Many of my patients come to me seeking help with the changes that can occur after childbirth. Following childbirth, a woman’s breast can grow to uncomfortable proportions or just the opposite can happen. A woman’s breast can actually lose volume and shrink, resulting in the breast appearing “deflated.” Additionally, a large number of women come to me seeking help with the post partum changes of their abdomen. During pregnancy the skin and abdominal wall muscles are stretched. Following childbirth, the abdomen can protrude and the skin can be loose or sag. In some cases, the abdominal muscles can be so weakened that the individual may look like she is still pregnant. Despite daily workouts including sit ups and crunches, a tummy tuck may be needed to restore these muscles. ing breast reduction. Both groups of women want to have breasts that are proportional to their body size with the most natural result possible. In certain situations, a breast lift is also needed to tighten lax skin. The laxity can be the result of pregnancy or weight loss. When a breast lift is needed, I utilize the lollipop scar technique. A breast lift procedure is very similar to a breast reduction. The only difference is that with a breast reduction, breast tissue is removed. Combined Breast/Tummy Tuck and Liposuction Procedures. Many of my patients who have breast surgery also have other procedures performed at the same time. This allows for one surgery and one recovery. Meet This Long Island Women Dr. Cynthia Paulis Physician and Writer Dr. Cynthia Paulis with Gloria Gaynor Where did you grow up? I grew up in Massapequa Park but lived 10 years in Washington, DC and Virginia for college and graduate school. I then moved to Missouri for medical school, New Jersey for internship and residency, and then spent four years in Oklahoma as a military physician caring for veterans and Native American Indians. I later moved to Texas to work in border towns and then came full-circle back to Massapequa Park. You’ve had an interesting career. Yes, I graduated with a business degree and ran my own company at the age of 25, recruiting physicians for rural areas. Eventually I decided to go to medical school and became board-certified in both family practice and emergency medicine and worked trauma centers. I was a medical director of an inner-city hospital in Dallas, Texas and was also a medical reporter for CBS in Oklahoma, with a 15-minute television spot every Wednesday at noon called “Ask the Doctor.” Tell us about your Hurricane Sandy experience. I had no heat or power for almost two weeks, but I was fortunate that I still had a 8r-0/(*4-"/%80."/rApril 2014 house. I volunteered during the day at Cedar Creek Park with the Red Cross and at night took refuge at Ace Hardware where they had heat, light and people. I would speak to the people who were coming in, asking them about their experiences. Some had four feet of water in their homes and lost everything but they were surprisingly still hopeful and all seemed to say the same thing: “I was lucky.” “We were blessed.” “We survived.” I started to write down their stories. What happened to these stories? A former writing teacher contacted me about a world-wide contest that Gloria Gaynor was running, looking for survival stories. Forty essays were selected to be included in her new book, We Will Survive, and mine was chosen. When she sang at Burn’s Park last July in front of 10,000 people, I had the great privilege of meeting her for the first time, and to my surprise, she gave a shout-out to four of us in the audience whose essays had been selected for her book. It was quite a thrill! I understand that Ms. Gaynor’s hit song, “I Will Survive,” helped you through medical school. I was in medical school during the 1980s, when there were not many women in the medical field and we faced a lot of challenges and prejudice and had to be twice as good as the male docs. I was the first female class president elected in the history of my medical school, so I faced a lot of obstacles from some of the oldguard professors. It was very tough and often times isolating but that song always played in my head and I knew I could survive it and get through, and I did. Of the 22 women out of a class of 135, we all graduated medical school and yes, we all did survive—and thrive! Most adults are still reading books, but their reading habits have changed dramatically over the last several years. A recent Pew study tells us that most American adults read at least one print book in the past year, but reading e-books is definitely a rapidly-growing trend—not surprising, since about half of American adults own either an e-Reader or tablet. The number of adults who read an e-book in the past year rose by 28%, up five percentage points from the previous year. However, since the number of readers who read ebooks exclusively is just 4%, most adults are also reading print books. Listening to audiobooks is also on the rise. Andrea Brancato: I read some on my Kindle – but I listen to a lot of books on audio – mostly from the Harborfields Public