weeks, receiving her weekly IV chemotherapy and all her daily tests and checks at home. Sharon watches for potential dangers, changes the course of treatment as the Boston team advises, consults on progress and enjoys a ballet demonstration on good days. Sharon now schedules her nursing visits around Ella’s school day – a welcome change as she returns to being a happy 2nd grader. 6 Home Health VNA. Experience. Caring. Home is the center of our care More than ever, people are living at home with serious medical conditions and complex treatment regimens. Home healthcare is now a specialty of care – with highly trained clinicians, up-to-the-minute plans of care shared across teams, and innovations in treatments all part of what is delivered in the home to thousands of patients in our care every day. How do we do this? With extensive training, communication, innovation in technology and always with compassion. Training Every single clinician of Home Health VNA received skills training and competency testing on all aspects of care delivery and on the latest techniques and technology used in patient care. An agencywide skills workshop brought every member of the medical care team up to date on the current advances, in addition to refreshing standards of practice. Hands-on labs and testing of the most critical and complex treatments mean Home Health VNA staff are prepared for varying patient conditions and treatment options. Our quality measures continue to improve as the focus on best practices across every aspect of our agency is emphasized. Information sharing for preparedness The ability to do this intense level of nursing care in the home is supported by the electronic information shared and referenced via our mobile computing systems that keep every nurse connected to the medication classifications and patient conditions. Before a nurse visits a patient, he or she has key information on the patient, the level of risk they may be facing, and what medication or treatment alerts they need to be prepared for. And seeing the patient in their home, our clinicians are relied upon to offer consistent communication across all the patient’s medical providers as well as those who are helping care for the patient at home. Technology for managing health Leveraging technology is important to be able to extend the reach of our nurses’ expertise and provide specialty care to more people. We are one of the few home health agencies in the nation piloting the new Bundled Payment for Care Improvement initiative for the Centers for Medicare and Medicaid Services. Patients with congestive heart failure typically require close vital sign monitoring to manage their disease. With technology grants in support of telehealth, we are now on the leading edge of healthcare, redesigning the way care is delivered and managed for patients, and improving quality of life. Telehealth equipment in the home allows the patient’s information to be relayed to the telehealth nurses at our offices. It alerts them to any type of warning signs, up-to-the-minute symptom or condition changes that may need intervention, and identifies urgent needs. The telehealth team becomes a core “control center” for keeping up with what is happening with the patient, reaching out to the patient and connecting with their home caregiver or physician as needed to address the problem. Caring for kids One of the few agencies in the state of Massachusetts, we provide expert nursing care to children from newborn to age 18, including those with acute and chronic conditions which may require complex IV therapy and oncology care, wound and ostomy care, cardiac treatments and other intensive forms of care. Our skilled medical teams work with parents and the child at home, where they are most comfortable; and nurses are able to educate the parents on treating the child within the environment in which they spend the most time. Medications that keep the heart pumping Throughout our lives, we take medicine as needed: to help relieve a headache, reduce pain or fight an infection, lower blood pressure, or help treat a medical condition. Patients with serious or complex medical issues, many of whom rely on our home healthcare, may be taking 20 or more different kinds of medications to treat the serious conditions or illnesses they are living with. Some medications are so life-critical that a power outage or shut-down of the pump that delivers it could be life ending. More than 100 types of medications are delivered via intravenous therapy, a form of treatment that was previously only delivered in a hospital setting. Every Home Health VNA nurse is trained in routine IV therapy, with specialized nurses delivering chemotherapy at home. In 2015, nearly 8,000 IV therapy treatments were provided. We are fortunate to have these kinds of treatments available and accessible to patients in their homes, but they require the close monitoring and reliability of a home health care team. This is the role of today’s Home Health VNA clinician and the reason that healthcare relies upon home healthcare. We are that trusted partner in health. 