This model sets the example for all HGB team members and helps them understand how we can all have a positive impact by working together. You can see this in action as we move to a Patient-Centered Medical Home model in our practices and as our physicians incorporate the resources offered at AL!VE to create a continuum of care that begins with prevention and keeping our patients healthy, through diagnosis and treatment when you need us more urgently. And we have come a long way since we first began seeing patients in the mansion that was gifted to the community by Ellen Green, on behalf of Henry Hayes. HGB’s more than 100 high-caliber physicians provide an extensive array of care services in more than 25 specialties. In recent years, HGB’s Board of Trustees expanded the hospital’s mission and vision beyond providing traditional hospital services to one that promotes health and wellbeing. In partnership with the community, several groups of hospital employees thoughtfully considered how HGB could have the greatest, positive impact in the region. Out of that was born AL!VE, HGB’s newest facility, an experience-based destination health park and a place unlike any other in Michigan. At the end of 2012, the most recent phase of AL!VE was opened. It included the fitness center, children’s recreation area, conference space and expansion of the walking path. AL!VE offers visitors the opportunity to discover, act on and achieve goals that will help them become their most vital and healthy. HGB also formalized a partnership with Sparrow Health System that allows the hospital to provide seamless care and build greater clinical integration to the benefit of every patient, especially those who need emergency stroke or heart care. The HGB team is proud of all that has been accomplished, and this annual report offers an inside look at these successes. We also look forward to sharing the many exciting initiatives that are on the horizon, which include the development of a breast care center, as well as the expansion of orthopedics and pain management programs, to name just a few. When you come to this small community hospital with a broad vision and bright future, you can be confident you are placing your health in capable hands. 2013 “Enduring great organizations are characterized by a fundamental duality. On the one hand, they have a set of timeless core values and a core reason for being that remains constant over long periods of time. On the other hand, they have a relentless drive for change and progress.” — Jim Collins, from his book, Good to Great 0 6 • TOGE THER IN HE ALTH TOGE THER IN HE ALTH • 07 Top 100 CAH Exceeding Expectations People’s Choice HGB was ranked as a top performing Critical Access Hospital according to iVantage Health Analytics Hospital Strength Index. It was one of only three rural hospitals in Michigan named to the top 100 list of 1,267 Critical Access Hospitals in the nation. The Hospital Strength Index is a comprehensive scorecard based on more than 50 measures that evaluate market conditions, clinical and operational performance, and financial and qualitative outcomes. HGB was awarded the Exceeding Patient Expectations Award from Avatar International, an industry leader in healthcare quality improvement services. It is the fourth year HGB was honored with the recognition. The award is based on patient surveys conducted by Avatar at approximately 400 hospitals nationwide. It is given to the facilities whose overall score exceeds expectations for whichever patient types they currently survey, gauging patient expectations about quality of care, reliability and personal service. HGB is proud to be named in four different categories for the Charlotte Shopping Guide’s 2013 People’s Choice Awards. • Hospital: HGB • Doctor: Scot Randall, D.O. • Conference Center: AL!VE • Workout Facility: MOVE Thank you to the Charlotte community for including HGB staff and facilities in your favorites! earning respect Executive Team Matthew Rush, FACHE, President and CEO Kim Capps, CFO Maureen Hillary, RN, CNO Patrick Salow, EVP, Physician Services and Compliance Jennifer Bucienski, VP, Human Resources Patrick Sustrich, VP, Health and Wellness, and AL!VE Medical Executive Team Anthony Marl, DO, Chief of Staff Todd Otten, MD, Vice Chief/MSQC Chair Richard Pinke, DO, Chief of Medicine Hugh Lindsey, MD, Chief Medical Officer Stephanie Almy, DO, Chief of Surgery Brian Bowditch, MD, Chief of Emergency Barrett Zink, DO, Past Chief of Staff Mark Cimmerer, MD, Member-at-Large Board of Trustees Fred Darin, OD, Chairman Charles Grundstrom, Vice Chair Denise Wheaton, Treasurer Kathy Tomlanovich, Secretary Thomas Eveland, Member-at-large Ed Dobbs, Trustee Sharon Kubica, Trustee Martin Latchaw, Trustee Robert Leeser, MD, Trustee Lori Pray, Trustee Dennis Swan, Trustee Barrett Zink, DO, Trustee Robert Fulton, Emeritus Trustee Virginia Lowe, Emeritus Trustee Richard Trumley, Emeritus Trustee Julia Weaver, Senior Citizen of the Year HGB’s longest serving volunteer, Julia Weaver, was presented with the Senior Citizen of the Year Award for service by the Michigan Office of Services to the Aging, Commission on Services to the Aging and Consumers Energy. They are frequent visitors and you can find them taking advantage of the many spaces throughout the health park that help them feel most “AL!VE.” Carol Monroe, 71, has resided in Charlotte for 34 years, ever since she moved here after her marriage to Leon in 