Be it known that ________________________________________________ has attended the 2015 Joint Congress, in Montréal, Québec, May 28 – 30, 2015 and has earned ____ credit hours under Section 1 of the Royal College of Physicians and Surgeons of Canada Maintenance of Certification (MOC) Program ____________________________ Dr. Jacques Lévesque __________________________ Dr. Christian Blais President of the CAR President of SCFR ____________________________ Deborah Murley President of CAMRT __________________________ Danielle Boué President of OTIMROEPMQ The 2015 Joint Congress is an Accredited Group Learning Activity (Section 1) as defined by the Maintenance of Certification (MOC) program of the Royal College of Physicians and Surgeons of Canada (RCPSC) and approved by the CAR for a maximum of 22.50 credit hours. Participants in the Coronary CT Angiography Simulation Workshops are eligible to claim a maximum of 9 credithours (3 credits per hour) under Section 3 of the RCPSC MOC program. The RCPSC recording system will automatically convert the credit-hours for this workshop to 3 credits per claimed hour (i.e., 3 hours x 3 credits = 9 credit-hours). Through an agreement between the Royal College of Physicians and Surgeons of Canada and the American Medical Association (AMA), physicians may convert Royal College MOC credits to AMA PRA Category 1 Credits™. Information on the process to convert Royal College MOC credits to AMA credits can be found at www.ama-assn.org/go/internationalcme. Participants can document their learning in the RCSPC MAINPORT portal at: www.royalcollege.ca Participants should only claim the credits commensurate with the extent of their participation in the activity. Technologists: The CAMRT will provide a Record of Attendance, indicating the number of continuous education credits, to each technologist attending the Congress by June 30, 2015.
