New CADET Member Forms & Dues
Becoming an Active Cadet is as easy as 1-2-3
Step 1. Complete two (2) Forms: Visit
http://www.capmembers.com/forms_publications__regulations/forms.cfm and complete:
● Form 15 (Application for Cadet Membership in Civil Air Patrol)
▪ Our CHARTER NUMBER is “PCR-CA-345”
▪ Our UNIT NAME is “Los Alamitos Cadet Squadron 153”
● Form 60 (Emergency Notification Data)
Step 2. Submit three (3) checks or money orders along with your CAPF15:
1. $40.00, payable to “National Headquarters, CAP”
2. Squadron Dues & Supplies, payable to “Civil Air Patrol.” Put your name & “SQ153” on the memo line.
▪ If you are joining in:
January or February, dues are $25.00 ($40 dues + $50 supplies = $75.00 total)
March or April, dues are $20 dues + $50 supplies = (total $70.00)
May or June, $15 dues + $50 supplies (total $65.00)
July or August, $10 dues + $50 supplies (total $60.00)
September or October, $5 dues + $50 supplies (total $55.00)
November or December, no dues + $50 supplies (total $50.00)
▪ Dues include pro-rated squadron dues.
▪ Supplies include: squadron pin, squadron patch, squadron t-shirt, squadron cover (hat), belt w/silver
buckle, & selected manuals & regulations.
Note: Annual squadron dues ($25) are collected every January from all SQ153 members.
3. BSA Venturing membership dues, payable to “Long Beach Area Council.” Put your name & “Crew
153” on the memo line.

February – $13.75
June – $8.75
October – $3.75
March – $12.50
July – $7.50
November – $2.50
April – $11.25
August – $6.25

Step 3. Receive your CAPID and Obtain Uniforms:
Upon receiving your Civil Air Patrol Identification Number (CAPID) – typically within 2 or 3 weeks after
submitting your application, go to http://capsq153.org/resources-2/cadet-programs/ and select “SQ153
Promotion Guide.” Complete the steps listed (especially #1-3). This will begin the process for you to
receive a free blues uniform (shirt, pants, cover, & belt only).
You may obtain your BDU and Blues uniform from a number of locations or purchase the uniforms
through the Squadron.
** If you decide to get uniform items from another source, please bring
them in for inspection to ensure they are the correct style/format. Keep all receipts!! **

OBTAINING YOUR UNIFORMS:
BDU – Battle Dress Uniform (Woodland Camouflage)
Submit a check or money order (payable to “Civil Air Patrol”), or cash, to the squadron’s Finance Officer in the
amount of $97.30 (includes tax & shipping; price as of 21JAN12).
This is for your BDU uniform & includes:
▪ BDU Shirt
▪ BDU Belt
▪ BDU Pants
▪ Name tape
▪ CAP tape
▪ CAWG patch
▪ Reverse US flag patch
▪ CAP cut-outs
NOTE: You must still purchase or obtain boots for this uniform. They must be all black, preferably all leather, &
absolutely no zippers, belts, or buckles. Some examples include:

Blues - Blue Service Uniform
Submit a payment as above, in the amount of $136.84 (DID NOT receive free uniform) or $16.73 (DID receive
free uniform) (includes tax & shipping; price as of 21JAN12).
This is for your blues uniform & includes:
▪ Light Blue Shirt
▪ Flight cap/cover
▪ Dark Blue Pants
▪ Name plate
▪ Airman insignia for flight cap
NOTE: You must still purchase or obtain shoes for this uniform. They must be all black, leather, low-quarters, and
absolutely no Corfam® or other type of “high gloss” – they must be polishable with black shoe polish.
