Dear Friends,
Our theme for the 2012 Annual Report is Community. The dictionary defines
community as a group of people held together by a common cause or
interest. The Foundation is then, by definition, a community. We only have
to look to our community of members, donors, volunteers, colleagues and
physicians to find inspiration and motivation.
Forty years ago the La Porte community came together to build ONE
hospital. A proud history of healthcare was created and continues today
as we celebrate the 40th Anniversary of IU Health La Porte Hospital. It is that
same community which made The Foundation a vital part of our healthcare
landscape; ever striving to make the place we have chosen to live, work,
raise a family and grow old a better place to call home.
We THANK YOU for being part of our Community! This report highlights
the impact of your gift - membership or charitable contribution. Be assured
100% of all gifts remain local to serve our community and 100% of your gift
is spent for the purpose intended. Please take the time to review our annual
report and contact us if you have any questions.

A Culture of Giving
A community is born and sustained through the work and gifts of others. This was
evident when the La Porte community came together to build La Porte Hospital.
This year IU Health La Porte Hospital is celebrating its 40th Anniversary.
The La Porte Hospital Foundation congratulates the hospital for 40 years of
providing exceptional healthcare to our community and would also like to
acknowledge the culture of giving that began at that time.
Ben Bortz was selected as Campaign Chairman and he set out into the
community with a goal to raise $3 million of the $8 million needed to build
La Porte Hospital. The community of La Porte heard the call and not only met
the challenge of “We Are Going to Build a Hospital” but exceeded it. Working
to achieve better healthcare for area residents, the community contributed over
$3.5 million through 7,700 pledges.
That same pride and culture of giving is still evident today as the community
continues to support the mission of The Foundation. The generous gifts from
many local organizations and individuals ensure healthcare and healthy lifestyles
in our community for years to come.
The La Porte Community should be proud of the accomplishments
of 40 years ago. The funds they raised to build La Porte Hospital
would equal more than $23 million in today’s dollars. That shows a
strong commitment and caring for the health of our community.
I am glad to see that same commitment and philanthropy
of giving exists today.

Many people answer the call to become a nurse,
but few have fully dedicated their lives to serving
their patients and community in a way that Linda
Satkoski has.
Linda, who received her Bachelor’s degree in
Nursing from the College of St. Francis and her
Master’s in Administration Degree from the
University of Notre Dame, began as a staff nurse
at Pine Lake Division (formerly Community Hospital)
in 1968. She recalls the day La Porte Hospital opened
on September 24, 1972, and the staff at E Street
(formerly Holy Family Hospital) and Pine Lake Division
had to transfer patients to the new hospital.
“We had discharged most of our patients before we
transferred to La Porte Hospital,” recalls Linda. “But I
can vividly remember climbing into the ambulance
with the last patient from Pine Lake Division.”
“When we came to La Porte Hospital, the thing I
remember most was the all-private rooms. Patients
had privacy and their own bathroom. It was like we
had moved into the Ritz Carlton,” laughed Linda.
“At Pine Lake Division we had some patient rooms
with four beds divided by curtains, and bathrooms
were limited.”
At La Porte Hospital, Linda quickly rose through the
ranks and was named Director of Patient Care Services
in 1985 and then Vice President, Patient Care
Services in 1996. Shortly after being named Senior
Vice President Physician Relations/Patient Care
Services in March 2005, Linda took over as President,
CEO/CNO of IU Health Starke Hospital in 2009. In June
2012, Linda was promoted to Chief Operating Officer
of IU Health La Porte Hospital System.
In between her duties of improving patient care,
Linda gave her time and support to The Foundation.
Much like her background in nursing, she felt this was
another way to give back and improve the health of
our community.
I joined The Foundation because it
was the right thing to do... But I quickly
realized it was a cause I believed in.
I saw what The Foundation did and how
it impacted our community.
What The Foundation gives back is
more than just for the necessities;
it goes above and beyond to make life better for our patients.
— Linda Satkoski, Foundation member since 1989
Linda has also gone above and beyond when
it comes to The Foundation. At all of The
Foundation special events, one would find
Linda there volunteering. Her passion for a
healthy community is evident by her
support of The Foundation’s Heart and
Endowment Campaigns. When it comes
to The Foundation’s Scholarship Fund and
assistance to the free community health
screenings, Linda will talk to anyone about
their importance and how they help to build a
better future for our patients and community.
“You just can’t ignore the needs that are met
in our community through the efforts of The
Foundation,” said Linda. “Because of the work
they and countless others who volunteer their
time do for our patients and community, I am
honored and proud to have been chosen as
an Honorary Life Member of The Foundation.”

