Rebate claims must be received by mail at the address shown on this form and cannot be received by email.
Submit a copy of the packing slip or invoice showing the product purchased and the purchase date.
Rebate form and supporting documents must be postmarked no later than January 30, 2009 and be received by February 15, 2009.
Please allow 8 to 10 weeks to process your rebate.
This form must be signed in order to be valid.
Purchased must be made between November 22, 2008 and December 31, 2008. Purchases made before or after these dates will not be eligible.
Any misrepresentations or fraudulent information disqualifies the claim.
Offer is not valid in conjunction with any other Blue Rooster Co. offer.
Blue Rooster dealers, distributors, resellers and their employees are not eligible to receive any rebates.
Limit ONE rebate claim per model, per person, household, family or address.
Any submission with invalid or undeliverable address will be rejected.