7 “When you stop chasing time, you can start making use of the time you do have for the things that are important to you – the better things in life.” For Barry, turning to hospice means enjoying the scones his wife bakes, joining his friends for walks through the neighborhood on his scooter, and watching his grandsons on the basketball court. In his last course of treatment for cancer, he found himself constantly feeling awful – unable to eat or not tasting what he did eat, so weakened and sick that the extra few months the treatment might bring him were not filled with the “life” he wanted to enjoy. Deciding to set up hospice was a chance to focus on how he wanted to live and arrange the resources that would help him do that. Today, his Merrimack Valley Hospice nurse Lisa visits weekly, scheduling his care around his life. She’s adapted his medication delivery to a time that works best for he and his wife’s daily plans and arranged her nursing visits to fit into his schedule. And she is responsive to Barry’s concerns and needs whenever they arise. They work together to improve the quality of his life, understand his wishes and reach his goals. “Some people wait too long to enjoy the benefits of hospice. I knew I wanted to get the most of life. Deciding on hospice, I chose how I wanted to live.” 8 Merrimack Valley Hospice. Experience. Dignity. Offering quality and comfort Hospice is about comfort and making the most of the time you have in ways that are most meaningful to you. It is having the resources needed – medical care, emotional support, symptom management, and guidance – to help you through an incredibly important time in life. Merrimack Valley Hospice has been creating new hopes and carrying out wishes for comfort, dignity and quality of life since 1980 and in York, Maine since 2013. With this experience, we provide patients and their loved ones with peace of mind and care that supports the needs, questions, and challenges that arise in the later stages of life. Wherever home is, our team of professionals and trained volunteers offer compassion, comfort and caring. Hospice is a specialty, designed to meet physical, emotional and spiritual needs. With expertise and a focus on this unique type of care, we guide people through with compassion and support to help create a better ending to their life’s journey. And when circumstances require a level of medical attention that exceeds the home setting, we can provide the care at Merrimack Valley Hospice House. Finding personal “Goals of care” It isn’t always easy to find the right path to follow when facing the later stages of an aggressive illness or medical condition. When patients begin to consult with Merrimack Valley Hospice, they begin to define what it is that they DO want, and in doing so, create their own terms for how they will live and what they might need to support those goals. Merrimack Valley Hospice provides them with the tools, support and care to reach those very personal goals. With a plan in place, the team is at the ready to ensure the patient and his or her family has exactly what they need along the course of their care. In 2012, York Hospital approached Merrimack Valley Hospice about offering not-for-profit hospice services in southern Maine when the all-volunteer organization Hospice of York was preparing to shut its doors. By fall of 2013, Merrimack Valley Hospice began operating as York Hospital Hospice, keeping the committed volunteers of the community and adding to them, the nursing and medical components of hospice. Now in its third year, York Hospital Hospice provides comprehensive end-of-life care to approximately 400 patients annually, as well as bereavement support and counseling to hundreds of families and caregivers. In addition, more than 28 volunteers provide companionship, respite and specialty services such as Reiki and massage for both patients and caregivers. Supporting hospice care in York Charitable support for the hospice program in the York region has been increasing, as more people find comfort for their loved ones nearing end of life at home or at Merrimack Valley Hospice House when an inpatient setting is needed. It is a privilege to extend our 35 years of experience to the 24 cities and towns in the southern Maine region from Kittery to Waterboro. For more information visit YorkHospitalHospice.org Today’s hospice Hospice care itself has evolved from decades before. In the early days, it was known as “the end” and to accept hospice meant refusing any medical treatment. Today, the philosophy of hospice is to truly make the most of the time available and find ways to help actively improve the quality of life during that time. Open access to medication means that some of the treatments patients are on can continue while they receive hospice care, to allow them to move positively toward their last days. 9 Grateful Family Shares the Experience of Hospice When I met my mother-in-law, Grace, for the first time in 1980, she was stunningly beautiful, welcoming, gracious, kind and confident. Thirty-five years later, my love and admiration for her never wavered. She was strong, confident, and faith-filled, even when she learned of a diagnosis of a terminal illness that would give her only 3-6 months to live. Prior to her illness, my husband and I had limited knowledge of hospice, but Grace had investigated and we scheduled an appointment. It was important for her to maintain her independence and stay in the home she loved as long as she could. Merrimack Valley Hospice enabled that. From the initial meeting until her last breath, this dedicated group of angels took great care of Grace, adjusting her level of care as her needs changed, always treating her with respect and honoring her wishes. During her final days, she entered Merrimack Valley Hospice House and was lovingly cared for – as was the family visiting by her bedside. It’s the little things that mattered – a word of comfort, a gentle caress, brushing her hair and just before she passed, a final grooming and dressing her in her favorite silk pajamas. In my mind, she looked as I remembered her from 35 years earlier – beautiful, her gorgeous hair flowing and dressed to impress as she would meet her maker. Each person’s experience with the loss of a loved one is