IU Health VNA Services:
providing compassionate care to our
COMMUNITY
When patients are facing end of life care, IU Health La Porte Hospital
VNA Hospice are there for them and their families. This program offers
compassionate care to people who are in the terminal stage of life and
provides much needed support to the families and caregivers.
Long before he was a Foundation Board Member, Carl Savich
experienced first hand the impact VNA Services has on families in our
community. In 1999, Carl’s wife Lori, experienced a grand mal seizure.
As she received medical treatment for this episode, the family learned
that Lori had a brain tumor. Over the course of the next year, Lori had
brain surgery to remove the tumor, followed by radiation treatments.
After a second surgery in 2000, she went into remission and the family
was able to enjoy four “normal” years before the tumor returned in 2004.
After countless unsuccessful medical treatments and surgery, the
Savichs were introduced to VNA Hospice in the summer of 2005.
Lori spent two months receiving VNA Hospice care before passing away
in late 2005. During those last months, Carl recalls knowing that there
was no “happy ending” in sight. Incredibly important to Carl was the
realization the Hospice Nursing Staff would do all they could to ensure
Lori was comfortable. Additionally, the VNA team offered round-theclock expertise to answer questions and provide support to Carl as he
cared for Lori.

“They were angels to Lori and me. The way they cared and looked after
her was incredible,” said Carl as he recalled the significant impact the
caregivers made. “The VNA services were not limited to physical care.
We received visits from Roger Ash, Hospice Chaplain as well. They truly
cared not only about Lori’s physical needs but provided emotional and
spiritual support to us both as well.”

The compassion and helpful

tremendous. When given the
opportunity to get involved and
give back – it’s important that
you take that opportunity!
– Carl Savich
IU Health La Porte Hospital VNA Services never turns
patients away, regardless of their ability to pay.
For the past eighteen years, The Foundation has
supported the endeavors of the IU Health La Porte
VNA Services through funds raised at the annual
Holiday at the Pops Concert and Hospice Mailing
Campaign. Through the VNA Fund, The Foundation
is able to provide financial assistance to make a
difference in people’s lives. This difference may seem
simple, but it is so important. The compassion and
support of The Foundation donors also helps to
provide for the little things that make the end of life
comfortable and manageable, such as providing
transportation services, chaplain visits and personal
care items to help ease the challenges that multiply
with a terminal patient.
“Looking back, I never really had a clear picture of
the importance of this service until after Lori passed
away. The compassion and helpful spirit of all the VNA
staff was tremendous,” stated Carl.

Each December, The Foundation hosts the Holiday at the Pops Concert to benefit
IU Health La Porte VNA Services (Home Health, Hospice and Skilled Care) and
other philanthropic selections. This event has become known for the joy it brings
to the community.
This night features beautiful symphonic music featuring local talent. The December
8, 2012 event includes the La Porte County Symphony Orchestra (LCSO) directed
by Maestro Philip Bauman, and performances by Kyrie Anderson, Cripple Creek,
William Wolf, MD and special guest artists of LCSO. The Foundation is also excited
to welcome IU Health La Porte Hospital Family Choir directed by Judge Kathleen
Lang, former Foundation and Hospital Board Member.
The IU Health La Porte Hospital Family Choir is comprised of community
members who are directly connected to either IU Health La Porte Hospital or to
The Foundation. We are excited for the first performance of this special group
of IU Health La Porte Hospital Colleagues, Volunteers, Board Members and
Foundation members.
His experiences through the services provided to
his family during their greatest time of need led to
Carl’s decision to join the La Porte Hospital
Foundation Board and give back to the community
he loves dearly. “When given the opportunity to get
involved and give back, it’s important that you take
that opportunity!”