Library. Jeanne Ferdinand: I definitely read more when I first got my Kindle. I found I could read faster with large size type. Joseph Reitzel: I read about the same. Lee Anne Vetrone-Timothy: I actually read less…but it makes traveling with a few books easier. Mark Rosenberg: I read more. The convenience of being able to use my Google Nexus has meant I can read more comfortably in bed. Theresa Bucherati Isaacs: It’s hard to say whether I read more, but I can read when I flat iron my hair! I think I probably read more when I have an actual book in my hands. Linda Ventimiglia Armyn: I was reading the same, but I recently went back to the real thing or the old school Kindle because my eyes can’t take the constant backlight. Brent Stone: I like reading the oldfashioned way. Joe Rothkopf: Definitely paper. I look at a screen too much as it is. Ellen Cohen: E-Reader more. Lyn Dobrin: E-Reader when on vacation; book at home. Joan Share Liss: Kindle or books…I still read the same amount. Jeffrey James: I’m afraid it’s books and only books. I grew up with a father who collected (although he would say accumulated) about 13,000 books and passed his love of them on to me. I don’t have nearly that many, but can’t imagine wanting an electronic contraption instead. Mike Bondroff: I refused to buy a Kindle for the longest time. Then my wife got me the Paperwhite two holidays ago. I probably read 10x as much as I used to! Ellen Dule Wright Torge: I love to own books–especially antique ones–but my husband bought me a Kindle two years ago and I definitely read a lot, lot more. I love to read on my Kindle at night when we go camping. Karen Lehrer Perry Weinstat: I read both and am an advocate of a “media mix” in most endeavors. Sometimes it’s a testosterone issue, at least initially, when you first go into perimenopause, or the beginning of menopause, the first thing to go down is testosterone,” says Finker. She also notes that stress is often the cause of hormone imbalance in many of her patients and explains that the stress hormone, cortisol, shares a similar pathway with progesterone. That means that when a woman is stressed her body shifts hormone production in the direction of cortisol and away from progesterone, which can subsequently produce menopausal symptoms. Dr. Finker has had success with using botanicals such as chaste tree berry and black cohosh which, she explains, helps to normalize levels of progesterone and estrogen. And, although Dr. Rabin seems to feel that these herbal substances primarily have a placebo effect, the Natural Medicines database, the most authoritative and comprehensive database on natural supplements, reports that there is preliminary research supporting the effectiveness of these substances. Another up-and-coming hormone that is being used to treat menopausal symptoms is testosterone. According to Dr. Marcus, testosterone is used for low levels of desire. However, for many years testosterone use was discour- aged because of the thought that it could turn into estrogen in the body. “I feel that they were looking to estrogen for help for some of the things that were bothering women. For many of them, testosterone may be an answer. Like energy, bone health, etc.” But testosterone is not for everyone. Dr. Rabin reports that some of her patients have experienced male-type side effects including a deepening of the voice, which could be permanent. What you can take home from all of the information that has developed in the past decade is that you no longer need to suffer in silence from menopausal symptoms. Despite the fact that HRT does increase disease risk for certain populations and may not be the answer to aging, there are some safe, viable options for managing specific symptoms when all the data is carefully weighed. Dr. Marcus feels that our bodies were not meant to live past our 40’s or 50’s and that, now that women are living much longer, it will take some form of hormones to allow women to have a good quality of life into their later years. “I think that this ‘one-size-fits-all-medicine’ really needs to shift, especially with something like this. But I don’t think that doctors need to be God in this case; I think that they need to explain all the risks and benefits to their patients and let them make their own determination.” ● Renee Sojcher, MS, MS, CHWC is a Health Behavior Change Specialist. She can be contacted on the web at www.WellHealthLLC.com and at [email protected] . To Soy or Not To Soy Soy is considered to be an active player in the biology of women’s health because it contains isoflavones, a class of compounds that acts as phytoestrogens in mammals. That means that because the molecular structure of soy’s isoflavones resembles the estrogen made in the human body, these compounds can bind to estrogen receptors and cause similar effects as the body’s own estrogen. It is for this reason that the use of soy has attracted much attention, and much controversy, particularly for women. Proponents of soy tout the positive effect that it has on heart health. Soy Soy has is high in protein but low in saturated fat and choattracted lesterol. And, although much the American Heart Association has reversed its attention, statement that soy has an and much effect on lowing cholesterol, it still recommends controversy, the use of soy as part of a particularly heart healthy diet. for women. Regarding breast can18r-0/(*4-"/%80."