A Message from the President...
Dear Residents,
Here’s to longer days and better weather! Now it’s time to
turn up the heat and celebrate fifty glorious years in parks
and recreation. Flashback to 1966 when a group of Lemont
residents got together with a vision to make Lemont even
better. Since April 16, 1966, when their dream became a reality,
the Lemont Park District (LPD) continues to grow. I am honored
to be a part of LPD, an agency rich in history with an exciting
future, as we celebrate our 50th Anniversary. After such an
impressive year in 2015, we look to exceed your expectation
this year and were off to a great start!
In January at the Illinois Association of Park Districts and the
Illinois Park and Recreation Association Conference in
Chicago, LPD was presented with the Illinois Distinguished
Accredited Agency award. This award gives recognition to
park and recreation agencies that go above and beyond in
dedicating themselves to offering quality programs and
services designed to fulfill their community’s needs. There are
just over fifty organizations who have achieved this award in
Illinois. This complete team effort took almost five years to
accomplish. The Board would like to make mention of two staff
members who were responsible in ultimately coordinating this
effort. A special thank you goes out to Executive Director,
Louise Egofske, and the Director of Recreation, Kim Passini.
LPD staff continue to display the necessary qualities,
motivation and passionate commitment to exceed expectations.
They are truly leaders in their profession. To top off this
amazing achievement, the district was also a front runner in
the conference’s Agency Showcase awards competition. The
Lemont Park District was presented with the first place award in
the Media Campaign category and brought home third place in
the Marketing Promotion category. A special thanks goes out to
our Sales and Marketing Director, Carrie Dellamano, and our
Marketing Coordinator, Carlene Dickman.
In honor of the District’s 50th, staff has prepared a grand
celebration. Festivities have been planned throughout along the
theme of “All you need is FUN!” The Annual Independence Day
Extravaganza on July 3rd should not be missed! In the spirit of
ongoing cooperation, the Lemont Park District, Village of
Lemont and Lemont Township are proud to announce that once
again our combined efforts will bring you one of the most
impressive fireworks display in the Chicagoland area. Also,
more family activities have been planned throughout the day
along with some flashback fun for the community to enjoy!
Nearly four years ago, LPD initiated a very comprehensive and
aggressive park replacement program for the betterment of
Lemont parks. If you have driven down McCarthy Road past
Northview Park, you may have noticed our park improvement
project in action. This regional park, with the input of local and
community residents, will be revitalized and boast something
for all ages! The mild winter has allowed the construction to
begin this March with a completion date scheduled for
sometime this fall. Additionally, Kensington Park and Rolling
Meadows Park will also be renovated in line with ADA
standards and the latest safety features found in the industry.
Also, a 50-foot-long pedestrian bridge will be installed at
Covington North Park. This will assist in making the park
more accessible to neighbors of the park.
Summertime’s around the corner and so is some great
entertainment! The Lemont Park District and the Village of
Lemont are pleased once again to bring you the Sunset Soiree
Free Concert Series. In celebration of
our anniversary we have added a sixth
concert and musical performances will
feature tunes through the decades.
Another FREE family fun event is our
Pack the Parks including a movie in the
park shown on our very own giant movie
screen purchased by the Lemont Park
Foundation. LPD would like to give a
special THANKS to the Park Foundation
and all they do for the community and
the district. We are pleased to bring the
community great opportunities for all
ages! From our Kids First Multi-sport
Camp and Jedi Engineering with
LEGO’S program to our Teen trips
including Navy Pier and China Town,
kids will have a ball with LPD this
summer. Our adults can get involved
with a Wine Glass Painting program or a
trip to see our Chicago White Sox Play!
And Seniors can enjoy Peach Picking
Trip, Cabaret Lunch Cruise, and a trip to
Michigan City Outlet Mall. Everyone’s
sure to have a great summer.
Finally, the Board and Staff would like
to wish James Davoren and his family
the best in their future endeavors.
Jim served as a Park Commissioner
for seven years, a very productive and
exciting time at the District. The opening
of The CORE, the re-dedication of the
outdoor pool and the initiation of an
aggressive park replacement program,
just to name a few. Jim resigned as of
February 2, 2016. Commissioner Peter
Korbakes has cordially accepted the
position of Board Treasurer and the
open Board Position is due to be filled
before summer.

