We’ve also faced some immense challenges—continuing to care for a growing population
with multiple chronic health conditions, such as diabetes and heart disease, current space
limitations at Duchesne and uncertain funding. Without a doubt, providing healthcare to
those most vulnerable continues to change and evolve.
As we move forward, our mission, values and focus on the patient will not change.
Saint Vincent and Duchesne Clinics are blessed in so many ways. Our donor and volunteer
support has profound, transformative power. In 2014, philanthropy provided 98 percent
of the Clinics’ net operating income. These gifts bring health, hope and opportunity.
We are forever thankful for our generous donors.
Through partnerships with visionary donors and the community, we are building a stronger,
sustainable future. To better meet patient and community needs, the Board of Directors is
exploring future collaboration opportunities. The end goal is to ensure better access to care.
In addition, there is renewed emphasis on the importance of continuing staff education and
training. As always, we strive to be good stewards of our resources, ensuring a healthy future.
We could never accomplish all we do without your faith in us. Your unwavering belief in

In Action
For 56 year old Susana Rodriguez Santos,
life hasn’t been easy.
With little education and no job, Susana lives rent free with
a church friend. She relies on the generosity of nearby food
kitchens and help from her son to cover medication costs.
The biggest challenge, however, is coping with diabetes, a
chronic disease that requires careful management through
exercise, nutrition and medication.
Since arriving to Duchesne Clinic two years ago, the care team
addressed her depression, high cholesterol and diabetes. And,
thanks to a recent partnership between Duchesne Clinic and
Riverview Health Services, Susana attends Standing Strong with
Diabetes, a free series of weekly diabetes self-management
education classes offered in Spanish. The opportunity to learn
about meal planning, monitoring, taking medication, reducing
risks, coping and problem solving skills has made all the
difference in Susana’s life.
Today, Susana sports a smile, oozes energy, volunteers
at the Salvation Army, and best of all, her A1C
(which is a measure of long-term blood
sugar levels) has dropped. She is ever
grateful to the clinic’s dedicated
care team—from the front door
receptionist to her nurse and
physician to the lab staff—
for making her feel welcome
and supported on her road
to health, hope and healing.

Breathing is something most of us take
for granted, but if you are diagnosed with
Chronic Obstructive Pulmonary Disease,
commonly known as (COPD), it can be
scary and unsettling.
For Frances Love, a Saint Vincent Clinic patient, this is just one
of her chronic health conditions the staff is addressing. Frances’
road to health began with a thorough head-to-toe examination
and a review of her medications. It was also discovered that
she has borderline diabetes. To tackle these health concerns,
her care providers Stephanie Kimbrel, ARNP, and Stephanie
Spacek, RN, encouraged regular checkups, exercising, good
nutrition and a “lung friendly” work and home environment.
And it is paying off.
“I had the opportunity to improve my health, and I listened to
my doctor’s advice,” Frances says. “I found a new job that’s free
from dust, cold and mold. At Saint Vincent, they really
care for my health and for me. It’s given me hope
for a healthier future.”
Today, you can see Frances walking all over
Leavenworth—to and from work and around
the indoor track and outdoor trail at the
Riverfront Community Center—with renewed
energy and a smile on her face. Best of all,
she’s breathing easier.

Although a fall at home didn’t land Bruce Johnson
in the emergency room—since he refused to go—it did alert
his cousin, Broderick Crawford, that something was amiss.
After a bit of prodding, Bruce agreed to bring Broderick to his next appointment at Duchesne Clinic.
Months before, Bruce—facing loss of employment
and health insurance along with a growing feeling
of hopelessness—arrived in a wheel chair at
Duchesne’s doors looking for health and hope.
Bruce’s doctor, Nancy Mikhaeel, MD, listened
and questioned him on his medical and life history.
The most obvious health concerns were diabetes,
high blood pressure and elevated cholesterol
levels, which were all addressed through
medication. Within weeks, his whole outlook
on life changed. He wanted to live a quality life,
and he felt human again.
But a vital piece of information was never shared with the clinic
until Broderick brought it to light during Bruce’s next appointment.
At one time, Bruce was a fully employed accountant with a good
salary and a beautiful home. This sudden life change was enough for
the doctor to probe further. The findings from a CT scan helped piece
together another mystery. Bruce had suffered from six mini strokes.
This discovery led to a shift to his treatment plan.
A referral was made to KU Neurology and thanks
to the intervention of Bruce’s cousin, he is now on
disability. In addition, he qualifies for KanCare
insurance, which allows him to receive healthcare
at a larger primary care clinic. With physical
therapy and the right medications, he’s now
walking with a cane and on his way to recovery.
Without a doubt, family plays a pivotal role in our
patients’ care as well as our community health
partners. It’s with this team approach that the
opportunity for health and hope prevails.