/rApril 2014 cer, there are still no definitive studies that prove whether soy is harmful or protective. And the jury is still out about whether soy affects thyroid function, especially in patients who suffer from hypothyroidism. Dr. Marcus and Dr. Rabin believe that the use of moderate amounts of unprocessed soy, such as tofu or edamame, can be beneficial for reducing menopausal symptoms but they warn against excess use. Other forms of soy considered to be highly beneficial are fermented soy products such as tempeh, miso, and natto. These foods contain high amounts of beneficial yeast, mold, and bacteria which are necessary for optimal digestion. Dr. Finker restricts soy in the majority of her patients. She believes that the soy that we have in the United States is far different from the soy found in Asia and it is her belief that soy disrupts thyroid function despite the fact that it could be useful to women who are looking to naturally raise their estrogen levels. Finker recommends the use of alternative ways of accomplishing the same goal of balancing hormones because not enough is known about whether or not soy is helpful or harmful. It’s quite impossible to think of her without it bringing a smile to your face, as she regularly shares her talent for heartfelt comedy with projects that span most every medium. Though she’s most well-known for her award-winning work as sassy, high-pitched Karen Walker on eight seasons of Will & Grace, she actually started out as a ballet dancer and theatrical actress in Chicago before launching a television and film career in Los Angeles in 1985. In the next twenty-eight(ish) years, Mullally has garnered a resume that boasts seven Broadway musicals (Grease, Young Frankenstein), a talk show (The Megan Mullally Show), a list of memorable guest spots that reads like a phone book (Parks and Recreation, 30 Rock, Happy Endings), film (The Kings of Summer, Anywhere But Here), and headlining roles in television projects such as Party Down and Adult Swim’s Childrens Hospital. Mullally is currently donning many hats…and shoes… and wigs…as both an actress in Off-Broadway’s Annapurna, (after an acclaimed run in L.A. last year) as the kooky Chief on Childrens Hospital, and as a musician with her two-person band, Nancy and Beth. She spoke with LONG ISLAND WOMAN about finding Karen Walker at the age of 40, starring in Off-Broadway’s Annapurna opposite real-life husband and Parks and Recreation funnyman Nick Offerman, and the many new ventures she now holds in the palm of her hands at the age of 55. Your mother was a model and your father was a contract player at Paramount. How was being from a family in the entertainment business effected your own career? Dad never got his big break, he just had teeny parts in different things. But in Oklahoma City where I was raised, he was the only theatrical, flamboyant, funny, eccentric person that I knew in the whole city. There wasn’t anybody else like my dad, because most people were pretty conservative. He would wear really fancy suits with ascots, and he actually purchased a 1937 Rolls-Royce Phantom III and would drive that around town in a Nehru jacket. He had a really crazy sense of humor, so I’m sure that rubbed off on me to a certain degree, considering most of the characters I play are really weird and eccentric. Was being in that environment in any way detrimental to your career? He didn’t even think that being an actor was a great idea because he thought that there was so much rejection. I started out in ballet, and I used to do musicals in the local summer stock theater during the summers, but I didn’t really know anything about acting when I got to college. It was scary to me. I tried to take an acting class and I was super intimidated by these very uninhibited kids. I was much more introverted, so it took me a long time to learn how to be more of an extrovert. What made you decide to transition from ballet to film and television? I loved being in a ballet company, and I studied in New York for a couple of summers at The School of American Ballet in the Juilliard building. I was very devoted to it in the 70s when it was the coolest thing. Ballet dancers were like rock stars. But I started realizing that I didn’t have enough real physical strength to be one of the great ballet dancers. I also started noticing that the ballets I liked the most, were the ones that required some storytelling. I liked the acting part of it where the character I was playing would have a mad scene and then die of love or something. Also, at a certain juncture things start to get not too pretty with your feet. Playing Karen on Will & Grace put you on the map and brought your comedy ability into the spotlight. How do you think your life would have been different