Registration is easy! On May 7th, Resident Registration will begin! Lemont Residents are able to register at the
Centennial Community Center, The CORE and Residents are also able to register from the comfort of their own
homes ONLINE on May 7th by utilizing our REAL-TIME WebTrac Online Reservation System. All you have to do is
visit www.LemontParkDistrict.org and click on the “Register Online” link at the top of the page and then follow the
prompts! Upon receipt of this brochure, residents of Lemont can also begin submitting their registration forms through
the mail or by dropping them off at the Centennial Community Center or The CORE. All resident registrations that
have been received by May 9th will be randomly processed that day. All registrations received after that time, will be
processed as they are received.

Proudly standing as a pinnacle in the heart of
Lemont Park District’s Centennial Campus, is
The Lemont Park District CORE Fitness & Aquatic
Complex (The CORE). The CORE opened its doors
on December 19, 2009 and has been packed with
visitors ever since. This amazing all-inclusive sport
and fitness facility is truly the center of it all...
the center of the Lemont Park District, of a great
Community and a better you!
Fitness Facility and Free Weights
The CORE is home to a state-of-the-art fitness facility which houses
over 100 pieces of the most innovative fitness equipment and free
weights available, including towel service, iPod docking stations and
video screens on most cardio machines. The cardio fitness equipment
includes cutting edge “Preva Technology” designed to help you set
your goals and track success. The CORE also has Personal
Training. Adjacent to the equipment and free-weight rooms is an
open area for stretching, warming up and cooling down. Here
Personal Trainers can have one-on-one meeting sessions with their
clients to discuss individual progress and goals. Providing the most
up-to-date equipment and impeccable service, is at the center of The
CORE. All guests ages11-17 who sign up for a CORE Membership
must provide proof of age and attend an orientation with their parents
before they will be allowed to work out in the fitness center.

Basketball, volleyball, even baseball with
indoor batting cages - you name it, this Gym
has it! The 3-court Gymnasium at The CORE
spans over 21,000 square feet maximizing
Court Time for all. “Open Gym” as well as
an array of other events take place here every
week. Encompassing the entire Gym is a
remarkable elevated indoor track. Extending
a 10th of a mile around, it allows ample
room for runners and walkers year-round.
Aquatics
Feel like a swim? You can jump
right in the 6-lane swimming
pool inside The CORE. This
modern indoor pool, in addition
to the outdoor Centennial Pool,
is ready to meet Lemont’s
aquatic needs year round. This
aquatic facility measuring nearly
8,000 square feet plays host to
lap swim, water aerobics, swim
meets, open swim and a variety
of other Park District aquatic
events! Come join us, the water’s
perfect!

fun-time fill! Childcare hours are
Mon.-Fri. 7:30 am-1:30 pm and
5 pm-8:30 pm (8 pm on
Fridays during the summer), Sat.
8 am-12 pm. The cost is $2.75 per
hour per child for residents and
$3.75 per hour per child for nonresidents. We also offer a punch
card option or a monthly childcare

forget? The Lemont Park District's Camp Centennial
experience offers a summer full of fun and activities such as
social & team building, sports, arts & crafts, cook outs, field
trips and swimming. Our counselors are well trained,
enthusiastic, friendly and CPR and first aid certified.

DAY CAMP INFORMATION
LPD Day Camp programs are full of activities. We offer
arts and crafts, sports and team building exercises. We
will be visiting the pool, take field trips once a week
and end each session with a cookout! Mandatory
Parents Meeting on May 25th at 6:30 pm in the
Centennial Building. Register in person or online. All
camp forms are available at the time of registration.
Please have all forms filled out and returned to
Lemont Park District by the first day of camp!

Counselor In Training (Ages 13 - 15)
Are you too old for day camp, but still enjoy the camp
experience? Become a Counselor in Training! Our CIT program
offers training in a leadership role. Counselors in training will
learn to plan and lead daily activities, field trips and camp
events. All CIT's will receive CPR & 1st aid training. The
Counselor in Training program offers an excellent opportunity
for individuals wanting to become future camp counselors.

