You are the heart of our 3 year strategic plan. Our vision to provide “Outstanding care – Every Person, Every Day” may look different for each person and family we work with. It is up to us to find out how we can create this outstanding care experience for you. Michael Delisle Board Chair We know our health care system is fragmented and at times can be challenging to find the services and care you need when you need it. It is for this reason that our organization, along with other local health care organizations, are working to create a seamless experience as you travel through the health care system – ensuring you receive fair access to services, personalized care coordination and quality service. We commit to work with you, your family caregivers, your family physician and others involved in your care to help you live a healthy and independent life in our community and in your home. Working With You to Achieve Your Health and Independence Goals One of my first tasks as CEO was to contribute to and finalize our new 3 year strategic plan. I am inspired by the direction of our plan. We commit to provide you with personalized care to meet your unique needs and to work with you to achieve your health and independence goals. We commit to connect you to the care you need, when and where you need it to achieve these goals. We will bring to life our directional plan and measure and report our progress openly. We know you want to stay in your home as long as possible. You and our community are relying on a strong home and community care system now and in the future. Our plan will move us in this direction. Gordon Milak Chief Executive Officer 4 Ontario Action Plan for Health Care WWCCAC Strategic Plan is based on Ontario’s Action Plan for Health. The Action Plan for Health Care in Ontario will ensure families get the best health care where and when they need it, while ensuring all Ontarians get better value for their health dollars. It will focus on three priorities aimed at preventing illness, increasing access to family doctors and supporting Ontario’s growing senior population. • Keeping Ontarians Healthy • Faster Access to Stronger Family Health Care • Access to the Right Care, at the Right Time, in the Right Place The Action Plan for Health Care in Ontario builds upon the significant progress that has been made in improving Ontario’s health care system since 2003 and is the next step in the Ontario government’s plan to protect health care for our children and grandchildren. The Waterloo Wellington CCAC’s Strategic plan builds on the foundation of change established by the Action Plan for Health Care in Ontario 5 Waterloo Wellington Health System Strategy Waterloo Wellington Health Services Plan for the Residents of Waterloo Wellington WWCCAC Strategic Plan is based on the Waterloo Wellington System Strategy which is founded on the Ontario Action Plan for Health. The Waterloo Wellington Local Health Integration Network (WWLHIN) sets the direction and makes decisions regarding local healthcare services, integration and funding to meet the needs of the residents of Waterloo Wellington. Health care providers funded by the WWLHIN include: • home care • mental health addictions hospitals • long-term care facilities • community health centres Mission: To lead a highquality, integrated health system for our residents. Vision: Better Health – Better Futures Core Value: Acting in the best interest of our residents’ health and well-being. 6 7 WWCCAC Strategic Plan Our Strategic Plan Design The WWCCAC Board of Directors led the strategic plan design over a 5 month period. The plan began with a review of an in-depth community needs assessment and an environmental scan wwccac.org/strategy and involved consultation of staff, 80 health system leaders and the public. This plan reinforces our commitment to ensuring safe, high quality seamless and coordinated care across the Waterloo Wellington region. 8 We will provide you with a positive and personalized care experience by providing you with timely assessments, information and care; connecting you to the services to meet your needs. We know when you or your loved one loses their independence to an illness or life changing sickness it can be overwhelming. Even though many services are available in our community you may not know how to find them or you may question which door is the right one to knock on for help. You have told us that when you need help, you want it to be timely and in the right place –preferably in your own community and at home. You have also told us that you would prefer us to work with you as opposed to provide care to you. You wish to be empowered as active and knowledgeable partners in the health care process. You are unique and have your own unique health and wellness goals. You want us to see you as the unique person you are and respect your wishes related to your care needs, your independence and health goals. 9 1.1 Provide you with Care When and where you need it: We commit to provide you with timely, personalized assessments, information, care and connections to the health and social services you need, when you need it. How we will do this: Your health and independence situation can change over time. Providing you with online information to research local health and social services by geography will help you stayed informed and empowered to make choices for your care needs as they change. When you need help to sort through various services and finding the right door to knock on, you can call 310CCAC and we will listen to your care needs and help connect you to the right health and social service. When you need care in your home we will ensure your care needs are assessed quickly and arrange for the right care based on your care needs. We work with your family and friend caregivers as we know just how important they are to your well-being. 1.2 Foster a Positive Care Experience: We will foster an informed and positive care experience for you and your family. How we will do this: We commit to listen to you and your family and respect your personal care wishes. We will seek to understand your personal needs, preferences and values. It is then that we will create together a care plan that helps you achieve the health and independence you deserve. Together we will evaluate how this plan is working to achieve your goals and adjust the plan as required. We will ask you to evaluate our interactions with you so we can learn and improve how we can create a positive experience for you. When your goals are met we will conclude our services knowing you can reach out to our organization at any time for assistance. 