How to Request a Transcript from DHS

All transcripts are sent electronically through Docufide Secure Transcript. Docufide charges $5 for each transcript sent,
including those for mid-year grades and final transcripts. Fees are to be paid via debit/credit card at the time of the
order.
To have transcripts sent, simply follow the steps below.
1. Register
Go to the DHS webpage http://dhs.dist113.org and click on the Docufide link or go to www.docufide.com.
Click on the “Students & Parents” tab at the top of the page.
Click on the “Register with Docufide” link under the Get Started heading.
Do a name search for Deerfield High School in IL; make sure you select the correct Deerfield High School
before continuing because there are a number in the system.
The first registration page will ask you for enrollment status, year of graduation, and student ID#: DO NOT
USE YOUR SOCIAL SECURITY NUMBER – USE YOUR STUDENT ID! Be sure to fill out all three of these fields.
The next registration page asks for additional info, only Name, Date of Birth and Gender are required but
you may provide more info if you like. When entering your name, enter your full, proper name as it
would appear on your transcript or in school records. If you go by Jim but your name is James, register
with James. If you have a hyphenated last name, like Harrison-Rose, use that if it appears that way on
your transcript, report card, etc.
Complete the registration process by inputting your e-mail and selecting a password. Docufide will send a
confirmation e-mail to your address with a link to complete registration. Use your DHS email account
(ending in “@student.dist113.org”).
You’re done! You can now request transcripts through Docufide.
2. Request a transcript for a college/university not on the Common Application
Go to www.docufide.com and click on the “Sign In” tab in the upper right hand corner to sign in using the
e-mail and password you used to register.
Search for your institution by name or state, click on the school to add it to your list of schools where you
would like your transcript sent.
Once you have selected a school, check to make sure it is the correct institution. If you are sending the
transcript to complete an initial application, leave the “Current Transcript” bubble checked. If you
need/want to have mid-year grades sent, check the “Next Grading Period Transcript” bubble. This
transcript will be sent as soon as first semester grades are posted in mid-February.
At this point you can save and add another school to your request by following the same process, or save
and continue to finish your request. Once you have completed all of your requests for this session, click
“Save & Continue” to finish the process, including acceptance of the user agreement and payment.
You will receive a series of three e-mails from Docufide; one confirming your request, one letting you
know when it was sent to the college and a final one letting you know the college has received it.
3. Request a Transcript for a Common Application school
First, you will need to create an account at www.commonapp.org and note your Common App ID.
You will need it to make sure your transcript gets to the right place.
Add any school you may apply to that is on Common App (see Common App instruction sheet).
Use the Education and School Forms sections of the Common App Online to invite your counselor
to be a recommender. Doing this will trigger an email message to your counselor providing
instructions on how to submit school forms online. Although this is one of the last tabs, you do
not need to have the other sections complete to do this. Be sure that you enter your counselor’s
name and email address correctly! Look on the DHS counseling website to verify their info.
Log into your Docufide account and click on the Common App tab.
You will be asked for your Common App ID – once you do enter this, you should see your name
and your counselor’s name.
Your transcript will be sent directly to the schools you have listed on your Common App account,
where it will be matched to your counselor’s recommendation form and submitted on your
behalf. For these to be matched, please allow one week from the time you enter your counselor’s
information on Common App (School Forms) for your counselor to process it before you request
the transcript.
Once you have requested your transcript to Common App, your transcript is ready for submission
to ALL of your Common App colleges that you have listed at that time. You do not need to send
one to each individual Common App school.
4. Request a transcript be sent somewhere other than a college/university (scholarship organizations, NCAA,
summer programs, etc.)
Go to www.docufide.com and click on the “Sign In” tab in the upper right-hand corner to sign in using the
e-mail and password you registered with.
Under “Select Destination” choose the destination you wish to have the transcript sent to. Unless you are
sending to the NCAA you will most likely click “Other”.
If you are requesting a transcript for the NCAA - once you click on the NCAA bubble you follow the same
process as you do when sending to a college/university.
If you select “Other” you will be instructed to fill out information on where you want the transcript sent,
including type of organization, the name, and address. You can have this transcript sent electronically by
PDF if the organization has that capability, check with them before choosing this option.
At this point you can save and add another request by following the same process, or save and continue
to finish your request. Once you have completed all of your requests for this session, click “Save &
Continue” to finish the order, including acceptance of the user agreement and payment.
In order to complete this process you must return the Docufide Transcript Authorization Form (TAF) via email or fax (Docufide needs your authorization to send a transcript to somewhere other than an
educational institution).
You will receive a confirmation e-mail from Docufide acknowledging your order and a follow-up contact
when the transcript has been sent.
**Please be very careful and double-check your work before submitting to Docufide!
Please allow at least one week for your transcript to be sent once you put the request into Docufide.