StagePlay: Musical Theater for Kids! (Ages 6 - 15)
No experience necessary as boys and girls are taught the
fastest game on two feet! Quality instruction will be provided by
area high school coaches and current college and high school
players. Campers will be taught the basic skills of passing,
catching and shooting. The final day of camp will be a full-field

was readily available, so much of today's youth leisure time is
spent staying connected through technology. What would

Lemont Dolphins
(Ages 6 - 18)
The Lemont Dolphins Swim Team is a competitive swim program
dedicated to teaching youth to swim and develop skills for competition on
the local and regional level. The Dolphins are committed to supporting all
swimmers at every level of competition, from dual meets to championship
level competitions. The team is open to anyone ages 6 - 18 who can swim
at least three lengths of the pool or 75 meters.
Once the season begins, the coaching staff will decide group placement
based on the individual swimmer's age and swimming ability. Dolphin
team members will be instructed on proper strokes mechanics, nutrition
and best training practices.
The team participates in both dual meets as part of the West Suburban
Swim Conference (WSSC) and will be registered with USA Swimming and

This program builds basic water safety for both parents and children. It helps
infants and younger children ages six months to three years old become
comfortable in the water so they are willing and ready to learn to swim.

Terrific Two’s
Preschool Plus
Terrific Two’s is a great way to introduce your child to a preschool
setting. One of the most important aspects of early development is
becoming comfortable with separation, encouraging independence
and fostering communication skills. Classes will follow a monthly
theme and include circle time with songs, a story and socialization
and playtime. Fine and gross motor skills will be enhanced through
games and crafts. Class will meet Tuesday and Friday mornings for
one hour and fifteen minutes.
Preschool Plus is an exciting opportunity to expand your child’s
educational, social & developmental horizons by adding one or two
days to our current programs. Children will experience an extended
curriculum including art, music, science and computers as well as
indoor and outdoor physical activities. Students that enroll in our
3 year old program will have the opportunity to add Friday.
Students who enroll in our 4 year old and Finally 5’s program will
have the opportunity to add Tuesday & Thursday for a 5-day
preschool week.
Young 3’s
Our Young 3’s classes were developed for children who will be three
years old between September 1st and December 31st. The “Young
3’s class meet Tuesday and Thursday mornings for one and a half
hours.
Three Year-Olds
Our Three Year-Old classes are designed for children who will be
three years old by September 1st. The three year-olds meet twice a
week for two hours.
Four Year-Olds
Our Four Year-Old class is for children who are four years old as of
September 1st. The 4 year old class meet three times a week for two
and half hours each day.
Finally Fives
Our Finally Fives class was developed for children who will turn
five years-old between September 1st and December 31st. Finally
Fives meet Monday, Wednesday and Friday for two and half hours
each day.
Preschool Enrichment Program
The “Preschool Enrichment Program” is available to students
currently enrolled in the Lemont Park District’s 4 Year Old and
Finally Fives preschool program. Students will attend their regular
preschool program during the morning - Monday, Wednesday and
Friday from 9 a.m. to 11:30 a.m. These students are able to extend
their preschool experience until 2:30 p.m. Students who enroll in the
Enrichment Program will experience a balance of Illinois CORE
standards of education in literacy, math and science along with
drama, music, science and creative movement. Students enrolled in
the Preschool Enrichment program will have a 30-minute supervised
lunch between their a.m. and p.m. programs.

t-shirts preferred. No baggy shorts, tights, socks, jewelry of
any kind, ballet shoes or street shoes. Long hair MUST be

safety and space restrictions, we ask that you watch from
outside the gymnasium through the windows.

Open Gym is not a class; it is a time for students to work on
whatever skills they would like or just to have fun with a friend.
However, the time will be structured for safety reasons.
Fireflies Gymnastics staff will be on duty for supervision and to
assist and/or spot. You must pay at the customer service desk
for each open gym time you attend. Receipt will be emailed to
you. However, PARENT must sign the child’s name in on the
sheet at the customer service desk with CHILD’S NAME & then

Keep your skills sharp this summer. The FIREFLIES is a year
round gymnastics program that competes in the I.P.D.G.C.
Optional Division. During the Fall session, we will be training to
compete at the OPTIONAL levels that starts in January. We are
in the I.P.D.G.C. (Illinois Park District Gymnastics Conference).
This conference provides gymnastics competition to girls from
participating park districts. If you are interested in being on the
Fireflies Gymnastics Team, come and keep your skills sharp
during the summer. If you want to make the Fall &
Winter/Spring Gymnastics Team, you MUST attend the second
session to work on your skills and get ready for competition
that starts early January, 2017. Mandatory that everyone
competes during the competition season. (TP)