unique, however, having the support and guidance of the wonderful people with Merrimack Valley Hospice and the Hospice House is something everyone deserves. Karen Pettoruto 10 Merrimack Valley Hospice House. Experience. C omfort. Created for a special time in life’s journey. Most hospice and palliative care is provided to patients in their own homes. But when it’s no longer possible for someone to remain at home for advanced care, Merrimack Valley Hospice House provides a comfortable, welcoming home-like alternative. Patients who may need more intensive pain management for a short time or require the oversight of a physician to help address complex symptoms may turn to the specialized care at the Hospice House and then return home. Merrimack Valley Hospice House is a place that brings trained hospice professionals together – physicians, registered nurses, nursing assistants, chaplains, social workers and volunteers – to provide highly specialized care to patients with advanced illness as well as support to their loved ones. Specialized care in a place like home One of only two inpatient facilities in Massachusetts to care for children as well as adults, Merrimack Valley Hospice House addresses specialized medical needs of those with advanced illness, with compassion and dignity. Patients may come to Merrimack Valley Hospice House after being cared for by a hospice provider at home or in an assisted living or skilled nursing facility. They may not have had any type of hospice care before coming, or perhaps they have been receiving only palliative services. Some families turn to Merrimack Valley Hospice House for respite, a break from care giving. Every patient is unique and an experienced admission team guides the transition to or from the Hospice House. The Compassionate Journey continues The need for subsidized care is ever-present as we accept those requiring inpatient levels of care at Merrimack Valley Hospice House. Our decision to support the caring and unique experience at the Hospice House is a meaningful extension of our desire to help make a difficult time a little easier for all.” Subsidized Care at Merrimack Valley Hospice House Our ability to offer subsidized care to those in financial hardship is possible from the charitable support we receive, including grants from foundations that recognize this ongoing and critical need. In 2015, eight foundations contributed to our subsidized care fund, including: Andover Home for Aged People, Artemas W. Stearns Trust, Eastern Bank Foundation, George C. Wadleigh Foundation, George P. Bishop Foundation, The High Pointe Foundation, Newburyport Society for the Relief of Aged Women and The Rogers Family Foundation. 11 With more than 30 years of experience as a nurse’s aide, Cheryl has seen and done it all, but each day, she arrives to Jack’s home brimming with energy to get him ready for the day. Whether the day holds a monthly gathering of his former colleagues, a walk outdoors with the family dog, or a visit to the doctor, it always includes his daily exercise routine. Cheryl provides in-home personal care, light housekeeping duties and transport to medical appointments with such comfort and ease that it might appear that she is one of the family. But it was less than a year ago that she first came to provide non-medical support and a watchful eye on Jack’s wife Dolores and him. Physical limitations had made his mobility more difficult, so having someone to provide assistance in the family home they enjoyed raising their family in supported their independence and comfort. When Cheryl arrived one morning and noticed Dolores was experiencing a serious health complication, she immediately called for emergency services and assisted the family in making sure Jack was taken care of during his wife’s hospitalization. That level of trust and partnership carried on following Dolores’ peaceful passing at home, and continues today as Cheryl makes sure Jack is healthy, comfortable, and has the details of his home in order. “She is an inspiration to me every day,” says Jack, as he counts out the arm presses he does to stay strong and healthy and remain independent in his home. 12 HomeCare, Inc. Experience. Independence. Experience. Quality. Home health aides are an important part of people’s lives, providing quality, non-medical support to clients to keep them happy and healthy in their home. They are often considered the heart and soul of home care and a true connection to our patients at home. The experience of our agency is backed by the long-standing commitment of our team. There are 62 aides who have more than 10 years of service with our agency, including 32 who have been with us for over 20 years and three who have been caring for those in our community for more than 30 years. This team is reliable, professional, and dedicated to those they serve. Our expertise and experience as an agency, and our reputation for exceptional training of staff, is valued not only by clients. HomeCare, Inc. is frequently consulted by the Massachusetts Home Care Aide Council to assist in establishing training programs for the entire state. The services of HomeCare, Inc. provide more than comfort and convenience. They truly meet the needs of those who require a level of support, monitoring and assistance in their homes. HomeCare, Inc. is part of the continuum of our services, supervised by a nursing team and coordinated with clinical services that someone may have. Maximizing the value of care Patients often qualify for a home health aide along with their skilled nursing care. Additional support is available through Kind Connections, the private pay program of HomeCare, Inc. and offers even greater value with an added level