I will be presented by my (mother/father/other) at a Scholarship Ball sponsored by CHARM
Outreach & Mentoring Services, Inc. on Sunday, July 31, 2011. I am asking you for your
support and patronage by purchasing an advertisement in the Souvenir Presentation to be
presented on the evening of the Scholarship Ball which will be held at the Drury Lane – Oak
Brook, Illinois. My ad sales from the Scholarship program will contribute towards a scholarship
for myself or another deserving young lady in our organization.

CHARM is a faith based 501c3 non-profit organization created by women exclusively for
women with the goal of transforming lives. The mentors of CHARM are committed to a realistic
approach purposed to positively influencing the lives of young girls, teens, young mothers, and
adults. CHARM’s motto , "meeting you where you are", simply means that no matter where you
are spiritually, socially, mentally, or emotionally, we will be there too.
The Souvenir Journal Advertisement Form outlining the pricing structure is attached. Also I’d
like to take this opportunity to extend an invitation for you to purchase a dinner seat at my
Scholarship table at a cost of $50 per seat.
