‘CAREGIVERS – Celtic Faire’, and mail the application to:

Proceeds from the Celtic Faire will benefit CAREGIVERS: Volunteers Assisting the Elderly a Charitable Foundation. Vendors must provide evidence of event
insurance with the submittal of this application. Vendors may begin setting up their booths at 6:00 am on Saturday, March 14th. The proposed Constitution
Park layout is noted on the reverse side of this application. The Caregivers and the event organizers reserves the right to deny any application based on
duplication of sales items with another vendor(s), or for other pertinent reasons. No alcohol, firearms, pornography, drug-related items/images, or tobacco
and related paraphernalia will be permitted in any booth (except for beer/wine in the beer garden). The CAREGIVERS: Volunteers Assisting the Elderly and
the Event Organizers also reserve the right to order the removal of any displayed items deemed inappropriate. Non-compliance with these conditions may
result in removal of your booth/products from the Celtic Faire.
Waiver of Liability and Agreement to Indemnify — As a participant in the 2015 Celtic Faire, I have read and understand the conditions of participation above,
and agree to assume all risks of participation and hereby waive, release, and discharge any and all claims for any and all liability, claims, demands, actions, or
suits against the CAREGIVERS: Volunteers Assisting the Elderly, the event organizers and the City of Camarillo, and the respective officials associated with
the 2015 Celtic Faire, which I may have, or which hereafter accrue to me, as a result of participation in this event. I agree for myself, my heirs, administrators,
executors, and assigns that I shall indemnify and hold harmless the CAREGIVERS: Volunteers Assisting the Elderly, the event organizers and City of Camarillo
from any and all claims, demands, actions, or suits arising out of, or in connection with my participation in this event. The waiver applies to every individual
that will be working in conjunction with our booth, no exceptions.