of care. HomeCare, Inc. is a medical model of home care, meaning that our skilled home health aides have extensive training as well as the support of a nursing team. Patients seen by Home Health VNA and Home Care, Inc. often have the same aide providing care throughout the week, scheduled as they need and coordinated with the services they are receiving. Aides report changes in patient conditions, concerns and issues promptly and can connect with the medical team seamlessly. Partner in caring for seniors Meeting the needs of an aging population, HomeCare, Inc. provides screenings, health clinics, and education and wellness programs in many local communities. We partner with councils on aging and elder services organizations to ensure that assistance for those living in their own homes is available, keeping people in the communities they are a part of. Keeping seniors active and healthy helps our community to stay strong. While we are here for people when they need health care, they also value the help we provide in keeping them healthy! We are the extra hands to stay independent Our home care team may be an additional, supportive component of the nursing or hospice care a patient is receiving, offering assistance between medical visits. They may be caring for one person or two in the household as needed. HomeCare, Inc.’s paraprofessional team of care attendants, home makers and aides help ensure the safety and independence of those living at home. HomeCare, Inc. offers daily living support, makes wellness checks, and provides a set of helping hands to allow clients to remain in their homes. Providing monthly wellness seminars the Atkinson (NH) Community Center, seniors learn about topics such as heart health, nutrition, stress management, and fall prevention. Andover Home for Aged People Since 2004, the Andover Home for Aged People has funded care that meets the needs of vulnerable seniors and low-income residents in the Town of Andover. Programs made possible by their generosity have included subsidized care at the Hospice House, foot health clinics for low-income seniors, the original construction and subsequent expansion of Merrimack Valley Hospice House. Their continued generosity toward the agencies of Home Health Foundation ensures that all residents of their community are cared for by the services provided in the places they call home. 13 Experience. C ommunity. Benefit Events 1 We host two major fundraising events annually to help financially support our ability to provide charity care as well as patient and family services that are not covered by insurance. The Legacy of Leading, a dinner event held in April, pays tribute to those who support the health and wellbeing of our community as well as the mission of our combined agencies. Our Food, Wine and Beer Pairing and Silent Auction is a fun-filled event each September featuring a festive theme-based atmosphere, activities and cuisine along with a silent auction and raffles to benefit Merrimack Valley Hospice. There are many ways members of the community help to support the incredible work of our agencies and the many people we serve. Sharing time, talent and resources, our supporters – individuals, organizations, and businesses – come together during the year for special events, as volunteers, or in their own fundraising efforts. Through the dedication of people just like you who want to make a positive difference in the lives of others, we are able to continue our mission of caring for all in the places they call home. The support of local businesses, through event sponsorship, is integral to helping us fund innovative programs, maintain quality care and ensure the wellbeing of our community. Volunteering Volunteering comes in many forms – as time spent with hospice patients and their families, assisting at the Hospice House, helping at events, sewing keepsake memories for someone who has lost a loved one, or checking in with patients by phone. Volunteers are important to our work and help extend the range of support to the greater community. Some volunteers were helped by Merrimack Valley Hospice or Home Health VNA and want to help others facing the challenges they overcame. Others are drawn to caring for those in their community and want to share their gift of time and talent, where they can see the difference they make in the lives of others. Volunteers are needed for all regions served including northeastern Massachusetts, southern New Hampshire and the York, Maine area. Friends of Merrimack Valley Hospice This auxiliary group of volunteers was created in 2014 by a passionate group of individuals touched by the care of Merrimack Valley Hospice and Merrimack Valley Hospice House. They help to raise awareness, support events held by the agency, as well as host their annual Spring Brunch fundraiser to raise proceeds in support of Merrimack Valley Hospice House and other services for hospice patients and their families. Mobile computing and real-time electronic information is vital to coordinate the care provided by the many members of every patient’s care team. In 2015, Home Health Foundation was able to purchase 500 Apple iPads to replace the previously leased EMR hardware and commence activation and roll-out. The new technology is smaller and lighter for transport, more energy efficient, and has better screen resolution. With military-grade cases, keyboards, and screen protectors, the investment is well protected. FINANCIAL Be financially successful to advance the mission and vision of our organization. 