The accreditation award recognizes Penacook’s dedication to continuous compliance with The Joint Commission’s state-of-the-art standards. LIZ COULTER, RPT is on a long-term mission to expand short-term care New rehab director wants to broaden Penacook’s message PENACOOK PLACE IS HIGHLY REGARDED for its long-term care. As someone from the area, Liz Coulter, RPT has long been well aware of its favorable reputation. But as Penacook’s new rehab director, she wants people in the Merrimack Valley and beyond to expand their perception and appreciation of the facility. Coulter wants them to know that in addition to long-term care, Penacook offers an equally praiseworthy short-term care program. “It’s something of a well-kept secret,” she says. She sees part of her job as helping to get the word out and making Penacook’s short-term care services, also known as subacute rehab. “When patients need rehabilitation for perhaps two or three weeks before they can Continued on p. 3 INSIDE: Your letters Short- and long-term care Celebrating years of service Penacook underwent a rigorous on-site survey in February 2013 which evaluated the organization for compliance with standards of care specific to the needs of residents, including infection prevention and control, leadership, and medication management. “In achieving Joint Commission accreditation, Penacook Place has demonstrated its commitment to the highest level of care for its residents that goes above and beyond federal and state requirements,” says Gina Zimmermann, M.S., senior executive director, Long Term Care Accreditation, The Joint Commission. “Accreditation is a voluntary process, and I commend Penacook Place for successfully undertaking this challenge to elevate its standard of care and instill confidence in the community it serves.” According to Julian Rich, president and CEO of Penacook, the process to prepare for accreditation took 18 months and involved a lot of work. “Our staff members did an incredible job getting us ready,” he says. As a testament to their diligence, the Joint Commission not only conferred accreditation, but also offered no recommendations for corrective actions in its follow-up report. “That is Continued on p. 5 Deficiency-free inspection once again The state’s Department of Public Health (DPH) conducts annual unannounced compliance inspections for all licensed skilled nursing facilities in the state. In what has become something of an annual tradition, Penacook Place again aced its inspection. Surveyors found no deficiencies at the facility during its 2012 inspection. Operating on behalf of Medicare, the DPH survey team assesses how well nursing facilities conform to state and federal regulatory and quality standards. During the multi-day, unannounced evaluations, inspectors check for compliance on dozens of items, including areas such as food services, residents’ rights, nursing care, and administration. They tour the facility, interview clients and families, review records, and speak with staff members to make their determination. “As the only not-for-profit nursing facility in Haverhill, our focus is on the residents and quality care,” says William J. Klueber, chair of Penacook’s executive board. “We always try to be a cut above for staffing and services, and that is reflected in our deficiency-free inspection.” While the board is pleased with survey results, Klueber says that Continued on p. 2 We like to share our letters, email, and Facebook posts. As a result of damage to my parents’ home caused by Hurricane Sandy, they were forced to evacuate. With no suitable nursing home options available in the area for my dad, I decided to reach out to you for assistance. I was both surprised and relieved when you told me that you were holding a bed for my dad while I transported him up from New York. We arrived after 10 p.m., and the admissions staff cheerfully came back to work to register my dad and get him into a room right away. During my many visits to Penacook Place, I found the entire staff in the memory unit to be very helpful, patient, compassionate, and caring. Dealing with these sometimes difficult patients (especially my dad) can be trying, but they always seemed to handle the situation pleasantly and with dignity for all concerned. Being in a service related industry, it is important to know how your staff performs when the boss isn’t around. I can assure you that your staff understands the Golden Rule identified in your mission statement: “To care for our residents as we would want to be cared for.” They cheerfully live by it each and every day. He adds that businesses and individuals who support Penacook Place through their donations share in the achievement. “Thanks to their support, we are able to maintain our mission.” Along with Joint Commission accreditation, deficiency-free inspections offer reassurance to residents and family members about the quality of the care they or their loved ones are receiving at Penacook. They provide an impartial and critical stamp of approval and help offer peace of mind. Continued from p. 1 return home after a hospitalization, we want them to think of Penacook as the go-to place.” Coulter is helping to oversee an expansion of the facility’s short-term care services. She is developing an orthopedic program to help patients recover from surgeries and injuries related to muscles, bones, and joints such as hips, knees, and shoulders. To develop the program, Coulter is coordinating specialized training of staff members so that they will have the skills to follow the protocols prescribed by doctors and be able to work with orthopedic patients. She is also establishing and enhancing relationships with area hospitals to seek their input and keep them informed about the expanded services. Likewise, the rehab director is planning to expand Penacook’s pulmonary program so that patients with chronic obstructive pulmonary disease (COPD), congestive heart fail- ure, and other ailments can get the care and support they need. Coulter is helping to coordinate additional training for staff. Part of the training includes teaching staff members how to educate the patients