10 Enable better health care outcomes, safe independence and sustainable value for money by adopting evidence-based, high quality approaches to care You value your health, your independence and ability to choice to live in your home as safely and as long as possible. You have told us that when you need help to regain your independence and health you want it to be the least intrusive, the most effective, expedited safest care possible. You also have told us that you want to have a say in the level of risk you live with in your home. As taxpayers you want to ensure that your dollars invested in health care result in safe care provided in the most economically way possible. We aim to do no harm and to ensure that the care you receive in the home and in the community is based on the best way to provide that care, ensuring you achieve the health outcomes you deserve. “We have an aging demographic in Ontario. In order to provide the best care for people who want to be in their homes, we have to change the system.” Kathleen Wynne, Premier of Ontario 11 2.1 Deliver Safe High Quality, Evidence Based Care: We will adopt evidence-based practices to improve the quality of your care, deliver better health outcomes and support value for money for long-term system sustainability. How we will do this: We will base our care provided to you on solid evidence, taking into consideration your individual situation, preferences and characteristics. We will not do something just because we have always done it that way. We commit to find new and better information and practices to make decisions and to provide care that will result in better health outcomes for you. Whenever possible we will provide you with care in the home and in the community as we know it is not only more economical but it is where you would like to receive care when possible. 2.2 Support Safe Independence: Partner with you and your health care team to provide the information, tools and support that enable you to live safely and independently in your home and community. How we will do this: You have the right to define what safe independence looks like to you. We will listen to you and work with you whenever possible to support you with your wishes. We will work along-side you and your family to provide you with information, aids and coaching on ways for you to live safely and independently at home. 12 Creating a more seamless and coordinated health care system by partnering with our patients, families and service providers and leveraging information technology As you travel through the health care system you will see many different health care professionals – your family physician, a medical specialist, hospital and our home care professionals. You have told us it is important that your care team share the information you have provided them so you do not have to repeat your story. It is important to you that we standardize our information and communications systems so we can share your information within your care team as you seek help. It is not enough that your care team shares information with one another but it is important that we work together with you to achieve your health goals. When you are very ill it is important not only to you but to your family to know you have the right team of professionals caring for you. When you need to move from one health care provider or organization to another, it is important there is a smooth handoff between providers and they work together to help you. In an age where technology is at our finger tips, we know you expect that you should not have to tell your story multiple times to multiple health care providers. 13 3.1 Partner for Seamless Care: Partner and collaborate with your primary care physician and team, with you and your family and other care providers to develop more seamless and coordinated care delivery methods. How we will do this: Over the next three years our organization will be actively involved with your primary care physician, hospital leaders and other community services leaders to improve how we work together to create a smooth and organized care experience. We will improve your care experience as you move from one care setting to another (e.g. hospital to home, home to a community program). We will focus primarily on those people with complex health conditions. 3.2 Leverage Information and Technology: Invest in and leverage our information and technology assets to enable seamless and cost-effective care and outcome measurement. How we will do this: We commit to use our current information and technology assets to their fullest potential in order to ensure that your information flows freely to those people on your health team. We will share our health assessments so other care team members do not repeat another assessment asking you similar questions. We will expedite referral for care to the appropriate care team member to improve your access to service. When new technology is needed to create a more seamless coordinated experience for you we will work to put in place this technology. 14 Enablers To Support Our Plan To support our priority areas we will also focus on the following enablers: OUR PEOPLE Investment in engaged and high performing staff in a healthy and safe work environment. In order to provide you with outstanding care, we rely on committed and passionate staff. We know that our staff team has chosen the home and community care as their careers because they believe helping people stay in their community and in their home is important. They are committed to the philosophy that you deserve the best health and independence possible. We commit to invest in ensuring we have high performing and highly engaged staff and we commit to provide a healthy and safe working environment for them. INNOVATION A Culture of Innovation and Accountability that drives Sustainable, Continuous Quality Improvement. As tax payers you have a right to know that the money directed to home and community care is proving to benefit those we serve. We must continuously improve how we provide care and we must look for ways to find creative and innovative solutions to help people stay in their home as long as possible and as independent as possible. We must also ensure that our home care system is delivering value for the money invested. TECHNOLOGY Integrated Information Systems to Connect Providers and Support Cost-Effective Care Delivery We will use our current information and technology assets to their fullest potential and work to find new ways to innovate with technology to support people to live safely in their homes as long as possible. 15 Our Objectives for 2016 • You and your loved one will know of to search for health and social service information independently and will be informed of the local services within your community and can easily access these services. • You and your family will know to contact CCAC directly at 310CCAC for additional help and support to find services • You will not have to repeat your health assessments and your family physicians will be informed of your home and community services you are receiving • The care you receive in your home will match your health and independence goals and you will be empowered to direct your care team • Your family members helping care for you will be supported. • Your information will flow freely amongst your care team so all understand your unique care needs and know your care plan.