if you had reached that level of fame before having hit the age of 40? I don’t know if I would have been ready for it. I think I had a lot of growing up to do. I was precocious in many ways, but emotionally kind of immature. I think everything happens in the timing that it happens in for a reason. It was exactly right. It’s funny, because (my husband) Nick is on the exact same trajectory. He got Parks & Rec when he was almost 40. The parallels are weird because he’s younger than I. Why do you think the role of Karen resonated so well with women? There’s something about her outspokenness. Karen wasn’t afraid of what anybody thought of her. That’s a powerful quality. We all know people who are very confident and don’t really care what other people think of them, but they are few and far between. Since I turned 50 I have felt like I’m the most who I am in terms of my appearance. Most of us succumb to all the pressure to feel like we have to look a certain way, be a certain way, stay looking young forever, weigh a certain amount, etc. Karen obviously cared a lot about her appearance but she didn’t give a flying you-know-what about what anybody thought of her. There’s something freeing about that. Of your many roles, choose a few that have held great meaning for you and explain your choices. Definitely Karen from Will & Grace. I did a play called The Receptionist at L.A.’s Odyssey Theatre in 2009 and I played the title role. It was only sixty pages, but I used to read it every day during the run of the play just because I loved it so much. I just loved playing older than myself, she’s a woman in her sixties from Newark and I had to do my best with the dialect. Maybe I did okay…(laughs). I recently did a few episodes of Showtime’s Web Therapy with Lisa Kudrow. I think I loved it because it was completely improvised. I also did a little indie movie called Smashed, and I played a principal of an elementary school. I loved that character because she’s extremely sweet and vulnerable. I don’t get to play very many roles in movies where I have any kind of serious scenes, and I like doing more dramatic stuff also. Your character on Childrens Hospital, Chief, is very quirky and at times hideous, to say the least. How many of her foibles are from your own imagination? A lot of it! The scripts are just jokes, so everything else is a filling in of the blanks. They give me hilarious things to do. For example, the wig I wear is a man’s wig from Hollywood Boulevard that I got for $39.99. And then she has those big, thick glasses and the walker. Her hunchback was my idea. We just tried to make her as disgusting as possible. I’m in the minority of Hollywood actresses too when I say that I also don’t wear any makeup as Chief. Now you’ve established yourself in front of the camera, what keeps bringing you back to theater? Theater is where I got all of my on-the-job training because I never took acting classes in college. There’s nothing like live theater. And I feel like Annapurna is such an incredible boon for Nick and I to stumble upon because it’s a two-person play. The cast is my husband and I. We carpool to work together. We share a dressing room. The set is a trailer so it’s he and I in a trailer for 90 minutes going up and down and every way you can think of emotionally, and it’s cathartic. There are comedic parts, but overall it’s more of a drama than anything else. When we did it in L.A. every night, by the end of the show people were audibly weeping in the audience. I don’t think I’ve ever been in a play where that happened. How did you originally come to Annapurna? We did it at the same theater where I had done The Receptionist. I had loved doing The Receptionist so much that I definitely wanted to come back and do another play. Then Nick threw his hat into the ring and said that he would also like to do a play. He also has a big theater background. (Odyssey Theatre’s Artistic Director) Beth Hogan had stumbled across this play by Sharr White. It only had one production done at a theater up in San Francisco. We really liked it, and then it was a really long time before we could get our schedules together to just sit down and read through it with Beth and with our director, Bart DeLorenzo, who also directed The Receptionist. When we read it we thought, “Oh yeah. This is good.” Of all the theater pieces I’ve ever done, it grew the most from opening night. I felt like the play just sort of kept unfolding with every performance in a way that I hadn’t expected. Every night it was something new for the actors. (DeLorenzo will also be directing the Off-Broadway production of Annapurna.) How has your experience acting opposite Nick changed since you first worked with him many years ago on stage in Chicago, and now working with him in Annapurna? We met doing a play, The Berlin Circle. It’s interesting, I’ve dated a couple of other guys over the years who were actors, and I always felt kind of shy acting in front of them. But because Nick and I met acting together, I never felt like that. I think it’s always fun for us to work together and we