Freedom 5K!
June 26th - 8:00 a.m. - Centennial Park
What is the Freedom 5K?
Join us for the Freedom Run as we pay tribute to Freedom
races past and run the original Freedom 5K course. Wear your
best red, white and blue and challenge yourself as you race
through the scenic and hilly streets of Lemont. And, KIDS, we
didn’t forget about you - come on out and take part in the “Kids
Dash in the Park” at Centennial Campus at the conclusion of
the race.
Start Time
The race will begin at 8:00 a.m. All runners must be present at
the start of the race. Jogging strollers are welcome but must
start at the rear of the race. The Kids Dash will take place at
approximately 8:45 a.m.
Registration and Entry Fee
Registration will be available on Active.com through
Wednesday, June 22nd. Register early... This event may sell

out. The cost of the Freedom 5K race is $25 for ages 6+ and

In the event of extreme weather conditions including heat indexes that exceed 82 degrees Fahrenheit, extreme humidity
and/or thunder or lightening, the race may be postponed and/or
cancelled.
If thunder or lightening is present at the time of the race start
time, the race will be delayed 30 minutes to allow the weather to
pass. If the weather does not pass within the 30-minute time
frame, the race will be cancelled.
If a runner must drop out of the race for any reason, they MUST
check in with a volunteer along the race route and give them
their name and bib number. The volunteer will contact the race
director on her cell phone and relay this information. This policy
will be communicated to runners via email, on flyers at packet
pickup and via announcements on race day.
Runners have two hours from the time they cross the start line to
complete the course. Runners that are not able to complete the
course within the two hour time limit will be removed from the
course and returned to the starting area.
Parking
Parking is available on the Lemont Park District campus and at
Old Quarry Middle School. Please enter through the Lemont
Park District entrance off 127th Street. NOTE: The entrance to
Old Quarry Middle School will be closed at 6:30 a.m. sharp.

Pickleball League
(Ages 35 +)
Join us for our First Annual Pickleball Ladder League! The
format of play is designed so that players play mostly with others
of similar skill levels. Two players must register to participate as
a team. (Please put both players names on the same registration
form to clarify who is on a team.) Payments are per player.
Your ranking on a ladder is based upon your winning percentage
(the ratio of points won to total points possible). Teams will move
up or down the ladder based upon their winning percentage, with
a two tier skill level single elimination tournament to conclude the
league. Regular season matches will be round robin depending
the amount of players at time of registration. League Payouts will
be given depending on the amount of players that register. (AM)

Rookies (Ages 4 - 6): This level encourages having fun on
the court with simple games and lead up drills to the forehand
and service.
Beginners (Ages Youth 7 - 11 and Adult 16 +)
No previous instruction is required. Teaching includes an
introduction to ground strokes, rally drills, elementary service,
footwork and scoring.
Advanced (Ages 12-15)
A wide range of basic strokes; forehand/backhand topspin on
the move, basic service, block volley and shot placement. Along
with these are an introduction to competitive points, games and
modified sets.
Player Ready ( Ages 13 +)
Emphasis is placed on steadiness, consistency, control and
placement of shots. Instruction includes angles, approach shots,
service and return of service, overhead and lob drills and some
emotional and mental match thoughts.
Adult Beginner ( Ages 16 +)
Little or no previous instruction. Introduction to strokes, stroke
improvement including serve, rally and net play.

This is a great league for anyone who enjoys playing
racquetball and having some friendly competition! All skill

the length of the season. League participants will split the
court costs for each match. Players may change dates and

recommended. All matches do NOT have to be played on
Saturdays, this is just reservation date and all matches will

PERMANENT COURT TIME
Permanent court time requests for racquetball, wallyball and
handball will be accepted beginning September 1st at 9:00 am
at Centennial Community Center. Requests for permanent court
time will be taken on a first come, first serve basis for the Sept.
2016-August 2017 season. Reservations will be taken yearround with an automatic end date of August 31st. Reservations
must be made in person, form filled out and full payment made.
All requests can be tailored to fit your schedule. Permanent
court time may last from 5 weeks to 52 weeks with discounts
offered. Scheduled leagues and park district programs take
precedence over permanent court time requests.