1979. They have lived in the same house built by Josephine and Darrel Johnson, owners of local business, Johnson Lumber. Retirees and HGB donors, Kathy and Scott McLeod were originally members of HGB’s Wellness Center when their daughter, Lindsay Blair, bought them a three-month membership in 2000. When the Wellness Center became MOVE at AL!VE they made the switch to the new location. Carol is ingrained in the community and likes to keep busy by staying involved. She helped get Eaton Clothing and Furniture Center off the ground and has worked with the organization in some capacity ever since. She is also very involved with the First Congregational Church in Charlotte and its community activities. Not much has slowed Carol down until health issues forced her to take an unplanned rest in 2012. “Had it not been for Lindsay’s foresight and enthusiasm, we may never have discovered the benefits of the Wellness Center, and then AL!VE,” Kathy said. Kathy and her family, which also includes Lindsay’s husband, Bill, and grandsons: Brady, 5, and Jordan, 2, all benefit from AL!VE. Two of the spaces they use most often are MOVE and GROW, the children’s enrichment and recreational area – and Brady and Jordan’s favorite spot at AL!VE. “Up until then, I had been pretty healthy,” Carol said. “There were normal things that came up but nothing like what I faced when I had to have emergency gallbladder surgery.” Carol had just moved her mother, who was 101 at the time, to Sensations, a local memory care residence. She wanted her close-by to more easily help with her care. That nearness worked in her favor when Carol staying vital “I like the slide,” exclaimed Brady. “I go down it eight times – no 100 times when I’m in GROW!” The slide is part of the four-story tree house in one of the play spaces in GROW. Pictured at AL!VE are, from left to right, Brady Blair, Kathy McLeod, Scott McLeod, Jordan Blair, Lindsay Blair and Bill Blair “I grew up in Detroit,” Carol said. “I had always wanted to live in a small town.” Kathy also uses JOURNEY for her women’s health appointments, and has enjoyed classes in RELISH, the demonstration teaching kitchen. Scott attends the Guys, Grilling and Golf men’s health event in June, and the entire family has participated in HGB’s annual HealthFest event now held at AL!VE in March. “AL!VE is multidimensional,” Lindsay said. “It’s not just for one type of person. It would be hard not to find something, an interest that couldn’t be fulfilled.” When asked about what they feel they have gained by being part of AL!VE, the McLeods and Blairs agreed it’s about health and family and community. “I think many people have that ‘aha’ moment when they realize they need to start doing more,” Kathy said. “Scott and I both lost parents, and we decided we needed to start taking better care of ourselves. We want to be around for many more years to come. Plus we need to be able to keep up with our grandchildren!” wasn’t feeling well one late October evening, but refused to go to the emergency department. Leon was almost relieved when they received a call that Carol’s mother had been taken to Hayes Green Beach Memorial Hospital to be treated for pneumonia. Carol’s mother was admitted to HGB that evening. Carol did go to the emergency department and went home with gallbladder surgery scheduled for early the following week. However, Dr. Hugh Lindsey, HGB chief medical officer and Carol’s surgeon, took one look at her the next day and decided to perform surgery immediately. It turns out Carol had a large gallstone and her gallbladder was infected. “I ended up being admitted and had a room right down the hall from my mother,” Carol said. She was so impressed by the personal care she and her mother received that Carol took all the names of those who cared for them, from pre-op to the team on the floor, and returned later with “goody bags” full of chocolate and treats for each person. Scott also mentioned that the AL!VE team members are part of that family atmosphere as well. “They are tremendous – friendly and helpful. We have great relationships with them too; they are people you want to have around.” “The staff provided real TLC – it’s that personal feeling that everyone cares,” Carol said. “No matter what I asked for, they went out of their way to provide, including the housekeeping and café staff. I had some strict diet restrictions due to allergies and health concerns. When I mentioned I liked pears, they pureed some just for me because I was still only on liquids. And continued to do that until I could eat more solid food. Even the chairs in the rooms were marvelous.” The McLeods and Blairs plan to continue to make the most of AL!VE’s programs and services for many more years. The housekeepers would stop by to chat with Carol when she wasn’t able to sleep at night. “Everyone was really kind, and so personal and professional; the care was just outstanding,” Carol said. “It’s about healthy habits and creating a way of life,” Lindsay said. “AL!VE offers many different options so you can’t get stuck with the same routines … it’s refreshing.” “And it’s not just what I experienced inside the hospital,” Carol said. “I had car trouble one day and the EMS paramedics who were nearby didn’t hesitate to offer their assistance and jumped my car’s battery.” “It’s unusual for a community the size of Charlotte to have a facility like this,” Scott said. “But Charlotte has a lot of assets: the hospital, Charlotte Performing Arts Center, the schools. AL!VE is the icing on the cake.” Because she lives in the Charlotte community, Carol also appreciates that she is familiar with many of the HGB staff and volunteers. But she would quickly recommend the hospital for its personal care to her mid-Michigan family and friends, even if they live outside Charlotte. Scott added, “Plus the activities are things we can all do together … it’s brought us closer.” 