are not able to purchase their medications. Being able to provide medications for a few days until we
organize other resources is very appreciated by the patients and the case managers. Some of them
do not have transportation. Especially during weekends, when public transportation is not an
option, the Samaritan Fund is able to cover the costs of taking someone home. The list of needs is
long and the Samaritan Fund assists us in closing this gap until other resources become available.
As case managers, our role is to work with the patient’s entire healthcare team to assess, plan and
help facilitate the right treatment plan. We want to ensure the patient has what he or she needs to
continue getting well when they leave the hospital.
Through this process we identify the patients who may not be able to continue their treatment plan
because of financial reasons. That is when your gifts to the Samaritan Fund become so important.
We are so grateful for your support. This fund has made a difference for so many of our patients.
We can tell you from seeing first hand, your giving changes lives!

Going above and beyond to support the mission of The Foundation
At the heart of any community or organization are its people. The La Porte Hospital Foundation
is fortunate to have a community of colleagues at IU Health La Porte Hospital who support and
believe in the mission of The Foundation. Among this much appreciated group of colleagues are
those who not only support The Foundation with their gifts, but also give their time and talents
by serving on one of The Foundation’s committees. In addition to the colleagues listed who
have given of their time to assist in the planning of events, membership drives and screening
services, The Foundation depends on countless volunteers. They are integral to the The
Foundation’s success.
This spirit of giving flows throughout the hospital from all departments. Each year, many
IU Health colleagues join together for the betterment of our community. The Foundation would
like to thank these colleagues for helping to plan and run our events.

Planned Giving
When people hear the words “estate planning,” they
often assume it’s an activity only for retirees or
near-retirees. But if you have a family, it’s never too
soon to create your estate plan.
Of course, estate planning can seem like a daunting
task. But you’ll find it easier to handle if you break it
down into three key areas: distributing your assets,
protecting your family and reducing estate taxes.
Let’s look at these topics:
Jonathan E. Salisbury,
AAMS, is a Financial
Advisor for Edward Jones.
He is a board member
for La Porte Hospital
Foundation. Jonathan has
been with Edward Jones
for eight years. He lives
in La Porte with his wife
Brandy, and two children.
• Distributing your assets — Obviously, it’s essential
that you let your family know just how and to whom
you’d like to see your assets distributed. At the very least,
you’ll need to draw up a will. If you were to die without
one, the state could end up distributing your resources,
and it might do so in a way you wouldn’t have wanted.
But even a will may not be sufficient. Many people
design a living trust, which provides them with more
flexibility in distributing assets. For example, you could
direct your living trust to disperse assets to children or
grandchildren at specific ages and allows assets to be
distributed without going through the time-consuming
and public probate process.
• Protecting your family — Estate planning isn’t
just about dollars and cents. It also involves taking
the necessary steps to preserve the welfare of your
family if you are not around or become incapacitated.
Consequently, you’ll need to name a guardian for your
minor children, someone who can step in and raise
them should anything happen to you and your spouse.
And when your children are adults, you’ll want to help
them with decisions that could prove agonizing.
For example, by creating a living will, you can state
whether you want your life prolonged if you ever face
a terminal illness or catastrophic brain injury and are no
longer able to make decisions for yourself. By drawing
up a healthcare power of attorney, you can name
someone to make healthcare choices for you if you are
unable to do so.
• Reducing estate taxes — Depending on the size of
your estate, your heirs may never have to worry about
estate taxes. But that’s hard to predict, especially given
the fact that federal estate laws have gone through
several changes in recent years, and may do so again.
Your best bet is to stay informed about the exemption
level the amount you can pass on to your heirs, free
of estate taxes and look for ways to reduce the size
of your taxable estate. You could, for instance, make
charitable gifts, thus moving these assets from your
estate. You may also want to consider arrangements
such as an irrevocable life insurance trust under which
you can transfer a life insurance policy out of your
estate and have the trust distribute the proceeds to
the beneficiaries you’ve chosen or a credit shelter trust,
which allows both you and your spouse to take full
advantage of both your estate tax exemptions.
A trust can be a complex instrument, so before
establishing one, you’ll need to consult with your tax
and legal advisors. In fact, you’ll want to consult with
them on all aspects of estate planning. It will take time
and effort, but it’s worth it to leave the type of legacy
you desire.
The Cornerstone Society recognizes people who have decided to enhance the future availability of healthcare in our
community by including the La Porte Hospital Foundation in their wills or other deferred gifts in their estate planning. If you
are interested in learning more about the Cornerstone Society or if you would like to inform The Foundation of your plans,