Whether its helping preserve a natural habitat
or making recycling available, the Lemont Park
District is connuously working on GREEN iniaves to help the environment!
Save the
Monarch Butterfly!
The monarch buerfly is one of the most recognizable
species of wildlife in all of America. They undertake
one of the world’s more remarkable and fascinang
migraons, traveling thousands of miles over many
germinaons from Mexico, across the US, to Canada.
The Lemont Park District has nave restoraon areas
that can be found at Bambrick Park and Centennial
Park where buerflies flock.
Would you like to have buerflies flock to your
backyard? Make it an oasis for monarchs and other
polinators by planng milkweed. Gardens should be
planted in sunny spots with some protecon from the
wind. Plant a variety of nectar plants with staggered
bloom mes to give buerflies and other pollinators
a connuous food source. Be sure to include a
combinaon early, middle and late blooming species.
Buerfly gardens can be anywhere... including
residenal areas, parks and schools. Happy Summer!
Northview Park Renovation
Spring/Summer 2016
Northview Park is one of the oldest parks in the District and
will undergo a comprehensive renovaon this year. Residents
can expect to see increased off-street parking, improved
drainage and walking paths that surround the enre park,
relocated basketball court, playground, swings and picnic
shelter. What’s new? A restroom facility, spray pad,
embankment dual slides and a zip-line.
Some green storm water management elements will be
included in the renovaon project. These features are
designed to help filter the storm water before it reaches our
streams and rivers. The planngs that will accompany these
elements are chosen because of their deeper root systems
which help manage the filtering process. The planngs will be
more natural in appearance than a formal landscape design.

at the NEW Northview Park!

Cancellations
Lemont Park District reserves the
right to cancel, postpone any classes
or rentals, or combine classes as
necessary. Classes may be cancelled
due to low enrollment. Don’t wait
until the last minute to register! All
cancellations for classes or rentals
must be made M-F, 9 am-5 pm, at
least 48 hours prior to the start of a
class. A $4 service fee will be
deducted from all refund requests for
each class that is cancelled. All
facility cancellations are subject to
cancellation fees.

As a service to our CORE Members,
childcare is available at The CORE.

We make every effort to ensure the

Helping Hand Program is designed
to provide financial assistance to
residents of the Lemont Park District
in need, to enable them to participate

attempt to provide a scholarship to a
needy individual or family.

Bench/Tree Program
Through the Memorial Tree and
Bench Program, friends and family
can provide a unique dedication to
loved ones by placing a tree or bench
in one of the beautiful parks in the
Lemont Park District. This historical
dedication provides a lasting
remembrance in honor of a loved
one and enhances the meaning of a
tree or bench to observers or park

LPD publishes program guides three
times a year: Summer, Fall and
Winter/Spring.

Class Numbers
Each program offered has a minimum number of participants needed
for the class to proceed. To maintain
our fiscal responsibility to the residents of Lemont, we will cancel
classes or programs that fail to meet
the minimum requirement. The
maximum number represents the
highest amount of registrants allowed
in the class.

Non-Residents
Lemont residents contribute to the
overall financing of LPD through
property taxes. Because of this, they
receive a resident discount rate for
classes, programs and memberships
and receive priority registration.
Non-residents must pay the nonresident rate.
NSF Checks
Checks returned for insufficient funds
will be assessed a $25 service fee.
Observation Policy
We love parents, but.... We ask that
anyone who is not registered for a
class remain outside of the class
room/gym during the program time.
This allows your child to enjoy class
without distraction.
Participant Illness/Attendance
Guidelines
Participants who have a fever or are
sick should not attend classes or
programs. It is the Lemont Park
District’s goal to provide a healthy,
safe place for our participants and
members. The District asks that all
participants and members be free
from the following symptoms for at
least 24 hours before attending the
District. Some examples of illness
are listed below but not limited to
fever, vomiting/diarrhea, excessive
coughing, green or yellow runny
nose, discharge in or around eyes,
questionable rash or any communicable Disease. District staff are not
medical care providers but reserve
the right to send participants home if
they are showing symptoms of being
ill.
Photo Policy
We like to take pictures of people
having fun! By registering for
classes/programs, joining The CORE
and attending special events at LPD,
you agree to let us use the photos in
our publications, advertisements and
on our web site. If you don’t want
your photo used, you must submit a
written request to the park
administration.
Photo Submissions
If you have great photos of you and
your family enjoying LPD events
and activities, we would love to use
them! Send your pictures (identifying
who, what, where & when) to