themselves so that they can better recognize the signs and symptoms of pulmonary and cardiac distress and avoid future hospitalizations. Caring for short-term patients is quite different than caring for long-term residents, and part of the training Coulter is developing involves helping staff adjust to the differences, such as changing their expectations. While training staff members across all disciplines is critical as Penacook expands its short-term care and seeks to more fully integrate the services throughout the organization, Coulter says that Penacook is well prepared for the task. “The degree of caring that exists here is extraordinary.” For example, she says that nurses and CNAs are Positive Satisfaction Surveys Each year we survey our residents and their families to see what they think about a number of different areas of our home. This year we scored an average of 4.4 on a scale of 1 to 5 with 5 being the highest satisfaction score. Top rated areas included: helpful business office staff (4.8), comfortable raising concerns (4.8), confidence in the quality of care and safety (4.7), residents are treated with dignity and respect (4.7) and staff respond in a sensitive manner to requests (4.7). She oversaw the transitional care program there and developed an orthopedic rehabilitation program. She also worked extensively in training and education at the facility. In addition to working with patients, Coulter says that it is important for a successful rehab program to reach out to and work with families. They are key sources of information as Penacook admits and begins to work with rehab patients. And they are an important ally in helping patients safely and successfully return to their homes. Coulter enjoys her role at Penacook and is looking forward to trumpeting the rehab programs to the community. “We do subacute rehab very well,” Coulter says. “Now, we are going to do more of it, and we look forward to helping even more patients make the transition back to their homes following an illness or hospitalization.” Before discharge, the rehab team conducts a home evaluation and checks factors such as mobility in the patient’s actual living space. Here too, families play an essential role. “It is important that the people closest to the patient are aware of and participate in the discharge plan,” Coulter says. While on site, Penacook’s therapists, in conjunction with families, can help remove or reduce obstacles in homes and discuss strategies to resolve other issues. Friends of Penacook Place As she was studying to be a physical therapist, Coulter was initially interested in pediatrics. She trained at New England Rehabilitation Hospital in Woburn as part of her college program. While there, she worked with many types of patients and was introduced to the geriatric population. “I discovered that I really enjoyed working with them,” she says, even though they are at the opposite end of the spectrum of her intended area of specialty. “They are tough. They work so hard to be as independent as possible. Julian Rich & Bill Klueber Quality short- and long-term care As you are aware from the lead article in this newsletter, we were recently awarded accreditation by the Joint Commission, which validates our commitment to quality care and exceptional service. Last fall, our third deficiencyfree survey in the past four years provided another indication of our determination to do the best for those we serve. In spite of the fact that we continue to be challenged by decreased funding and increased regulations, our amazing staff works harder than ever to enhance the lives of our residents. In December, we began a partnership with Covenant Health Systems so that we could benefit from their expertise, resources, and experience in senior care. While we have enjoyed success as an independent, standalone non-profit facility, our future depends on developing Jean Jovell, LPN retired as the charge nurse of our Memory Care Unit after working at Penacook for 31 years. Margaret Gomez, housekeeping supervisor, retired after 19 years, and Dale Clayton, RN retired as nurse manager of the Hillview Terrace after 15 years. These special, dedicated women were part of our family and worked hard every day to care for those who call Penacook home. They will be missed, and we wish them the best in their retirement and future endeavors. relationships and affiliations that will build upon our strong foundation. One of our current challenges is to expand our services to those in our community that require post-acute and rehabilitative services. While many of our residents and patients have chosen Penacook as their home, we find that most of our admissions are now for rehab and other short-term services after a hospitalization. It is important to note that the federal government is encouraging facilities like Penacook to increase their capacity to provide post-hospital care to those who are able to return to their home following an illness. Now that Penacook has achieved accreditation, Toothaker will assume the dayto-day responsibilities of complying with The Joint Commission’s standards and maintaining excellence. “We developed a mandate to pursue accreditation over two years ago,” says William J Klueber, chair of Penacook’s executive board. He says that it had been under discussion for some time before that, and that he and the other board members are proud of the staff’s preparation and performance. “They really came through,” he says. While Penacook has a welldeserved reputation for quality care, The Joint Commission accreditation adds the recognition and imprimatur of a prestigious third-party organization. The dining service department provided delectable hors d’oeuvres, door prizes were awarded, and each guest went home with a commemorative wine glass. Students from Whittier Vocational-Technical High School, under the instruction of teacher Earl Corr, volunteered their services to construct a pergola on the patio at Penacook Place. Julian Rich, president/CEO, spoke briefly regarding the state of long-term care, our dedicated short-term rehab unit, and our recent attainment of Joint Commission accreditation.