COMMUNITY
Improving the Health of Our
Preventative healthcare is vital to creating a healthier community. Routine check ups are one
of the most important steps an individual can take to ensure proper health. Sadly, many people either cannot
afford or do not have access to routine physicals.
The La Porte Hospital Foundation knows that early detection of disease could save a person’s life. The Foundation
Board of Directors agrees; each year they allocate funds for free community health screenings.
“Over the years, the Wellness Outreach Team had the opportunity to positively impact thousands of community
members through our free screenings and Lil’ F.I.S.H. Club®,” said Katie Sarver, Wellness Outreach Program
Manger, IU Health La Porte Hospital. “We would not be able to provide such high quality screenings and services
without the generous and continued support of The Foundation’s donors.”

comes together for brighter smiles
Most trips to the dentist are born out of the
obligation we learned in our youth... brush,
floss and visit your dentist twice a year.
But more than 100 million Americans
cannot visit the dentist because they lack
dental insurance and can’t afford dental care
or treatment. Due to financial constraints,
some have put off dental care for so long
that their dental health has become critical,
even life-threatening. “People without
dental insurance put off dental care until
they have no other option,” explains Robert
Grace, DDS. “They think they have to decide
between paying for dental care or paying
their mortgage and other bills. So they take
care of their families and hope that the
situation improves.”

Dr. Grace was in private practice for 23 years

community have a better choice. Dr. Grace
continues, “The situation worsens and due
to the pain of decay or infection, they have
no choice but to visit a dentist. They sit for
an exam and they find out how much the
cost of treatment will be, and they never
go back. The treatment is cost prohibitive
and without insurance they can’t afford
the treatment that their dentist advises.
In most cases in many communities, there
is no other option. There is no Plan B.”

What a perfect way to describe IU Health La Porte Hospital Auxiliary. Through the years, the Auxiliary has been a
foundation of volunteerism and support. It has proudly aided the hospital’s growth into the landmark of healthcare
in our community.
IU Health La Porte Hospital Auxiliary was created by the merging of Holy Family Hospital and Community Hospital
auxiliaries over 40 years ago. From the very beginning, the newly merged auxiliary supported the new La Porte
Hospital by giving $125,000 toward the new building’s fundraising drive. Forty-five years later, the Pink Ladies, as the
auxiliary was once known, consists of both men and women from our community proudly wearing their red jackets;
Candy Stripers are now referred to as Volunteens; and the gift shop once known as the Pink Geranium is now the
Wishing Well Gift Shop.
Although the names and faces have changed, the spirit of paying it forward by serving the hospital remains strong.
IU Health La Porte Hospital’s Volunteer Program will continue to be one of our community’s most precious resources.