really enjoy it. It’s been three plays, a few television shows, and a few movies now. However, doing Annapurna was the absolute. It goes far beyond anything else we’ve ever done together. It’s very comprehensive emotionally, and no emotional stone is left unturned. I’m very excited to bring it to New York. What have you learned about each other from working on Annapurna? It’s funny because you would think that during the run of the L.A. production we would have talked about the play a lot, but we never really did. We just did it. We would run lines in the car on the way there and then finish running lines once we got to the theater. Then we’d go to the dressing room and chat about other things. Sometimes after the play we’d be like, “Oh, that was a heavy one tonight.” We would never deconstruct or talk about acting. We’re not like that. If I have learned anything, it’s that I know for a fact if it were any other actor in that role, it would have taken me a lot longer to get there. But since Nick and I have been together now for over thirteen years, I can just start laying into him on day one. I would feel really awkward doing that with an actor I just met that day. I wouldn’t want to be screaming and yelling at some poor guy that I just met twenty minutes ago. Nick and I just cut to the chase because we’ve been together for so long. How do you separate work from your personal life? It just seems to be okay. We don’t really talk about work too much, just in the context of how it’s affecting our lives. We’re both so busy all the time. We try to keep our weekends free so that we can spend them together and not be working. Last year was really tough because Nick has a stage show called American Ham, a one-man humorous show, and I’ve got this new band called Nancy and Beth, and so sometimes we’re off doing our shows in separate places. How did Nancy and Beth come to be? I went to Austin for a couple of weeks to hang out with Nick when he worked on an indie movie called Somebody Up There Likes Me. Stephanie Hunt, my band partner, had a supporting role in the movie. They ended up filming me in this little cameo role and so we met. Stephanie is thirty years younger than I am, and we just had a funny connection. We THE LONG ISLAND WOMAN INTERVIEW M E G A N M U L L A L LY Megan Mullally with her husband, Nick Offerman hit it off. She showed me around Austin, and we both discovered that we love shopping for vintage clothes. At one point she mentioned that she played ukulele and wrote songs, and I said, “Oh, I want to hear your songs.” On the very last day I was getting ready to leave and she brought her ukulele and said, “I’ll play you a song but you have to sing part of it with me.” She taught me this part to sing with her, and then the minute that we started singing it together we just laughed. “Oh my God, we sound so good together. We really blend!” She ended up moving out to L.A., and Nick decided he should have someone open for his American Ham show. I said, “Have Stephanie open for you.” So he said, “Okay.” Stephanie said to me, “Well, I want you to sing with me on one of the songs.” I did, and then it kept exponentially increasing. The next time we did three songs together, and then five, and then we started a band. Why call the band Nancy and Beth if that’s not either one of your names? (Laughs) I had made a list of about thirty different potential band names that I thought were fun or funny or interesting. I emailed them all to Stephanie and they were all like, “The Somethings”… except for Nancy and Beth, which was my favorite, and I just buried it in the middle of the list. She instantly emailed me back with her choice: Nancy and 22r-0/(*4-"/%80."/rApril 2014 Beth. It was meant to be. You’re playing in your band on the weekends or you are shopping for vintage clothing. What else would we find you doing in your down time? I haven’t had very much of it lately which is sort of weird. I’m not really sure how to account for my time. I’ve had a show in development at the Independent Film Channel that I am working on quite extensively, but hasn’t been picked up, and then I’ve been working on the band a lot. Now I’ve got two other television shows that are getting developed. It’s a little crazy. If you had a weekend in which you could do whatever you wanted to, what would you find yourself doing? Nick and I are homebodies. We like to hang out with our three dogs at home in L.A. One of the things that we like the most is just getting the chance to go out to the movies. When we have a free day, sometimes we’ll go to a theater and see three movies in a row. We never get to go, so we just smash them all into one sitting. We like to go to Venice in L.A., and have dinner and walk along the canals. It’s so romantic. We’re mellow. We’re not partiers. We like to be together, which is nice. Even now I still feel like I’ve only known him for a month. When I see him I think, “Oh, there’s that cool guy that I want to try to crack up,” or I want to hear what he has to say, or try to think of interesting things that happened during the day to tell