Picnic Permit Reservations
Picnic permit reservations will begin
on January 18, 2016, for the 2016
season. All reservations are taken on
a first-come, first-serve basis.
Reservations must be made in
person. All fees are due when
making reservations. Persons using
Park District picnic sites are required
to provide the Park District with
insurance information regarding any
rentals of inflatable apparatus,
carnival games or other rental items
to be placed on District property.
Refunds
No refunds will be issued after a class
has begun. Participants will receive a
full refund if the park district cancels a
program. Refunds are mailed out only
on the second Tuesday of each month.
Refunds for trips can’t be given once
the registration deadline has passed.
Note: If you cancel a class more than
48 hours before it starts, you will
receive a full refund minus a $4
service fee. The $4 fee will be
charged for each class cancelled.
Resident Rate
The Lemont Park District reserves

Minimum/Maximum Class Numbers Each program offered
has a minimum number of participants needed for the class to
proceed. To maintain our fiscal responsibility to the residents of
Lemont, we will cancel classes/programs that fail to meet the
minimum requirement. The maximum number represents the
highest amount of registrants allowed in the class.
Cancellations We reserve the right to cancel, postpone, or
combine classes as necessary. Classes may be cancelled due
to low enrollment. So, don’t wait until the last minute to
register! If a guest would like to cancel their registration for a
class or program, cancellations must be made M-F, 9 am - 5 pm,
at least 48 hours prior to the start of a class. A $4 service fee will
be deducted from all refund requests for each class that is
cancelled.
Wait List When maximum enrollment has been reached for
a particular class or program, we will begin a wait list.
Refunds No refunds will be issued after a class has begun.
Participants will receive a full refund if the park district cancels a
program. Refunds are mailed out only on the second Tuesday of
each month. Refunds for trips can’t be given once the registration
deadline has passed. Note: If you cancel a class more than 48
hours before it starts, you will receive a full refund minus a $4
service fee. The $4 fee will be charged for each class cancelled.
Misc. Registration Info You may register for yourself and your
immediate family only! Program Waivers must be signed for all
programs and class by the starting date. Please fill out the
registration form in it’s entirety as this may result in a delay in
processing. If you have never registered for a program at the
Lemont Park District, you are required to register in person the
first time.

Read carefully!
The Lemont Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Lemont Park District continually strives to reduce such risks and insists that all participants follow safety
rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities
contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered
an illness, injury or impairment, to consult a physician before undertaking any physical activity.
Warning of Risk
Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when
participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must understand that certain risks, dangers and injuries due to acts of God, inclement weather, slipping, falling, equipment failure, failure of supervision, premise defects and all other circumstances inherent to recreational activities/programs exist. In this regard, it must be
recognized that it is impossible for the Lemont Park District to guarantee absolute safety.
Waiver & Release of All Claims & Assumption of Risk
Please read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming
the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might
sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation vehicles when provided).
I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to
me or my child/ward) as a result of participating in this program/activity against the Lemont Park District, including its officials, agents,
volunteers and employees (hereinafter collectively referred as Lemont Park District).
I do hereby fully release and forever discharge the Lemont Park District from any and all claims for injuries, damages, or loss
that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connecting with, or in
any way associated with this program/activity. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering a minor participant, I further attest that I have read the above to my
minor child/ward.
Any faxed registration forms/signatures will be considered the same as an original form. Participation will be denied if the signature of
an adult participant or the parent/guardian are not on the front of this waiver.

During the course of park district activities and programs, videos and/or photographs are
occasionally taken of participants. The signee understands the Lemont Park District may use
these photos and videotapes on their website, the LPD Facebook page, in their publications
and for promotional purposes. All items remain the property of the Lemont Park District.
