On Friday, July 15, 2016 at 1112 hours, I was dispatched to the Butler Township Police Department,
3510 Sudachi Drive in Butler Township, Montgomery County, Ohio regarding a vehicle crash.
Upon arrival, I met with the driver of Unit #2, Tarry Snow. Mr. Snow was also the owner of the
vehicle. Mr. Snow stated he was involved in an ACDA crash on US Route 40 at Union Airpark Blvd on July
13, 2016.
Mr. Snow stated the driver of Unit #1 did not provide any insurance information and gave a name of
"Tommy" (unknown last name.) The driver of Unit #1 did provide a cell phone number. Mr. Snow stated
the driver of Unit #1 left the scene in a hurry and he had not been able to make contact with the
driver with the cell phone number that was provided. Mr. Snow stated he had called multiple times,
even from a different number, in the event someone would answer the phone and by not recognizing his
phone number. Mr. Snow stated he had never received a call back and no one ever answered the phone.
Mr. Snow described the driver as a white male, short brown hair, and approximately 20-22 years of age.
The vehicle was described as a black Ford Escape with Ohio license plate number FSV5033.
Using his cell phone, Mr. Snow took a picture of the license plate number and the front of the vehicle
of Unit #1. Mr. Snow showed me the picture.
I checked the back of Mr. Snow's vehicle, and I found an imprint of a license plate that clearly
displayed the license plate number of FSV5033. I ran the license plate number through LEADS, and found
the vehicle belonged to a Tyler S. Wise.
Mr. Wise is a white male, 22 years of age with brown hair. As I was running the license plate through
LEADS, Mr. Snow saw the picture of the vehicle owner on my computer screen and said, "that's the guy."
I have made several attempts to make contact with Mr. Wise via public service on July 15, 2016 and was
unable to make contact. The voice mail on the cell phone had not been set up, and I have not been able
able to leave a message. The calls were made from the police department, so the phone number would
have shown up if the cell phone had caller I.D..
On July 16, 2016, I called Mr. Wise several times and the phone would only ring, and there was still
no voice mail set up. I contacted dispatch to see if they had a different phone number for Mr. Wise
from their past contacts with him. Dispatch had the same phone number that I had been calling and
their last contact was October 2015.

On July 22, 2016 I completed citation number 40673 for ACDA to Mr. Wise. The citation was forwarded to
Vandalia Municipal Court to have the citation served per the request of Detective Sink.