him about. It seems that middle-aged women are often questioned about two things: not having children, and being in a relationship with someone younger than them. Have you personally found that to be the case? One of the only real remaining taboos for a woman is not having children. For a man it’s like, “Eh, you don’t have children.” For a woman, it’s almost like people really don’t know what to say to you because they just assume that you desperately wanted children but were unable to have them, so there must be some tragedy there that they don’t want to unearth. For me, I never had a burning desire to have children. When I met Nick, it was the only time that I really tried, but I was already 44 by the time we got engaged. It was a little late in the game. I also got my first dog as an adult when I was 41, and even though it’s not the same as an actual child, it does sort of give you an idea of what you would be like as a parent. For some reason I felt that maybe I wouldn’t be a good parent. The age difference has never really entered into anything for Nick (age 43) and I. I’ve never really noticed a difference. He never misses any pop culture references. I think that we look about the same age. He looks a little older and I look a little younger, so it works out. The new taboo is not having plastic surgery. It used to be a taboo to have plastic surgery. Especially if you’re an actress and you haven’t done anything to your face, people just look at you like they feel so sorry for you. They think there must be something wrong with you if you didn’t do it. So what’s wrong with you? Why haven’t you done it? (Laughs) I think part of it, is just that my mom never did anything, so it’s not something that I was brought up with. But also in a more philosophical way, I’ve always just wanted To advertise: YrBET!MJXPNBOPOMJOFDPN Featuring Dilip D. Linda and Gary Mancuso’s hearts beat fast as they waited for the birth of their first child. Linda was a week late. Finally on July 22nd baby Danielle arrived. Thirty-six hours of labor ended with Gary proudly holding his 7-pound, 4-ounce baby girl. Until the Mercy Hospital delivery room nurse noticed something. Danielle’s lips were blue. Baby Danielle was swept to intensive care. Cardiologist, Dr. Milton Reitman, rushed in, did an EKG, and ordered her transferred to St. Francis Heart Hospital. By the time she was just seven days old, 7-pound Danielle had endured THREE open-heart surgeries. You see, Danielle was born with a congenital heart defect: Transposition of the great vessels. Danielle’s aorta and By the time pulmonary artery were reversed. It means the hoses pumping she was just her blood and oxygen were connected to the wrong places. Gary and Linda were given two options in 1987. The Mustard seven days procedure would rebuild Danielle’s heart muscle, and she’d old, 7-pound likely live another few years. The newer Switch procedure would “switch” or reconnect her vessels to the right places. Danielle had Switch also meant reconnecting every microscopic capillary and ventricle to a heart the size of a walnut. The St. Francis endured doctors had only done one Switch before, a few months earlier, on a little boy. Linda remembers a slim 1% chance of Switch THREE success. But they were told if Danielle could survive the Switch, open-heart she’d have a shot at a normal life. Linda says Gary thought she’d know better what was best for surgeries. the baby she had carried for nine months. Linda thought, “The world can be cruel. I wanted her to have the best opportunity to survive and thrive.” And so the Mancusos asked the doctors to begin moving her heart’s tiny hoses. Linda recalls the doctors left Danielle’s little chest open for days, so they could do gentle CPR when needed. It worked. But then eight months later, Danielle needed 2 more open-heart surgeries to close part of a valve that should have closed naturally. Linda admits they were paranoid parents Danielle’s first year, always worried she’d get hurt. But Linda says God shifted their focus to the positive by sending baby sister Nicole. On the other hand, Danielle knew no fear. She played soccer and basketball in school, until the day she came home with a permission slip for something new – fencing. Linda asked, “You’re going to have a sword?” “No Mom. They call it a weapon!” which did nothing to calm her parents until Danielle insisted, “If the doctor says it’s OK, then it’s OK.” And it was. And it is. Two months ago Danielle was a guest at St. Francis’ Heart Ball. She brought a message to parents of other kids with health challenges. “Don’t tell them they can’t do something; that they’re not good enough. Don’t ever give up on them. Cause the minute you do they’ll give up on themselves.” Linda and Gary never gave up. Neither has Danielle. After college, Danielle became a certified EKG technician. Ironic? Maybe not, that now she’s taking care of other hearts, with her strong, brave heart. ● News 12’s Carol Silva has been bringing Long Islanders their local news for more than 25 years weekdays starting at 5 AM on News 12 Long Island.