14 • TOGE THER IN HE ALTH TOGE THER IN HE ALTH • 15 Community Generosity Non-profit organizations in Charlotte, Mich., have long enjoyed the philanthropic support of community members. HGB has taken a leadership role in a unique approach by collaborating with other local individuals and organizations to create a strategic generosity plan. Guided by a bold vision the plan’s intent is to inspire the “power of one” across the community and to create a sustainable set of practices for an engaged and supportive citizenry. Whether it’s the power of one hour, one person, one dollar or one project, the power of one can transform communities. The generosity plan has several components: •Operations •Resources •Communications engaging support •Measurement Operations includes strategies for initiating generous actions … celebrating generosity, integrating generosity into health care, education, workplaces and churches; and building systems that connect. Generosity Guild Volunteering and civic engagement are the foundation for a strong nation, vibrant communities, and personal happiness. Local residents who volunteer at HGB are part of its Generosity Guild. It refers to those who share an interest in giving their time and talents to help patients, families, and the staff who devote themselves to health care. HGB has long relied on the support and assistance provided by its volunteers. The first volunteer group started very soon after the hospital was founded in 1933. HGB’s Generosity Guild includes many branches: • Courtesy Volunteers help in clinical departments by greeting, guiding, assisting with discharges, and aiding hospital staff. • Auxiliary Volunteers expertly manage all aspects of the hospital gift shop, as well as host many creative events to raise money for hospital needs. In fiscal year 2013, TRUST, the women’s health suite, in JOURNEY was brought AL!VE by the Auxiliary through their donation of $75,000. They also donated $25,000 toward the purchase of two blanket warmers at HGB. • HGB Chaplains provide non-denominational support for patients, families, and staff, to help cope with the emotional challenges of medical crises. • Office Volunteers provide assistance behind the scenes with the countless administrative demands of providing hospital services. • AL!VE Volunteers help deliver experiences at HGB’s newest facility, focused on health and well-being. More than 150 individuals are part of the hospital’s Generosity Guild, many choosing to serve in multiple roles. Additionally, the hospital provides an eight-week summer teen program and offers occasional internships for college students. The Generosity Guild is one way the hospital manages health care costs. Well over 6,000 hours of time was donated in the past year by the Generosity Guild. We all benefit when community generosity is thriving. The Resources component includes strategies for engaging individual philanthropists, grants, local organizations and corporate partners. Communications involves all the ways to tell generosity stories. And Measurement includes the various ways the community can measure and track local trends in generosity. The community conducted a generosity survey at festivals, in coffee shops and local papers. Stories of generosity now appear regularly in local publications. Local charitable assets groups have begun to connect with youth groups. A local time bank initiative was established, creating systems through which people are able to put their time, energy and wisdom into motion for another person. And Hayes Green Beach created a Power of One Fund, to set aside 1 percent of its annual net income for a fund to seed generosity initiatives. The original vision for community generosity included a concept for a fund that would support collaborative projects and fund multiple organizations. Since 2008, a local committee of community volunteers, including HGB representatives, has pursued this vision, realizing it at last in 2012 when the Charlotte Generosity Fund was established. Initial donations came from a new annual generosity celebration. During the last year, the Charlotte Generosity Fund framework was shaped by adapting the Seattle Foundation’s Seven Elements of a Healthy Community to provide the basis for how grants would be made available from the fund. The vision for the upcoming year is to build a plan for local surveys and prioritization, so that community needs in the seven elements are linked to grants through the Charlotte Generosity Fund. The community is recognizing HGB’s efforts. In a recent survey conducted by Professional Research Consultants, more than 40 percent of the 600 respondents named HGB as the hospital that does the most to give back to the community. HGB was mentioned twice as many times as the next hospital named. 16 • TOGE THER IN HE ALTH TOGE THER IN HE ALTH • 17 Donor List OAK: $250,000 AND ABOVE Eaton Federal Savings Bank HGB Auxiliary Sparrow Mr. and Mrs. Richard Trumley HICKORY: $50,000 – $249,999 Keith and Connie Bazaire Crandell Bros.
