Session Abstract:
The landmark Supreme Court decision regarding the Affordable Care Act is of
exceptional significance for case managers and health care providers. Basic
economic and federal fiscal issues will be discussed, along with the implications
for health care reform. The speaker will also review the state-by-state decisions on
Medicaid expansion and the impact on providers.

1. Identify the major points of the Supreme Court’s decision on the Affordable
Care Act
2. Recognize the economic and fiscal issues involved in health care reform

Session Abstract:
You may often refer to the “Next of Kin Law,” the spousal-impoverishment clause
for Medicaid, and the rules for the Georgia Advance Directive, but have you seen
the actual law or know where to find it? Have you heard of the POLST or the new
Assisted Living Community changes in Georgia? Why is all of this important? After
time, these laws often get misquoted as fewer and fewer of us see them ourselves
and we depend on hearsay. This presentation will show you the actual laws and
apply them to real-life cases. Some of it should be a “refresher” course while we
have added some new content/updates for 2013. You will either leave with new
knowledge or empowered in your current knowledge by seeing the laws firsthand.

1. State two different parts in the Georgia “Next of Kin” Law
2. Identify two resources for locating different bills in Georgia
3. Name two parts of the POLST form in Georgia

Session abstract:
This presentation will review the origin of the DNR order and look at the impact of
TV shows on a lay person’s decision-making in regards to end-of-life issues. In
particular, this presentation will examine how wording can influence a caregiver’s
ability to allow a loved one to die peacefully without heroic efforts. Finally, this
presentation will educate the audience on what a natural death is, including looking
at proponents for and opponents against using AND (Allow Natural Death).

Session Abstract:
With rising unemployment, greater numbers of uninsured individuals and
increasing community diversity, case managers face new challenges in discharge
planning on a daily basis. Case managers must have an extensive knowledge of
local, state, national and international resources to meet the increasing needs of
their patient population. In this session, the panelists will discuss a patient
scenario that required the development of a creative discharge plan. Discussion
will include the multiple barriers to discharge that were encountered and how
they were addressed.
Learning Objectives:
1. Describe the role of the case manager in the discharge planning process for
the case study patient
2. Identify the barriers to discharge for the selected scenario
3. Describe the available community resources utilized to address the barriers to
discharge and how these translate to daily case management practice
