Recurring Claims – Monthly Payments
Recurring claims are expenses that you incur on a monthly basis and
cost the same each month. Example: Health Insurance, Medication
1. Fill out the Recurring Claim Form found on MidAmerica’s website.
2. After paying your first bill, provide a copy of the bill, along with the
form, to MidAmerica either electronically or by paper mail. The bill
should state name of the expense, amount and frequency.
3. MidAmerica will send a check each month.
4. Participants are required to submit monthly documentation as proof
of continued payment, such as a cancelled check or receipt.
5. $5 fee for each reimbursement, not to exceed $30 per year.
