Note:
 To facilitate OSA to manufacture sufficient quantities of souvenir items, we suggest that
orders should be placed before 30 June 2014.
The sum of the order has to be settled in full when order is placed. OSA will try to fulfill
all the items ordered by individual alumni, but if for any reason your order could not be
fulfilled in full, OSA would refund the amount of the unfulfilled portion of your order.
Personal Particulars collected in this form will be treated as strictly confidential, and will
only be used for advising you the goods collection arrangement, and will not be used for
other communications with you.
In case alumni like to place order in currency other than HK$, please contact us on
(852)2448-0899 or through e-mail [email protected] to sort out the arrangement.

The tentative earliest collection date of souvenir items is mid August 2014. OSA would
inform you the exact date(s) for goods collection. Most probably, items collection
would be scheduled at some weekends in Queen Elizabeth School and/or during the
QES60 Gala Dinner on 22 November 2014.
 As OSA has very limited man-power, we cannot establish routine arrangement to deliver
souvenirs to you (by postal service or any other usual courier means). However, if
alumni finds special but practical delivery service is needed, we suggest that you can
contact us on (852)2448-0899 or through e-mail [email protected] to sort out the
arrangement (and unavoidably, incurred cost and necessary surcharge has to be borne by
the alumni).
