PROGRAM
INFORMATION
CLINIC INFORMATION
Your child must be between the ages of 7-14 by December 31, 2011. She will
participate in three October clinics at Bank of America Stadium. Clinics will
begin at 12:00 p.m. and end promptly at 4:00 p.m. The TopCats will provide
a positive, productive and structured environment in which your child can
excel. The focus will be on learning the basics of cheer/dance and the material for the Halftime
Performance. Because of limited time available to practice as a group, attendence at all clinics
is recommended in order for your child to perform during halftime of the Washington Redskins
vs. Carolina Panthers game on October 23, 2011.
PROGRAM FEES
A $260 program fee is required for each child and must accompany this form. If a second child
from the same family attends, the fee is $225 for that second child. All fees are non-refundable
after September 16, 2011. You may pay by personal check or money order made payable to the
Carolina Panthers and mailed to: Carolina Panthers, Attention: Junior TopCats, 800 South Mint
Street, Charlotte, N.C. 28202.

ITEMS TO BRING
Child should wear a comfortable shirt, (she will receive her Junior TopCats shirt at the first
clinic), black jazz or sweat pants and sneakers, all suitable for cheer/dance.
TICKET INFORMATION
Until Friday, September 9, 2011, you may purchase October 23, 2011 game tickets on a first come, first serve basis,
with your completed Junior TopCats Registration Form. Tickets are $52 each and located in the 500 level (upper deck).
Tickets will be available for pick-up at the first clinic on Saturday, October 15, 2011, during registration.
You can also purchase tickets on a first come, first serve basis by calling Ticketmaster at 800-745-3000 or visiting the
Carolina Panthers website at www.panthers.com.
Note: You will need a game ticket to watch the halftime performance. If you choose to have your child sit with you in the
stands following the halftime performance, you must purchase a ticket for your child.

AGREEMENT
I have read the above Guidelines and understand the Carolina Panthers Junior TopCats Program is a membership organization with all
fees and costs being paid to the Carolina Panthers. I am aware these fees are non-refundable after September 16, 2011. I agree to adhere
to the above Guidelines and understand that not adhering to the above may result in the dismissal of my child from the program.

WAIVER
In consideration of my child being allowed to participate in the Carolina Panthers Junior TopCats Program, and on behalf of myself and my child, as well as our agents, personal
representatives, heirs and assigns, I agree as follows:
1. I recognize that the Carolina Panthers Junior TopCats Program is a participatory cheer/dance clinic and that there are certain risks inherent in cheer/dance clinics and in this cheer/
dance clinic. Such risks include, but are not limited to, strained muscles, sprained or broken arms and legs, and heat exhaustion. With this knowledge, my child and Iassume all risks of
participation in this cheer/dance clinic. I also certify that my child can participate in the activities at the cheer/dance clinic and is in good health and does not suffer from any physical
infirmities or chronic illness which might affect my child’s ability to participate in the cheer/dance clinic or which might be adversely affected by such participation.
2. I fully and forever release, discharge and hold harmless Panthers Football, LLC, the Carolina Panthers, the TopCats, the National Football League and each of its member clubs, NFL
Properties, LLC, their agents and employees, and all associated sponsors and their agents and employees (collectively, the “Released Parties”), from any and all claims for property
damage or personal injury that may arise from my child’s participation in the cheer/dance clinic, including, but not limited to, any and all claims arising from the negligence or
carelessness of the Released Parties, and any and all claims arising from and Acts of God.
3. I agree that I will not take any legal action against the Released Parties for personal injury, property loss, or any other loss sustained by me or my child as a result of my child’s
participation in the cheer/dance clinic. I also agree to indemnify and release, discharge and hold harmless the Released Parties for any and all such claims, judgments, and
costs, including reasonable attorneys’ fees, incurred in connection with any legal action against persons or entities other than the Released Parties brought by me or my child, our
agents, our personal representatives or our heirs arising out of my child’s participation in the cheer/dance clinic.
4. I also agree that it is possible that an emergency situation may arise and that in the case of emergency, I grant permission for my child to be given medical treatment at a local
hospital and I agree that my child may undergo surgery or other medical procedures in an emergency situation in the event I cannot be contacted.
5. I agree that any legal disputes regarding the interpretation of this document will be determined in accordance with the laws of the State of North Carolina. I further certify that my
child’s participation in this cheer/dance clinic and my signature on this document is completely voluntary and that I am acting of my own free will. I have read this document and I am
willing to sign it, fully understanding that by signing it, my child and I give up many important legal rights that may otherwise be available.
6. I grant the Carolina Panthers the right to copyright and/or publish or use photographic or video footage of my child, in which she may be included in whole or in part or composite. I
understand that the photographs and video footage in which she appears will not be used for resale. Any still or video shots will be used strictly to promote the Junior TopCats annual
program.