16 Historic Town Hall Meeting for staff In October, we hosted the first ever Town Hall Meeting of all agency staff. The executive management team shared the vision and goals for our future and unveiled the Pillars of Excellence. It was also an opportunity for all staff to ask questions, share ideas and interact with one another. With more than 80% of our employees working remotely, bringing 520 of them together in two sessions for this historic meeting was a unique opportunity. Our staff is behind this long history of caring and is truly proud to help us confidently and together move into the next 120 years! 2015 Highlights Our ability to accept all patients, regardless of the complexity of their care or their ability to pay, is possible through the charitable support we receive from our donors who allow us to uphold our mission of caring. The generosity of our donors ensures that we will remain a trusted resource for many years to come. Charity Care: $2,820,739 Charity care is provided under the organization’s established sliding fee scale policy. It includes the deficit created by unreimbursed and unpaid costs of services provided in patients’ own residences as well as at Merrimack Valley Hospice House. Mortimer 25 Celebrating 120 years of caring for our community 1918 1933 The Spanish Influenza epidemic hits Boston and spreads throughout Massachusetts. Visiting nurses are overwhelmed with thousands of new patients. During the Great Depression the Federal Government reduces the work schedule of all nurses to 8 hours a day/48 hours a week from 12 hours day/72 hours a week. Also provides funds for nurses to provide care in the home to recipients on employment relief. 1922 The average cost of an automobile was $2,000. Most nurses walked or traveled by bicycle to patient homes. 1895 Lawrence General Hospital adds a district nursing program to its school of nursing and nurses begin caring for the sick children of local mill working families. 1940 World War II creates a need for visiting nurses to provide care to the wives and children of servicemen in active duty. 1917 Nurses volunteer for the war effort and sail for Europe creating a shortage of visiting nurses nationally and locally. 1910 The Massachusetts Board of Registration authorizes licensure by examination of Registered Nurses. Applicants must be 21 years of age and of good moral character. Fee is $5.00. 1914 The fee for a nursing visit is 75 cents for those who can pay. 1946 Nurses worked a 48 hour work week at an average nurse salary of $175.00 per month or $2,100.00 annually. Our offices in York, Maine are the most recent in a history of ensuring continuity of not-for-profit home health and hospice care, ensuring continued mission-driven care for the 110+ towns we now serve. 26 2000 2012 Home Health VNA acquires VNA of Greater Newburyport, formerly a program of Anna Jaques Hospital. 1984 1986 GLHH&NS reorganizes, changes their name to Visiting Nurse Association of Greater Lawrence, creates the parent company Home Health Foundation and establishes HomeCare, Inc. to provide in-home support services. Visiting Nurse Association of Greater Lawrence renamed VNA HomeCare, Inc. 1992 1968 Home Health Visiting Nurse Services of Northern Essex, Inc. is renamed to Home Health VNA. Home Health VNA is established in Haverhill with seed money from the United Fund, later known as the United Way. 1957 The cost of a nursing visit increases to $3.50 with an additional $1.00 for care for an additional person in the same house. 1965 Social Security establishes Medicare and Medicaid and extends home health care coverage to Americans over 65. 1979 Lawrence General Hospital Visiting Nurse Department separates from the Hospital and the agency is named Greater Lawrence Home Health & Nursing Services (GLHH&NS). 1980 Hospice of Greater Lawrence is formally organized. Robert Klie is elected President of the Board. Hospice of Greater Lawrence, an all volunteer organization, merges with GLHH&NS becoming a division of the agency. 2001 Home Health VNA goes online with the first electronic medical record in the region to keep pace with emerging clinical demands and the complex needs of patients. 2005 1988 1983 Merrimack Valley Hospice partners with York Hospital to operate as York Hospital Hospice opening an office in York, Maine and serving 13 southern communities. Home Health VNA acquires Staff Builders in Peabody and Overlook VNA in Chelmsford. Hospice of Greater Lawrence changes its name to Merrimack Valley Hospice and is separately incorporated under the Home Health Foundation. 2009 Merrimack Valley Hospice House opens as the first 14 suite hospice residence in the region. Home Health VNA merges with Seacoast VNA in North Hampton. 1995 2013 VNA HomeCare merges with Home Health VNA of Haverhill and an office is retained on Merrimack Street. 1999 Home Health Foundation consolidates offices and moves to 360 Merrimack Street in Lawrence. Home Health VNA acquires Commonwealth Care in Lowell. Merrimack Valley Hospice House expands to 21 suites. 2015 The agencies celebrate 120 years! Vehicle Donations Donate for Charity, our recycling partner, offers an opportunity to support our agencies by donating a car, truck, van, boat, RV, or motorcycle by calling 800-933-5593. Donors receive a tax deduction of the auction sale price of the vehicle or $500, whichever is the greater amount. Income-generating Gifts You can also use cash and appreciated property to establish a charitable remainder trust or charitable gift annuity that will provide income and tax benefits to you, while benefiting one of Home Health Foundation’s agencies. Bequests A simple way to support is through a bequest in a will or living trust. Any part, percentage, or component of estate or trust assets can be designated to Home Health Foundation or one of its agencies and will help to reduce inheritance taxes.