Collaboration offers
Duchesne launches
Metabolic Syndrome Pilot Project
Metabolic syndrome is a health condition that everyone’s
talking about. According to the American Heart Association,
47 million Americans have it. That’s roughly a staggering
one out of every six people. And this syndrome is more
common among Hispanics, African-Americans, Asians and
Native Americans, runs in families and increases with age.
With Duchesne’s large Hispanic and older adult population,
our caregiving team is excited to partner with Riverview
Health Services to help reduce these risk factors to
improve health outcomes. Thanks to funding provided by
the William G. McGowan Charitable Fund, the United Way
of Wyandotte County, Unified Government and Hollywood
Casino, Duchesne patients—who meet the risk factors—
are invited to participate in a comprehensive education
and peer support program.
The program, facilitated by Melinda Krautmann Borelli,
MS, BSN, RN, who is bilingual, provides education and
support to patients by promoting healthy lifestyle
strategies. The program is offered in a small group setting
and in Spanish. Individual appointments are available for
Hope & Healing
English speaking patients and for those who are better
suited for individual education.
“Metabolic syndrome is not a disease,” Melinda says.
“Instead, it’s a group of risk factors—high blood pressure,
high blood sugar, unhealthy cholesterol levels and
abdominal fat. When combined, they set the stage for
serious health problems—doubling the risk of blood vessel and heart disease, which can lead to heart attacks and strokes and increasing the risk for diabetes by five.
The good news is that metabolic syndrome can be
controlled, largely with changes to your lifestyle.”
Patients learn skills such as meal planning and managing
portions, low impact physical activities and stress
management to achieve better health outcomes.
“It is so rewarding to see our patients take control of their
health and the impact it is already making in their lives and
the lives of their family members,” Melinda adds. “They
now understand what their numbers (blood sugar, blood
pressure, A1C, lipids, etc.) mean and the positive impact they
have on their own health—both physically and mentally.”

volunteer and prayer support of mission-minded individuals
wanting to bring health, hope and opportunity to those who are
most poor and vulnerable.
Here’s how you can get involved:
Become a Health & Hope Ambassador. Simply invite
10 people to a Health & Hope Tour, which is offered on
a monthly basis at both clinics.
Coordinate a supply drive. Our patients always need first aid
and/or over-the-counter medications.
Help with the Health & Hope Breakfast. Be a Table Captain
and bring nine friends to this free, one-hour fundraising
breakfast (Spring 2016), join the coordinating committee,
or help with day-of event logistics.
Volunteer. Serve on the Development Committee, or become
a medical volunteer. Medical providers volunteer on site once or
twice a month. We’re in special need of general/family practice
and gynecology specialists.
Join our Leadership Society! This elite group of dedicated
champions pledge long-term support for the clinics, ensuring
a sustainable future.
Consider a financial gift. Make a cash gift (envelope enclosed),
donate appreciated securities, or consider planned giving.

At Duchesne Clinic, we are
improving patient health by
moving the dial on healthcare.
Through chronic disease management,
preventive care, patient education and medication
assistance, we have helped 880 Duchesne patients
get control of their diabetes just last year. We base
this on our patients’ A1C score, which is a measure of
long-term blood sugar levels. Over the last 18 months,
our patients’ A1C numbers have dropped an average
of six points. This is a key indication that our efforts
with patient-centric care and management, patient
empowerment and diabetes education are having a
positive effect on our patients’ health and wellness.

Leadership Society
Changes make a positive financial impact
It was a transformational year for the fundraising team at
the clinics. With support from the Board, Saint Vincent and
Duchesne Clinics embraced an innovative way to fundraise by
building and strengthening donor and community relationships.
And, we’re seeing a positive impact to our bottom line.
The clinics hosted two one-hour fundraising breakfast events—
Celebration of Health & Hope (in October 2014 and May
2015). We netted a total of $220,000, which is an increase
over our largest Caritas Celebrate fundraising event. It drew
more than 400 people and resulted in 20 new Leadership
Society members.
The Celebration of Health & Hope has become our signature
fundraising event to ensure future sustainability. We thank our
donors, volunteers and our Leadership Society, which includes
our long-term donors, for their faith and support.
Farewell…The final Caritas Celebrates fundraising event
was held on April 26, 2014 and has been replaced with an
annual Health & Hope Breakfast fundraiser.

2014 and 2015 Snowflake Streaks
Net a Whopping $76,000
The initial idea of a one-mile fundraising walk for Saint Vincent Clinic in
January 2014 had created a flurry of support in Leavenworth County.
The community rallied around the first annual Snowflake Streak, netting
$28,000, with the forecast looking even better for 2015. In 2015, the
level of enthusiasm, team spirit, friendly competition and involvement
from team captains and walkers quadrupled. There was an increase
in new teams and sponsorships—netting $48,000, which was a
63 percent increase from the previous year! Besides building new
community connections and collaboration, our patients benefitted the most.
Ron Romig, the 2015 Snowflake Streak Grand Marshal, reflected,
“It’s rewarding to see so many area businesses, civic groups, schools,
and families join forces to help those in our community who have
no health insurance. Together we are making a difference!”
