Terms & Conditions of the Company Credit/ Debit Card AutoDebit Service
Dengan pertimbangan Syarikat menerima kebenaran ini, saya bersetuju dengan
syarat-syarat yang berikut:

own responsibility to resolve it with my/ our Credit/ Debit Card company
(including but not limited to any problems due to a breakdown or malfunction or mechanical defect of the computer system or equipment of the
Credit/ Debit Card company).
Autodebit adalah disediakan oleh Syarikat hanya semata-mata untuk kemudahan dan faedah saya/ kami. Oleh demikian, untuk sebarang masalah atau
pertikaian yang timbul akibat pemerosesan/ pendebitan tersebut, adalah
menjadi tanggungjawab saya/ kami untuk menyelesaikannya dengan syarikat
Kad Kredit/ Debit saya/ kami (termasuk tetapi tidak terhad kepada sebarang
masalah akibat kerosakan atau pincang tugas atau kerosakan mekanikal sistem
komputer atau kelengkapan syarikat Kad Kredit/ Debit.
5. Saya akan memastikan pihak Syarikat diberi notis pemberitahuan secara
bertulis ke atas sebarang perubahan, kehilangan atau penggantian Kad Kredit/
Debit saya/ kami, atau pembatalan arahan sekurang-kurangnya sebulan
sebelum premium seterusnya. Perubahan sedemikian atau pembatalan akan
bermula hanya selepas pihak Syarikat telah memberi pengesahan penerimaan
notis pemberitahuan tersebut.
5. I will ensure that the Company is notified in writing of any changes, loss
or replacement of my/ our Credit/ Debit Card, or cancellation of this
authorisation at least one month before the next premium(s) is due. Such
changes or cancellation will become effective only after the Company has
duly acknowledged receipt of such notification.

10. Receipts will not be issued for premium payments made through the
Auto-Debit service via Credit/ Debit Card. I shall refer to my Credit/ Debit
Card statement and annual confirmation letter (statement for Income Tax)
from the Company for confirmation of payments.

dan surat pengesahan tahunan (penyata untuk cukai pendapatan) dari pihak
Syarikat untuk mengesahkan pembayaran.
11. In the event that any money charged to or debited from the above
Credit/ Debit Card account is refundable by the Company for any reason,
the Company is authorised to do a reverse entry to the Credit/ Debit Card
and shall thereafter be fully discharged from all obligations pertaining to
the same. Should any dispute or issue arise regarding any payment or
refund of money paid pursuant to this authorisation, both the Policy Owner
and Credit/ Debit Cardholder shall refer only to each other for remedies and
resolutions.
11. Dalam peristiwa di mana wang dikenakan atas atau didebitkan dari akaun
Kad Kredit/ Debit di atas sebarang sebab, pihak Syarikat dibenarkan untuk
mengembalikan kemasukan wang tersebut ke Kad Kredit/ Debit dan seterusnya
bebas sepenuhnya dari sebarang kewajipan berkenaan perkara tersebut.
Sekiranya terdapat sebarang pertikaian atau isu yang timbul dari pembayaran
atau pengembalian wang yang dibayar berikutan kebenaran ini, kedua-dua
Pemegang Polisi dan Pemegang Kad Kredit/ Debit akan merujuk hanya kepada
satu sama lain untuk mengatasinya.
12. I hereby authorise the Company to release relevant information
inclusive of the Credit/ Debit Card number, policy number and the premium
amount to the processing bank for purpose of processing the Cardholder’s
billing and payment of bills via this auto-debit facility.
12. Saya dengan ini membenarkan pihak Syarikat untuk mendedahkan segala
maklumat termasuk nombor Kad Kredit/ Debit, nombor polisi dan jumlah
premium kepada bank pemerosesan untuk tujuan memproses bayaran
menerusi perkhidmatan Autodebit.