GRANT NAME
In our communities, one doesn’t have to look very far to find partners who
share in The Foundation’s sense of community. In an effort to expand the
La Porte Hospital Foundation’s ability to fulfill its mission of supporting
health care and healthy lifestyles in our community, the community itself
is our greatest partner. Through local philanthropic gifts and grants, we
receive more than money. We receive affirmation that we are part of a
strong local network focused on distinct local needs.
A community is made up of groups with shared interest within a society.
Improving the health of those in our community is clearly the tie that binds
this community together. This is evident through funding from sources
such as Tri-Kappa, The Matt Moore Foundation, 2 BigHearts Foundation,
Michigan City United States Bowling Congress Women’s Bowling
Association (MCUSBCWBA) and Unity Foundation of La Porte County.
The funding provided through these partnerships extends the reach
of The Foundation, impacting services such as prenatal care at the IU Health
Community Health Center, cardiovascular screenings, mammography
screenings and services, and other health and wellness benefits to the
members of our communities most in need.
Even through difficult economic times, MCUSBCWBA Chairperson Marilyn
Schnick continues to seek support for women’s health in our county. “We
think it is important to try to help out wherever we can, however we can,”
she says. “We know times are tough, but we keep trying.”
It is this spirit, this culture of philanthropy that The Foundation sees as
one of the many things that makes this a special and unique community.
Whether through charitable giving, volunteering, or partnering in numerous
other ways, our community comes together during even the most difficult
times and carries a legacy of philanthropy that has spanned generations.

Through my annual membership gift, I have helped patients who otherwise
would not be able to afford medical care, scholarship recipients further their
education, and children grow their knowledge of good health habits. Being
a member of The Foundation, I am part of something bigger than myself.
Together with your help, we can increase the number of lives impacted by
the funds membership dollars support. Through our membership, we can
purchase equipment and supplies to enhance patient care, provide the
community with health screenings, offer dental screenings for students and
so much more.
If you are not currently “IN” The Foundation, please consider an annual
membership today. The Foundation’s vision to be the leader in health
related philanthropy to support a healthy community can be achieved.
Many partners are needed to accomplish this vision. Your partnership
is needed to accomplish this vision! We thank all members
of The Foundation and we want you all to say “I’m IN”!

I support the La Porte Hospital Foundation because it is the
leading organization in our community to support IU Health
La Porte Hospital and community wellness. Of course the
hospital restores the health of the ill and injured. It also
supports wellness, so patients recover more fully. They also
work to prevent illness in our community in numerous ways.
A healthy community is one in which we can enjoy our family,
friends, and be effective workers. Thanks to The Foundation,
our hospital and community can continue to be leaders in

These proceeds help fund healthcare scholarships for the communities served
by Indiana University Health La Porte Hospital. Since 1983, our scholarships and
awards have provided financial assistance to students pursuing careers in health
science occupations. Through these scholarships, area students are encouraged
to pursue health careers and colleagues are encouraged to “go to next level”
of education.
“The Foundation’s golf tournament has continued to positively impact local
students to achieve their education goals for many years due to the hard work
of our committee, chaired by Board Member John Leinweber,” said Liz Kaminski,
Foundation Board Chair. “On behalf of The Foundation, we truly appreciate the
CAR SEAT
FITTING
TRAINING
support of the many companies, community members and Briar Leaf Golf Club
for their underwriting and participation.”
As part of the Safe Kids Program, IU Health La Porte
Hospital colleagues assist the community by
providing child car seats & booster seats to families
who would not otherwise be able to afford or have
access to them. Additionally, the Safe Kids program
hosts Child Passenger Safety Trainings. During these
trainings, colleagues learn how to properly install,
fit and adjust child safety seats to ensure they meet
safety standards. The Safe Kids program is supported
by The Foundation through grant funding provided
by Indiana University School of Medicine.
